    Hernioplasty in One-Day Surgery: result of 228 self-adhesive prosthesis

    Aim: To evaluate the outcomes of inguinal hernia repair with ProGrip® mesh in same-day surgery Methods: Follow-up data was collected at 24 hours and 30 days after surgery. Results: In one year, 228 patients underwent surgical repair of unilateral inguinal hernia. At 24 hours after surgery, 50.64% of patients reported some degree of pain and 66.3% were able to move around the house with few limitations. Thirty days after surgery, 94.39% of patients had returned to their routine activities. Conclusions: The use of the ProGrip® mesh is associated with low post-operative pain and rapid recovery

    Frequency doubling in the cyanobacterial circadian clock

    Organisms use circadian clocks to generate 24-h rhythms in gene expression. However, the clock can interact with other pathways to generate shorter period oscillations. It remains unclear how these different frequencies are generated. Here, we examine this problem by studying the coupling of the clock to the alternative sigma factor sigC\textit{sigC} in the cyanobacterium Synechococcus elongatus\textit{Synechococcus elongatus}. Using single-cell microscopy, we find that psbAI\textit{psbAI}, a key photosynthesis gene regulated by both sigC\textit{sigC} and the clock, is activated with two peaks of gene expression every circadian cycle under constant low light. This two-peak oscillation is dependent on sigC\textit{sigC}, without which psbAI\textit{psbAI} rhythms revert to one oscillatory peak per day. We also observe two circadian peaks of elongation rate, which are dependent on sigC\textit{sigC}, suggesting a role for the frequency doubling in modulating growth. We propose that the two-peak rhythm in psbAI\textit{psbAI} expression is generated by an incoherent feedforward loop between the clock, sigC\textit{sigC} and psbAI\textit{psbAI}.     231 Colecistectomias Laparoscopicas em Ambulatório: Que Resultados?

    ntroduction: Laparoscopic cholecystectomy is the gold standard procedure for the treatment of lithiasic gallblader pathology and acute cholecystitis. Whether or not it should be done as ambulatory surgery is still being discussed. The present study aims to analyze the quality and safety of laparoscopic cholecystectomy conducted by the Ambulatory Unit of Hospital de Braga and compare the results with those from other European surgical centers performing LC as ambulatory surgery. Material and Methods: Observational prospective study of patients submitted to laparoscopic cholecystectomy in ambulatory surgery during a period of 26 months. Data regarding patients' demography, peri- and postoperative complications, surgical time, time in recovery room, and readmission rates was collected. Results: A total of 231 patients were subjected to Laparoscopic cholecystectomy in the ambulatory unit with overnight stay (time to discharge less than 24 hours). Three patients presented with intra-operative complications, which needed conversion to laparotomy, and four patients were admitted after surgery. The mean time for the procedure was 58 minutes and the mean time for recovery was 19h19 minutes. Postoperative morbidity was 7.8% with 2 nonscheduled admission having occurred. Discussion: One of the controversies regarding laparoscopic cholecystectomy as an outpatient procedure is the need for overnight hospital surveillance. In our unit, we have started LC on an outpatient basis with an overnight stay. Using this protocol, over the past two years, we have operated 231 patients and our results show that this is a completely safe technique. Conclusions: Data from the study suggests that LC is a safe technique when performed in ambulatory practice, having similar results to other european surgical centers

    Characteristics of Dysphagia in Infants with Microcephaly Caused by Congenital Zika Virus Infection, Brazil, 2015.

    We summarize the characteristics of dysphagia in 9 infants in Brazil with microcephaly caused by congenital Zika virus infection. The Schedule for Oral Motor Assessment, fiberoptic endoscopic evaluation of swallowing, and the videofluoroscopic swallowing study were used as noninstrumental and instrumental assessments. All infants had a degree of neurologic damage and showed abnormalities in the oral phase. Of the 9 infants, 8 lacked oral and upper respiratory tract sensitivity, leading to delays in initiation of the pharyngeal phase of swallowing. Those delays, combined with marked oral dysfunction, increased the risk for aspiration of food, particularly liquid foods.
