    Differences between pharmacists’ perception of counseling and practice in the era of prescription drug misuse

    Objective: This study was conducted to assess pharmacists\u27 practices when counseling patients on their prescription medications, and their preferences for training. Methods: Five focus group discussions of community pharmacists (n=45, with seven to eleven participants in each group) were conducted in a major metropolitan city in the southern United States. Participants were recruited via email using a list of community pharmacists provided by the Texas State Board of Pharmacy. All focus group discussions were structured using a moderator guide consisting of both discrete and open-ended questions. Qualitative analysis software was used to analyze the data with a thematic analysis approach. Results: The participants in this study had a high self-efficacy regarding their ability to counsel on both new and opioid prescriptions. Many pharmacists experienced the same barriers to counseling and agreed on the components of counseling. However, the themes that emerged showed that the participants exhibited only a partial understanding of the components of counseling. The themes that emerged in the thematic analysis were perceived confidence and discordant counseling practices, inadequate infrastructure, lack of comprehensive counseling, inconsistent use of the Prescription Drug Monitoring Program (PDMP), and pharmacists\u27 desired training/assistance. Conclusions: Community pharmacists are in a unique position to help combat the opioid crisis; however, there has been very little research on the pharmacist-patient interaction in this context. With policy changes, such as the PDMP mandate, going into effect across the country, it is important to capitalize on the potential community pharmacists have in ameliorating the opioid crisis in the United States

    DEVELOPMENT AND EVALUATION OF IN SITU GELLING GASTRORETENTIVE FORMULATIONS OF MELOXICAM

    Objective: The aim of this study was to develop a novel gastro retentive oral floating in situ gelling system for controlled release of Meloxicam. Meloxicam is an NSAID that inhibits cyclooxygenase (COX) synthesis and has analgesic and antipyretic effects. Methods: Four polymer based floating in situ gelling systems of Meloxicam were prepared by dissolving varying concentrations of different ingredients including sodium alginate, HPMC K100M, calcium carbonate, sodium citrate. The prepared gels were characterized for solution viscosity, pH, gelling capacity, floating lag time, floating duration and in-vitro release study. Results: The formulations possessed satisfactory pH value ranging from 7.25±0.09 to 8.12±0.49. All the formulations showed instant gelation maintaining integrity for at least 12 h. Maximum drug release was shown by formulations of batch G1 (94.38%). Conclusion: The study demonstrated that a stomach specific in-situ gel of Meloxicam could be prepared using the floating mechanism to increase the residence time of the drug in the stomach and improve bioavailability and thus improve patient compliance.     Improvement of attention span and reaction time with hyperbaric oxygen treatment in patients with toxic injury due to mold exposure

    It is, by now, well established that mold toxins (mycotoxins) can cause significant adverse health effects. In this study, 15 subjects who developed an attention deficit disorder (ADD) and slowing of reaction time at the time of exposure to mold toxins were identified. Deficits in attention span and reaction time were documented not only by taking a careful history, but also by performing a Test of Variables of Attention (TOVA). The TOVA test provides an objective measure of these two variables. It was found that mold-exposed subjects show statistically significant decreases in attention span and significant increases in reaction time to stimuli compared to controls. After ten sessions of hyperbaric oxygen treatment (HBOT), a statistically significant improvement was seen in both measures. This preliminary study suggests promising outcomes in treating mold-exposed patients with hyperbaric oxygen

    Detoxifying effect of Nelumbo nucifera and Aegle marmelos on hematological parameters of Common Carp (Cyprinus carpio L.)

    The objective of this study was to investigate the efficacy of Nelumbo nucifera and Aegle marmelos on common carp exposed to sub-lethal concentrations of combined heavy metals (5 ppm) under laboratory conditions. The fish were treated with Nelumbo nucifera (500 mg/kg bwt) and Aegle marmelos (500 mg/kgbwt) for 30 days as a dietary supplement. The blood biochemical parameters of the fish were evaluated by analyzing the level of red blood cells (RBC), packed cell volume (PCV), hemoglobin concentration, glucose, cholesterol, iron and copper. The findings of the present investigation showed significant increase in hemoglobin (p<0.001), RBC (p<0.01) and PCV (p<0.01) of herbal drug-treated groups compared with metal-exposed fish. Conversely, glucose and cholesterol level in blood of common carp showed significant reduction compared with heavy-metal-exposed groups. All the values measured in Nelumbo nucifera and Aegle marmelos treated fish were restored comparably to control fish.
