    Effectiveness of the biophysical barriers on the peridural fibrosis of a postlaminectomy rat model: An experimental research

    WOS: 000292284100010PubMed: 21392759Purpose: Long term results after surgical treatment of disc herniation have shown that epidural and/or peridural fibrosis formed during the healing process after surgical intervention. We conducted this experimental study to evaluation of the effectiveness of the bioresorbable barriers (ADCON-L and Seprafilm (R) Adhesion Barrier) on formation of the peridural fibrosis in rat model performed laminectomy. Methods: Thirty-two male Wistar albino rats 250-350 g body weight were distributed into three groups (CONTROL, AL group received ADCON-L; SAB group received Seprafilm (R) Adhesion Barrier). A dorsal laminectomy at L3, L4, L5 was performed, and then except those of the CONTROL group, the experimental material was left on the dura mater. Six weeks later spinal column of all rats was totally removed between the T10 and L5 levels, and peridural fibrosis, and dural adhesions were evaluated histologically and graded. The results were compared statistically by using the chi-square (chi(2)) test. Also three random regions were examined, and the fibroblast cells were counted. The fibroblast count results were statistically analysed by using the One-Way ANOVA test. Results: The variation of histopathological grades was statistically significant regarding the comparison of the all groups obtained from the chi(2) test (chi(2) = 16.40; p = 0.003). However, the variation in the mean values of the fibroblast count result was not statistically significant obtained from the One-Way ANOVA test (F = 2.114; p > 0.05). Conclusion: Our study results suggest that Seprafilm (R) Adhesion Barrier and ADCON-L can be effective in reducing the prevalence of the postoperative peridural adhesions in rat laminectomy model. On the other hand, the fibroblast densities of the experimental groups were not different between groups. So, we could say that these materials can act as a foreign body in long term period in rat.     Incidental pulmonary nodule frequency in Turkey

    Introduction: Pulmonary nodules are common incidental findings on computed tomography (CT). In Turkey, there is no available data about the follow-up of the frequency of incidental nodules. Our aim is to assess the frequency and size distrubition of incidental pulmonary nodule in our country. Materials and Methods: Between January 2015 and December 2016, computed tomographies, taken of all outpatient and emergency department that recorded in the screening database were examined retrospectively. Nodules and their characteristics (number, size, density, localization) and relationship between age and gender were evaluated. Results: The age range of the cases was mean 58.99 +/- 16.20 years, 256 (42.5%) were women and 347 (57.5%) were men. A total of 288 (48.25%) cases had 420 nodules. Solid nodule was present in 184 cases (30.5%). The number of cases with one solid nodule was 119 (64.7%). There were 124 solid nodules (55.36%) of >= 4-= 6-= 8 mm diameter. Nodule frequency increased statistically significantly with the age (p = 0.001). Conclusion: The frequency of incidental nodule was found higher than in our country than in developed countries

    A novel structural risk index for primary spontaneous pneumothorax: Ankara Numune Risk Index

    Background: In this study, we aimed to reveal a novel risk index as a structural risk marker for primary spontanoeus pneumothorax using body mass index and chest height, structural risk factors for pneumothorax development. Methods: Records of 86 cases admitted between February 2014 and January 2015 with or without primary spontaneous pneumothorax were analysed retrospectively. The patients were allocated to two groups as Group I and Group II. The patients were evaluated with regard to age, gender, pneumothorax side, duration of hospital stay, treatment type, recurrence, chest height and transverse diameter on posteroanterior chest graphy and body mass index. Body mass index ratio per cm of chest height was calculated by dividing body mass index with chest height. We named this risk index ratio which is defined first as ‘Ankara Numune Risk Index’. Diagnostic value of Ankara Numune Risk Index value for prediction of primary spontaneous pneumothorax development was analysed with Receiver Operating Characteristics curver. Results: Of 86 patients, 69 (80.2%) were male and 17 (19.8%) were female. Each group was composed of 43 (50%) patients. When Receiver Operating Characteristics curve analysis was done for optimal limit value 0.74 of Ankara Numune Risk Index determined for prediction of pneumothorax development risk, area under the curve was 0.925 (95% Cl, 0.872–0.977, p < 0.001). Conclusions: Ankara Numune Risk Index is one of the structural risk factors for prediction of primary spontaneous pneumothorax development however it is insufficient for determining recurrence

    The effect of ACE2 receptor, IFN-?, and TNF-? polymorphisms on the severity and prognosis of the disease in SARS-CoV-2 infection

    To investigate the effect of genetic variations in the angiotensin converting enzyme (ACE), interferon (IFNG) and tumor necrosis factor (TNF-alpha) genes on the severity of coronavirus disease (COVID-19). Between September and December 2021, 33 patients with COVID-19 were included in this prospective study. The patients were classified and compared according to disease severity: mild&moderate (n = 26) vs severe&critical (n = 7). These groups were evaluated to assess possible relationships with ACE, TNF-alpha and IFNG gene variations using univariate and multivariable analyses. The median age of the mild&moderate group was 45.5 (22-73), and that of the severe&critical group was 58 (49-80) years (p = 0.014). Seventeen (65.4%) of the mild&moderate patients and 3 (42.9%) of severe&critical patients were female (p = 0.393). According to results of univariate analysis, the percentage of patients with the c.418-70C>G variant of the ACE gene was significantly higher in the mild&moderate group (p = 0.027). The ACE gene polymorphisms, c.2312C>T, c.3490G>A, c.3801C>T, and c.731A>G, were each only seen in separate patients with critical disease. The following variants were observed more frequently in the mild&moderate group: c.582C>T, c.3836G>A, c.511+66A>G, c.1488-58T>C, c.3281+25C>T, c.1710-90G>C, c.2193A> G, c.3387T>C for ACE; c.115-3delT for IFNG; and c.27C>T for TNF. It can be expected that patients carrying the ACE gene c.418-70C>G variant may present with a mild clinical manifestation of COVID-19.     The Cytotoxic Effect of Nasodren Nasal Spray (Cyclamen Europaeum Extract) in L929 Fibroblastic Cell Culture

    Objective: Nasodren nasal spray (Cyclamen europaeum extract), is used in some countries as an alternative therapeutic agent for sinusitis. The aim of this study was to ivestigate the possible cytotoxic effects of the drug on cell morphology and viability in L929 mouse fibroblast cell culture model. Material and Methods: Nasodren solution was diluted to the dilutions of 1:25, 1:50, 1:100, 1:200, and 1:400 (w/v), and these dilutions were grouped from I to V, respectively. The control group was prepared by using saline in a 1:1 ratio. L929 fibroblastic cell culture material was incubated in 96-cell incubation medium. Following a 12-hour incubation period, the material was taken into fresh culture medium and was stored for five days after treatment with various dilutions of the test material. Cell morphology was observed under microscope on 1(st), 2(nd), 3(rd), 4(th), and 5(th) days. Cell viability was evaluated by 3-(4.5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide colorimetric assay method on the same days. Results: Cells maintained normal fibroblastic shape and morphology in Group V and the control group during the incubation period. Normal round ring shape was apparently lost in Groups I-TV. Cell viability decreased in Groups I-TV, whereas cell viability increased in Group V and the control group. Conclusion: Results of this study indicate that higher concentrations of Cyclamen europaeum have toxic effects on cell viability in vitro. There is need for in vivo animal studies before safely prescribing this drug as the first line treatment of sinusitis.WoSScopu

    Czy istnieje związek między średnią objętością płytek krwi a zaawansowaniem choroby wieńcowej?

    Background: Platelet activation and aggregation play key roles both in the pathogenesis of atherosclerosis and in the developmentof acute thrombotic events. Platelet volume is a marker of platelet activation and function, and is measured usingmean platelet volume (MPV).Aim: To determine the relationship between MPV and angiographic Gensini and SYNTAX scores, which give information about the severity and complexity of coronary artery disease (CAD).Methods: This study included 435 consecutive patients undergoing elective coronary angiography. The complete blood countand biochemical examination of blood were obtained after 12 h of fasting. The independent association between MPV andthe severity of CAD was statistically evaluated using PASW Statistics 18 for Windows.Results: Mean age of the study population was 58.4 ± 9.3 years, of whom 196 were female (45.1%) and 239 male (54.9%). Of the patients, 63.2% had CAD, 31.7% had diabetes mellitus, 61.8% had hypertension, 56.6% had hyperlipidaemia, and 38.6% were smokers. Mean Gensini score was 20.7 ± 31.1. According to Gensini scores, 160 of the patients (36.8%) hadnormal coronary arteries (Gensini score: 0), 134 of the patients (30.8%) had minimal CAD (Gensini score: 1–19), and 141 ofthem (32.4%) had severe CAD (Gensini score ≥ 20). Mean MPV values were 8.4 ± 1.0 fL in the group that had no CAD,8.7 ± 1.0 fL in the group with minimal CAD, and 9.3 ± 1.5 fL in the group with severe CAD. According to Spearman correlationanalysis, the positive relationship found between MPV and Gensini score was statistically significant (p &lt; 0.001,r = 0.290). Likewise, SYNTAX score was also associated with MPV (p &lt; 0.001, r = 0.504).Conclusions: We determined a positive correlation between MPV and Gensini and SYNTAX scores. Therefore, this simple haematology test can be used in determining cardiovascular disease burden besides other risk factors during routine clinical practice. For further information about this topic, large-scale studies are needed.Wstęp: Aktywacja i agregacja płytek krwi odgrywają istotną rolę w patogenezie miażdżycy, a także w rozwoju ostrych zdarzeń zakrzepowo-zatorowych. Objętość płytek krwi, opisywana jako średnia objetość płytek (MPV), jest wyznacznikiem aktywnościpłytek i ich funkcji.Cel: Celem badania było określenie zależności między MPV a wskaźnikiem Gensini i SYNTAX score, będącymi liczbową miarąstopnia zaawansowania choroby wieńcowej.Metody: Badanie przeprowadzono u 435 chorych, u których wykonano planową koronarografię. Krew do badań morfologicznychi biochemicznych pobrano na czczo (12 h od ostatniego posiłku). Niezależny związek między MPV i stopniem nasilenia choroby wieńcowej oceniono za pomocą programu PASW Statistics 18 dla systemu Windows.Wyniki: Średni wiek pacjentów wynosił 58,4 ± 9,3 roku; 196 (45,1%) osób stanowiły kobiety, a 239 (54,9%) mężczyźni. Wśród badanych u 63,2% zdiagnozowano chorobę wieńcową, u 31,7% — cukrzycę, u 61,8% — nadciśnienie tętnicze, a u 56,6% — hiperlipidemię. Wśród badanych 38,6% osób paliło tytoń. Średni wynik Gensini score był równy 20,7 ± 31,1. Według Gensini score 160 (36,8%) chorych miało prawidłowe tętnice wieńcowe (Gensini score: 0), u 134 (30,8%) pacjentów występowały minimalne oznaki choroby wieńcowej (Gensini score: 1–19), a u pozostałych 141 (32,4%) osób stwierdzono zaawansowaną chorobę wieńcową (Gensini score: ≥ 20). Wartości MPV u pacjentów bez choroby wieńcowej wynosiły średnio 8,4 ± 1,0 fl,w grupie z minimalnym zaawansowaniem choroby wieńcowej — 8,7 ± 1,0 fl, a w grupie z zaawansowaną chorobą wieńcową— 9,3 ± 1,5 fl. Analiza korelacji Spearmana pokazała pozytywny związek między MPV i Gensini score (p &lt; 0,001;r = 0,290). Podobnie wynik SYNTAX score wiązał się z MPV (p &lt; 0,001; r = 0,504).Wnioski: Stwierdzona dodatnia korelacja między Gensini i SYNTAX score a MPV wskazuje, że w rutynowej praktyce lekarskiej proste badanie hematologiczne można wykorzystać do oceny obciążenia chorobami układu sercowo-naczyniowego, opróczinnych czynników ryzyka. Aby potwierdzić tę zależność i uzyskać dodatkowe informacje, należy przeprowadzić badanie w większej grupie chorych

    Single-step hysteroscopic myomectomy for submucous leiomyoma

    Objective: Leiornyomas are most commonly observed benign tumors in the female genital tract. Depending on the size, number, and location, the complete resection of Type 0, 1, and 2 leiomyomas by hysteroscopy can be completed in a single-step or multi-step procedure. The purpose of this study is to document the cases of hysteroscopic myomectomy performed via the resectoscopic technique in the gynecology department of a university hospital. Moreover, we assessed the applicability of single- or multi-step hysteroscopic myomectomy with respect to the diameter of the leiomyoma. Materials and Methods: We retrospectively reviewed the records of hysteroscopic myomectomy performed between 2012 and 2018. According to the diameter of the submucous leiomyomas, we divided 46 patients into 2 groups. Group 1 (n=25) consisted of patients with submucous leiomyomas = 3 cm in diameter. We recorded the number of removed leiomyomas and completed hysteroscopy sessions. Results: Myomectomy was completed by single-step hysteroscopy in all the patients of group 1, whereas eight patients in group 2 needed multiple sessions of hysteroscopy. None of the patients in group 1 had fluid overload; however, two patients in group 2 had mild asymptomatic hyponatremia. Conclusion: The success of hysteroscopic myomectomy depends on the diameter, localization, and number of the leiomyomas. This study revealed that Type 0, 1, and 2 leiomyomas of less than 3 cm can be resected by single-step hysteroscopy. For larger leiomyomas, the possibility of need for further sessions should be shared with the patients

    Evaluation of the neurotoxicity of DMSO infused into the carotid artery of rat

    WOS: 000300640000013PubMed: 21907339Introduction: Despite the explanations put forth in many studies regarding histopathological evidence of the inflammatory stage related with the infusion of dimethyl sulfoxide (DMSO) in the vessel wall and its lumen, there has been no research to evaluate its neural toxicity when it is infused via the intracarotid route. This study was designed to evaluate the possible neurotoxic effects of DMSO on the closer and distant brain tissue and carotid artery when it was slowly infused into the internal carotid arteries of the rats. Methods: The right common carotid artery bifurcation was exposed through a midline neck incision, and then except those of the control group animals (n = 5), the experimental material (normal saline, n = 5 or anhydrous DMSO, n = 10) was infused into the internal carotid artery of the Wistar albino rats. After the experimental materials were administered intra-arterially, brain tissues were harvested for histopathological and biochemical studies at 72 h for investigation of the acute stage changes and on 10th day for investigation of the chronic stage changes. Internal carotid arteries of both sides were also removed for histopathological evaluation. During sacrification of the rats, whole body blood of them are collected for biochemical evaluation. Results: There was no statistically significant difference between the groups regarding comparison of the mean values of the hippocampal neuronal cell counts and the carotid artery diameters in both acute and chronic stages. Also, mean values of the lipid peroxidation levels of harvested brain tissues and serums of the collected bloods were similar in control, saline and DMSO groups. Conclusion: This experimental study suggested that DMSO has no toxic effect on the neural and arterial tissues of rats when it is slowly infused into the carotid artery. (C) 2011 Elsevier Ltd.     Effect of the novel type coronavirus on the mental health of patients

    BACKGROUND AND AIM: The novel Coronavirus disease (COVID-19) emerged in China in 2019 and significantly affected the entire world. The pandemic and the restrictions led to an increase in the frequency of mental health disorders (i.e., depression and anxiety) and insomnia. The aim of the study was to evaluate mental health disorders in patients who were hospitalized because of COVID-19. The patients were evaluated 6 months after hospital discharge. METHODS: A total of 114 patients who were hospitalized at Ufuk University Hospital with the diagnosis of COVID-19 were consented to participate in the posttreatment questionnaire about demographics and a Depression, Anxiety, and Stress Scale-21. The Pittsburgh Sleep Quality Index was also given to the patients to answer. RESULTS: In our study, anxiety symptoms were detected in 34 (29.8%) individuals, stress symptoms in 9 (7.9%) individuals, and depressive symptoms in 17 (14.9%). When gender, education level, age, and marital status were evaluated, there was no significant relationship observed between depression, anxiety and stress, and sleep disorders. Only those with a history of psychiatric illness had significantly higher levels of anxiety, stress, and depression (p=0.040, p=0.047, and p=0.009, respectively). Sleep quality was poor in 88 (77.2%) patients and good (normal) in 26 (22.8%) patients. CONCLUSIONS: The results show that the sleep quality of the patients deteriorated and the symptoms of stress and anxiety increased during the COVID-19 pandemic period.     Quantative computerized tomography assessment of lung density as a predictor of postoperative pulmonary morbidity in patients with lung cancer

    Cubuk, Sezai/0000-0003-4277-6360WOS: 000362069800031PubMed: 26380765Background: The aim of this study was to evaluate the pulmonary reserve of the patients via preoperative quantitative computerized tomography (CT) and to determine if these preoperative quantitative measurements could predict the postoperative pulmonary morbidity. Methods: Fifty patients with lung cancer who underwent lobectomy/segmentectomy were included in the study. Preoperative quantitative CT scans and pulmonary function tests data were evaluated retrospectively. We compare these measurements with postoperative morbidity. Results: There were 32 males and 18 females with a mean age of 54.4 +/- 13.9 years. Mean total density was -790.6 +/- 73.4 HU. The volume of emphysematous lung was ( 5.41% (P=0.014) and lung density <-787.5 HU (P=0.009) were independent prognostic factors associated with postoperative pulmonary morbidity. Conclusions: In this study, the patients with a lower lung density than -787.5 HU and a higher volume of emphysema than 5.41% were found to be at increased risk for developing postoperative pulmonary morbidity. More stringent precautions should be taken in those patients that were found to be at high risk to avoid pulmonary complications
