The role of proinflammatory cytocines in depressive syndrome of patients with cardiac insuficiency: interaction with hypothalamic neurohormons and prognostic value
There is a greater incidence of depressive symptomsin heart failure patients than in general population. Recent studies indicate that proinflammatory cytokines are being activated in greater degree both in patients with depression and in heart failure patients. Nevertheless their correlation with dypressive symptoms and their interaction with hypothalamic-pituitary axis remain elusive. In this thesis, correlation of several proinflammatory factors, such as IL6, IL10, TNFa, soluble FAS Ligand and FASR, with clinical findings, neurohormonal factors and depressive symptomatology was studied. Apart from U/S, 6MWT and NYHA class, clinical evaluation included the administration of validated questionnaires such as Kansas City Cardiomyopathy Questionnaire and Minnesota Living with Heart Failure Questionnaire. During the follow up period, major cardiovascular events (hospital admission due to these events or death) were also recorded. The most important neurohormonal factors of interest were cortisol, prolactin and BNP, whereas depressive symptoms were evaluated with Zung and Beck validated scales. A statistically significant correlation of TNF-a, soluble FAS Ligand και IL-10 with Zung and Beck scores was noticed. Correlations of TNF-a and soluble FAS Ligand with prolactin and cortisol were also reached statistical significance. Contrary to TNF-a and soluble FAS Ligand, IL10 was negatively correlated to prolactin and cortisol. In addition, the incidence of major cardiovascular events was also correlated to prolactin levels, questionnaires scores and proinflammatory cytokines concentrations. Central nervous and immune systems interactions through cytokines may deteriorate heart failure through positive neurohormonal feedback loops. Prolactin may also act as a pathogenetic link between the central 120 neuroendocrine systems and the development and progression of chronic heart failure (CHF). Whether prolactin elevation plays a causal role in CHF or is just a bystander remains to be elucidated. In the first case, bromocriptine may merit further evaluation as a therapeutic option in CHF patients with raised prolactin levels. This inexpensive therapy also interferes with serotonin receptors and could further have a role in depression. What is already known is that the factors that have been studied in this thesis interact at the level of hypothalamus. Τέλος, το σημαντικότερο ίσως εύρημα ήταν ότι τόσο τα επίπεδα προλακτίνης όσο και οι απαντήσεις στα διάφορα σκορ εκτίμησης της ποιότητας της ζωής, αλλά και οι συγκεντρώσεις των προφλεγμονωδών παραγόντων παρουσίασαν στατιστικά σημαντική διαφορά μεταξύ της ομάδας των ασθενών που εμφάνισαν σοβαρό καρδιακό συμβάν και αυτών που είχαν καλύτερη πρόγνωση κατά τη διάρκεια της παρακολούθησης.Οι κυτταροκίνες επομένως ενδέχεται να αποτελούν το συνδετικό κρίκο μεταξύ των λειτουργιών του κεντρικού νευρικού συστήματος και του ανοσοποιητικού και η αλληλεπίδρασή τους να επιδεινώνει την καρδιακή ανεπάρκεια μέσω ανατροφοδοτούμενων νευροορμονικών μηχανισμών. Σχετικά με τη σημασία επίσης της προλακτίνης ως πιθανό παθοφυσιολογικό μεσολαβιτή μεταξύ των κεντρικών νευροορμονικών συστημάτων και της εξέλιξης της καρδιακής ανεπάρκειας, παραμένει άγνωστο το κατά πόσο η παραπάνω συσχέτιση αναδεικνύει μια αιτιακή σχέση ή η αύξηση των επιπέδωντηςείναιδευτεροπαθής.Στηνπρώτηπερίπτωσηη βρωμοκρυπτίνη θα πρέπει να διερευνηθεί περαιτέρω ως θεραπευτική επιλογή σε ασθενείς με καρδιακή ανεπάρκεια και αυξημένα επίπεδα προλακτίνης και ιδιαίτερα στην υποοομάδα με καταθλιπτικά συμπτώματα αφού εκτός των άλλων αυτή η οικονομική θεραπεία επηρεάζει και τη λειτουργεία των υποδοχέων της σεροτονίνης. Το γεγονός είναι ότι επειδή οι παράγοντες που μελετήθηκαν είναι γνωστό ότι αλληλοεπιδρούν σε επίπεδο υποθαλάμου, οι εξακρίβωση των μηχανισμών, ιδιαίτερα στο επίπεδο του παρακοιλιακού πυρήνα του υποθαλάμου, αποτελεί μία πολλά υποσχόμενη προσέγγιση στην αντιμετώπιση της καρδιακής ανεπάρκειας

Κλινικές μελέτες έχουν δείξει ότι η συχνότητα της καταθλιπτικής συνδρομής είναι μεγαλύτερη σε ασθενείς με καρδιαγγειακά νοσήματα και ιδιαίτερα σε ασθενείς με χρόνια καρδιακή ανεπάρκεια. Πρόσφατα δεδομένα υποστηρίζουν την άποψη ότι το σύνολο των προφλεγμονωδών κυτταροκινών ενεργοποιείται υπέρμετρα σε ασθενείς με καταθλιπτική συνδρομή. Επίσης είναι γνωστό ότι παρόμοια ενεργοποίηση των κυτταροκινών έχει παρατηρηθεί στη χρόνια καρδιακή ανεπάρκεια. Ωστόσο δεν έχει διερευνηθεί η συσχέτιση των προφλεγμονωδών κυτταροκινών με την εμφάνιση συμπτωμάτων κατάθλιψης σε ασθενείς με χρόνια καρδιακή ανεπάρκεια καθώς και η αλληλεπίδρασή τους με τον άξονα υποθαλάμου- υπόφυσης.
Στην παρούσα διατριβή μελετήθηκε η συσχέτιση γνωστών προφλεγμονωδών παραγόντων, όπως των κυτταροκινών IL6, IL10, TNFa και των soluble FAS Ligand και FASR, με την κλινική κατάσταση, νευροορμονικούς παράγοντες και καταθλιπτικά συμπτώματα. Στην πρώτη περίπτωση η βρωμοκρυπτίνη θα πρέπει να διερευνηθεί περαιτέρω ως θεραπευτική επιλογή σε ασθενείς με καρδιακή ανεπάρκεια και αυξημένα επίπεδα προλακτίνης και ιδιαίτερα στην υποοομάδα με καταθλιπτικά συμπτώματα αφού εκτός των άλλων αυτή η οικονομική θεραπεία επηρεάζει και τη λειτουργεία των υποδοχέων της σεροτονίνης. A comparison framework for the evaluation of illumination compensation algorithms
Abstract—This paper presents a new comparison framework, with the view to help researchers in selecting the most appropriate illumination compensation algorithm to serve as a preprocessing step in computer vision applications. The main objective of this framework is to reveal the positive and negative characteristics of the algorithms, rather than providing a single metric to rank their overall performance. The comparison tests, that comprise the proposed framework, aim to quantitatively evaluate the efficiency of algorithms in diminishing the effects of illumination in images. The proposed framework utilizes synthetic images, with artificial illumination degradations, which are enhanced by the tested algorithms. It represents a useful tool for the selection of illumination compensation algorithms as preprocessing in other applications, due to a) its quantitative nature, b) its easy implementation and c) its useful estimations regarding many algorithm characteristics. Genetic diversity of Thaumetopoea pityocampa in Greece: the role of Quaternary changes in Aegean Sea
Thaumetopoea pityocampa, the winter pine
processionary moth, is one of the most important pests
of pine trees in the Mediterranean region. To learn more
about the refugial areas within the greater refugium
of Greece, samples from 15 local populations were
collected. Analysis of the Cytochrome Oxidase I region
identified 15 haplotypes. One strongly supported clade
was found, separating the Aegean island of Lesvos from
the mainland populations. Mdiv analysis showed that
this clade diverged from the general clade 155,000 y.a.,
suggesting the sea level changes during the Quaternary
Period as a possible reason for its seclusion. Additionally,
comparisons with similar studies in this region revealed
a possible colonization of the eastern Aegean islands
from the Turkish mainland rather than from Greece.
However, no strong isolation-by-distance events were
detected among the mainland populations, which could
be attributed to the joint effect of regular gene flow and
the lack of insurmountable geographic barriers. Finally,
regarding population structure, Bayesian analysis as well
as neutrality tests pointed towards an ongoing population
expansion which verifies the potential invasiveness of
this pest species, something that will have to be dealt with
under the influence of climate change
EVALUATION OF FLOW DYNAMICS IN BIFURCATIONS BY FUSION OF 3-DIMENSIONAL ANGIOGRAPHY AND TWO-VESSEL OPTICAL COHERENCE TOMOGRAPHY
Clinical and neurohormonal correlates and prognostic value of serum prolactin levels in patients with chronic heart failure
Hypothalamic axis deregulation is associated with clinical severity and
depression in chronic heart failure (CHF). We investigated the
relationship of serum prolactin, an indicator of hypothalamic axis
function, to neurohomonal/immune activation and depressive symptoms in
CHF as well as its prognostic value.
Serum prolactin was determined in 180 patients with advanced CHF (aged
65 12 years, mean LVEF 27 7) along with natriuretic peptides (BNP),
inflammatory cytokines, endothelial adhesion molecules, 6 min walk test
(6MWT), and the Zung self-rating depression scale (SDS). Patients were
followed for all-cause death or hospitalization for cardiovascular
reasons for up to 8 months. Prolactin levels were significantly
correlated with NYHA class (r 0.394, P 0.001), LVEF (r 0.314, P 0.001),
6MWT (r 0.353, P 0.001), BNP (r 0.374, P 0.001), Zung SDS (r 0.544, P
0.001), interleukin-6 (IL-6) (r 0.451, P 0.001), IL-10 (r 0.426, P
0.001), tumour necrosis factor (TNF)- (r 0.310, P 0.001), soluble Fas (r
0.333, P 0.001), soluble Fas-ligand (r 0.517, P 0.001), soluble
intercellular adhesion molecule-1 (ICAM-1) (r 0.409, P 0.001), and
soluble vascular cell adhesion molecule-1 (VCAM-1) (r 0.480, P 0.001).
During follow-up, 119 patients (66) died or were hospitalized for
cardiovascular events after a median time of 72 days (range 5220 days);
these patients had higher baseline prolactin levels (10.2 5.7 vs. 6.7
4.3 ng/mL, P 0.001), and a prolactin value 4.5 ng/mL was associated with
a higher rate of death or hospitalization (116 7 vs. 181 11 days, P
0.0001). In multivariate analysis, prolactin levels remained an
independent predictor of death or hospitalization (4.5 vs. 4.5 ng/mL;
odds ratio, 0.368; 95 confidence interval 0.1480.913; P 0.031), along
with BNP (P 0.001) and 6MWT (P 0.020).

Joint ESH excellence centers' national meeting on renal sympathetic denervation: A Greek experts' survey
Objective: The efficacy of renal sympathetic denervation (RDN) has been
affirmed by a number of recent clinical studies, despite controversies
in this field over the last five years. Therefore, it is of paramount
importance that hypertension experts debate the merits of RDN by
revealing and expressing their personal beliefs and perspectives
regarding this procedure. Methods: A cross-sectional survey was
conducted among Greek leaders of the Hypertension Excellence Centers
with the use of a closed-type questionnaire specifically designed to
elicit information and evaluate the respondent’s views and perspectives
about RDN efficacy, safety and ideal target patient population. Results:
A total of 36 participants completed the survey. Based on the results,
RDN was considered efficient (91.7%) and safe (94.5%), while the
overwhelming majority of the participants felt confident in the
long-term efficacy (88.9%) of the intervention and that it lacks
reliable predictors of blood pressure response (94.5%). Patients with
resistant (91.7%), ultra-resistant (94.4%), and uncontrolled
hypertension (80.6%) were suggested as ideal candidates for RDN.
Establishing a close co-operation between interventionalists and
hypertension experts was considered essential to ensure the efficacy
(97.2%) as well as the safety (97.3%) of the procedure. Conclusion:
The vast majority of Greek hypertension experts surveyed were convinced
of the efficacy and safety of RDN based on the preponderance of
available scientific and clinical data. Identification of the ideal
patient group remains controversial. Respondents generally agreed on the
necessity of building close collaborative relationships between
interventionalists and hypertension experts in order to improve RDN
