    How to improve palliative care teaching in times of COVID-19?

    Introducción : Los cuidados paliativos, según la OMS, tratan de mejorar la calidad de vida del paciente y de los familiares, previniendo, aliviando dolor y tratando síntomas psicosociales y espirituales. La docencia no es específica y la muerte siempre se trata de manera superficial dado que los docentes no están bien formados; además, cada universidad tiene libertad de introducir la materia de la manera que desee. En cuanto a las redes sociales, en la actualidad son una herramienta útil como método interactivo de información y conocimiento, y se ha demostrado que el uso de ellas en la educación genera aspectos positivos en la formación de los estudiantes. Objetivo : Realizar una propuesta para la mejora de la docencia universitaria en los cuidados paliativos. Metodología : Se llevaron a cabo dos búsquedas bibliográficas independientes en distintas bases de datos ( Medline Complete , ERIC , Phychology and Behavioral Sciences Collection (PsycINFO), Education Research , Library Information Science & Technology Abstracts and CINAHL Complete ). Se realizó una revisión sistemática de métodos docentes en cuidados paliativos y otra sobre el uso de las redes sociales en la docencia en enfermería. Resultados : Tras la búsqueda inicial, conseguimos un total de 43 artículos que incluimos para su posterior estudio. Significativamente hallamos que los criterios de la asignatura de cuidados paliativos no están integrados de una manera uniforme, por lo que los estudiantes, dependiendo del país al que pertenecen, adquieren una formación dispar. En la segunda búsqueda 16 artículos fueron finalmente seleccionados. Entre los temas tratados destacan los beneficios de emplear las redes sociales en la educación de enfermería y la manera en la que perciben el uso de estas plataformas. Conclusiones : Los cuidados paliativos son necesarios y es imprescindible que los profesores y los futuros profesionales tengan acceso a una formación de calidad, globalizada. Además, los planes de estudio deben cumplir con los requisitos necesarios para que los estudiantes no sientan frustración y miedo a estos cuidados y a la muerte. Por otro lado, las redes sociales son consideradas como una estrategia innovadora y un recurso efectivo en la formación de estudiantes de enfermería.Introduction : Palliative care according to WHO seeks to improve the quality of life of the patient and family members by preventing, relieving pain and treating psychosocial and spiritual symptoms. As for social media, these are currently a useful tool as an interactive method of information and knowledge, and it has been shown that their use in education generates positive aspects in the training of students. Objective : To make a proposal for the improvement of university teaching in palliative care. Methodology : Two independent bibliographic searches were carried out in different databases (Medline Complete, ERIC, Phychology and Behavioral Sciences Collection (PsycINFO), Education Research, Library Information Science & Technology Abstracts and CINAHL Complete). First, a systematic review of teaching methods in palliative care was carried out, and the second on the use of social networks in nursing teaching. Results : After the initial search, we obtained a total of 43 articles that we included for further study. Significantly we found that the palliative care subject criteria are not uniformly integrated, so that students, depending on the country to which they belong, acquire disparate training. In the second search 16 articles were finally selected. Among the topics discussed are the benefits of using social networks in nursing education and the way in which they perceive the use of these platforms. Conclusions : Palliative care is necessary and it is essential that teachers and future professionals have access to quality, globalized training. In addition, the curricula must meet the necessary requirements so that students do not feel frustration and fear of this care and death. On the other hand, social networks are considered as an innovative strategy and an effective resource in the training of nursing students.Medicin

    Cost-effectiveness analysis of antimuscarinics in the treatment of patients with overactive bladder in Spain: A decision-tree model

    <p>Abstract</p> <p>Background</p> <p>Fesoterodine, a new once daily antimuscarinic, has proven to be an effective, safe, and well-tolerated treatment in patients with overactive bladder (OAB). To date, no analysis has evaluated the economic costs and benefits associated with fesoterodine, compared to antimuscarinics in Spain. The purpose of this analysis was to assess the economic value of OAB treatment with fesoterodine relative to extended release tolterodine and solifenacin, from the societal perspective.</p> <p>Methods</p> <p>The economic model was based on data from two 12-week, randomized, double-blind, and multicenter trials comparing fesoterodine and tolterodine extended released (ER). Treatment response rates for solifenacin were extracted from the published literature. Discontinuation and efficacy were based on the results of a 12-week multinational randomized clinical trial extrapolated to 52 weeks. Changes in health related quality of life were assessed with the King's Health Questionnaire, which was transformed into preference-based utility values. Medical costs included (expressed in € 2010) were antimuscarinics, physician visits, laboratory tests, incontinence pads and the costs of OAB-related comorbidities, fractures, skin infections, urinary tract infections, depression, and nursing home admissions associated with incontinence. Time lost from work was also considered. Univariate sensitivity analyses were also performed.</p> <p>Results</p> <p>At week 12, continents accounted for 50.6%, 40.6% and 47.2% of patients in the fesoterodine, tolterodine, and solifenacin groups, respectively. By week 52, the projected proportions of patients remaining on therapy were 33.1%, 26.5% and 30.8%, respectively. The projected quality- adjusted life years (QALY) gain (compared to baseline) over the 52-week simulation period were 0.01014, 0.00846 and 0.00957, respectively. The overall treatment cost was estimated at €1,937, €2,089 and €1,960 for fesoterodine, tolterodine and solifenacin, respectively. Therefore, treatment with fesoterodine resulted in similar overall costs and greater QALY gain than treatment with either tolterodine or solifenacin. Sensitivity analysis showed that these results were robust to all changes performed.</p> <p>Conclusions</p> <p>The results of this economic analysis suggest that fesoterodine is a cost-effective alternative to tolterodine and solifenacin for the treatment of patients with OAB in Spain. Fesoterodine provides additional health benefits while maintain a similar level of costs being a cost-effective treatment strategy from a societal perspective.</p
