    Histological modification in TRAM flap in rats treated with pentoxifylline

    PURPOSE: To investigate the blood vessels' concentration in TRAM flap's rat model, in the presence of pentoxifylline.METHODS:32 male, Wistar-EPM rats were divided into two groups. Control group (C): 0.5 ml of saline, intraperitoneally, once a day, for seven days before flap elevation; PTX group (P): pentoxifylline (20mg/kg/day), intraperitoneally, for seven days before flap elevation. After that, they were submitted to a caudal unipedicle TRAM flap. On the fifth postoperative day, percentages of flap necrosis were determined via the paper template method and Tram flap's zone IV skin biopsies were taken for histological analysis.RESULTS:the mean percentage of flap necrosis in group C was 58.7 % and in group P, 31.1 (Wilcoxon test; p = 0.003). Mean capillary vessels number in zone IV's skin in C group was 33.4 and in P group was 71.9 (p=0.008).CONCLUSIONS: Pentoxifylline was effective reducing the necrosis in the caudal unipedicle TRAM flap in the rat as well as increasing the number of capillaries in an ischemic zone (zone IV).UNIFESP Surgery DepartmentUNIFESP Pathology DepartmentUNIFESPUNIFESP, Surgery DepartmentUNIFESP, Pathology DepartmentUNIFESPSciEL

    Introduction: facial fractures, as well as the extensive injuries of the head and neck region, are the most frequent surgical urgencies of the daily routine of the plastic surgeon. The treatment of these injuries in the emergency room, in most hospitals, does not have available medical and multiprofessional staff or even an adequate diagnostic structure. Given this scenario, many patients do not have the best course of action in the acute phase of the trauma and need to deal with the sequelae until they are referred to specialized centers. Objective: organize a care protocol through a smartphone application in order to optimize the referral of patients who suffered facial trauma to the reference centers. Results: the content of the application was distributed into the seven most frequent complications of facial trauma, with a simple presentation and in the form of an algorithm. Conclusion: The Facial Trauma application has proved to be an easy reference tool for the initial evaluation of patients with facial trauma in care units that do not have an on-site specialist.Introdução: as fraturas dos ossos da face, assim como os ferimentos extensos da região da cabeça e pescoço, são as intercorrências cirúrgicas de urgência mais frequentes no cotidiano do cirurgião plástico. O tratamento dessas entidades nas unidades de atendimento de pronto socorro, na maioria dos hospitais, não tem disponível equipe médica e multiprofissional treinada para diagnóstico adequado. Com a evolução dos smartphones, facilitou-se o acesso dos profissionais da saúde a informações sobre a literatura disponível, principalmente através do uso de aplicativos. Objetivo: organizar um protocolo assistencial através de um aplicativo para smartphone a fim de otimizar o encaminhamento dos pacientes que sofreram traumatismo facial aos centros de referência. Método: contéudo foi criado a partir de literatura atual no manejo dos traumatismos faciais; o desenvolvimento foi realizado através de ferramenta pré-programada para criação de aplicativos. Resultados: o conteúdo do aplicativo foi distribuído nas sete intercorrências mais frequentes dos traumatismos faciais, com uma apresentação simples e em forma de algoritmo.
