    Resistance-Associated Substitutions/Variants Correlate to Therapeutic Outcomes of Novel Direct-Acting Antivirals in Different HCV Genotype Treated Individuals

    The expanded classification of hepatitis C virus (HCV) genome into various genotypes and numerous subtypes significantly correlates to therapeutic outcomes of interferon-free direct-acting antivirals (DAAs) in HCV treated patients. In particular, genotypes 3 and 4 are still harder to treat, and higher sustained virologic response (SVR) rates are not achieved in some difficult-to-treat specific populations (i.e., HCV subtype 1a patients, compensated and decompensated cirrhotic patients, HCV/HIV co-infection, and prior treatment failure with pegylated interferon plus ribavirin and first-generation protease inhibitor based therapeutic regimens). Furthermore, the pre-existing and treatment-emergent resistance associated substitutions (RAS) at specific amino acid positions within the viral quasispecies may increase the chances of viral breakthrough (HCV RNA remains lower limit of quantification, but increased to 100 IU/mL or 1.log10 during DAAs therapy), viral relapse (undetectable viral load at the end of treatment but positive within the follow-up of 6 months), and discontinuation of therapy in treated individuals. Although the clinical importance of RAS is not entirely elucidated, it is believed that such substitutions decrease the therapeutic efficacy of DAAs in treated individuals. Similarly, the emergence of multiclass hepatitis C virus resistance to interferon-free DAAs failure in real-world experiences demands eagerly tailored second-line anti-hepatitis C therapies.     Recent Advances in Angiogenesis Assessment Methods and their Clinical Applications

    Angiogenesis, a natural phenomenon of developing new blood vessels, is an integral part of normal developmental processes as well as numerous pathological states in humans. The angiogenic assays are reliable predictors of certain pathologies in particular tumor growth, metastasis, inflammation, wound healing, tissue regeneration, ischemia, cardiovascular, and ocular diseases. The angiogenic inducer and inhibitor studies rely on both in vivo and in vitro angiogenesis methods, and various animal models are also standardized to assess qualitative and quantitative angiogenesis. Analogously, the discovery and development of anti-angiogenic agents are also based on the choice of suitable angiogenic assays and potential drug targeted sites within the angiogenic process. Similarly, the selection of cell types and compatible experimental conditions resembling the angiogenic disease being studied are also potential challenging tasks in recent angiogenesis studies. The imaging analysis systems for data acquisition from in vivo, in vitro, and in ova angiogenesis assay to preclinic, and clinical research also requires novel but easy-to-use tools and well-established protocols.     IoT-Based Remote Health Monitoring System Employing Smart Sensors for Asthma Patients during COVID-19 Pandemic

    COVID19 and asthma are respiratory diseases that can be life threatening in uncontrolled circumstances and require continuous monitoring. A poverty stricken South Asian country like Bangladesh has been bearing the brunt of the COVID19 pandemic since its beginning. The majority of the country's population resides in rural areas, where proper healthcare is difficult to access. This emphasizes the necessity of telemedicine, implementing the concept of the Internet of Things (IoT), which is still under development in Bangladesh. This paper demonstrates how the current challenges in the healthcare system are resolvable through the design of a remote health and environment monitoring system, specifically for asthma patients who are at an increased risk of COVID19. Since on-time treatment is essential, this system will allow doctors and medical staff to receive patient information in real time and deliver their services immediately to the patient regardless of their location. The proposed system consists of various sensors collecting heart rate, body temperature, ambient temperature, humidity, and air quality data and processing them through the Arduino Microcontroller. It is integrated with a mobile application. All this data is sent to the mobile application via a Bluetooth module and updated every few seconds so that the medical staff can instantly track patients' conditions and emergencies. The developed prototype is portable and easily usable by anyone. The system has been applied to five people of different ages and medical histories over a particular period. Upon analyzing all their data, it became clear which participants were particularly vulnerable to health deterioration and needed constant observation.     More than 70% of our planet is covered by extremely cold environments, nourishing a broad diversity of microbial life. Temperature is the most significant parameter that plays a key role in the distribution of microorganisms on our planet. Psychrophilic microorganisms are the most prominent inhabitants of the cold ecosystems, and they possess potential cold-active enzymes with diverse uses in the research and commercial sectors. Psychrophiles are modified to nurture, replicate, and retain their active metabolic activities in low temperatures. Their enzymes possess characteristics of maximal activity at low to adequate temperatures; this feature makes them more appealing and attractive in biotechnology. The high enzymatic activity of psychrozymes at low temperatures implies an important feature for energy saving. These enzymes have proven more advantageous than their mesophilic and thermophilic counterparts. Therefore, it is very important to explore the efficiency and utility of different psychrozymes in food processing, pharmaceuticals, brewing, bioremediation, and molecular biology. In this review, we focused on the properties of cold-active enzymes and their diverse uses in different industries and research areas.     Design and study of a miniaturized millimeter wave array antenna for wireless body area network

    A miniaturized millimeter wave (mmWave) antenna for wireless body area networks is proposed in this paper. The antenna is found to be operational in the V-band, around the 60 GHz frequency range, with high efficiency of up to 99.98% in free space simulations. A multilayer, thin substrate was implemented in the design to enhance radiation efficiency and gain. The antenna seems to be most suitable for small electronic devices and wireless body area network (WBAN) applications because of its low profile and lighter weight concept. To enhance its performance, several arrays of different orders were created. The Parallel-Fed and Tapered Feed Line methods were followed to design the planar arrays with 1 Ă— 2, 1 Ă— 4, and 2 Ă— 2 elements in the primary design. Free space results were compared, and a 2 Ă— 2 element array was found to be the most balanced according to the simulations. To justify the eligibility of these designs for WBAN applications, a virtual human body model was created within the 3D computer-simulated environment and the simulations were repeated, where four equal-spaced distances were taken into account to identify the antenna and its array behavior more accurately. Simulations returned optimistic results for the 2 Ă— 2 element planar array arrangement in almost all parameters, even when placed close to the human body at any distance greater than 2 mm

    Background: Obsessive-Compulsive Disorder (OCD) is a debilitating condition marked by the presence of intrusive obsessions and repetitive compulsions. The primary care setting often serves as the first line of contact for individuals grappling with mental health issues, making it a crucial frontier in the early detection and management of OCD. Therefore, the accurate diagnosis of OCD in such settings is essential for effective management. Objective: This review article aims to provide a comprehensive overview of the diagnostic process for OCD, emphasizing the clinical presentation, differential diagnosis, and various diagnostic tools available. Additionally, it explores current strategies for managing OCD, including pharmacological and psychotherapeutic interventions. Methodology: For this review, a comprehensive literature search was conducted using Google Scholar and PubMed databases. Keywords such as "Diagnosis," "obsessive compulsive disorder," and "management" were employed to narrow down relevant studies. Both qualitative and quantitative research papers were included, while non-English publications and those lacking peer-review were excluded. Results: Core symptoms of OCD include obsessions and compulsions, with the Y-BOCS being a standard measure for diagnosis. Differential diagnosis is essential to distinguish OCD from other conditions. SSRIs have been recognized as first-line pharmacological treatments. CBT, particularly Exposure and Response Prevention, remains a potent psychotherapeutic intervention. Emerging treatments like DBS and TMS offer hope for those unresponsive to conventional treatments. Combination therapies have shown enhanced efficacy in certain cases. Conclusion: The meticulous diagnosis of OCD requires recognizing its core symptoms, ruling out other conditions, and leveraging validated clinical tools. A multi-faceted management approach combining pharmacological and psychological treatments ensures optimal patient outcomes, with ongoing research introducing promising new interventions

The purpose of our study was to evaluate the particular identification, management, encountering, and referral practices related to dermatological illnesses in family care. We also looked into the challenges and opportunities that family doctors experience in family medicine and saw a few of the paediatric dermatological diseases that family doctors may encounter.  Finding areas of weakness in the clinical therapy of certain dermatological disorders, however, will be aided by assessing how family doctors treat particular ailments.
