    The implementation of social dialogue in the institution of higher education in the development of social partnership

    Increasing information awareness of the development of social partnership is a necessary and vital result of twenty years of development of social partnership in Russia. Conferences (roundtable discussions) of stakeholders of organizations, members of social partnership at the regional level can be the means of increasing information awareness. The staff of the University has a powerful resource in this area. The paper describes the project of the conference on exchange of experiences in research and development of social partnership in the region. Professional playground of the University is the best venue for such conference.Повышение информационной осведомленности о развитии социального партнерства является необходимым и насущным результатом двадцатилетнего развития социального партнерства в России. Средством повышения информационной осведомленности может быть конференция (круглый стол) стейкхолдеров организации, участников социального партнерства на региональном уровне. Персонал вуза обладает мощным информационным ресурсом в этой области. В статье описан проект конференции по обмену опытом исследования и развития социального партнерства в регионе. Профессиональная площадка вуза является оптимальным пространством для проведения такой конференции

    Educational needs and opportunities for adaptation of students with disabilities and persons with health limitations in the tertiary education system

    The article is devoted to the definition consideration of the educational needs of persons with health limitations (hereinafter – HL) and disabled persons in the tertiary education system; the aspects in which these needs are expressed. The article highlights the adaptation possibilities of disabled people in the tertiary education system, as well as the specifics of the organization and goals of an educational institution for teaching people with HL. The authors identify the barriers of social relations faced by students with HL, disabled students and their teachers at universities; analyze the degree of involvement of a student with HL in the educational process. The paper pays attention to the main approaches to the organization of educational activities of persons with HL and disabled people, namely: competence-based, project-based and activity-based approaches. The main types of inclusion, identified on the basis of medical indicators, as conditions for organizing the training of students with HL and disabled students are considered. It is determined that the education organization at the university of students with HL allows to adapt educational programs to the special needs of students, depending on the state of health of students with HL and of disabled students

    Observation of Resonant Diffusive Radiation in Random Multilayered Systems

    Diffusive Radiation is a new type of radiation predicted to occur in randomly inhomogeneous media due to the multiple scattering of pseudophotons. This theoretical effect is now observed experimentally. The radiation is generated by the passage of electrons of energy 200KeV-2.2MeV through a random stack of films in the visible light region. The radiation intensity increases resonantly provided the Cherenkov condition is satisfied for the average dielectric constant of the medium. The observed angular dependence and electron resonance energy are in agreement with the theoretical predictions. These observations open a road to application of diffusive radiation in particle detection, astrophysics, soft X-ray generation and etc.. `Comment: 4pages, 4figure

    Efficacy of Angio-Seal Device for Primary And Complicated Haemostasis: a Multicentre Retrospective Assay

    Background. Femoral approach in endovascular surgery remains relevant being, however, associated with the risk of fatal and non­fatal complications. Modern techniques for instrumental haemostasis achieved with the Angio­Seal device (Terumo Corporation, Japan) provide an opportunity to reduce the number of fatal and non­fatal complications. Aim. A multicentre assay of the efficacy and safety of instrumental puncture closure using Angio­Seal devices (Terumo Corporation, Japan) in various settings and clinical situations.Materials and methods. A multicentre retrospective analysis of 1088 use cases of the Angio­Seal device  (Terumo Corporation, Japan) for “primary” and “complicated” instrumental haemostasis after femoral access  has been conducted for the years 2018–2020. The mean patient age was 62.3 ± 11.6 years, including 845 men (78.0%) and 243 women (22.0%). Five centres for routine and emergency endovascular radiology participated in the study.Results. The Angio­Seal device (Terumo Corporation, Japan) demonstrated a high method efficiency, with a  97.0% success rate of “primary” instrumental haemostasis (1055 of 1088 total successful haemostatic  procedures) in various clinical settings. Arterial thrombosis at the approach side had a 3.0% complication rate  (5 of 33 observations), all other complication types associated with haemorrhagic events (8 cases) or device malfunction and damage (20 cases).Discussion. The assay conducted suggests that the incidence of “complicated” instrumental haemostasis  relates to technical drawbacks of the device application that required advance planning. The key identified  prerequisites for a successful Angio­Seal application (Terumo Corporation, Japan) are the alignment of anchor tube­delivery through soft tissue into the vessel lumen and its positioning inside the vessel.Conclusion. The constructive features and instrumental haemostatic technique realised in the Angio­Seal  device (Terumo Corporation, Japan) enable an effective and safe primary sealing of arterial puncture site in 97% cases

    Haemoptysis as Complication of Wire-Guided Pulmonary Vein Orifice Cryoballoon Ablation in Complex Anatomy: a Clinical Case Series

    Background. Cryoballoon ablation of pulmonary vein orifices is an effective measure in atrial fibrillation but associates with risks of interventional complications, including haemoptysis. The described clinical case series presents novel evidence to revisit the complication sources.Materials and methods. Four haemoptysis-complicated cryoballoon ablations of pulmonary vein orifice have been analysed retrospectively. Interventions completed in a common clinical protocol with no additional postoperative complications.Results. Fluoroscopy revealed a loop-formed balloon-supporting end of guidewire located in peripheral lower lobe of right lung. Postoperative chest computed tomography also depicted the main lung blood volume confined in right lower lobe, thus indicating pulmonary damage by the guidewire.Discussion. Pulmonary vein anatomy is diverse and can greatly sophisticate the cryoballoon ablation procedure with a peculiar balloon navigation in left atrium. An optimal pulmonary vein orifice occlusion can be attained with guidewire, albeit incurring complications in form of haemoptysis.Conclusions. The appliance advancement towards lung periphery associates with a higher risk of haemoptysis and bleeding. Perioperative haemoptysis and bleeding are most likely associated with mechanical injury to minor veins and require further investigation

    XUV digital in-line holography using high-order harmonics

    A step towards a successful implementation of timeresolved digital in-line holography with extreme ultraviolet radiation is presented. Ultrashort XUV pulses are produced as high-order harmonics of a femtosecond laser and a Schwarzschild objective is used to focus harmonic radiation at 38 nm and to produce a strongly divergent reference beam for holographic recording. Experimental holograms of thin wires are recorded and the objects reconstructed. Descriptions of the simulation and reconstruction theory and algorithms are also given. Spatial resolution of few hundreds of nm is potentially achievable, and micrometer resolution range is demonstrated.Comment: 8 pages, 8 figure

    Pancreatic Perivascular Epithelial Cell Tumor (PEComa) with Liver Metastasis: a Case Report and Literature Review

    Aim: to present a clinical case of pancreatic PEComa with liver metastases with an emphasis on morphological features, radiology diagnostic methods and features of treatment tactics, allowing practitioners to get an idea about this rare mesenchymal tumors composed of “perivascular epithelioid cells”, and a review of literature data on pancreatic PEComa, including 32 author's cases.General statements. A 22-year-old woman who was surgically treated for pancreatic head PEComa with infiltration of the duct of Wirsung, common bile duct, duodenal wall, focal invasion into the blood and lymphatic vessels, and perineural space infiltration. Synchronously, metastases were detected in both liver lobes, for which she took Everolimus for 6 years under the control of radiology methods with dose adjustment and frequency of administration. At the A.V. Vishnevsky National Medical Research Center of Surgery, the patient underwent right-sided hemihepatectomy and atypical resection of II-III liver segments.Conclusion. To determine clearer criteria for the diagnosis and differential diagnosis of PEComa, to identify criteria for the malignancy of these tumors, to develop treatment tactics and further dynamic monitoring, a set of statistical data of significant group and randomized clinical trials are needed. Pancreatic PEComas in this group are extremely rare and often have a benign course. The presented clinical case demonstrates the most malignant form of this tumor with localization in the pancreas and liver metastases

    Diagnostic and Treatment Errors in Biliary Cystadenomas and Cystadenocarcinomas: Clinical Cases

    Background. Biliary cystadenomas and cystadenocarcinomas are rare cystic tumors of the liver. Complicated differential diagnostics for simple cysts often leads to errors in surveillance of patients with these tumors. Cystadenoma and cystadenocarcinoma should be suspected upon detection of single or multilocular cystic neoplasms of the liver with septa and blood flow loci  in the cyst wall, especially in middle-aged women. The localization of the tumor is critical. The most common localization is segment IV of the liver. Urgent intraoperative biopsy is required to determine the extent of surgery.Case description. Clinical observations with analysis of the examination and treatment data of two female patients aged 38 and 56 were presented. Both clinical observations illustrate the underestimation of the preoperative examination data that served as a ground for diagnosis of liver cysts with inadequate extent of surgery. In the first case, the resection was incomplete, and, as such, the biliary cystadenoma recurred in the resection area, the capsule of the neoplasm was ruptured and an encysted fluid collection was formed. In the second case, lack of histological examination of the excised neoplasm, due to confidence in its morphological verification as a cyst, resulted in cystadenoma recurrence in the resection zone with metastasis to the contralateral lobe of the liver.Conclusion. Hepatic cystadenomas and cystadenocarcinomas are often misdiagnosed as simple cysts. These tumors should be suspected in central localization of the tumor in the liver, especially in young women. The clinical and instrumental symptomatology and radiological semiotics of the disease require careful evaluation. The recurrence of a cystic lesion in the resection zone in a patient previously operated for a hepatic cyst serves as an additional signal for detecting biliary cystadenoma. Rational strategy for surgical management of cystic liver lesions should include hepatectomy within healthy tissues (both anatomical and atypical) with mandatory intraoperative ultrasound and urgent histological examinations
