    Open Access to Resource Management in Multimedia Networks

    The paper is dedicated to mechanisms for open access to resource management in the Internet Protocol (IP) multimedia networks. First we present the concept of IP Multimedia Subsystem (IMS) and explain the IMS functional architecture, principles of quality of service management and service control in IMS. Then we describe the idea behind the opening of network interfaces for third parties so that others besides the network operator can create and deploy services. Open Service Access (OSA) and Parlay appear to be the technologies for value-added service delivery in multimedia networks. In the paper we take a closer look to the Parlay/OSA interfaces that allow third party applications to access the resource management functions in IMS. OSA "Connectivity Manager" interfaces and OSA "Policy Management" interfaces are considered. Parlay X Web Services interfaces provide a higher level of abstraction than Parlay/OSA interfaces and gain an amazing amount of support among service developers.     Immune response – genesis, duration, and strength in patients with moderate and severe coronavirus infection of different age groups

    Acquired (adaptive) immunity is a major factor determining effective immune response against a few infectious diseases. The immune response during recovery from COVID-19 is complex, involving both cellular and humoral adaptive immunity. The purpose of the study is to determine the intensity and effectiveness of the immune response at the end of the second year after discharge from the hospital in patients who have suffered from moderate and severe forms of coronavirus infection. A study among 2683 patients who suffered from moderately severe and severe coronavirus SARS-CoV2 infection with recorded complications which have not received a vaccine against SARS-nCoV-2 was performed. In the studied group of patients there were no deaths. In the whole cohort, the share of underlying prehospital comorbidity was also analyzed. The immune response induced because of moderate and severe infection with COVID-19 could serve as source of protection from recurrent severe infection for patents of different ages with various comorbidities

    ï»żTobacco smokers as target group for complicated coronavirus infection

    The aim of current study was to determine, retrospectively, possible correlations between smoking and the incidence, course severity, intubation rate, and mortality (by gender and age) in patients treated for complicated coronavirus infection in the internal medicine clinic at UMHATEM ”N. I. Pirogov” Sofia for the period 01.03.2020–31.12.2020. In a prospective study, the recovery period and immunogenesis in smokers and non-smokers within a one-year period after hospital discharge was investigated. The applied methods were: 1) computed tomography and blood gas analysis 2) chemiluminescent immunoassay for the qualitative determination of total IgM, IgA and IgG anti-SARS-CoV2 AB. Results showed that the part of non-smokers with a positive PCR test is significantly higher compared to the group of former and current smokers. The data obtained from the study confirmed that Covid infection is much more severe among smokers and former smokers with a higher levels of inflammatory markers noticed among the smoking group

    Background: UMHATEM „N.I. Pirogov“ Sofia, is one of the largest and busiest hospitals for emergency medical care in Bulgaria. It is the legal successor of the former Institute of Emergency Medicine „N.I. Pirogov“, and it can be said that at the moment it is the only diagnostic- therapeutic, prophylactic and scientific-educational structure of this type within the Bulgarian healthcare system. The concept of adequate functioning and development of this type of hospital does not consider structuring an Infectious Diseases diagnostic-treatment unit. This reality makes necessary the formation of a radically different organization for the admission and treatment of patients in the hospital, both for those with symptoms of Covid 19 and for all other emergency patients. The organization created in this way must absolutely guarantee safety for both streams of patients. In the conditions of a pandemic, in case of a real threat to public health, the main task of triage in the Emergency Department is to establish indications for urgent hospitalization, or to refuse it in the absence of indications. The characteristic course of the disease, the prolonged treatment, the manifestations within the so-called „post-Covid syndrome“, require serious planning not only of the diagnostic-treatment and rehabilitation period, but also adequate monitoring in the first months after the patient‘s discharge. Within the national reorganization measures, during the determined periods, the main changes concerning the MED (Multi-profile emergency department) of Pirogov are implemented, with an emphasis on the formation of a specialized triage for the diagnosis and clinical evaluation of patients with a coronavirus infection. The main goal is the adequate diagnosis, treatment and follow-up of patients with coronavirus infection who have passed through the organized Covid-triage in a period of extreme pressure on the emergency structures and on the hospital system in the country as a whole. Objective: For a MED, which at the time of declaring an epidemic situation does not have a concept for the diagnosis and treatment of infectious diseases, to systematize the main urgently implemented organizational and structural changes, which turned out to be absolutely necessary to meet a newly emerging epidemiologically significant infectious disease. Aims: To systematize the organizational changes imposed by the situation and urgently implemented in the MED (multi-profile emergency department).     Pattern-generation and pattern-transfer for single-digit nano devices

    Single-electron devices operating at room temperature require sub-5 nm quantum dots having tunnel junctions of comparable dimensions. Further development in nanoelectronics depends on the capability to generate mesoscopic structures and interfacing these with complementary metal–oxide–semiconductor devices in a single system. The authors employ a combination of two novel methods of fabricating room temperature silicon single-electron transistors (SETs), Fowler–Nordheim scanning probe lithography (F-N SPL) with active cantilevers and cryogenic reactive ion etching followed by pattern-dependent oxidation. The F-N SPL employs a low energy electron exposure of 5–10 nm thick high-resolution molecular resist (Calixarene) resulting in single nanodigit lithographic performance [Rangelow et al., Proc. SPIE 7637, 76370V (2010)]. The followed step of pattern transfer into silicon becomes very challenging because of the extremely low resist thickness, which limits the etching depth. The authors developed a computer simulation code to simulate the reactive ion etching at cryogenic temperatures (−120 °C). In this article, the authors present the alliance of all these technologies used for the manufacturing of SETs capable to operate at room temperatures
