Fertility Preservation: A Key Survivorship Issue for Young Women with Cancer
Fertility preservation in the young cancer survivor is recognized as a key survivorship issue by the American Society of Clinical Oncology and the American Society of Reproductive Medicine. Thus, health care providers should inform women about the effects of cancer therapy on fertility and should discuss the different fertility preservation options available. It is also recommended to refer women expeditiously to a fertility specialist in order to improve counseling. Women’s age, diagnosis, presence of male partner, time available and preferences regarding use of donor sperm influence the selection of the appropriate fertility preservation option. Embryo and oocyte cryopreservation are the standard techniques used while ovarian tissue cryopreservation is new, yet promising. Despite the importance of fertility preservation for cancer survivors’ quality of life, there are still communication and financial barriers faced by women who wish to pursue fertility preservation
Paracoccidioidomicosis diseminada con insuficiencia suprarrenal: reporte de un caso de autopsia
RESUMEN
La paracoccidioidomicosis producida por el Paracoccidioides brasiliensis es una micosis sistémica que afecta principalmente los pulmones y puede comprometer mucosas, piel, sistema reticuloendotelial, hueso y glándulas suprarrenales. Esta enfermedad es una importante causa de insuficiencia suprarrenal en Brasil y en estudios posmortem se encuentra compromiso de estas hasta en el 80% de los casos.
Se presenta el caso de un campesino, con masa de seis centímetros de diámetro en ojo izquierdo, hospitalizado por alteración del estado de conciencia, deshidratación e hipotensión. Las pruebas de laboratorio indican alteración electrolítica. Recibe manejo con líquidos endovenosos sin mejoría y posteriormente fallece. El estudio de autopsia evidencia compromiso pulmonar, linfático y suprarrenal por Paracoccidioides brasiliensis.

SUMMARY
Paracoccidioides brasiliensis causes a systemic infection that primarily affects the lungs, but can also target mucous membranes, skin, reticuloendothelial system, bone, and adrenal glands. This disease constitutes an important cause of adrenal insufficiency in Brazil and studies posmortem show that the adrenal glands are compromised in as many as 80% of cases.
This is the case of a farmer who presented with a mass of 6 cms in diameter in his left eye. He was hospitalized for altered state of consciousness, dehydration, and hypotension. Laboratory tests indicated electrolytes disturbance. He received management with intravenous fluids without improvement and subsequently died. The autopsy evidenced lung, lymphatic, and adrenal compromise by Paracoccidioides brasiliensis.

Level of compliance in the implementation of the audit to improve the quality of care in health in the physiotherapy service of the IPS Fomesalud de Bucaramanga
La Auditoria es una herramienta de gestión de la calidad de atención en salud y es
un proceso obligatorio para las IPS pública y privadas en la búsqueda de la
calidad esperada; por lo tanto, es indispensable su implementación como parte de
un proceso de aprendizaje organizacional encaminado hacia la acreditación. El
modelo a utilizar fue definido por el Ministerio de la Protección Social en el Decreto
1011 de 2006 por medio del cual se creo el Sistema Obligatorio de Garantía de
Calidad, con un enfoque de mejoramiento continuo y atencion centrada en el
Usuario.
Con el propósito de diagnosticar los avances en la implementación del mismo, se
decidió realizar una auditoría externa al PAMEC de una institución privada y se
seleccionó la IPS FOMESALUD, mediante un estudio descriptivo y evaluativo de
corte transversal soportado en la normatividad, para lo cual se diseñó un plan de
auditoría que incluyó una lista de verificación y se aplicaron una serie de
entrevistas que permitieron determinar el cumplimiento de cada uno de los pasos
de la ruta crítica.
Se pudo detectar que la institución requiere reforzar sus acciones, ya que a pesar
de haber realizado la autoevaluación y detección de las oportunidades de mejora,
la priorización de las mismas no generó la información suficiente respecto a las
fallas de calidad, que permitiera centrar sus esfuerzos en proponer planes de
mejora de los procesos que impactan con más fuerza al usuario y determinar las
herramientas de medición y seguimiento adecuados, estructurando indicadores
para monitorear dichas acciones.
Por consiguiente la IPS debe involucrar al equipo de trabajo tanto asistencial como
administrativo para generar y mantener una cultura de autocontrol frente al
compromiso de implementar estos planes y así generar un aprendizaje
organizacional que logre estandarizar los procesos y mejorar la calidad de la

In order to diagnose the progress in its implementation,

adequate measurement and monitoring tools, structuring indicators

A Systematic Review of Reproductive Counseling in Cases of Parental Constitutional Reciprocal Translocation (9;22) Mimicking BCR-ABL1
We aim to determine the spectrum of cytogenetic abnormalities and outcomes in unbalanced offspring of asymptomatic constitutional balanced t(9;22) carriers through a systematic literature review. We also include a case of a constitutional balanced t(9;22) carrier from our institution. Among the 16 balanced t(9;22) carriers in our review, 13 were maternal and 3 were paternal. Of the 15 unbalanced translocation cases identified, 13 were live births, one was a missed abortion, and one resulted in pregnancy termination. The spectrum of established syndromes reported among the live births was the following: trisomy 9p syndrome (6/13), dual trisomy 9p and DiGeorge syndrome (3/13), dual 9q subtelomere deletion syndrome and DiGeorge syndrome (1/13), 9q subtelomere deletion syndrome (1/13), and DiGeorge syndrome (1/13). One unbalanced case did not have a reported syndrome. The phenotype of the unbalanced cases included cardiac abnormalities (5/13), neurological findings (7/13), intellectual disability (6/10), urogenital anomalies (3/13), respiratory or immune dysfunction (3/13), and facial or skeletal dysmorphias (13/13). Any constitutional balanced reciprocal t(9;22) carrier should be counseled regarding the increased risk of having a child with an unbalanced translocation, the spectrum of possible cytogenetic abnormalities, and predicted clinical phenotype for the unbalanced derivative
La economía solidaria como alternativa de fortalecimiento para la reconciliación y la paz, en el marco del desarrollo social e inclusivo en el postconflicto colombiano
El actual acuerdo de paz entre la Guerrilla de las FARC-EP y el Estado se constituye como una etapa histórica de construcción de paz y el restablecimiento de derechos fundamentales de los grupos (campesinos, afros, indígenas, sociedad civil) afectados por el conflicto interno armado colombiano desde la década de los sesenta hasta la actualidad. Es así como el presente estudio descriptivo parte de un Diseño metodológico de corte Fenomenológico Hermenéutico, Paradigma Interpretativo y Enfoque Cualitativo busco mediante el método hermenéutico analizar los aportes de la Economía Solidaria como alternativa de fortalecimiento de la reconciliación y la paz, en el marco del desarrollo social e inclusivo en el postconflicto colombiano. Los miomas parásitos son infrecuentes y de causa incierta. Surgen a partir de un mioma que se separa del útero y se adhiere a una superficie extrauterina en busca de un nuevo aporte vascular. Hace poco se sugirió que se forman a partir de fragmentos uterinos o de miomas que quedan en la cavidad abdomino-pélvica después de fragmentación por lo que se les llama “iatrogénicos”. Los cirujanos deben tener en cuenta la reciente aparición de esta complicación relacionada con el aumento en la frecuencia de los procedimientos laparoscópicos. 7 páginasRheumatoid arthritis (RA) is an inflammatory disease characterized by joint destruction, deformity, lower functionality, and decrease in life expectancy. Wingless signaling pathway (Wnt) has been recently involved in bone homeostasis. Studies suggest that overexpression of the pathway inhibitors, like the Dickkopf 1 protein (DKK1), has been implicated in bone destruction. The objective of this study is to compare circulating levels of DKK1 in different groups of patients with disease activity (remission, low, moderate, high activity,) and functionality status. Three hundred seventy-nine patients with RA were evaluated between March 2015 and November 2016. Disease activity was evaluated by disease activity score 28 with C-reactive protein (DAS28CPR), simplified and clinical disease activity scores (SDAI, CDAI), routine assessment of patient index data 3 (RAPID3), functional status using Multidimensional Health Assessment Questionnaire (MD-HAQ), and the Steinbrocker functional classification. DKK1 levels were measured by ELISA. The mean age was 60.7 ± 13.9 years. Disease duration was 13.2 ± 10.9 years. Higher levels of DKK1 were not associated with disease activity by CDAI (p = 0.70), SDAI (p = 0.84), DAS28CRP (p = 0.80), or RAPID3 (p = 0.70). Interestingly higher levels of DKK1 were significantly associated to lower functional status evaluating by the Steinbrocker classification (p = 0,013), severe disability by MD-HAQ (p = 0,004), and variables associated with joint destruction like osteoporosis, higher titles of rheumatoid factor, smoking, and increased hospital admissions related to RA. Higher levels of DKK1 were found in patients with lower functional status. This association was not found in patients with greater disease activity by CDAI, SDAI, DAS28, and RAPID3. This could be explained by more structural damage; DKK1 could be used as a biomarker of joint destruction in RA.La artritis reumatoide (AR) es una enfermedad inflamatoria caracterizada por destrucción articular, deformidad, menor funcionalidad y disminución de la esperanza de vida. La vía de señalización Wingless (Wnt) se ha implicado recientemente en la homeostasis ósea. Los estudios sugieren que la sobreexpresión de los inhibidores de la vía, como la proteína Dickkopf 1 (DKK1), se ha implicado en la destrucción ósea. El objetivo de este estudio es comparar los niveles circulantes de DKK1 en diferentes grupos de pacientes con actividad de la enfermedad (remisión, actividad baja, moderada, alta) y estado funcional. Trescientos setenta y nueve pacientes con AR fueron evaluados entre marzo de 2015 y noviembre de 2016. La actividad de la enfermedad se evaluó mediante la puntuación de actividad de la enfermedad 28 con proteína C reactiva (DAS28CPR), puntuaciones de actividad de la enfermedad clínica y simplificada (SDAI, CDAI), evaluación de rutina de datos del índice del paciente 3 (RAPID3), estado funcional utilizando el Cuestionario de evaluación de salud multidimensional (MD-HAQ) y la clasificación funcional de Steinbrocker. Los niveles de DKK1 se midieron por ELISA. La edad media fue de 60,7 ± 13,9 años. La duración de la enfermedad fue de 13,2 ± 10,9 años. Los niveles más altos de DKK1 no se asociaron con la actividad de la enfermedad por CDAI (p = 0,70), SDAI (p = 0,84), DAS28CRP (p = 0,80) o RAPID3 (p = 0,70). Curiosamente, niveles más altos de DKK1 se asociaron significativamente con un estado funcional más bajo evaluado por la clasificación de Steinbrocker (p = 0,013), discapacidad severa por MD-HAQ (p = 0,004), y variables asociadas con destrucción articular como osteoporosis, títulos más altos de factor reumatoide, el tabaquismo y el aumento de los ingresos hospitalarios relacionados con la AR. Se encontraron niveles más altos de DKK1 en pacientes con un estado funcional más bajo. Esta asociación no se encontró en pacientes con mayor actividad de la enfermedad por CDAI, SDAI, DAS28 y RAPID3. The burden of the most common rheumatic disease in Colombia
10 páginasBackground Estimating the burden of rheumatic diseases (RDs) requires proper evaluation of its lethal and nonlethal consequences. In Colombia, it is possible to find local data and Global Burden of Disease (GBD) reports that collect information from varied contexts and apply complex statistical models, but no on-site estimations are available. Methods This was a descriptive study on the burden of RD based on occurrence and mortality data in the general population during 2015, including information and prevalence estimations from the Community Oriented Program for the Control of Rheumatic Diseases (COPCORD) study. Disability-adjusted life years (DALYs) were estimated by combining measures of years of life lost (YLL) and years lived with disability (YLDs). For disability weight estimations among cases, different COPCORD responses were mapped using flowcharts to show the severity distribution according to GBD. All model parameters and results were validated through an expert consensus panel. Results Low back pain (LBP) was the RD with the greatest burden of disease, costing 606.05 (95% CI 502.76–716.58) DALYs per 100,000 inhabitants, followed by osteoarthritis (292.11; 95% CI 205.76–386.85) and rheumatoid arthritis (192.46, 95% CI 109.7–239.69). Conclusions The burden of RD is as high in Colombia as in other countries of the region. The results offer an interesting tool for optimizing healthcare system design as well as for planning the distribution of human and economic resources to achieve early diagnosis and adequate care of these diseases
