    Posture and gait assessment in spastic patients: Key points for practice

    IntroductionPosture and gait are frequently disturbed in patients suffering from diseases affecting central nervous system. Whatever the origin of these troubles (neuromotors, sensory, coordination or cognitive deficiencies), they can lead to major impact on activities, participation and quality of life. Thus they represent an important rehabilitation goal for patients.MethodIn order to assess consequences of neuromotor deficiencies, posture and gait evaluations have two main aspects. A first part of assessments are designed to study posture and gait troubles and to search for their determining factors; in a therapeutic point of view, they can help to choose treatment target(s) and tools and to measure their efficacy on gait or posture troubles. The second part is intended to assess the impact of gait and posture disturbances on activity, participation and quality of life; these evaluations allow measuring the overall efficacy of the treatment in daily living.ResultsThere are multiple clinical and instrumental evaluation tools to deal with these two aspects. The use of these measurements in daily practice must be considered with regards to their feasibility, practical aspects and usefulness.DiscussionWe will make an overview of some of these posture and gait assessment tools in neurological disease, keeping in mind their utility in clinical practice

    Return to Baseline After an Interpretation Training as a Dynamic Predictor for Treatment Response in Social Anxiety Disorder

    Background: Despite considerable research efforts, consistent predictors of cognitive behavioral therapy (CBT) outcome for social anxiety disorder (SAD) are scarce. A dynamic focus on individual symptom reactivity and resilience patterns may show promise in predicting treatment response. This pilot study is the first to investigate whether rate of return to baseline after a one-session positive interpretation training indicates resilience and predicts CBT-response among individuals with SAD. Method: Participants (N = 39) completed an interpretation bias assessment before and after training, and once a day for three days after the training, followed by a six-week CBT-program. Participants completed SAD-assessments pre-treatment, during treatment, and post-treatment. Return to baseline was operationalized as the individualized slope of negative and positive interpretations across interpretation bias assessments. Results: Intention-to-treat analyses showed no significant relation between both negative and positive interpretation bias and CBT-response. Similarly, for completers-only, most analyses also showed no such relationship. Conclusion: These findings suggest that slower return to baseline as a resilience index does not have predictive value for CBT-outcome in individuals with SAD. Future studies should incorporate experience-sampling to capture subtle changes in interpretation bias.</p

    A Holder Continuous Nowhere Improvable Function with Derivative Singular Distribution

    We present a class of functions K\mathcal{K} in C0(R)C^0(\R) which is variant of the Knopp class of nowhere differentiable functions. We derive estimates which establish \mathcal{K} \sub C^{0,\al}(\R) for 0<\al<1 but no K∈KK \in \mathcal{K} is pointwise anywhere improvable to C^{0,\be} for any \be>\al. In particular, all KK's are nowhere differentiable with derivatives singular distributions. K\mathcal{K} furnishes explicit realizations of the functional analytic result of Berezhnoi. Recently, the author and simulteously others laid the foundations of Vector-Valued Calculus of Variations in L∞L^\infty (Katzourakis), of L∞L^\infty-Extremal Quasiconformal maps (Capogna and Raich, Katzourakis) and of Optimal Lipschitz Extensions of maps (Sheffield and Smart). The "Euler-Lagrange PDE" of Calculus of Variations in L∞L^\infty is the nonlinear nondivergence form Aronsson PDE with as special case the ∞\infty-Laplacian. Using K\mathcal{K}, we construct singular solutions for these PDEs. In the scalar case, we partially answered the open C1C^1 regularity problem of Viscosity Solutions to Aronsson's PDE (Katzourakis). In the vector case, the solutions can not be rigorously interpreted by existing PDE theories and justify our new theory of Contact solutions for fully nonlinear systems (Katzourakis).     Neutral Iron Emission Lines From The Day-side Of KELT-9b -- The GAPS Programme With HARPS-N At TNG XX

    We present the first detection of atomic emission lines from the atmosphere of an exoplanet. We detect neutral iron lines from the day-side of KELT-9b (Teq ∼\sim 4, 000 K). We combined thousands of spectrally resolved lines observed during one night with the HARPS-N spectrograph (R ∼\sim 115, 000), mounted at the Telescopio Nazionale Galileo. We introduce a novel statistical approach to extract the planetary parameters from the binary mask cross-correlation analysis. We also adapt the concept of contribution function to the context of high spectral resolution observations, to identify the location in the planetary atmosphere where the detected emission originates. The average planetary line profile intersected by a stellar G2 binary mask was found in emission with a contrast of 84 ±\pm 14 ppm relative to the planetary plus stellar continuum (40 ±\pm 5%\% relative to the planetary continuum only). This result unambiguously indicates the presence of an atmospheric thermal inversion. Finally, assuming a modelled temperature profile previously published (Lothringer et al. 2018), we show that an iron abundance consistent with a few times the stellar value explains the data well.
