    Oral self-injuries: clinical findings in a series of 19 patients

    Objectives: Self-injury (SI) is defined as a behavioral disturbance consisting of a deliberate harm to one’s own body without suicidal intent, it is not uncommon and ranges in severity from simple nail-biting to more extreme forms of self-mutilation. The head neck region may be the target of such lesions. SI is associated with several medical conditions, of which it can represent the first clinical sign. Aim of this paper is to describe a series of oral SI, giving special emphasis to the clinical findings, etiology and the management of lesions. Material and Methods: A total of 19 patients with oral SI were prospectively examined; attention was paid to the occurrence and characterization of oral lesions. The management of the lesion also varied depending on the patient medical history, on the etiology of the psychiatric behavior, and on the severity, frequency, and method of inflicting injury. Periodic examinations were performed (after two weeks, three months and six months) and registered. Results: All the patients healed gradually and healing was conditioned by the disease underlying. The treatment consisted of behavior modification in 11 cases, pharmacological treatment in 11 cases, psychotherapy in 2 cases, mouth guard in 9 cases, surgery in 2 cases, extractions in 1 case. Conclusions: Oral SI are uncommon in the clinical practice. They may be associated with a known disease or may be the consequence of this, but often they may be the first sign of a psychiatric disorder

    Oral Microbiota and Salivary Levels of Oral Pathogens in Gastro-Intestinal Diseases: Current Knowledge and Exploratory Study

    Various bi‐directional associations exist between oral health and gastro‐intestinal diseases. The oral microbiome plays a role in the gastro‐intestinal carcinogenesis and fusobacteria are the most investigated bacteria involved. This paper aims to review the current knowledge and report the preliminary data on salivary levels of Fusobacterium nucleatum, Porphyromonas gingivalis and Candida albicans in subjects with different gastro‐intestinal conditions or pathologies, in order to determine any differences. The null hypothesis was “subjects with different gastro‐intestinal diseases do not show significant differences in the composition of the oral microbiota”. Twenty‐one subjects undergoing esophagastroduodenoscopy or colonscopy were recruited. For each subject, a salivary sample was collected before the endoscopy procedure, immediately stored at ‐20°C and subsequently used for genomic bacterial DNA extraction by real‐time PCR. Low levels of F. nucleatum and P. gingivalis were peculiar in the oral microbiota in subjects affected by Helicobater pylori‐negative chronic gastritis without cancerization and future studies will elucidate this association. The level of C. albicans did not statistically differ among groups. This preliminary study could be used in the future, following further investigation, as a non‐invasive method for the search of gastrointestinal diseases and associated markers

    Multidisciplinary approach in the management of a complex case: Implant-prosthetic rehabilitation of a periodontal smoking patient with partial edentulism, malocclusion, and aesthetic diseases

    Complex periprosthetic cases are considered as challenges by clinicians. Clinical and radiographic parameters should be considered separately to make the right choice between an endodontically or periodontally compromised treated tooth and implant. Therefore, in order to decide whether the tooth is safe or not, data that have to be collected are specific parameters of both the patient and the clinician. In addition, the presence of periodontal, prosthetic, and orthodontic diseases requires patients to be set in multidisciplinary approach. The aim of this case report is to describe how the multidisciplinary approach could be the best way to manage difficult cases of implant-prosthetic rehabilitation.     Polydatin Induces Differentiation and Radiation Sensitivity in Human Osteosarcoma Cells and Parallel Secretion through Lipid Metabolite Secretion

    Osteosarcoma is a bone cancer characterized by the production of osteoid tissue and immature bone from mesenchymal cells. Osteosarcoma mainly affects long bones (femur is most frequently site) and occur in children and young adults with greater incidence. Here, we investigated the role accomplished by polydatin, a natural antioxidative compound, in promoting osteogenic differentiation alone or after radiation therapy on osteosarcoma cells. In vitro, polydatin significantly induced cell cycle arrest in S-phase and enhanced bone alkaline phosphatase activity. Moreover, the differentiation process was paralleled by the activation of Wnt-β-catenin pathway. In combination with radiotherapy, the pretreatment with polydatin promoted a radiosensitizing effect on osteosarcoma cancer cells as demonstrated by the upregulation of osteogenic markers and reduced clonogenic survival of tumor cells. Additionally, we analyzed, by mass spectrometry, the secretion of sphingolipid, ceramides, and their metabolites in osteosarcoma cells treated with polydatin. Overall, our results demonstrate that polydatin, through the secretion of sphingolipids and ceramide, induced osteogenic differentiation, alone and in the presence of ionizing therapy. Future investigations are needed to validate the use of polydatin in clinical practice as a potentiating agent of radiotherapy-induced anticancer effects

    Aim: The primary aim of this study were to: investigate value and possible advantages in terms of survival nanotite surface implants used in critical bone conditions such as the severely atrophic posterior maxilla simultaneously reconstruction with sinus floor elevation technique utilizing nonabsorbable membrane and anorganic bovine bone as only graft material, and examine the reliability of resonance frequency analysis measurements to assess and monitor implant stability also in such cases. Materials and Methods. Five consecutive patients presenting severely atrophic posterior maxillae were treated with 12 Nanotite surface implants in conjunction with sinus lifting using nonasorbable membrane and Bio-oss as only graft material. Resonance frequency analysis measurements was carried out at implant placement and four months and three years postoperatively. Results: At 3-year follow up the survival rate of nanotite surface implants was 100%. RFA mean value was 58.75 implant stability quotient at implant placement time and gradually increased during the examination time ending with an average ISQ of 75 at 3 year follow up. Conclusions: Althoug the present investigation is based on a limited sample, its results support the possible benefits of utilizing nanotite surface implants in case of implant placement and simultaneous reconstruction of the severely atrophic posterior maxilla through sinus elevation technique. Resonance frequence analysis system appears to be a reliable method to assess and monitor implant stability also in case case of severely atrophic posterior maxilla simultaneously grafted with anorganic bovine bone alone
