    Relationship of global assessment of change to AUSCAN and pinch and grip strength among individuals with hand osteoarthritis

    SummaryObjectiveThis study assessed the utility and construct validity of a new patient global assessment of symptom change for hand osteoarthritis (OA) by examining its associations with change over time in grip strength, pinch strength, and AUStralian CANadian Osteoarthritis Hand Index (AUSCAN).MethodsParticipants (N=531, 80% female, mean age=68) were part of a study on the Genetics of Generalized Osteoarthritis (GOGO) and completed two assessments (average 4 years apart). At the second assessment, participants described change in their right and left hand pain, aching, and stiffness on a 15-point scale with descriptors ranging from “Great deal worse” to “Great deal better”. Linear regression models examined associations of global change scores with changes in hand strength and AUSCAN, controlling for age, gender, number of hand joints with OA, and time between assessments.ResultsBoth right and left hand global assessment of change scores were significantly associated with change in AUSCAN, grip strength, and right hand pinch strength (P<0.05), and approached significance for left hand pinch strength (P=0.06). The strongest associations were between global change scores and AUSCAN change (right hand: β=0.29, P<0.001; left hand: β=0.27, P<0.001). Associations of change scores with grip and pinch strength were stronger among participants with greater radiographic OA severity at baseline.ConclusionResults support the validity of this new global assessment of symptom change. This measure is particularly useful for assessing change over time when no baseline data are available. Additional research should examine this measure's responsiveness in the context of clinical trials

    Making sense of the Sense Model: translation priming with Japanese-English bilinguals

    Many studies have reported that first language (L1) translation primes speed responses to second language (L2) targets, whereas L2 translation primes generally do not speed up responses to L1 targets in lexical decision. According to the Sense Model (Finkbeiner, Forster, Nicol & Nakamura, 2004) this asymmetry is due to the proportion of senses activated by the prime. Because L2 primes activate only a subset of the L1 translations senses, priming is not observed. In this study we test the predictions of the Sense Model by using Japanese-English cognates, which allow us to manipulate the number of senses that words have in each language. Contrary to the predictions of the Sense Model, our results replicated the typical asymmetrical priming effects, suggesting that it is not the total activation of senses that drives the priming effect. Rather the results are more in line with theories that postulate slower, and thus ineffective, activation of semantics by L2 primes

    Gravitational Waves in Open de Sitter Space

    We compute the spectrum of primordial gravitational wave perturbations in open de Sitter spacetime. The background spacetime is taken to be the continuation of an O(5) symmetric instanton saddle point of the Euclidean no boundary path integral. The two-point tensor fluctuations are computed directly from the Euclidean path integral. The Euclidean correlator is then analytically continued into the Lorentzian region where it describes the quantum mechanical vacuum fluctuations of the graviton field. Unlike the results of earlier work, the correlator is shown to be unique and well behaved in the infrared. We show that the infrared divergence found in previous calculations is due to the contribution of a discrete gauge mode inadvertently included in the spectrum.Comment: 17 pages, compressed and RevTex file, including one postscript figure fil

    Environmental Impact Assessment: Detecting Changes in Fish Community Structure in Response to Disturbance with an Asymmetric Multivariate BACI Sampling Design

    One of the primary challenges to detecting anthropogenic environmental impacts is the high degree of spatial and temporal variability inherent in natural systems. Planned or routine events that result in disturbance to populations and communities provide an opportunity for scientists to apply well-replicated and statistically powerful sampling designs to assess subsequent biological effects. For example, a thick layer of sessile invertebrates is the prominent biotic feature of intertidal and shallow subtidal portions of offshore petroleum platforms in southern California. Given the central role of such invertebrates in providing food and shelter, their presence can reasonably be expected to influence associated fish community structure. At one platform on the San Pedro Shelf, invertebrate biomass was completely removed from support pilings and horizontal crossmembers to a depth of 20 m with high-pressure water during a standard “hydrocleaning” event in November 2007. Three nearby platforms remained undisturbed, providing a unique opportunity to test for disturbance-related changes in the local fish assemblage and the overall time course of community recovery. The potential impact of the abrupt and intense removal of the invertebrate layer was assessed with survey data collected periodically for one year prior- and one year post-hydrocleaning in a modified Before-After-Control-Impact (BACI) design. Asymmetrical multivariate analyses of variance revealed a significant effect of disturbance to fish, driven largely by reductions in the abundance of numerically dominant blacksmith (Chromis punctipinnis). Nevertheless, the system was surprisingly resilient, recovering to pre-disturbance conditions within ten months. Our results demonstrate that a well-replicated BACI sampling design can detect even subtle biological changes in response to disturbance, a key step towards developing a mechanistic understanding of community disassembly in the face of increasingly frequent and intense perturbations

    Cosmological constraints on the generalized holographic dark energy

    We use the Markov ChainMonte Carlo method to investigate global constraints on the generalized holographic (GH) dark energy with flat and non-flat universe from the current observed data: the Union2 dataset of type supernovae Ia (SNIa), high-redshift Gamma-Ray Bursts (GRBs), the observational Hubble data (OHD), the cluster X-ray gas mass fraction, the baryon acoustic oscillation (BAO), and the cosmic microwave background (CMB) data. The most stringent constraints on the GH model parameter are obtained. In addition, it is found that the equation of state for this generalized holographic dark energy can cross over the phantom boundary wde =-1.Comment: 14 pages, 5 figures. arXiv admin note: significant text overlap with arXiv:1105.186

    ELECTROSTATIC BODY-MOTION REGISTRATION AND THE HUMAN ANTENNA-RECEIVER EFFECT: A NEW METHOD FOR INVESTIGATING INTERPERSONAL DYNAMICAL ENERGY SYSTEM INTERACTIONS

    This paper documents that it is possible to measure electromagnetic fields created by physical movements of the human body-termed electrostatic body-motion effects-using readily available EEG amplifiers, and that it possible to measure the human body's capability to serve as an antenna and/or receiver for these electrostatic movements-termed the human antenna-receiver effect. Following the observation by Green et al (1991)1 that small body-motions could be detected by electrometers attached to copper walls, three experiments were conducted measuring the effects of hand-motions and foot-motions using DC amplifiers (the Synamps System by Neuroscan). Clear hand-motion and foot-motion effects could be recorded using a standard electrode box as an antenna. The electrostatic motion effect was attenuated as a function of distance of the motions from the electrode box, and by placing a wire mesh shield over the electrode box. The human body was discovered to funcrion as a strong antenna and/or receiver for electrostatic body-motions. The findings indicate that electrostatic body-motions and the human antenna-receiver effect are easily measurable, and may serve as a new method for investigating interpersonal dynamic energy system interactions in psychology, medicine and healing

    Relationship of limb length inequality with radiographic knee and hip osteoarthritis

    SummaryObjectiveThis study examined the relationship of limb length inequality (LLI) with radiographic hip and knee osteoarthritis (OA) in a large, community-based sample.MethodsThe total study group comprised 926 participants with radiographic knee OA, 796 with radiographic hip OA, and 210 (6.6%) with LLI ≥2cm. The presence of radiographic OA was defined as Kellgren/Lawrence (K/L) grade ≥2. Multiple logistic regression models were used to examine the relationship of LLI with hip and knee OA, while controlling for age, gender, race, body mass index, and history of hip or knee problems (joint injury, fracture, surgery, or congenital anomalies).ResultsIn unadjusted analyses, participants with LLI were more likely than those without LLI to have radiographic knee OA (45.1% vs 28.3%, P<0.001) and radiographic hip OA (35.2% vs 28.7%, P=0.063). In multiple logistic regression models, knee OA was significantly associated with presence of LLI (adjusted Odds Ratio [aOR]=1.80, 95% Confidence Interval [95% CI] 1.29–2.52), but there was no significant relationship between hip OA and LLI (aOR=1.20, 95% CI 0.86–1.67). Among participants with LLI, right hip OA was more common when the contralateral limb was longer than when the ipsilateral limb was longer (30.3% vs 17.5%, P=0.070).ConclusionLLI was associated with radiographic knee OA, controlling for other important variables. Future research should examine the relationship of LLI with hip or knee OA incidence, progression, and symptom severity, as well as the efficacy for LLI corrective treatments in OA

    Exploring the effects of demonstration and enactment in facilitating recall of instructions in working memory

    Across the lifespan the ability to follow instructions is essential for the successful completion of a multitude of daily activities. This ability will often rely on the storage and processing of information in working memory, and previous research in this domain has found that self-enactment at encoding or observing other-enactment at encoding (demonstration) improves performance at recall. However, no working memory research has directly compared these manipulations. Experiment 1 explored the effects of both self-enactment and demonstration on young adults’ (N=48) recall of action-object instruction sequences (e.g. ‘spin the circle, tap the square’). Both manipulations improved recall, with demonstration providing relatively larger boosts to performance across conditions. More detailed analyses suggested that this improvement was driven by improving the representations of actions, rather than objects, in these action-object sequences. Experiment 2 (N=24) explored this further, removing the objects from the physical environment and comparing partial demonstration (i.e. action-only or object-only) with no or full demonstration. The results showed that partial demonstration only benefitted features that were demonstrated, while full demonstration improved memory for actions, objects and their pairings. Overall these experiments indicate how self-enactment, and particularly demonstration, can benefit verbal recall of instruction sequences through the engagement of visuo-motor processes that provide additional forms of coding to support working memory performance.</p

    Screening of mental health and substance users in frequent users of a general Swiss emergency department

    BACKGROUND: The objectives of this study were to determine the proportions of psychiatric and substance use disorders suffered by emergency departments' (EDs') frequent users compared to the mainstream ED population, to evaluate how effectively these disorders were diagnosed in both groups of patients by ED physicians, and to determine if these disorders were predictive of a frequent use of ED services. METHODS: This study is a cross-sectional study with concurrent and retrospective data collection. Between November 2009 and June 2010, patients' mental health and substance use disorders were identified prospectively in face-to-face research interviews using a screening questionnaire (i.e. researcher screening). These data were compared to the data obtained from a retrospective medical chart review performed in August 2011, searching for mental health and substance use disorders diagnosed by ED physicians and recorded in the patients' ED medical files (i.e. ED physician diagnosis). The sample consisted of 399 eligible adult patients (≥18 years old) admitted to the urban, general ED of a University Hospital. Among them, 389 patients completed the researcher screening. Two hundred and twenty frequent users defined by &gt;4 ED visits in the previous twelve months were included and compared to 169 patients with ≤4 ED visits in the same period (control group). RESULTS: Researcher screening showed that ED frequent users were more likely than members of the control group to have an anxiety, depressive disorder, post-traumatic stress disorder (PTSD), or suffer from alcohol, illicit drug abuse/addiction. Reviewing the ED physician diagnosis, we found that the proportions of mental health and substance use disorders diagnosed by ED physicians were low both among ED frequent users and in the control group. Using multiple logistic regression analyses to predict frequent ED use, we found that ED patients who screened positive for psychiatric disorders only and those who screened positive for both psychiatric and substance use disorders were more likely to be ED frequent users compared to ED patients with no disorder. CONCLUSIONS: This study found high proportions of screened mental health and/or substance use disorders in ED frequent users, but it showed low rates of detection of such disorders in day-to-day ED activities which can be a cause for concern. Active screening for these disorders in this population, followed by an intervention and/or a referral for treatment by a case-management team may constitute a relevant intervention for integration into a general ED setting

    Study of NAP adsorption and assembly on the surface of HOPG

    NAP is an octapeptide that has demonstrated a neuroprotective/therapeutic efficacy at very low concentrations in preclinical studies and in a number of clinical trials. Yet little is known about its structural organization at low concentrations. Here, we have employed atomic force microscopy to investigate NAP peptide assembly on graphite in aqueous media at nanomolar concentration. High spatial resolution scans of NAP assemblies reveal their fine structure with clearly resolved single NAP units. This observation leads us to conclude that NAP molecules do not form complex self-assembled structures at nanomolar concentration when adsorbed on graphite surface
