    Audiometric Characteristics in Patients With Noise-Induced Hearing Loss After Sodium Enoxaparin Treatment

    OBJECTIVES:The aim of this study was to evaluate the effect of sodium enoxaparin treatment on patients with noise-induced hearing loss. METHODS: Sixty patients with noise-induced hearing loss were included and randomly divided into two numerically equal groups. Group A underwent therapy with sodium enoxaparin for 10 days, followed by an additional 10 days of treatment after 10 days of no treatment. Group B received placebo as a control. Before treatment, at the end of treatment, and 2 months after the end of treatment, all patients underwent evaluation by laboratory tests, pure tone audiometry, transient evoked otoacoustic emissions (TEOAEs) testing, distortion product otoacoustic emissions (DPOAEs) testing, and auditory brain stem response testing. RESULTS: In contrast to group B, at the end of the treatment in group A pure tone audiometry showed a significant (p < 0.05) improvement of the audiometric thresholds at 0.5, 1, 2, 4, and 8 kHz. Depending on the air and bone conduction thresholds, TEOAEs and DPOAEs, which had previously been absent, were evoked at the frequencies examined. These improvements were confirmed at last follow-up. We found no significant differences in auditory brain stem responses or laboratory results. CONCLUSIONS: These preliminary data encourage further studies to collect additional evidence on the effect of sodium enoxaparin in preventing the development of noise-induced hearing loss

    Lo scopo di tale pubblicazione è di offrire una panoramica generale sulle leggi e sulle procedure che disciplinano la denuncia dei lavori per autorizzazione sismica e per deposito sismico delle opere civili sul territorio nazionale e, in particolare, nella Regione Campania. Si intende cioè offrire una lettura semplificata di tali leggi, fornendo indicazioni pratiche e dirette ai professionisti. Il presente lavoro vuole essere di supporto sia per gli operatori del settore delle costruzioni, quali tecnici e costruttori, sia per i committenti per facilitare l’orientamento in un iter burocratico purtroppo spesso poco chiaro e appesantito. Tale lavoro parte da un inquadramento legislativo nazionale, ma inevitabilmente focalizza sulle leggi regionali della Campania poiché ogni regione, nel corso degli anni, si è disciplinata da sé in merito alle procedure di deposito sismico e autorizzazione sismica

    We present an extreme rare case of extramedullary nasal plasmacitoma that arise from nasal septum. The mass surgically removed was analyzed by a pathologist who diagnosed an extramedullary nasal plasmacytoma. The patient did not present systemic involvement. A short cycle of radiotherapy was performed after the surgery. At 9-month follow-up, the patient is recurrence free

    Cost-effectiveness analysis of revisional Roux-en-Y gastric bypass: laparoscopic vs. robot assisted

    There is controversy over the possible advantages of the robotic technology in revisional bariatric surgery. The aim of this study is to report the experience of a high-volume bariatric center on revisional Roux-en-Y gastric bypass with robot-assisted (R-rRYGB) and laparoscopic (L-rRYGB) approaches, with regards to operative outcomes and costs. Patients who underwent R-rRYGB and L-rRYGB between 2008 and 2021 were included. Patients’ baseline characteristics and perioperative data were recorded. The primary endpoint was the overall postoperative morbidity. A full economic evaluation was performed. One-way and two-way sensitivity analyses were performed on laparoscopic anastomotic leak and reoperation rates. A total of 194 patients were included: 44 (22.7%) L-rRYGB and 150 (77.3%) R-rRYGB. The robotic approach was associated with lower overall complication rate (10% vs. 22.7%, p = 0.038), longer operative time, and a reduced length of stay compared to L-rRYGB. R-rRYGB was more expensive than L-rRYGB (mean difference 2401.1€, p < 0.001). The incremental cost-effective ratio (ICER) was 18,906.3€/complication and the incremental cost-utility ratio was 48,022.0€/QALY (quality-adjusted life years), that is below the willingness-to-pay threshold. Decision tree analysis showed that L-rRYGB was the most cost-effective strategy in the base-case scenario; a probability of leak ≥ 13%, or a probability of reoperation ≥ 14% following L-rRYGB, or a 12.7% reduction in robotic costs would be required for R-rRYGB to become the most cost-effective strategy. R-rRYGB was associated with higher costs than L-rRYGB in our base-case scenario. However, it is an acceptable alternative from a cost-effectiveness perspective

    Defining a Digital Strategy in a BIM Environment to Manage Existing Reinforced Concrete Bridges in the Context of Italian Regulation

    Regulatory activity concerning the management of existing bridges has recently been affected by updates, for instance, in Italy, which calls for a speedy and pragmatic approach based on new technologies such as building information modeling (BIM), when dealing with the survey and risk classification as well as the evaluation and monitoring of structural safety. This paper focuses on the development and integration of a digital solution, based principally on the specific framework developed by the authors, which supports BIM modeling and information management activities, in the structural setting under investigation, through the use of several technologies and tools, namely BIM-authoring, CDE platform and visual programming, in addition to programming in Python. Starting from the organization of a specific BIM object library and the initial data, inserted by means of a custom-made input environment, it was possible to reproduce digital models of bridges in accordance with specific information requirements following the new Level of Information Need setting. The applicability of the proposal is tested on two judiciously chosen real-life cases with different characteristics. Through this implementation, a series of advantages emerge, including expediting traditional procedures for BIM modeling, accessibility and traceability of information— which are constantly updated to support the monitoring of structural safety over time—and the decision-making process related to the bridge management context
