Laparoscopic Nephrectomy in Pediatric age
Objectivos: Numerosas séries têm sido publicadas documentando a experiência em nefrectomias por via transperitoneal e retroperitoneal. Este artigo pretende documentar as primeiras 10 nefrectomias
e nefroureterectomias laparoscópicas via transperitoneal realizadas no Hospital D. Estefânia.
Material e Métodos: Revisão retrospectiva de cinco nefrectomias e cinco nefroureterectomias laparoscópicas
realizadas no período de Fevereiro a Setembro
de 2010 em crianças com idades compreendidas
entre os 2 e os 18 anos. Todos os doentes
foram referenciados por complicações de rins não funcionantes. Os procedimentos, via transperitoneal, foram unilaterais, com recurso a 3 portas. Foi igualmente efectuada a análise da duração da cirurgia,
taxa de conversão, tempo de internamento
e complicações associadas à técnica.
Resultados: Todos os rins foram removidos com
sucesso por via laparoscópica, sem necessidade de conversão em nenhum dos casos. O tempo operatório médio foi de 103 minutos. Cinco doentes tiveram alta ao 2º dia de pós-operatório, três doentes ao 3º dia
de pós-operatório e dois doentes, pela complexidade médico-cirúrgica inerente tiveram internamentos mais prolongados. Não se verificaram complicaçõesno per- ou pós-operatório, com retoma precoce da actividade normal em todos os doentes.
Conclusão: A nefrectomia laparoscópica na criança parece ser uma alternativa viável e segura à cirurgia aberta. Consideramos necessário a aquisição de mais experiência para reduzir os tempos operatórios e definir a sua verdadeira eficácia. A experiência
adquirida com outros procedimentos laparoscópicos transperitoneais foi determinante na opção por esta via (versus retroperitoneal)
Karyotypic conservatism in samples of Characidium cf. zebra (Teleostei, Characiformes, Crenuchidae): Physical mapping of ribosomal genes and natural triploidy
Basic and molecular cytogenetic analyses were performed in specimens of Characidium cf. zebra from five collection sites located throughout the Tietê, Paranapanema and Paraguay river basins. The diploid number in specimens from all samples was 2n = 50 with a karyotype composed of 32 metacentric and 18 submetacentric chromosomes in both males and females. Constitutive heterochromatin was present at the centromeric regions of all chromosomes and pair 23, had additional interstitial heterochromatic blocks on its long arms. The nucleolar organizer regions (NORs) were located on the long arms of pair 23, while the 5S rDNA sites were detected in different chromosomes among the studied samples. One specimen from the Alambari river was a natural triploid and had two extra chromosomes, resulting in 2n = 77. The remarkable karyotypic similarity among the specimens of C. cf. zebra suggests a close evolutionary relationship. On the other hand, the distinct patterns of 5S rDNA distribution may be the result of gene flow constraints during their evolutionary history
Prevalence and determinants of child undernutrition and stunting in semiarid region of Brazil
OBJECTIVE : To analyze the evolution in the prevalence and determinants of malnutrition in children in the semiarid region of Brazil. METHODS : Data were collected from two cross-sectional population-based household surveys that used the same methodology. Clustering sampling was used to collect data from 8,000 families in Ceará, Northeastern Brazil, for the years 1987 and 2007. Acute undernutrition was calculated as weight/age < -2 standard deviation (SD); stunting as height/age < -2 SD; wasting as weight/height < -2 SD. Data on biological and sociodemographic determinants were analyzed using hierarchical multivariate analyses based on a theoretical model. RESULTS : A sample of 4,513 and 1,533 children under three years of age, in 1987 and 2007, respectively, were included in the analyses. The prevalence of acute malnutrition was reduced by 60.0%, from 12.6% in 1987 to 4.7% in 2007, while prevalence of stunting was reduced by 50.0%, from 27.0% in 1987 to 13.0% in 2007. Prevalence of wasting changed little in the period. In 1987, socioeconomic and biological characteristics (family income, mother’s education, toilet and tap water availability, children’s medical consultation and hospitalization, age, sex and birth weight) were significantly associated with undernutrition, stunting and wasting. In 2007, the determinants of malnutrition were restricted to biological characteristics (age, sex and birth weight). Only one socioeconomic characteristic, toilet availability, remained associated with stunting. CONCLUSIONS : Socioeconomic development, along with health interventions, may have contributed to improvements in children’s nutritional status. Birth weight, especially extremely low weight (< 1,500 g), appears as the most important risk factor for early childhood malnutrition
