    High plasma levels of adrenomedullin in collagen diseases

    Background: Adrenomedullin (ADM), a potent vasorelaxant/hypotensive peptide, was shown recently to be over-expressed in inflammatory rheumatic diseases. Objectives: The aim of this study was to investigate the value of ADM as a laboratory marker of disease activity in juvenile rheumatoid arthritis (JRA) and pediatric onset- systemic lupus erythematosus (SLE) and its relation to other markers of disease activity such as clinical scores, the ESR and tumor necrosis factor-α (TNF-α). Methods: The study included 24 patients with JRA, 17 with childhood onset- SLE, as well as, 19 with rheumatic arthritis and twenty clinically healthy age- and sex- matched subjects. Clinical evaluation for disease activity was performed using the clinical activity score index in JRA, and SLE-DAI in SLE. Subjects were investigated to verify the diagnosis and disease activity. Plasma ADM and serum of TNF-α levels were then assayed. Results: Serum TNF-α and plasma ADM levels were significantly higher in JRA and SLE patients than in rheumatic arthritis patients and healthy controls. Though serum TNF-α and plasma ADM levels were both higher in JRA (73.88 ± 11.6 pg/ml and 156.5 ± 22.4 pg/ml, respectively) compared to SLE (48.82 ± 7.5 pg/ml and 85.12 ± 15.7 pg/ml, respectively), the difference was of statistical significance only in ADM. Both serum TNF-α and plasma ADM levels were significantly higher in systemic onset-JRA (139.75 ± 18.5 and 260.25 &#177 28.6 pg/ml, respectively) compared to the pauciarticular-onset type (33.8 ± 3.04 and 93.4 ± 9.35 pg/ml, respectively), but comparable to the polyarticular onset cases (69.97 ± 8.45 and 149.87 ± 21.15 pg/ml, respectively). Positive correlations were noticed between plasma ADM and activity score index (r=0.72), ESR (r=0.59) and serum TNF-α (r=0.64) in JRA. The serum TNF- α was not influenced by the site of lupus activity unlike plasma ADM that was higher in subjects suffering from lupus arthritis or cardiovascular manifestations. The afore-mentioned markers correlated positively to the ESR in SLE but not to the SLE-DAI. With a cut-off value of TNF-α = 31 pg/ml and that for ADM = 80 pg/ml calculated from the results of the included rheumatic arthritis patients, ADM appeared to be a more sensitive marker of activity in JRA and SLE compared to TNF-α. Conclusion: Plasma ADM was over-expressed in JRA and SLE. It correlated with the clinical and biochemical activity markers in JRA suggesting that it can be used as an indicator of disease activity.     Characterization of graphene oxide-ziziphus seeds and its application as a hazardous dye removal adsorbent

    Abstract The zizphus seeds are considered as a biomaterial residues that has been used for removing of organic industrial waste such as 2-((10-octyl-9,10-dihydroanthracene-2-yl) methylene) malononitrile (PTZS-CN) dye from aqueous solutions utilizing graphene oxide-Ziziphus (GO-Ziziphus). A batch study explored the impacts of various experimental circumstances, including solution pH, initial dye concentration, temperature, and contact time. General order, nonlinear pseudo-first order and pseudo-second order, elvoich model and intraparticiple diffusion were utilized to analyze the kinetic data. The adsorption kinetics of dye onto GO-ziziphus adsorption was best mentioned by nonlinear pseudo-first order. Similarly, the intra-particle diffusion plots revealed one exponential line throughout the adsorption process. The Freundlich, Dubinin-Radushkevich, and Langmuir models were employed to examine isothermal data. It provided the best fit of the dye adsorption isothermal data onto GO-ziziphus Freundlich models. Besides, the calculated free energies showed that the adsorption progression was physical adsorption. Thermodynamic calculations revealed that dye adsorption onto GO-ziziphus was exothermic and spontaneous. The combined results indicated that GO-ziziphus powder might be used to treat dye-rich wastewater effectively

    Integration of Mn-ZnFe2O4 with S-g-C3N4 for Boosting Spatial Charge Generation and Separation as an Efficient Photocatalyst

    The disposal of dyes and organic matter into water bodies has become a significant source of pollution, posing health risks to humans worldwide. With rising water demands and dwindling supplies, these harmful compounds must be isolated from wastewater and kept out of the aquatic environment. In the research presented here, hydrothermal synthesis of manganese-doped zinc ferrites’ (Mn-ZnFe2O4) nanoparticles (NPs) and their nanocomposites (NCs) with sulfur-doped graphitic carbon nitride (Mn-ZnFe2O4/S-g-C3N4) are described. The samples’ morphological, structural, and bonding features were investigated using SEM, XRD, and FTIR techniques. A two-phase photocatalytic degradation study of (0.5, 1, 3, 5, 7, 9, and 11 wt.%) Mn-doped ZnFe2O4 NPs and Mn-ZnFe2O4/(10, 30, 50, 60, and 70 wt.%) S-g-C3N4 NCs against MB was carried out to find the photocatalyst with maximum efficiency. The 9% Mn-ZnFe2O4 NPs and Mn-ZnFe2O4/50% S-g-C3N4 NCs exhibited the best photocatalyst efficiency in phase one and phased two, respectively. The enhanced photocatalytic activity of the Mn-ZnFe2O4/50% S-g-C3N4 NCs could be attributed to synergistic interactions at the Mn-ZnFe2O4/50% S-g-C3N4 NCs interface that resulted in a more effective transfer and separation of photo-induced charges. Therefore, it is efficient, affordable, and ecologically secure to modify ZnFe2O4 by doping with Mn and homogenizing with S-g-C3N4. As a result, our current research suggests that the synthetic ternary hybrid Mn-ZnFe2O4/50% S-g-C3N4 NCs may be an effective photocatalytic system for degrading organic pollutants from wastewater

    Global economic burden of unmet surgical need for appendicitis

    Background There is a substantial gap in provision of adequate surgical care in many low- and middle-income countries. This study aimed to identify the economic burden of unmet surgical need for the common condition of appendicitis. Methods Data on the incidence of appendicitis from 170 countries and two different approaches were used to estimate numbers of patients who do not receive surgery: as a fixed proportion of the total unmet surgical need per country (approach 1); and based on country income status (approach 2). Indirect costs with current levels of access and local quality, and those if quality were at the standards of high-income countries, were estimated. A human capital approach was applied, focusing on the economic burden resulting from premature death and absenteeism. Results Excess mortality was 4185 per 100 000 cases of appendicitis using approach 1 and 3448 per 100 000 using approach 2. The economic burden of continuing current levels of access and local quality was US 92492millionusingapproach1and92 492 million using approach 1 and 73 141 million using approach 2. The economic burden of not providing surgical care to the standards of high-income countries was 95004millionusingapproach1and95 004 million using approach 1 and 75 666 million using approach 2. The largest share of these costs resulted from premature death (97.7 per cent) and lack of access (97.0 per cent) in contrast to lack of quality. Conclusion For a comparatively non-complex emergency condition such as appendicitis, increasing access to care should be prioritized. Although improving quality of care should not be neglected, increasing provision of care at current standards could reduce societal costs substantially
