    A Real-Life Turkish Experience of Ruxolitinib in Polycythemia Vera

    Introduction:Ruxolitinib is a small -molecule inhibitor of the JAK1/2 pathway. This study aimed to reveal the results and side-effect profile of the use of ruxolitinib as a treatment option in polycythemia vera (PV).Methods:A total of 34 patients with PV from 18 different centers were included in the study. The evaluation of the response under treatment with ruxolitinib was determined as a reduction in spleen volume (splenomegaly size: ≥35%) by imaging and control of hematocrit levels (≤45%) compared to baseline.Results:While the number of patients in which a reduction in spleen volume and hematocrit control was achieved was 19 (55.9%) at 3 months of treatment, it was 21 (61.8%) at 6 months. Additionally, while the number of side effects was negatively correlated with the reduction in spleen volume (Spearman’s rho: -0.365, p=0.034), a decrease in the hematocrit level was positively correlated (Spearman’s rho: 0.75, p=0.029). Those without a reduction in spleen volume experienced more constipation (chi-square: 5.988, Fisher’s exact test: p=0.033).Conclusion:This study shed light on the use of ruxolitinib in PV and the importance of splenomegaly on studies planned with larger patient groups

    Firdevsî-İ Rûmî’s Suleyman-nâme 67th skin (review-critical text)

    In this master's thesis, 67th volume of Süleyman-nâme belonging to Firdevsî-i Rûmî was studied. Manzum and mixed civilized work Hz. It describes the life and men of Suleiman. Two copies of the Topkapı Palace Museum Treasure Library H.1535 and the Topkapı Palace Museum Treasury Library K.892 have been identified and a text has been created from these two journals. 81th volume of the Suleyman-name of Firdevsî-i Rûmî, one of the most productive writers of his time, is present today. A number of studies have been conducted on 16 of these 81 volumes. In the study titled "Firdevsi-i Rumi and Suleyman-name of 67th volume (Examination-Critical Text)" composed of three parts; In the first part, information about the life and works of Firdevsî-i Rûmî was given. In the second part, Suleyman-name's 67th volume was examined in terms of form and content. In the third part, the text was transcribed with the Arabic alphabet from Arabic alphabet to Latin alphabet. As a result of the work done, information was given about the sources of Süleyman-nâme.Bu yüksek lisans tezinde Firdevsî-i Rûmî’ye ait Süleymân-nâme’nin 67. cildi çalışılmıştır. Manzum ve mensur karışık olan eser Hz. Süleymân’ın hayatı ve mucizelerini anlatmaktadır. Topkapı Sarayı Müzesi Hazine Kitaplığı H.1535 ve Topkapı Sarayı Müzesi Hazine Kitaplığı K.892’de kayıtlı olan iki nüsha tespit edilmiş ve bu iki nüshadan hareketle tenkitli metin oluşturulmuştur. Yaşadığı dönemin en üretken yazarlarından olan Firdevsî-i Rûmî’nin Süleymân-nâme’sinin 81 cildinin yeri bugün belirlidir. Bu 81 ciltten 16 tanesi üzerine çeşitli çalışmalar yapılmıştır. Üç bölümden oluşan “Firdevsî-i Rûmî’nin Süleymân-nâme’sinin 67. Cildi (İnceleme-Tenkitli Metin)” adlı çalışmada; birinci bölümde Firdevsî-i Rûmî’nin hayatı ve eserleri hakkında bilgi verilmiş, ikinci bölümde Süleymân-nâme’nin 67. Cildi şekil ve muhteva açısından incelenmiş, üçüncü bölümde ise metin transkripsiyonlu bir şekilde Arap harfli Türk alfabesinden Latin harflerine aktarılmıştır. Yapılan çalışmanın sonucunda Süleymân-nâme’nin kaynakları hakkında bilgiler verilmiştir

    Combination of Opportunistic Infections in a Case of Chronic Inflammatory Demyelinating Polyradiculopathy

    Cytomegalovirus infections are asymptomatic when they appear in childhood and adolescence. However, cytomegalovirus causes severe infections in immuno-suppressed patients. The virus may lead to primary infections or it may result in secondary infections in the form of reactivation or reinfection when the immune system is suppressed since it may be latent in various parts of the body. Leishmaniasis is transmitted by phlebotomuses and caused by Leishmania, an intracellular protozoon. In visceral leishmaniasis, protozoons caught by macrophages are kept in the reticuloendothelial system. Patients develop hepatosplenomegaly, anemia, leucopenia, and thrombocytopenia due to bone marrow involvement. The disease has a poor outlook in immuno-suppressed individuals and patients receiving immuno-suppressive treatment. Leishmaniasis is endemic in Aegean and Mediterranean regions of Turkey, but sporadic in other regions. A rare case of cytomegolovirus infection accompanied by kala-azar development during treatment with steroids for chronic inflammatory demyelineating polyradiculopathy is reported

    Aim: The aim of this study was to determine how to eliminate the treatment uncertainties, and correct and prevent inappropriate treatment in patients with renal failure. Material and Methods: We retrospectively evaluated the biochemistry department’s records of 438 patients with creatinine values >1.5 mg/dL that were followed-up at our hospital for the last five years. Demographics, type of renal dysfunction, related risk factors (use of nephrotoxic agents, surgical procedures, comorbidity, etc.), dialysis treatment, complications, and clinical outcome of these patients were recorded and analyzed. Results: The most important result of the study is that the quality of the medical data recorded was very poor. The most common type of acute renal injury was prerenal acute renal injury. Among the cases, the most common etiological factors were dehydration and use of nephrotoxic agents. Surgical procedures and comorbid conditions facilitated the development of renal dysfunction, and all complications observed were more common and serious in the elderly patients. Among the patients with chronic renal injury, more of those with diabetic nephropathy required hospitalization. Interestingly, nosocomial infections were the most common cause of mortality in the patients hospitalized due to renal dysfunction. Conclusion: Only a few medical records were suitable for retrospective evaluation. We think that if the quality of hospital data collection/storage systems could be improved, the quality of research data obtained in such studies would likewise improve and these data will bring preventive and corrective approach to mortality and morbidity. © 2019, Duzce University Medical School. All rights reserved.Amaç: Bu çalışmanın amacı, böbrek hasarı olan hastalarda tedavi belirsizliklerinin nasıl giderileceğinin belirlenmesi ve tedavideki uygunsuzlukların düzeltilmesi ve önlenebilmesidir. Gereç ve Yöntemler: Kreatinin değeri >1,5 mg/dL olan ve hastanemizde son beş yılda takip edilen 438 hastanın biyokimya anabilim dalı kayıtları geriye dönük olarak değerlendirilmiştir. Hastaların demografik özellikleri, böbrek fonksiyon bozukluğu tipleri, ilişkili risk faktörleri (nefrotoksik ajanların kullanımı, cerrahi prosedürler, komorbidite, vb.), diyaliz tedavisi, komplikasyonlar ve klinik sonuçları kayıt altına alındı ve analiz edildi. Bulgular: Çalışmanın en önemli bulgusu, kaydedilmiş olan tıbbi verilerin kalitesinin çok düşük olmasıdır. En sık görülen akut böbrek hasarı tipi prerenal akut böbrek hasarı idi. Olgular arasında en yaygın etiyolojik faktörler dehidratasyon ve nefrotoksik ajanların kullanımı idi. Cerrahi işlemlerin ve eşlik eden hastalıkların böbrek fonksiyon bozukluğu gelişimini kolaylaştırdığı, gözlenen tüm komplikasyonların yaşlı hastalarda daha sık ve ciddi seyirli olduğu belirlendi. Kronik böbrek hasarı olan hastalar arasında, diyabetik nefropatisi olanların çoğunun hastaneye yatması gerekiyordu. İlginç olarak, böbrek fonksiyon bozukluğu ile izlenen yatan hastalarda en önemli mortalite nedeninin nozokomiyal infeksiyonlar olduğu belirlendi. Sonuç: Retrospektif değerlendirme için sadece birkaç tıbbi kayıt uygundu. Hastane veri toplama/depolama sistemlerinin kalitesinin iyileştirilebilmesi durumunda, bu tür çalışmalarda elde edilen araştırma verilerinin kalitesinin de aynı şekilde gelişeceğini ve bu verilerin mortalite ve morbidite konus

It is characterized by thrombocytopenia due to thrombocyte destruction mediated by auto-antibodies; however, cytotoxic and defective regulatory T-lymphocytes play an important role in its pathogenesis. While childhood ITP is usually acute, self-limiting and generally seasonal in nature, ITP in adults is usually chronic; its relation with seasons has not been studied. Aims: We investigated whether months and/or seasons have triggering roles in adults with ITP. Study Design: Descriptive study. Methods: A retrospective case review of adult patients with primary ITP diagnosed at various University Hospitals in cities where Mediterranean climate is seen was performed. Demographic data, date of referral and treatments were recorded. Corticosteroid-resistant, chronic and refractory cases were determined. Relation between sex, corticosteroid-resistant, chronic and refractory ITP with the seasons was also investigated. Results: The study included 165 patients (124 female, mean age=42.8 +/- 16.6). Most cases of primary ITP were diagnosed in the spring (p=0.015). Rates of patients diagnosed according to the seasons were as follows: 35.8% in spring, 23% in summer, 20.6% in fall, and 20.6% in winter. With respect to months, the majority of cases occurred in May (18.2%). Time of diagnosis according to the seasons did not differ between genders (p=0.699). First-line treatment was corticosteroids in 97.3%, but 35% of the cases were corticosteroid-resistant. Steroid-resistant patients were mostly diagnosed in the spring (52.1%) (p=0.001). ITP was chronic in 52.7% of the patients and they were also diagnosed mostly in the spring (62.7%) (p=0.149). Conclusion: This is the first study showing seasonal association of ITP in adults and we have observed that ITP in adults is mostly diagnosed in the spring. The reason why more patients are diagnosed in the spring may be due to the existence of atmospheric pollens reaching maximum levels in the spring in places where a Mediterranean climate is seen
