Implantat-assoziierte Infektionen bei langstreckigen Instrumentalisierungen der Wirbelsäule – eine retrospektive Analyse von 46 Patienten

Diese retrospektive unizentrische Studie untersucht implantatassoziierte Infektionen bei langstreckigen Spondylodesen in Bezug auf die vorhandenen Risikofaktoren, das Erregerspektrum und die erfolgte Therapie. Dabei fand eine Unterscheidung der Fälle bezüglich des Implantaterhalts (Gruppe 1) und der Implantatentfernung (Gruppe 2) statt. Die Ergebnisse der Studie sollen eine Therapieentscheidung unterstützen. Wir konnten zeigen, dass der Zeitpunkt der Infektion in Bezug auf die Indexoperation entscheidend für die weitere Behandlung ist. Handelt es sich um eine Spätinfektion (>90 Tage nach Indexoperation), ist eine Implantatentfernung signifikant öfter notwendig geworden als bei frühzeitigen Infektionen (Gruppe 1: 18 Patienten, Gruppe 2: 14 Patienten, p= 0,031). Im Fall der Spätinfektionen war eine Kontrolle der Infektion vermehrt lediglich durch die Materialentfernung zu erreichen

Optimization, Evidence-Based Improvement, Datafication: Systematic Review on the Relation Between Data and School Development in the International Discourse

Zahlen, Daten und Algorithmen optimieren Lehren und Lernen zunehmend. In der Schule werden sie u. a. genutzt, um Gestaltungs- und Steuerungsanforderungen an Schulentwicklungsprozessen zu unterstützen. Dabei kommt ein kritischer Blick auf das Verhältnis von Optimierung, Evidenzbasierung und Datafizierung vielfach zu kurz. Ziel des Beitrages ist es daher, dieses Verhältnis auf Basis eines systematischen Reviews aus Sicht kritischer Medienpädagogik in den Blick zu nehmen und für den Diskurs aufzuarbeiten. Verfolgt wird die Fragestellung: Wie gestaltet sich das Verhältnis von Daten und Schulentwicklung im internationalen Diskurs? Die Ergebnisse zeigen, dass Schwerpunktsetzungen auf evidenzbasierter Schulentwicklung liegen. Leerstellen werden in Auseinandersetzungen mit technisch erzeugten, unsystematisch erhobenen Daten gesehen. Während meist die ‹bestmögliche› Nutzung digitaler Daten fokussiert wird, werden tiefgreifende Transformationsprozesse von Schule und Schulentwicklung wenig thematisiert.Numbers, data and algorithms are increasingly optimizing teaching and learning. In schools, they are used for example to support design and control requirements in the light of school improvement. In doing so, a critical view on the relation between optimization, evidence-based improvement and datafication comes up short. Therefore, this paper aims to examine this relation based on a systematic review and to make it fruitful for the discourse within critical media education. We focus on the following question: How could the relation between data and school development in an international discourse be described? The results show that the focus is on evidence-based school improvement respectively school development. But we can see gaps regarding the ongoing discussion of technically generated, unsystematically collected data. While publications somehow describe the ‘best possible’ use of digital data, deep transformation processes of schools and school development are still rarely addressed

Prerogative of interpretation: Digitisation and education in programmes and funding guidelines of Germany and the EU

Der vorliegende Beitrag befasst sich mit diskurs- und machtpolitisch hervorgerufenen Deutungshoheiten in Bezug auf Forschung zu und über Digitalisierung und Bildung in Programmatiken und Förderrichtlinien Deutschlands und der EU. Er gründet auf der Annahme, dass Forschungs- und Wissenschaftssysteme seit geraumer Zeit in enger Abhängigkeit zu Förderprogrammen stehen, welche auf unterschiedlichen politischen Ebenen als so genannte Förderinstrumente diverser Bildungsbereiche und der Wissenschaft selbst fungieren (sollen). Bei empirischer Betrachtung werden Deutungshoheiten infolge dieser Hinwendung zu und gleichzeitigen Abwendung von Begriffen und Konzepten sichtbar – nicht nur im Zusammenhang mit Diskursen um Digitalisierung und Bildung. Folge sind die (Über-)Betonung einzelner Konzepte und die zurückhaltende Repräsentation anderer, was für die Medienpädagogik qualifikatorische Engfassungen bedeutet, die auf empirischer Basis sowie vor dem Hintergrund präsenter Konzepte zu zeigen sind.This article is concerned with the prerogative of interpretation in relation to digitisation and education in the programmes and funding guidelines of Germany and the EU that has been created by political discourse and power. It is based on the assumption that the research and science system have for some time been closely dependent on funding programmes, which (should) function at different political levels as so-called funding instruments of various educational sectors and of science itself. On empirical observation, this turning towards and simultaneous rejection of terms and concepts makes prerogative of interpretation visible - not only in connection with discourses on digitisation and education. The consequence is the (over-)emphasis on individual concepts and the restrained representation of others, which means qualification narrowness for media education, which must be shown on an empirical basis and against the background of present concepts

Three of the EETs (i.e., 8,9-, 11,12- and 14,15-EET) are rapidly hydrolysed by the enzyme soluble epoxide hydrolase (sEH). Here, we investigated the role of sEH in nociceptive processing during peripheral inflammation. RESULTS: In dorsal root ganglia (DRG), we found that sEH is expressed in medium and large diameter neurofilament 200-positive neurons. Isolated DRG-neurons from sEH(-/-) mice showed higher EET and lower DHET levels. Upon AA stimulation, the largest changes in EET levels occurred in culture media, indicating both that cell associated EET concentrations quickly reach saturation and EET-hydrolyzing activity mostly effects extracellular EET signaling. In vivo, DRGs from sEH-deficient mice exhibited elevated 8,9-, 11,12- and 14,15-EET-levels. Interestingly, EET levels did not increase at the site of zymosan-induced inflammation. Cellular imaging experiments revealed direct calcium flux responses to 8,9-EET in a subpopulation of nociceptors. In addition, 8,9-EET sensitized AITC-induced calcium increases in DRG neurons and AITC-induced calcitonin gene related peptide (CGRP) release from sciatic nerve axons, indicating that 8,9-EET sensitizes TRPA1-expressing neurons, which are known to contribute to mechanical hyperalgesia. Supporting this, sEH(-/-) mice showed increased nociceptive responses to mechanical stimulation during zymosan-induced inflammation and 8,9-EET injection reduced mechanical thresholds in naive mice. CONCLUSION: Our results show that the sEH can regulate mechanical hyperalgesia during inflammation by inactivating 8,9-EET, which sensitizes TRPA1-expressing nociceptors. Therefore we suggest that influencing the CYP450 pathway, which is actually highly considered to treat cardiovascular diseases, may cause pain side effects.Peer Reviewe
