    Abstract 13987: Underutilization of Oral Anticoagulant Therapy in At-Risk Patients With Atrial Fibrillation—Insights From a Multistate Healthcare System

    Introduction: Oral anticoagulant (OAC) therapy significantly reduces the risk of thromboembolism among at-risk patients with atrial fibrillation (AF). Current guidelines provide strong support for an OAC in men and women with AF and CHA2DS2-VASc scores of \u3e2 and \u3e3, respectively. In spite of this, previous data has suggested that up to 40% of these patients are not treated in accordance with guideline recommendations. Hypothesis: We hypothesized that OAC therapy continues to remain significantly underutilized among at-risk patients with AF in real-world settings. Methods: We sought to evaluate the prevalence of OAC underuse and contributing factors in an ambulatory population of at-risk AF patients within a large multistate healthcare system. EHR and coding (ICD-10) data were used to identify patients with AF, calculate their CHA2DS2-VASc score, and define their current antithrombotic regimen. Demographics were assessed to allow for comparison between those receiving an OAC from those who were not. Chi square or Fisher exact tests were used to examine differences between groups. Results: Data was pulled from our EHR on 8/1/18, identifying 147,455 unique patients with AF, of which 102,728 (76.3%) had a CHA2DS2-VASc score \u3e2 (excluding female gender) (Table). Compared to those on an OAC, patients on antiplatelet therapy were more likely to have coronary artery disease, peripheral vascular disease, and prior MI (p Conclusions: In a contemporary, non-registry setting, OAC underuse remains substantial among at-risk patients with AF. Further investigation into tools that facilitate implementation of guideline-directed medical therapy is needed to limit preventable thromboembolic events in this population

    Abstract 14012: Opportunities to Improve the Efficacy and Safety of Oral Anticoagulant Therapy in Atrial Fibrillation—Insights From a Multistate Healthcare System

    Introduction: Vitamin K antagonists (VKAs) effectively reduce thromboembolic risk in atrial fibrillation (AF), but are limited by a narrow therapeutic window. Patients with reduced time in the therapeutic range (TTR) also face an increased risk of bleeding and ischemic events. Based in part on this, current guidelines give preference to direct-acting oral anticoagulants (DOACs) over VKAs in AF. Hypothesis: We hypothesized that DOACs are underutilized among those on oral anticoagulant therapy and that TTR remains suboptimal for large numbers of individuals on VKAs in real-world settings. Methods: We sought to evaluate a) the breakdown of OAC type and b) TTR for those on VKAs in an ambulatory population of at-risk AF patients within a large multistate healthcare system. EHR and coding (ICD-10) data were used to identify patients with AF, calculate their CHA2DS2-VASc score, and define their current antithrombotic regimen. For those on a VKA, TTR was assessed with the Rosendaal method and reported as mean values. Demographics were assessed to allow for comparison between those receiving a DOAC and a VKA, as well as, those with high (\u3e70%) vs. low ( Results: Data was pulled from our EHR on 8/1/18, identifying 147,455 unique patients with AF, of which 102,728 (76.3%) had a CHA2DS2-VASc score \u3e2 (excluding female gender). Among these at-risk patients, 61,698 (60.1%) were receiving an OAC, of which 47.8% were on a VKA and 52.2% were on a DOAC. The mean TTR was 56.3%, with 37.1%, 49.9% and 60.8% with TTRs \u3e70%, \u3e60%, and \u3e50%, respectively. Patients on a DOAC were more likely to be female and less likely to have heart failure, coronary artery disease, peripheral vascular disease, diabetes and renal disease (p70% were more likely to be male and less likely to have heart failure, diabetes, and renal disease (p Conclusions: In a contemporary, non-registry setting, VKAs continue to be used in nearly half of at-risk patients on an OAC for AF, with a suboptimal TTR in nearly two thirds. Further investigation is needed into tools that facilitate interchange from a VKA to a DOAC, particularly among those with a suboptimal TTR

    Seeing the way: visual sociology and the distance runner's perspective

    Employing visual and autoethnographic data from a two‐year research project on distance runners, this article seeks to examine the activity of seeing in relation to the activity of distance running. One of its methodological aims is to develop the linkage between visual and autoethnographic data in combining an observation‐based narrative and sociological analysis with photographs. This combination aims to convey to the reader not only some of the specific subcultural knowledge and particular ways of seeing, but also something of the runner's embodied feelings and experience of momentum en route. Via the combination of narrative and photographs we seek a more effective way of communicating just how distance runners see and experience their training terrain. The importance of subjecting mundane everyday practices to detailed sociological analysis has been highlighted by many sociologists, including those of an ethnomethodological perspective. Indeed, without the competence of social actors in accomplishing these mundane, routine understandings and practices, it is argued, there would in fact be no social order

    Engaging diverse communities participating in clinical trials: case examples from across Africa

    <p>Abstract</p> <p>Background</p> <p>In the advent of increasing international collaborative research involving participants drawn from populations with diverse cultural backgrounds, community engagement becomes very critical for the smooth conduction of the research. The African Malaria Network Trust (AMANET) is a pan-African non-governmental organization that sponsors and technically supports malaria vaccine trials in various African countries.</p> <p>Case description</p> <p>AMANET sponsored phase Ib or IIb clinical trials of several malaria vaccine candidates in various Africa countries. In Burkina Faso, Mali and Tanzania trials of the merozoite surface protein 3 -- in its Long Synthetic Peptide configuration (MSP3 LSP) -- were conducted. In Mali, the apical membrane antigen 1 (AMA1) was tested, while a hybrid of glutamate rich protein (GLURP) and MSP3 (GMZ2) was tested in Gabon. AMANET recognizes the importance of engaging with the communities from which trial participants are drawn, hence community engagement was given priority in all project activities conducted in the various countries.</p> <p>Discussion and evaluation</p> <p>Existing local social systems were used to engage the communities from which clinical trial participants were drawn. This article focuses on community engagement activities employed at various AMANET-supported clinical trial sites in different countries, highlighting subtle differences in the approaches used. The paper also gives some general pros and cons of community engagement.</p> <p>Conclusions</p> <p>Community engagement enables two-way sharing of accurate information and ideas between researchers and researched communities, which helps to create an environment conducive to smooth research activities with enhanced sense of research ownership by the communities.</p

    Genomic diversity of bacteriophages infecting Microbacterium spp

    The bacteriophage population is vast, dynamic, old, and genetically diverse. The genomics of phages that infect bacterial hosts in the phylum Actinobacteria show them to not only be diverse but also pervasively mosaic, and replete with genes of unknown function. To further explore this broad group of bacteriophages, we describe here the isolation and genomic characterization of 116 phages that infect Microbacterium spp. Most of the phages are lytic, and can be grouped into twelve clusters according to their overall relatedness; seven of the phages are singletons with no close relatives. Genome sizes vary from 17.3 kbp to 97.7 kbp, and their G+C% content ranges from 51.4% to 71.4%, compared to ~67% for their Microbacterium hosts. The phages were isolated on five different Microbacterium species, but typically do not efficiently infect strains beyond the one on which they were isolated. These Microbacterium phages contain many novel features, including very large viral genes (13.5 kbp) and unusual fusions of structural proteins, including a fusion of VIP2 toxin and a MuF-like protein into a single gene. These phages and their genetic components such as integration systems, recombineering tools, and phage-mediated delivery systems, will be useful resources for advancing Microbacterium genetics

    Individual patient data meta-analysis of self-monitoring of blood pressure (BP-SMART): a protocol.

    INTRODUCTION: Self-monitoring of blood pressure is effective in reducing blood pressure in hypertension. However previous meta-analyses have shown a considerable amount of heterogeneity between studies, only part of which can be accounted for by meta-regression. This may be due to differences in design, recruited populations, intervention components or results among patient subgroups. To further investigate these differences, an individual patient data (IPD) meta-analysis of self-monitoring of blood pressure will be performed. METHODS AND ANALYSIS: We will identify randomised trials that have compared patients with hypertension who are self-monitoring blood pressure with those who are not and invite trialists to provide IPD including clinic and/or ambulatory systolic and diastolic blood pressure at baseline and all follow-up points where both intervention and control groups were measured. Other data requested will include measurement methodology, length of follow-up, cointerventions, baseline demographic (age, gender) and psychosocial factors (deprivation, quality of life), setting, intensity of self-monitoring, self-monitored blood pressure, comorbidities, lifestyle factors (weight, smoking) and presence or not of antihypertensive treatment. Data on all available patients will be included in order to take an intention-to-treat approach. A two-stage procedure for IPD meta-analysis, stratified by trial and taking into account age, sex, diabetes and baseline systolic BP will be used. Exploratory subgroup analyses will further investigate non-linear relationships between the prespecified variables. Sensitivity analyses will assess the impact of trials which have and have not provided IPD. ETHICS AND DISSEMINATION: This study does not include identifiable data. Results will be disseminated in a peer-reviewed publication and by international conference presentations. CONCLUSIONS: IPD analysis should help the understanding of which self-monitoring interventions for which patient groups are most effective in the control of blood pressure
