    Loneliness and internet addiction in doctors and medical students: a cross-sectional study

    Background: Our aim in this study is to determine the level of internet addiction in medical school students and primary care physicians and the factors affecting them.Methods: A 68 physicians and 167 students participated in this descriptive study. The participants were administered the young ınternet addiction test, UCLA loneliness scale and asked to respond to survey questions about their ssociodemographic characteristics, and the relations were evaluated with appropriate statistical methods.Results: It has been identified that playing games online, surfing the internet aimlessly and loneliness score affected internet addiction regardless of other variables (p<0.05). A statistically significant correlation between addiction level and age (r= (-0.341), p<0,001), loneliness score (r=0,284, p<0,001) and online time (r=0,387, p<0,001) has been discovered.Conclusions: In this study, it was found statistically significant that students with internet addiction stayed online for long periods of time and it seemed in line with the criteria observed in substance use disorders such as longer duration of use than intended and not being able to spare time for other activities

    Dialogue of civilizations in a multipolar world: toward a multicivilizational-multiplex world order

    In this article, I explore the relationship between the new multipolar trends related to the emerging powers and the idea of dialogue of civilizations. My starting point is to understand multipolarity as part of a broader epoch making process of transformation of contemporary international society beyond its Western-centric matrix. In the first part of this article, I therefore argue for an analytical understanding that emphasizes the emergence of a new multipolar world of civilizational politics and multiple modernities. In the second part of the article, I reflect on how to counter the risk inherent in the potential antagonistic logic of multipolarity by critically engaging the normative Huntingtonian construction of a multicivilizational-multipolar world order. I argue that the link between dialogue of civilizations and regionalism could represent a critical issue for the future of global peace. In particular, multiculturally constituted processes of regional integration are antidotes to the possible negative politicization of cultural differences on a global scale and can contribute to the emergence of a new cross-cultural jus gentium. These elements are critical to the construction of a realistic dialogue of civilizations in international relations while preventing the risks inherent in its growing multipolar configuration. They shape what, drawing on Amitav Acharya's work, could be named a multicivilizational-multiplex world order

    An Evaluation of Reverse Mentoring: The Intern Movie Case

    Pek çok yönetim uygulaması gibi mentorluk da kendisini değişen şartlara göre yeniden düzenlemektedir. Tersine mentorluk, bilişim teknolojilerinin gelişmesi, çalışma koşullarının farklılaşması, iş hayatına giren yeni kuşakların önceki kuşaklardan farklı beklentilere sahip olması gibi sebeplerle yeni bir mentorluk anlayışı olarak ortaya çıkmıştır. Bu çalışmada tersine mentorluğa ilişkin izler taşıdığı düşünülen ve 2015 yılında gösterime girmiş olan The Intern (Stajyer) filmi incelenmiştir. Filmin başrolleri arasındaki ilişki tersine mentorluk işlev ve alt işlevleri bağlamında ele alınmıştır. Nitel araştırma yöntemleri benimsenerek yürütülen çalışmada analiz tekniği olarak betimsel analiz tercih edilmiştir. Yapılan analiz ile birlikte; bilgi paylaşımı, açığa çıkarma ve görünürlük, meydan okuma ve zorluklar, sosyal ağlar, kabul ve onaylama, arkadaşlık, cesaretlendirme ve rol model alt işlevlerinin filmde işlendiği sonucuna ulaşılmıştır. Koçluk, yetenek gelişimi ile destek ve geribildirim alt işlevlerinin ise filmde yer almadığı görülmüştür.Like many management practices, mentoring reorganizes itself according to changing circumstances.     The Moderating Role of General Self-Efficacy on the Effect of Employees’ Dark Personality Traits on Burnout

    Bu çalışmanın amacı çalışanların karanlık kişilik özelliklerinin (narsisizm, Makyavelizm ve psikopati) tükenmişliğe etkisinde genel öz-yeterliliğin düzenleyici rolünün test edilmesidir. Bu kapsamda Sakarya’da faaliyet gösteren bir özel sektör işletmesinden toplamda 182 çalışan araştırmaya dâhil olmuştur. Çalışanların karanlık kişilik özelliklerinin tükenmişliğe etkisinde genel öz-yeterliliğin düzenleyici rolünün tespit edilmesi için Hayes’in (2013) PROCESS modelinden yararlanılmıştır. Araştırma bulgularına göre sadece narsisizmin tükenmişliğin toplam skoruna ve duygusal tükenmeye etkisinde, genel öz-yeterliliğin anlamlı bir şekilde düzenleyici etkisi olduğu tespit edilmiştir. Harici değişkenlerin (Karanlık Üçlü toplam skor, Makyavelizm, psikopati) tükenmişliğe etkisinde, genel öz-yeterlilik düzenleyici rol oynamamıştır.The purpose of this study is to test the moderating role of general self-efficacy on the effect of dark personality traits (narcissism, Machiavellianism, and psychopathy) on burnout. In this context, a total of 182 employees from a private sector enterprise operating in Sakarya participated in the study. Hayes's (2013) PROCESS model was used to test the moderating role of general self-efficacy on the effect of dark personality traits on burnout. According to the findings of the study, it was found that general self-efficacy had a significant moderating role on the effect of narcissism only on burnout global and emotional exhaustion. General self-efficacy did not play a moderating role on the effect of other variables (Dark Triad global, Machiavellianism, and psychopathy) on burnout

    STUDIES ON MECHANICAL PERFORMANCE OF CYNARA SCOLYMUS/ POLYETHYLENE TEREPHTHALATE NONWOVEN COMPOSITES

    Global warming, energy consumption and the desire to obtain products from renewable sources have led to a resurgence of interest in plant-derived products. Plant fibers with some interesting inherent properties like cost, effectiveness, renewable, available in high quality, have low fossil-fuel energy requirements, and can offer good mechanical properties has attracted it use for composite materials. Hemp, jute, flax, bagasse, coconut fiber, and bamboo are among the natural fibers which has attracted attention because of their low cost compared to synthetic fibers such as glass, carbon etc . Natural fiber reinforced composites are finding successful application in the automotive, electronics, and engineering sectors.In this study, Artichoke (Cynara scolymus) fibers  are obtained from the West Mediterranean Region of Turkey. Firstly the stem of plant was removed than the fibers were gained. Nonwoven surfaces were obtained at different percentages by to the mix Cynara scolymus / polyethylene terephthalate (PET) fibers.The mechanical properties (elongation (%),tensile strength (N), modulus of elasticity (MPa)) of composite materials were investigated. The mechanical properties of non-woven surface composites of Cynara scolymus / PET fibers were observed with increases in elongation value, strength value and elasticity modulus values compared to a 100% PET based nonwoven surface. In addition, the contact angle values of nonwoven surface composites based on Cynara scolymus/PET fibers were  decreased

    Low dose sultamicillin in acute sinusitis

    Objectives: to study the effectiveness of low dose sultamicillin in the treatment of acute sinusitis. Methods: a total of 108 patients, between 16-56 years of age (mean 32.8), suffering from acute sinusitis took part in the trial. Patients received orally 2×375 mg sultamicillin, and compared with patients receiving 3×500 mg amoxicillin. The first control was made between the 5th and 7th days. A patient was considered clinically cured when all pretreatment signs and symptoms of infection were eliminated. Clinical improvement was defined as the partial disappearance of pretreatment signs and symptoms. In either result, study drugs were reconstituted for additional 5 days. Failure was defined as no change or worsening of signs and symptoms; and study drug was changed. The second control was made between 10-12th days, and the third was four weeks later. Results: The clinical success (improvement + cure) rate was (17+11)/42 (66.6%) and (28+21)/66 (74.2%) for amoxicillin and sultamicillin respectively, at first control. All improved patients were cured at the second control. No significant side-effects were noted in either amoxicillin or sultamicillin treated patients. All side effects were gastrointestinal, 11.9% and 3.0% in the same order. Conclusions: Low dose sultamicillin was comparable to amoxicillin; sultamicillin has fewer side effects than amoxicillin (p>O.05)

    Gender Difference in Apnea and Hypopnea Component in Obstructive Sleep Apnea

    Introduction We aimed to analyze the apnea and hypopnea structure separately with demographic parameters and sleep architecture in men and women with sleep apnea. Materials and Methods Patients referred for snoring, witnessed apnea and/or day time sleepiness to Dışkapı Yıldırım Beyazıt Research and Educational Hospital Sleep Center and gone under polysomnography (PSG) between December 2010 and June 2012 were taken in order. PSG reports were analyzed retrospectively. The patients with sleep efficiency less than 40% were excluded. The BMI, neck circumference (NC), abdominal circumference (AC) and PSG values were recorded. Results Totally 406 patients (250 male, 156 female patients) were studied. NC was found more in males whereas AC and BMI were found significantly more in females. Mean age, apne-hypopnea index (AHI), oxygen desaturation index (ODI) for 3% were similar in two genders. Percentage of total light sleep (Stage 1+2) was significantly more in males while Stage 3 (slow wave sleep: SWS) was more in females. Total apneas were significantly more in males and hypopneas were significantly more in females. The factors associated with AHI were NC and BMI in males and AC and BMI in females. Discussion We found that, females are more hypopneic and men are more apneic, in a study group of similar apne-hypopnea indexed patients. The different distribution of fat in genders seems to effect the apnea/hypopnea predominance. The clinical significance of the apnea and hypopnea indexes separately can be related with SWS percentage. Prospective studies are needed to evaluate the effect of apneas and hypopneas on morbidity and mortality in both genders

    Invasive aspergillosis (IA) is an infectious disease caused by Aspergillus species, is usually seen in patients with immune deficiency and has been associated with high mortality rate. IA most commonly affects the lungs, but Aspergillus can also spread throughout the body and cause infection in other organs. Voriconazole is the first-line antifungal agent in the treatment of invasive aspergillosis. Other antifungal agents including isavuconazole, lipid amphotericin formulations, itraconazole, caspofungin, micafungin, and posaconazole can be used to treat IA in patients who can not take voriconazole or who have not responded to voriconazole.     Sleep in Mechanically Ventilated Patients in the Intensive Care Unit

    Objective Sleep abnormalities are common in critically ill patients. Polysomnography (PSG) is the gold standard in assessing sleep quality. The aim of this prospective study was to monitor the sleep pattern in mechanically ventilated patients with PSG who were admitted to our medical intensive care unit. Materials and Methods This study was conducted in the Medical Intensive Care Unit of an University Hospital. Patients with endotracheal intubation and mechanical ventilation for at least 24 hours were included in the study. They were monitored for 18 hours per day by continuous PSG. Sleep parameters were recorded; [total sleep time (TST), sleep efficiency (SE) and sleep stages]. Results Records of 12 patients were evaluated. There were nine males and three females. Median age of patients were 72.5 years (min-max=31-92). Median APACHE II was 19 (min-max=10-27). Median sleep time was 489.5 minutes (180-1105), median SE was 77.1% (24.9-96.5) and median arousal number was 147.5/TST (14-450). While REM sleep and non REM stage 3 sleep time and proportion were found to be decreased, non REM stage 2 sleep time and proportion were increased. Conclusion We have shown that mechanically ventilated patients have changes in sleep architecture and that they have severe sleep fragmentation. Future research should address the cause of these problems by using methodology for comprehensive assessment of sleep-disrupting factors and by examining the dynamic effects of changes in illness severity on sleep quality

    Importance of Labarotory Parameters in Obstructive Sleep Apnea

    Objective Obstructive sleep apnea (OSA) is associated with intermittent hypoxia. OSA leads to increased sympathetic activation, oxidative stress, vascular endothelial dysfunction, coagulation disorders and metabolic dysregulation. These disturbances increase the the risk of inflammation and cardiovascular diseases. The purpose of this article is to review the laboratory parameters of OSA patients without any comorbidities. Materials and Methods This retrospective study of consecutive 675 patients who had polysomnography, was conducted on one hundred and thirty patients who did not have any comorbidities. Laboratory values of patients were evaluated. Patients were grouped according to apnea-hypopnea index (AHI). Group 1 (n=17) AHI 30. Results There were 88 men (67.7%) and 42 women (32.3%) in the study. Mean age, body mass index, Epworth score and AHI were 41.6±11.3 (16-75), 29.6±6.3 (17.1-65.7) 9.9±5.5, 6.2±11.34, respectively. Serum glucose, cholesterol and triglyceride levels were higher in group 4 (p=0.03, p=0.04, p=0.02, respectively). Uric acid and fibrinogen levels were higher in patients with higher AHI (p=0.038). Conclusion Our study indicates that increased blood glucose, uric acid and dyslipidemia are associated with OSA regardless of comorbidities
