Gender Differences in Smoking Among an Urban Emergency Department Sample.
BackgroundUrban emergency department (ED) patients have elevated smoking and substance use compared with the general population. We analyzed gender differences in smoking among an urban ED sample and assessed the contribution of substance use, demographic, and couple factors.MethodsWe conducted a secondary analysis of data obtained from a cross-sectional, observational survey (N = 1037 participants) on drinking, drug use, and intimate partner violence (IPV). Gender-specific logistic regression models for current (past 30-day) smoking and multinomial regression models for smoking intensity (light: ⩽5 cigarettes per day [CPD]; moderate: 6 to 10 CPD; heavier: >10 CPD) were estimated.ResultsSmoking prevalence was higher among men than women (35.5% vs 18.9%; P < .001). Substance use (frequency of intoxication, marijuana, amphetamine, and cocaine use), demographic (food insufficiency, unemployment), and couple-related factors (having a spouse/partner who smoked, IPV involvement, being in a same-gender couple) were differentially associated with current smoking and level of intensity among men and women.ConclusionsEmergency department staff should consider the impact of polysubstance use, food insufficiency, unemployment, and whether both partners in the couple smoke when screening patients for smoking and formulating cessation treatment plans. Women in same-gender relationships and those who have experienced IPV involvement may require additional referral
Effect of lower sodium intake on health: systematic review and meta-analyses
Objective To assess the effect of decreased sodium intake on blood pressure, related cardiovascular diseases, and potential adverse effects such as changes in blood lipids, catecholamine levels, and renal function.
Design Systematic review and meta-analysis.
Data sources Cochrane Central Register of Controlled Trials, Medline, Embase, WHO International Clinical Trials Registry Platform, the Latin American and Caribbean health science literature database, and the reference lists of previous reviews.
Study selection Randomised controlled trials and prospective cohort studies in non-acutely ill adults and children assessing the relations between sodium intake and blood pressure, renal function, blood lipids, and catecholamine levels, and in non-acutely ill adults all cause mortality, cardiovascular disease, stroke, and coronary heart disease.
Study appraisal and synthesis Potential studies were screened independently and in duplicate and study characteristics and outcomes extracted. When possible we conducted a meta-analysis to estimate the effect of lower sodium intake using the inverse variance method and a random effects model. We present results as mean differences or risk ratios, with 95% confidence intervals.
Results We included 14 cohort studies and five randomised controlled trials reporting all cause mortality, cardiovascular disease, stroke, or coronary heart disease; and 37 randomised controlled trials measuring blood pressure, renal function, blood lipids, and catecholamine levels in adults. Nine controlled trials and one cohort study in children reporting on blood pressure were also included. In adults a reduction in sodium intake significantly reduced resting systolic blood pressure by 3.39 mm Hg (95% confidence interval 2.46 to 4.31) and resting diastolic blood pressure by 1.54 mm Hg (0.98 to 2.11). When sodium intake was 0.05). There were insufficient randomised controlled trials to assess the effects of reduced sodium intake on mortality and morbidity. The associations in cohort studies between sodium intake and all cause mortality, incident fatal and non-fatal cardiovascular disease, and coronary heart disease were non-significant (P>0.05). Increased sodium intake was associated with an increased risk of stroke (risk ratio 1.24, 95% confidence interval 1.08 to 1.43), stroke mortality (1.63, 1.27 to 2.10), and coronary heart disease mortality (1.32, 1.13 to 1.53). In children, a reduction in sodium intake significantly reduced systolic blood pressure by 0.84 mm Hg (0.25 to 1.43) and diastolic blood pressure by 0.87 mm Hg (0.14 to 1.60).
Conclusions High quality evidence in non-acutely ill adults shows that reduced sodium intake reduces blood pressure and has no adverse effect on blood lipids, catecholamine levels, or renal function, and moderate quality evidence in children shows that a reduction in sodium intake reduces blood pressure. Lower sodium intake is also associated with a reduced risk of stroke and fatal coronary heart disease in adults. The totality of evidence suggests that most people will likely benefit from reducing sodium intake
Ion Exchange for Nutrient Recovery Coupled with Biosolids-Derived Biochar Pretreatment to Remove Micropollutants
Wastewater, especially anaerobic treatment effluent, contains high ammonia nitrogen (NH4-N) and inorganic orthophosphate (PO4-P), which necessitate additional treatment to meet stringent discharge regulations. Ion exchange regeneration is a process that can be adopted for not only removing but also recovering nutrients. However, recovering nutrients by ion exchange from nutrient-rich effluents that also contain micropollutants (which typically pass through anaerobic treatment as well) may result in subsequent problems, since micropollutants could end up in ion exchange effluent, regenerant, or recovered fertilizer products. Micropollutant removal by a nonselective adsorbent, such as biosolids-derived biochar, before nutrient recovery processes would mitigate potential risks. The objective of this research was to evaluate the capability of biosolids-derived biochar as a pretreatment step for separating micropollutants from nutrient-rich water before ion exchange for nutrient recovery. In the presence of ammonium and phosphate, both pristine and regenerated biosolids-derived biochar could effectively adsorb triclosan (TCS) and estradiol (E2), and to a lesser extent, sulfamethoxazole (SMX) in batch sorption experiments. On the other hand, nutrient ions were not effectively adsorbed by biosolids-derived biochar. A continuous flow-through system consisting of columns in series filled with biochar, LayneRT, and then clinoptilolite was operated to test selective removal of micropollutants and nutrients in a flow-through system. The biochar column achieved more than 80% removal of influent TCS and E2, thereby reducing the chances of micropollutants being adsorbed by ion exchangers. Sulfamethoxazole removal through the biochar column was only 50%, indicating that biosolids-derived biochar would have to be optimized in the future for hydrophilic micropollutant removal. Influent nutrients were not effectively removed by the biochar column, but were captured in their respective selective ion exchanger columns. This research revealed that biosolids-derived biochar could be employed before ion exchange resins for removal of micropollutants from nutrient-rich water
Andersen-Tawil syndrome: report of 3 novel mutations and high risk of symptomatic cardiac involvement.
IntroductionAndersen-Tawil syndrome (ATS) is a potassium channelopathy affecting cardiac and skeletal muscle. Periodic paralysis is a presenting symptom in some patients, whereas, in others, symptomatic arrhythmias or prolongation of QT in echocardiographic recordings will lead to diagnosis of ATS. Striking intrafamilial variability of expression of KCNJ2 mutations and rarity of the syndrome may lead to misdiagnosis.MethodsWe report 15 patients from 8 Polish families with ATS, including 3 with novel KCNJ2 mutations.ResultsAll patients had dysmorphic features; periodic paralysis affected males more frequently than females (80% vs. 20%), and most attacks were normokalemic. Two patients (with T75M and T309I mutations) had aborted sudden cardiac death. An implantable cardioverter-defibrillator was utilized in 40% of cases.ConclusionsKCNJ2 mutations cause a variable phenotype, with dysmorphic features seen in all patients studied, a high penetrance of periodic paralysis in males and ventricular arrhythmia with a risk of sudden cardiac death

Immunosuppressive effect and global dysregulation of blood transcriptome in response to psychosocial stress in vervet monkeys (Chlorocebus sabaeus).
Psychosocial stressors - life events that challenge social support and relationships - represent powerful risk factors for human disease; included amongst these events are relocation, isolation and displacement. To evaluate the impact of a controlled psychosocial stressor on physiology and underlying molecular pathways, we longitudinally studied the influence of a 28-day period of quarantine on biomarkers of immune signalling, microbial translocation, glycaemic health and blood transcriptome in the wild-born vervet monkey. This event caused a coordinated, mostly transient, reduction of circulating levels of nine immune signalling molecules. These were paralleled by a massive dysregulation of blood transcriptome, including genes implicated in chronic pathologies and immune functions. Immune and inflammatory functions were enriched among the genes downregulated in response to stress. An upregulation of genes involved in blood coagulation, platelet activation was characteristic of the rapid response to stress induction. Stress also decreased neutrophils and increased CD4 + T cell proportions in blood. This model of psychosocial stress, characterised by an immune dysregulation at the transcriptomic, molecular and cellular levels, creates opportunities to uncover the underlying mechanisms of stress-related diseases with an immune component, including cardiovascular diseases and susceptibility to infections

Characterization of a continuous feline mammary epithelial cell line susceptible to feline epitheliotropic viruses.
Mucosal epithelial cells are the primary targets for many common viral pathogens of cats. Viral infection of epithelia can damage or disrupt the epithelial barrier that protects underlying tissues. In vitro cell culture systems are an effective means to study how viruses infect and disrupt epithelial barriers, however no true continuous or immortalized feline epithelial cell culture lines are available. A continuous cell culture of feline mammary epithelial cells (FMEC UCD-04-2) that forms tight junctions with high transepithelial electrical resistance (>2000Omegacm(-1)) 3-4 days after reaching confluence was characterized. In addition, it was shown that FMECs are susceptible to infection with feline calicivirus (FCV), feline herpesvirus (FHV-1), feline coronavirus (FeCoV), and feline panleukopenia virus (FPV). These cells will be useful for studies of feline viral disease and for in vitro studies of feline epithelia
Magnetic fluctuations and superconducting properties of CaKFe4As4 studied by 75As NMR
We report As nuclear magnetic resonance (NMR) studies on a new
iron-based superconductor CaKFeAs with = 35 K. As
NMR spectra show two distinct lines corresponding to the As(1) and As(2) sites
close to the K and Ca layers, respectively, revealing that K and Ca layers are
well ordered without site inversions. We found that nuclear quadrupole
frequencies of the As(1) and As(2) sites show an opposite
temperature () dependence. Nearly independent behavior of the Knight
shifts are observed in the normal state, and a sudden decrease in in
the superconducting (SC) state clearly evidences spin-singlet Cooper pairs.
As spin-lattice relaxation rates 1/ show a power law dependence
with different exponents for the two As sites. The isotropic antiferromagnetic
spin fluctuations characterized by the wavevector = (, 0) or (0,
) in the single-iron Brillouin zone notation are revealed by 1/ and
measurements. Such magnetic fluctuations are necessary to explain the
observed temperature dependence of the As quadrupole frequencies, as
evidenced by our first-principles calculations. In the SC state, 1/ shows
a rapid decrease below without a Hebel-Slichter peak and decreases
exponentially at low , consistent with an nodeless two-gap
