EuReCa ONE—27 Nations, ONE Europe, ONE Registry A prospective one month analysis of out-of-hospital cardiac arrest outcomes in 27 countries in Europe
AbstractIntroductionThe aim of the EuReCa ONE study was to determine the incidence, process, and outcome for out of hospital cardiac arrest (OHCA) throughout Europe.MethodsThis was an international, prospective, multi-centre one-month study. Patients who suffered an OHCA during October 2014 who were attended and/or treated by an Emergency Medical Service (EMS) were eligible for inclusion in the study. Data were extracted from national, regional or local registries.ResultsData on 10,682 confirmed OHCAs from 248 regions in 27 countries, covering an estimated population of 174 million. In 7146 (66%) cases, CPR was started by a bystander or by the EMS. The incidence of CPR attempts ranged from 19.0 to 104.0 per 100,000 population per year. 1735 had ROSC on arrival at hospital (25.2%), Overall, 662/6414 (10.3%) in all cases with CPR attempted survived for at least 30 days or to hospital discharge.ConclusionThe results of EuReCa ONE highlight that OHCA is still a major public health problem accounting for a substantial number of deaths in Europe.EuReCa ONE very clearly demonstrates marked differences in the processes for data collection and reported outcomes following OHCA all over Europe. Using these data and analyses, different countries, regions, systems, and concepts can benchmark themselves and may learn from each other to further improve survival following one of our major health care events
SERVICIILE PUBLICE DE AMBULANȚĂ DIN ROMÂNIA- EVALUARE DE ANSAMBLU
Public ambulance services in Romania are public health units of strategic importance, with legal personality, coordinated by the specialized department of the Ministry of Health, functioning under the direct subordination of the Ministry of Health, which provides the necessary funds for its operation, according to the provisions of art. 100 of Law 95/2006. The present analysis aimed in providing evidence of the activity of ambulance services, as well as future directions for action and recommendations to support the decision making in the Romanian health sector. In this respect, it was planned to carry out a descriptive, transversal study, based on activity data and resources used at the level of public ambulance services in Romania. In 2017 the public ambulance services in Romania carried out a total of 2,942,988 interventions, structured as follows: 352,286 Major Emergencies with the Life Threat (Red Code) 1,701,182 potentially life-threatening emergencies (yellow code) 523,582 non-acute problems - consultations at home (Green Code) 365,938 non-assisted medical transport that does not require medical assistance and vital functions monitoring, the carriage being carried out with A1 / A2 ambulance type with only the driver. If the county based analysis showed great differences in the volume of activity performed by each public ambulance service, an analysis of the served population grouped on the eight development regions results in a balanced and homogeneous situation, without significant differences, a situation that would justify a new approach to the organization / reorganization of public ambulance services. • The biggest problem is that when the number of requests increases significantly due to different code situations, can not be reached the shortest time at all requests due to staff shortages • The ambulance services face a large staff shortage at national level, compared to the norm approved by MoHOrder 1788/2006, based on the number of population statistically declared. • Staff policy dysfunctions are reflected both in the shortage of physicians and staff as well as in the health of those employees exposed to high levels of fatigue and stress. Analiza de față și-a propus furnizarea de evidențe privind activitatea serviciilor de ambulanţă, dar și direcții viitoare de acțiune și recomandări care să sprijine luarea deciziei în sectorul sanitar din România. În acest sens a fost planificată desfășurarea unui studiu descriptiv, de tip transversal, realizat pe date de activitate și de resurse utilizate la nivelul serviciilor publice de ambulanță din România. În anul 2017 serviciile publice de ambulanţă din România au efectuat un număr total de 2.942.988 intervenţii, structurate astfel: 352.286 urgenţe majore cu ameninţarea vieţii (Cod roşu); 1.701.182 urgenţe cu potenţial de ameninţarea vieţii (Cod galben); 523.582 probleme non acute- consultaţii la domiciliu (Cod verde); 365.938 transport sanitar neasistat care nu impune asistenţă medicală şi monitorizarea funcţiilor vitale, transportul fiind realizat cu autosanitare de tip A1/A2 şi echipaj format din ambulanţier Dacă la analiza pe județe au rezultat diferențe mari în ceea ce privește volumul de activitate depus de fiecare serviciu public de ambulanță, la o analiză care vizează populația deservită grupată pe cele 8 regiuni de dezvoltare, rezultă o situație echilibrată și omogenă, fără diferențe semnificative, situație care ar justifica o nouă abordare în privința organizării/reorganizării activității serviciilor publice de ambulanță. Cea mai mare problemă este că, în perioadele când numărul de solicitări înregistrează creșteri semnificative, datorate diverselor situații de coduri, nu se poate ajunge în timpul cel mai scurt la toate solicitările, datorită deficitului de personal. Serviciile de ambulanţă se confruntă cu un mare deficit de personal la nivel național, față de normativul aprobat prin OMS 1788/2006, raportat la numărul populației declarate statistic. Disfuncțiile privind politica de personal se reflectă atât în deficitul de medici și personal de specialitate cât și în starea de sănătate a respectivilor salariați, expuși unui nivel ridicat de oboseală și stres.
