    Long-Term (10-Year) Gastrointestinal and Genitourinary Toxicity after Treatment with External Beam Radiotherapy, Radical Prostatectomy, or Brachytherapy for Prostate Cancer

    Objective.To examine gastrointestinal (GI) and genitourinary (GU) toxicity profiles of patients treated in 1999 with external beam radiotherapy (RT), prostate interstitial brachytherapy (PI) or radical prostatectomy (RP). Methods. TThe records of 525 patients treated in 1999 were reviewed to evaluate toxicity. Late GI and GU morbidities were graded according to the RTOG late morbidity criteria. Other factors examined were patient age, BMI, smoking history, and medical co-morbidities. Due to the low event rate for late GU and GI toxicities, a competing risk regression (CRR) analysis was done with death as the competing event. Results. Median follow-up time was 8.5 years. On CRR univariate analysis, only the presence of DM was significantly associated with GU toxicity grade >2 (P = 0.43, HR 2.35, 95% Cl = 1.03–5.39). On univariate analysis, RT and DM were significantly associated with late GI toxicity. On multivariable analysis, both variables remained significant (RT: P = 0.038, HR = 4.71, CI = 1.09–20.3; DM: P = 0.008, HR = 3.81, 95% Cl = 1.42–10.2). Conclusions. Late effects occur with all treatment modalities. The presence of DM at the time of treatment was significantly associated with worse late GI and GU toxicity. RT was significantly associated with worse late GI toxicity compared to PI and RP

    The Carnian Humid Episode of the late Triassic: a review

    From 1989 to 1994 a series of papers outlined evidence for a brief episode of climate change from arid to humid, and then back to arid, during the Carnian Stage of the late Triassic Epoch. This time of climate change was compared to marine and terrestrial biotic changes, mainly extinction and then radiation of flora and fauna. Subsequently termed, albeit incorrectly, the Carnian Pluvial Event (CPE) by successive authors, interest in this episode of climatic change has increased steadily, with new evidence being published as well as several challenges to the theory. The exact nature of this humid episode, whether reflecting widespread precipitation or more local effects, as well as its ultimate cause, remains equivocal. Bed-by-bed sampling of the Carnian in the Southern Alps (Dolomites) shows the episode began with a negative carbon isotope excursion that lasted for only part of one ammonoid zone (A. austriacum). However, that the Carnian Humid Episode represents a significantly longer period, both environmentally and biotically, is irrefutable. The evidence is strongest in the European, Middle Eastern, Himalayan, North American and Japanese successions, but not always so clear in South America, Antarctica and Australia. The eruption of the Wrangellia Large Igneous Province and global warming (causing increased evaporation in the Tethyan and Panthalassic oceans) are suggested as causes for the humid episode

    Fish Assemblage Relationships with Physical Habitat in Wadeable Iowa Streams

    Fish assemblages play a key role in stream ecosystems and are influenced by physical habitat. We analyzed fish assemblages and physical habitat at 93 randomly selected sites on second- through fifth-order wadeable Iowa streams to explore fish assemblage relationships with reach-scale physical habitat in this agriculturally dominated landscape. Sites were sampled using DC electrofishing and the wadeable streams physical habitat protocol of the U.S. Environmental Protection Agency\u27s Environmental Monitoring and Assessment Program. In all, 82 species were collected, with species richness at sites averaging 14. Over 80% of the sites had fish assemblages rated as fair (53%) or poor (32%) based on a fish index of biotic integrity (FIBI). Ordination separated sites from the two major river drainages along an axis of impairment, with sites in the Missouri River drainage exhibiting lower FIBI scores than sites in the Mississippi River drainage. Physical habitat at most sites exhibited fine substrates, eroding banks, and low-gradient, nonmeandering channel and was dominated by glides. Thirty physical habitat variables describing channel morphology, channel cross section and bank morphology, fish cover, human disturbance, large woody debris, relative bed stability, residual pool, riparian vegetation, and substrate differed significantly between sites with FIBI scores rated as poor and those with FIBI scores rated as good or excellent. Eighteen physical habitat variables were significant predictors of fish assemblage metrics and FIBI in multiple linear regression models, with adjusted R 2 values ranging from 0.12 to 0.58. Seventy percent of the model coefficients reflected substrate (40%), residual pool (21%), and fish cover (9%) variables. Fish assemblages in wadeable Iowa streams are strongly associated with the quality of physical habitat. Thus, understanding and addressing the determinants of physical habitat are crucial for managing streams in Iowa and other agricultural regions

    Simple Nudges for Better Password Creation

    Recent security breaches have highlighted the consequences of reusing passwords across online accounts. Recent guidance on password policies by the UK government recommend an emphasis on password length over an extended character set for generating secure but memorable passwords without cognitive overload. This paper explores the role of three nudges in creating website-specific passwords: financial incentive (present vs absent), length instruction (long password vs no instruction) and stimulus (picture present vs not present). Mechanical Turk workers were asked to create a password in one of these conditions and the resulting passwords were evaluated based on character length, resistance to automated guessing attacks, and time taken to create the password. We found that users created longer passwords when asked to do so or when given a financial incentive and these longer passwords were harder to guess than passwords created with no instruction. Using a picture nudge to support password creation did not lead to passwords that were either longer or more resistant to attacks but did lead to account-specific passwords

    The Speed of Sound in Methane under Conditions of the Thermal Boundary Layer of Uranus

    We present the first direct observations of acoustic waves in warm dense matter. We analyze wavenumber- and energy-resolved X-ray spectra taken from warm dense methane created by laser-heating a cryogenic liquid jet. X-ray diffraction and inelastic free electron scattering yield sample conditions of 0.3±\pm0.1 eV and 0.8±\pm0.1 g/cm3^3, corresponding to a pressure of ∌\sim13 GPa and matching the conditions predicted in the thermal boundary layer between the inner and outer envelope of Uranus. Inelastic X-ray scattering was used to observe the collective oscillations of the ions. With a highly improved energy resolution of ∌\sim50 meV, we could clearly distinguish the Brillouin peaks from the quasi-elastic Rayleigh feature. Data at different wavenumbers were used to obtain a sound speed of 5.9±\pm0.5 km/s, which enabled us to validate the use of Birch's law in this new parameter regime.Comment: 7 pages, 4 figures with supplementary informatio

    The Consensus from the Mycobacterium avium ssp. paratuberculosis (MAP) Conference 2017.

    On March 24 and 25, 2017 researchers and clinicians from around the world met at Temple University in Philadelphia to discuss the current knowledge of Mycobacterium avium ssp. paratuberculosis (MAP) and its relationship to human disease. The conference was held because of shared concern that MAP is a zoonotic bacterium that poses a threat not only to animal health but also human health. In order to further study this problem, the conferees discussed ways to improve MAP diagnostic tests and discussed potential future anti-MAP clinical trials. The conference proceedings may be viewed on the www.Humanpara.org website. A summary of the salient work in this field is followed by recommendations from a majority of the conferees
