    Awareness and Knowledge Regarding Cervical Cancer Screening and its Association with Demographic Characteristics

     Objective: To assess awareness and knowledge regarding cervical cancer screening, Pap smear screening, HPV infection, vaccine and their association with demographic characteristics. Methodology: This is a questionnaire based cross sectional study with a sample size of 100 female patients. This study was conducted in (OB/GYN OPD) in tertiary care setting of Military Hospital and Combined Military Hospital, Rawalpindi. Duration of study was 1 month April 2018. Results: Only 44.4% of patients had heard about cervical cancer. Awareness regarding PAP smear and HPV vaccination was present in around 20% patients. The results of the study revealed that higher socio-economic status (p=0.048) and education (p=0.03) was linked to more awareness regarding cervical cancer. Educational status had a linear relationship. Conclusion: The lack of knowledge to cervical cancer occurs in higher socio economic class also. There is a need of more education for increased awareness in all patients to enhance uptake of preventive strategie

    Accuracy of Uterine Artery Doppler in Second Trimester in Determining Pre-Eclampsia

    Background: To determine the accuracy of uterine artery doppler in the second trimester (18-24 weeks) in determining preeclampsia.Methods: In this descriptive study, a total of 89 women with singleton pregnancy attending routine second-trimester anomaly scan were included. Uterine artery doppler sonography was conducted in the women coming for routine anomaly scan. Flow velocity waveforms of right and left uterine artery were imaged with the patient in the semi-recumbent position and the uterine artery was identified in the longitudinal scan, lateral to the uterus. Resistive index, pulsatility index and presence and absence of early diastolic notch and its depth in terms of notch index were calculated.Results: Uterine artery doppler finding in second trimester in determining preeclampsia showed abnormal findings in 41.57% women in which 24.32% had type I (Resistive index >0.58), 45.95% (17/37) type II (Mild notching of uterine artery at beginning of diastole) and 29.73% had type III [(Severe notching + abnormal Resistive index (>0.58)]. After the follow-up of each patient, we found that pre-eclampsia was developed in 28.09% (25/89) cases. The accuracy, sensitivity, specificity, positive and negative predictive values of uterine artery doppler were 75.28%, 80%, 73.44%, 54.05% and 90.38% respectivelyConclusion: By detecting abnormal uterine artery Doppler indices (high RI) between 18-24 weeks of pregnancy can identify those women who are at risk for development of preeclampsia

    Impact of Personality Trait of Emotional Stability on Oral Parafunctional Habits

    OBJECTIVES To find the impact of emotional stability on oral parafunctional habits. METHODOLOGY A Cross-sectional descriptive study was conducted at the College of Dentistry, Sharif Medical and Dental College, Lahore, over five months, from July to November 2021. Data was collected using a medical questionnaire and a ten-item personality inventory scale (TIPI). The medical questionnaire comprised two sections with 11 items. The first section had six demographic statements, including age, gender, marital status, occupation, educational level and medical condition. The second section consisted of a pre-validated parafunctional habits questionnaire. RESULTSThere was a non-significant difference in the scores of the personality trait of emotional stability across categories of oral parafunctional habits of nail-biting (p=0.093), tooth grinding (p=0.192), tooth clenching (p=0.055), biting on hard objects (p=0.17) and chewing gum (p=0.116). CONCLUSION Emotional stability was most prevalent in individuals who denied having the habit of nail-biting, teeth grinding, tooth clenching and biting hard objects but neither agreed nor disagreed with having the habit of chewing gum

    Diagnostic accuracy of strain ultrasound elastography in differentiating benign and malignant thyroid nodules, taking histopathology as gold standard

    Background: Elastography is a method recently being used in the evaluation of thyroid nodules by comparing tissue elasticity. Strain and shear wave elastography are two types of elastography still being used in clinical practice. Two kinds of elasticity can be assessed by strain elastography. First, colors around and within the nodules are evaluated and visually scored according to the 4-5 scale scoring systems. The aim of this study was to determine the diagnostic accuracy of strain ultrasound elastography in differentiation between benign and malignant thyroid nodules, taking histopathology as gold standardMethods: Total of 101 patients with palpable thyroid nodule of any size of age 30-70 years of either gender were included. Patients with previous thyroid surgery and already proven histopathology were excluded. Strain ultrasound elastography was performed in every patient by using a highresolution unit with a linear array probe centred at 7.5 MHz. Strain ultrasound elastography was performed in every patient by a consultant radiologist in the presence of researcher and benign or malignant thyroid nodules was noted. Strain USG elastography findings were compared with histopathology report.Results: All the patients were subjected to strain ultrasound elastography. USG supported the diagnosis of malignant thyroid nodules in all 52 patients. Histopathology confirmed malignant thyroid nodules in 47 (true positive) cases where as 5 (false positive) had no malignant lesion on histopathology. In USG negative patients, 46 were true negative while 3 were false negative. Overall sensitivity, specificity, positive predictive value, negative predictive value and diagnostic accuracy of strain ultrasound elastography in differentiation between benign and malignant thyroid nodules, taking histopathology as gold standard is 94.0%, 90.20%, 90.38%, 93.88% and 92.08% respectively.Conclusions: According to this study, strain ultrasound elastography is the non-invasive modality of choice with high diagnostic accuracy in diagnosing malignant thyroid nodules

    Impact of brand hate on consumer well-being for technology products through the lens of stimulus organism response approach

    Consumer well-being is a micromarketing concept that emphasizes on contributions of marketing activities in social welfare. The major objective of the current study is to analyze the impact of self-incongruence on brand dissatisfaction, brand hate, and consumer well-being. This study has utilized the Self-incongruity Theory and the Stimulus-Organism-Response model to test the impact of self-incongruity on anti-consumption and consumer voice behaviors, and subsequent effects on consumer well-being. Data were collected from young consumers of technology products from major cities of Pakistan. A total of 592 consumers answered a paper-and-pencil questionnaire using purposive sampling technique. The data were analyzed by partial least square structural equation modeling. The findings of this study reveal that functional and symbolic incongruity predict brand hate and dissatisfaction, which is positively related with brand retaliation. Brand retaliation is negatively related with consumer well-being. This study offers implications for product designers, marketers, advertisers and other stakeholders to improve congruence between what young consumers of technology products expect and what brands are offering to mitigate negative attitudes and behaviors and increase consumer well-being

    Impact of natural killer cells on outcomes after allogeneic hematopoietic stem cell transplantation: A systematic review and meta-analysis.

    Background: Natural killer (NK) cells play a vital role in early immune reconstitution following allogeneic hematopoietic stem cell transplantation (HSCT). Methods: A literature search was performed on PubMed, Cochrane, and Clinical trials.gov through April 20, 2022. We included 21 studies reporting data on the impact of NK cells on outcomes after HSCT. Data was extracted following the PRISMA guidelines. Pooled analysis was done using the meta-package (Schwarzer et al.). Proportions with 95% confidence intervals (CI) were computed. Results: We included 1785 patients from 21 studies investigating the impact of NK cell reconstitution post-HSCT (8 studies/1455 patients), stem cell graft NK cell content (4 studies/185 patients), therapeutic NK cell infusions post-HSCT (5 studies/74 patients), and pre-emptive/prophylactic NK cell infusions post-HSCT (4 studies/77 patients). Higher NK cell reconstitution was associated with a better 2-year overall survival (OS) (high: 77%, 95%CI 0.73-0.82 vs low: 55%, 95%CI 0.37-0.72; n=899), however, pooled analysis for relapse rate (RR) or graft versus host disease (GVHD) could not be performed due to insufficient data. Higher graft NK cell content demonstrated a trend towards a better pooled OS (high: 65.2%, 95%CI 0.47-0.81 vs low: 46.5%, 95%CI 0.24-0.70; n=157), lower RR (high: 16.9%, 95%CI 0.10-0.25 vs low: 33%, 95%CI 0.04-0.72; n=157), and lower acute GVHD incidence (high: 27.6%, 95%CI 0.20-0.36 vs low: 49.7%, 95%CI 0.26-0.74; n=157). Therapeutic NK or cytokine-induced killer (CIK) cell infusions for hematologic relapse post-HSCT reported an overall response rate (ORR) and complete response (CR) of 48.9% and 11% with CIK cell infusions and 82.8% and 44.8% with NK cell infusions, respectively. RR, acute GVHD, and chronic GVHD were observed in 55.6% and 51.7%, 34.5% and 20%, and 20.7% and 11.1% of patients with CIK and NK cell infusions, respectively. Pre-emptive donor-derived NK cell infusions to prevent relapse post-HSCT had promising outcomes with 1-year OS of 69%, CR rate of 42%, ORR of 77%, RR of 28%, and acute and chronic GVHD rates of 24.9% and 3.7%, respectively. Conclusion: NK cells have a favorable impact on outcomes after HSCT. The optimal use of NK cell infusions post-HSCT may be in a pre-emptive fashion to prevent disease relapse

    Proteome level analysis of drug-resistant Prevotella melaninogenica for the identification of novel therapeutic candidates

    The management of infectious diseases has become more critical due to the development of novel pathogenic strains with enhanced resistance. Prevotella melaninogenica, a gram-negative bacterium, was found to be involved in various infections of the respiratory tract, aerodigestive tract, and gastrointestinal tract. The need to explore novel drug and vaccine targets against this pathogen was triggered by the emergence of antimicrobial resistance against reported antibiotics to combat P. melaninogenica infections. The study involves core genes acquired from 14 complete P. melaninogenica strain genome sequences, where promiscuous drug and vaccine candidates were explored by state-of-the-art subtractive proteomics and reverse vaccinology approaches. A stringent bioinformatics analysis enlisted 18 targets as novel, essential, and non-homologous to humans and having druggability potential. Moreover, the extracellular and outer membrane proteins were subjected to antigenicity, allergenicity, and physicochemical analysis for the identification of the candidate proteins to design multi-epitope vaccines. Two candidate proteins (ADK95685.1 and ADK97014.1) were selected as the best target for the designing of a vaccine construct. Lead B- and T-cell overlapped epitopes were joined to generate potential chimeric vaccine constructs in combination with adjuvants and linkers. Finally, a prioritized vaccine construct was found to have stable interactions with the human immune cell receptors as confirmed by molecular docking and MD simulation studies. The vaccine construct was found to have cloning and expression ability in the bacterial cloning system. Immune simulation ensured the elicitation of significant immune responses against the designed vaccine. In conclusion, our study reported novel drug and vaccine targets and designed a multi-epitope vaccine against the P. melaninogenica infection. Further experimental validation will help open new avenues in the treatment of this multi-drug-resistant pathogen
