    A Pharmacist Education Initiative Apoteker Cilik to Improve Medication Safety Knowledge among Elementary School Students in Gresik, Indonesia

    Pharmacists have the responsibility to ensure that medicines are obtained, used, stored, and disposed in the best and safest way particularly by a vulnerable population such as children. Pharmacists should adapt to a childā€™s level of health understanding to engage children in medication education. Therefore, a pharmacist-led program tailored to children in community settings was initiated in two elementary schools in Gresik, SD Muhammadiyah 1 Gresik and SD Muhammadiyah Manyar. An animated film Apoteker Cilik was shown to students of grade 4, 5, and 6 to promote the proper management of medications in line with the national campaign DAGUSIBU and to acquaint with the roles of pharmacists. A short questionnaire consisting of six questions related to medication use and safety (how to get, use, store, and dispose medicines) was utilized to assess the level of childrenā€™s knowledge. There was a significant difference between levels of knowledge before and after the program (p < 0.05) although some schoolchildren already had good baseline knowledge of medications. This local initiative may contribute to improving medication awareness, so more educational programs using colourful visuals and interactive games may be conducted with this vulnerable population as children will bring their medication-taking behaviour throughout their adult lives

    In Vitro Gentamicin Release from Bioactive BHAG(ELENA) Implant Against Staphylococcus aureus

    Osteomyelitis is a bone-related infectious desease which is difficult to treat, because the antibiotic reaches the target is lower than the MIC and bacteria canā€™t be eradicated. This condition can cause the bacteria become resistant. To solve this problem, we can use local antibiotics as BHAG(ELENA) pellets implant, which can release gentamicin (GEN) continuously for more than a day with a concentration greater than MIC. BHAG(ELENA) pellet that have made contain BHA : GEL = 20 : 2 (dry state); GEN 10% and cross-linking with glutaraldehyde (GA) 0,5%, cylindrical weigh 100 mg; 4 mm in diameter and 3.2 mm thick. The release of GEN from BHAG(ELENA) pellet were tested in vitro, by soaking the pellet in phosphate buffer saline pH 7.4 at temperature 370C. The sample were sampled every day until 28 days. Then, the sample were tested by agar diffusion method that contain Staphylococcus aureus. Results showed that inhibition zone diameter greater than MIC GEN to S. aureus. Within 28 days, the release of GEN provide a total activity 99,24%, it showed that after 28 days, the pellets are still actively inhibit the bacterial growth. Furthermore, required to be tested in animal study (in vivo) with a defect in the femoral bone then filled with BHAG(ELENA) pellets as drug delivery system of GEN and bone fillers.     Potential interaction of fluroquinolone with other drugs have been established in the several reports. The absorption of fluroquinolones were almost entirely inhibited by concomitant administration of di and trivalent cations such magnesium and aluminium in the antacids were significantly decreased the absorption of ciprofloxacin through formation of a complex. The effect of antacid (Antasida DOEN) 1.4 ml/kg body weight doses which containing polyvalent cation Mg (OH)2 and Al(OH) 3 on the absorption of ciprofloxacin. were examed in healthy rabbit. Twelve subjects were given 23 mg/ml BW ciprofloxacin alone as control. On day 8 the six subjects were given antacid and ciprofloxacin concomitant and the other were given antacid two hours after ciprofloxacin. The absorption parameters of ciprofloxacin were determined by spectrofluorometric method. Those parameters were Cmax, tmax and AUC, the administration of ciprofloxacin and antacids concomitantly result Cmax, tmax and AUC were 1.27Ā±0.45Āµg/ml; 110Ā±48.99 minutes and 248.63Ā±94.77Āµg.minutes/ml respectively in a significant decrease in ciprofloxacin absorption (p<0.05) and the administration of antacids two hours after ciprofloxacin result Cmax, tmax and AUC were 1.37Ā±0.65Āµg/ml;75Ā±36.74 minutes and 245.09Ā±100.46 Āµg.minutes/ml respectively in a significant decrease in ciprofloxacin absorption (p<0.05).     Objective. The present study was designed to investigate the potency of BHA-GEL-GA-GEN pellet as gentamicin (GEN) delivery system matrix which has been cross-linked with glutaraldehyde (GA) 0.5%. Methods. The composition of Bovine hydroxyapatite (BHA) compared to the composition of Gelatin (GEL) is 10 : 1. Pellets are cylindrical with 4.0 mm diameter and weighed 100.0 mg. GEN releasing is evaluated by in vitro and in vivo examinations based on the concentration of GEN in 28 days. Results. Based on in vitro examination, it indicates 86% of GEN are released gradually in 28 days. Meanwhile, in vivo examination by implanting the pellet on rabbit femur indicates C max of GEN release on proximal area reached 24.32 ļ­g/L in 14 days and 26.97 ļ­g/L in 28 days for distal area. X-ray examination indicates the pellets have circular form as its defects at the second day. At the 28 th day examination, the pellets have transformed into compact form which filled the defects. This result indicates the pellets have been degraded and compatible with surrounding tissue. GEL is a type-1 collagen which is the main component of softcallus (osteoid). It undergoes calcification and transformed into hardcallus (osteocyte). High availability of calcium provided by BHA as delivery system accelerated this remodeling process. Conclusion. It can be concluded that in 28 days BHA-GEL-GA-GEN as local delivery system releases higher concentration of GEN compared to MIC Staphyllococcus aureus. BHA and GEL serve as bone remodeling template in osteomyelitis treatment.     The Effect of 20-20-20 Rule Dissemination and Artificial Tears Administration in High School Students Diagnosed with Computer Vision Syndrome

    Computers and gadgets are modern tools for assisting Senior High School studentsā€™ work. However, using gadgets more than 2 hours a day may cause computer vision syndrome (CVS), which can interfere with academic activities. CVS main symptom is dry eyes. CVS could be prevented by applying the 20-20-20 rule and artificial tears. A total of 100 students from two public high schools in Kediri with 50 students from each school were selected for this community service. All students diagnosed with CVS participated in the dissemination of 20-20-20 rule and artificial tear administration. The pre-test and post-test scores, the Tear Break Up Time Test (TBUT) and the Ocular Disease Surface Index (OSDI) were measured within two weeks. TBUT and OSDI are parameters for measuring dry eye. Likewise, interviews were conducted to assess the adherence to the 20-20-20 rule and artificial tea administration. The results showed the knowledge level increased (p 0.05). Applying the 20-20-20 rule and artificial tears could be used for CVS treatment
