    THE ISOKINETIC MUSCLE ASYMMETRY OF THE THIGH AT 1 YEAR AFTER ANTERIOR CRUCIATE LIGAMENT RECONSTRUCTION WAS SIGNIFICANTLY ASSOCIATED WITH GAIT ASYMMETRY

    Objective: To study the correlation between muscle strength asymmetry and gait asymmetry in 1 year after Anterior Cruciate Ligament Reconstruction ACLR. Methods: Twenty-five ACLR patients were enrolled in the Department of Sports Medicine, Peking University Third Hospital. Data of isokinetic muscle strength test one year after ACLR were collected. The concentric and eccentric strength of extensor and flexor muscles at 60°/s, 180°/s and 300°/s on the uninjured side and the injured side were measured respectively, and the peak value of muscle strength was analyzed. The three dimensional motion information and ground reaction force during gait were collected, and the peaks of three dimensional joint angle and moments during gait stance phase were calculated by inverse dynamics analysis. The paired-samples T test was used to analyze the difference of gait parameters and isokinetic muscle strength peaks. Spearman correlation analysis was used to study the correlation between bilateral asymmetry index of isokinetic muscle strength and gait asymmetry index. Results: One year after ACLR, the isokinetic muscle strength peaks of the flexor and extensor muscles on the injured side were significantly lower than those on the uninjured side【60°/s extensor concentric, the injured side: (1.22 ± 0.4)Nm·kg-1, uninjured side: (1.73 ± 0.42)Nm·kg-1, bilateral difference: (-0.5 ± 0.39)Nm·kg-1, P \u3c 0.01; 60°/s flexor concentric, injured side: (0.84 ± 0.19)Nm·kg-1, uninjured side: (1.05 ± 0.23)Nm·kg-1, bilateral difference: (-0.21 ± 0.14)Nm·kg-1, P \u3c 0.01】. Compared with the uninjured side, the injured side showed insufficient knee extension at the time of maximum knee extension during stance phase 【injured side: (5.25 ± 4.17) °, uninjured side: (2.24 ± 3.11) °, bilateral difference: (3.01 ± 2.44) °, P \u3c 0.01】, and the peak extension moment decreased significantly 【injured side: (0.1 ± 0.09) Nm·kg-1·m-1, (0.15 ± 0.07) Nm·kg-1·m-1, (-0.05 ± 0.06) Nm·kg-1·m-1, P \u3c 0.01】. One year after ACLR, the asymmetry of 180°/s isokinetic extensor concentric strength was significantly correlated with the asymmetry of peak flexion moment (R = 0.449, P = 0.024). The asymmetry of 60°/s isokinetic extensor concentric strength was significantly correlated with the asymmetry of peak internal rotation moment (R = 0.421, P = 0.036). One year after ACLR, asymmetries of 180°/s, 300°/s isokinetic extensor concentric strength and 60°/s isokinetic flexor eccentric strength were significantly correlated with peak asymmetries during stance phase. Conclusion: There is a significant correlation between isokinetic muscle strength asymmetry of knee and gait asymmetry. This study suggests that ACLR patients still need regular rehabilitation training to improve muscle strength and motor function 1 year after ACLR, so as to reduce the risk of reinjury and secondary injury

    Iliopsoas fibrosis after revision of total hip arthroplasty revealed by 68Ga-FAPI PET/CT: a case report

    BackgroundTotal hip arthroplasty (THA) is a well-established surgical procedure that has been extensively validated to alleviate pain, enhance joint function, improve the ability to perform daily activities, and enhance overall quality of life. However, this procedure is associated with certain complications, among which skeletal muscle fibrosis is a frequently overlooked but significant complication that can lead to persistent pain. Currently, there is no effective method for diagnosing skeletal muscle fibrosis following total hip arthroplasty.Case reportWe report a 75-year-old male patient who complained of left groin pain after revision total hip arthroplasty. Serological examinations, X-rays, and bone scan results were all normal. However, during the 68Ga-FAPI PET/CT examination, we observed significant radiotracer uptake along the iliopsoas muscle. This abnormal uptake pattern suggested potential biological activity in this specific area. Combined with physical examination, the patient was diagnosed with iliopsoas fibrosis.ConclusionsThe presented images indicated that the uptake pattern was an important indicator for diagnosis, and the prospect of fibroblast activation protein in the diagnosis of skeletal muscle fibrosis has shown certain application value

    The effects of blurred visual inputs with different levels on the cerebral activity during free level walking

    ObjectiveThe aim of this study was to evaluate the effects of blurred vision on electrocortical activities at different levels during walking.Materials and methodsA total of 22 healthy volunteers (all men; mean age: 24.4 ± 3.9 years) underwent an electroencephalography (EEG) test synchronous with free level walking. Visual status was simulated by goggles covered by the occlusion foil targeted at a Snellen visual acuity of 20/60 (V0.3), 20/200 (V0.1), and light perception (V0). At each of these conditions, the participants completed barefoot walking for five blocks of 10 m. The EEG signals were recorded by a wireless EEG system with electrodes of interest, namely, Cz, Pz, Oz, O1, and O2. The gait performances were assessed by the Vicon system.ResultsDuring walking with normal vision (V1.0), there were cerebral activities related to visual processing, characterized as higher spectral power of delta (Oz and O2 vs. Cz, Pz, and O1, p ≀ 0.033) and theta (Oz vs. Cz and O1, p = 0.044) bands in occipital regions. Moderately blurred vision (V0.3) would attenuate the predominance of delta- and theta-band activities at Oz and O2, respectively. At the statuses of V0.1 and V0, the higher power of delta (at V0.1 and V0, Oz, and O2 vs. Cz, Pz, and O1, p ≀ 0.047) and theta bands (at V0.1, Oz vs. Cz, p = 0.010; at V0, Oz vs. Cz, Pz, and O1, p ≀ 0.016) emerged again. The cautious gait pattern, characterized by a decrease in gait speed (p &lt; 0.001), a greater amplitude of deviation from the right ahead (p &lt; 0.001), a prolonged stance time (p = 0.001), a restricted range of motion in the hip on the right side (p ≀ 0.010), and an increased knee flexion during stance on the left side (p = 0.014), was only detected at the status of V0. The power of the alpha band at the status of V0 was higher than that at V1.0, V0.3, and V0.1 (p ≀ 0.011).ConclusionMildly blurred visual inputs would elicit generalization of low-frequency band activity during walking. In circumstance to no effective visual input, locomotor navigation would rely on cerebral activity related to visual working memory. The threshold to trigger the shift might be the visual status that is as blurred as the level of Snellen visual acuity of 20/200

    A Particle Filtering Method for Muscle Tissue Segmentation from MRI Images

    We have developed a particle filtering method for muscle tissue reconstruction using the level set representation of contours, which makes it possible to handle topological change of the muscle contour. The inputs of the framework are the selected images which are manually outlined for specified muscle tissue. The contours are calculated using an energy term to predict the likelihood of muscle contour. The priors used in the energy term are acquired using an interpolation method between every two selected images.     Injury patterns in a large-scale ski resort in the host city of 2022 Winter Olympic Games: a retrospective cross-sectional study

    Objectives The aims of the study are to describe the injury patterns among recreational skiers and snowboarders in China and to provide primary data to guide the reconstruction of regional healthcare facilities to deal with the increasing number of participants in snow sports.Design Retrospective cross-sectional study.Methods A retrospective study was performed in Wanlong Ski Resort in Chongli, China. Data of all injured skiers and snowboarders treated in the resort clinic during the 2018–2019 season were collected. Patients’ information, including sex, age, equipment, skill level and injured body part, was analysed.Results A total of 753 sports injuries were recorded. The estimated incidence of injury was 1.98 per 1000 skier days. 453 cases (60.2%) were associated with skiing. The mean age of skiers was older than snowboarders (35.1±14.5 vs 29.0±8.9, p&lt;0.01). Injury not involving others constituted 67.9% of all injuries. The most common injured body part among skiers was lower extremity, contrary to injury in the upper extremity among snowboarders. Head and cervical injury was identified in 13.7% of skiers and 13.6% of snowboarders.Conclusion The incidence of skiing/snowboarding injury in China was similar to other countries.     Effects of visual restoration on gait performance and kinematics of lower extremities in patients with age-related cataract

    Abstract. Background:. Visual inputs are critical for locomotor navigation and sensorimotor integration in the elderly; however, the mechanism needs to be explored intensively. The present study assessed the gait pattern after cataract surgery to investigate the effects of visual restoration on locomotion. Methods:. The prospective study recruited 32 patients (70.1 ± 5.2 years) with bilateral age-related cataracts in the Department of Ophthalmology at Peking University Third Hospital from October 2016 to December 2019. The temporal-spatial gait parameters and kinematic parameters were measured by the Footscan system and inertial measurement units. Paired t-test was employed to compare data normally distributed and Wilcoxon rank-sum test for non-normally distributed. Results:. After visual restoration, the walking speed increased by 9.3% (1.19 ± 0.40 m/s vs. 1.09 ± 0.34 m/s, P=0.008) and exhibited an efficient gait pattern with significant decrease in gait cycle (1.02 ± 0.08 s vs. 1.04 ± 0.07 s, P=0.012), stance time (0.66 ± 0.06 s vs. 0.68 ± 0.06 s, P=0.045), and single support time (0.36 ± 0.03 s vs. 0.37 ± 0.02 s, P=0.011). High amplitude of joint motion was detected in the sagittal plane in the left hip (37.6° ± 5.3° vs. 35.5° ± 6.2°, P=0.014), left thigh (38.0° ± 5.2° vs. 36.4° ± 5.8°, P=0.026), left shank (71.9° ± 5.7° vs. 70.1° ± 5.6°, P=0.031), and right knee (59.1° ± 4.8° vs. 56.4° ± 4.8°, P=0.001). The motor symmetry of thigh improved from 8.35 ± 5.30% to 6.30 ± 4.73% (P=0.042). Conclusions:. The accelerated gait in response to visual restoration is characterized by decreased stance time and increased range of joint motion. Training programs for improving muscle strength of lower extremities might be helpful to facilitate the adaptation to these changes in gait

According to the different description of gait characteristics, a set of models was established according to various people to present the movement of lower limbs. By the introduction of algorithm in machine learning, the FCM clustering algorithm is used to cluster the sample set and create a set of models, and then the SVM algorithm was used to identify the new samples, so as to complete the normal and abnormal motion function identification. The multimodel presented in this paper was carried out into the analysis of the anterior cruciate ligament deficiency. This method demonstrated being effective and can provide auxiliary analysis for clinical diagnosis

    Silencing of microRNA-101 prevents IL-1ÎČ-induced extracellular matrix degradation in chondrocytes

    Introduction: Extracellular matrix (ECM) degradation leads to malfunction of the cartilage in osteoarthritis (OA). Inflammatory cytokine interleukin-1 beta (IL-1 beta) functions in ECM degradation and prevents ECM synthesis by down-regulating the key transcription factor, Sox9, and consequently inhibiting ECM gene expression. Evidence reveals that microRNAs (miRNA) have been associated with OA, but little is known of their function in chondrocyte ECM degradation. This study aimed to identify possible miRNAs that mediate IL-1 beta-induced down-regulation of Sox9 as well as its known down-stream genes, collagen type II and aggrecan. Methods: The miRNAs were predicted based on three classical databases. The expression levels of the predicted miRNAs were assessed in IL-1 beta stimulated chondrocytes by real-time PCR. A luciferase reporter was used to test the binding of the miRNAs to the 3&apos; untranslated regions (3&apos;UTR) of Sox9. The predicted miRNAs were transfected into chondrocytes to validate their relationship with Sox9. Functional analysis of the miRNAs on chondrocytes ECM degradation was performed at both the mRNA and protein levels after miRNA transfection and IL-1 beta treatment. Results: Six miRNAs were predicted to target Sox9, and their expression in IL-1 beta-stimulated chondrocytes was revealed by real-time PCR. The luciferase reporter assay indicated that only miR-101 could bind to the 3&apos; UTR of Sox9. The expression of Sox9 was likewise negatively regulated by miR-101 in rat chondrocytes. Functional analysis showed that miR-101 could aggravate chondrocyte ECM degradation, whereas miR-101 inhibition could reverse IL-1 beta-induced ECM degradation. Conclusion: miR-101 participates in IL-1 beta-induced chondrocyte ECM degradation.
