    Intersigmoid hernia. A forgotten diagnosis, a systematic review of the literature over anatomical, diagnostic, surgical, and medicolegal aspects

    Introduction. Intersigmoid hernia is a hernia of the small bowel into the intersigmoid fossa. It is well known to be a rare condition. Recent reports reveal that the preoperative differentiation of intersigmoid hernias is difficult and the diagnosis is often confirmed during the laparotomic exploration. Due to the vague clinical manifestation in most cases, the surgical treatment is frequently delayed. Materials and Methods. In this study, we systematically reviewed the literature up to 2019 covering 114 studies and 124 patients with an intersigmoid hernia. &e purpose of this work is to improve the understanding of the anatomical aspects, clinical presentation, diagnosis, and treatment of intersigmoid hernia so as to assist the preoperative differentiation of these hernias when presented as acute abdomen in the emergency department. Results. &e diameter of the intersigmoid recess was reported with mean 2.65 cm (range 1–10 cm, SD 1.15 cm) and the length of the incarcerated small intestine was between 3 cm (min) and 150 cm (max): mean 25.25 cm, SD 35.04 cm. &e diameter of the sigmoid recess was greater in patients who underwent resection due to strangulation (mean 3.31 cm, SD 1.53 cm) compared to those who underwent only reduction of the hernia (mean 2.35 cm, SD 0.74 cm). &e time from onset to operation was less in patients undergoing resection surgery due to throttling (mean 3.03 days, SD 3.01 days) compared to those who underwent only a reduction of hernia incarceration (mean 8.49 days, SD 6.83 days). Conclusion. Intersigmoid hernia is often a forgotten diagnosis and a clinical challange due to its anatomical characteristics

    Metal Nanoparticles Deposited on Porous Silicon Templates as Novel Substrates for SERS

    In this paper, results on preparation of stable and uniform SERS solid substrates using macroporous silicon (pSi) with deposited silver and gold are presented. Macroporous silicon is produced by anodisation of p-type silicon in hydrofluoric acid. The as prepared pSi is then used as a template for Ag and Au depositions. The noble metals were deposited in three different ways: by immersion in silver nitrate solution, by drop-casting silver colloidal solution and by pulsed laser ablation (PLA). Substrates obtained by different deposition processes were evaluated for SERS efficiency using methylene blue (MB) and rhodamine 6G (R6G) at 514.5, 633 and 785 nm. Using 514.5 nm excitation and R6G the limits of detection (LOD) for macroporous Si samples with noble metal nanostructures obtained by immersion of pSi sample in silver nitrate solution and by applying silver colloidal solution to pSi template were 10–9 M and 10–8 M respectively. Using 633 nm laser and MB the most noticeable SERS activity gave pSi samples ablated with 30000 and 45000 laser pulses where the LODs of 10–10 M were obtained. The detection limit of 10–10 M was also reached for 4 mA cm–2-15 min pSi sample, silver ablated with 30000 pulses. Macroporous silicon proved to be a good base for the preparation of SERS substrates

    IL15RA and SMAD3 Genetic Variants Predict Overall Survival in Metastatic Colorectal Cancer Patients Treated with FOLFIRI Therapy: A New Paradigm

    Simple SummaryThere is an increasing scientific interest in the study of the interaction between the immune system and drugs in cancer that can affect the efficacy of an anti-cancer treatment. This study was undertaken to better understand if the genetic characteristic of a cancer patient's immune system can predict the tumor response to the treatment and the duration of survival. The topic was studied on 335 metastatic colorectal cancer patients treated with a first-line chemotherapy (FOLFIRI regimen, irinotecan-5-fluorouracil-leucovorin). The research highlighted two markers, IL15RA-rs7910212 and SMAD3-rs7179840, significantly associated with the patient's survival. When considering IL15RA-rs7910212 and SMAD3-rs7179840 in combination with other two genetic markers previously investigated (NR1I2-rs1054190, VDR-rs7299460), we built up a highly predictive genetic score of survival. The herein identified markers must be further validated, but still represent good candidates to understand how much a patient with a metastatic colorectal cancer can benefit from a chemotherapy with FOLFIRI regimen.A new paradigm in cancer chemotherapy derives from the interaction between chemotherapeutics, including irinotecan and 5-fluorouracil (5-FU), and the immune system. The patient's immune response can modulate chemotherapy effectiveness, and, on the other hand, chemotherapeutic agents can foster tumor cell immunogenicity. On these grounds, the analysis of the cancer patients' immunogenetic characteristics and their effect on survival after chemotherapy represent a new frontier. This study aims to identify genetic determinants in the immuno-related pathways predictive of overall survival (OS) after FOLFIRI (irinotecan, 5-FU, leucovorin) therapy. Two independent cohorts comprising a total of 335 patients with metastatic colorectal cancer (mCRC) homogeneously treated with first-line FOLFIRI were included in the study. The prognostic effect of 192 tagging genetic polymorphisms in 34 immune-related genes was evaluated using the bead array technology. The IL15RA rs7910212-C allele was associated with worse OS in both discovery (HR: 1.57, p = 0.0327, Bootstrap p-value = 0.0280) and replication (HR: 1.71, p = 0.0411) cohorts. Conversely, SMAD3 rs7179840-C allele was associated with better OS in both discovery (HR: 0.65, p = 0.0202, Bootstrap p-value = 0.0203) and replication (HR: 0.61, p = 0.0216) cohorts. A genetic prognostic score was generated integrating IL15RA-rs7910212 and SMAD3-rs7179840 markers with inflammation-related prognostic polymorphisms we previously identified in the same study population (i.e., PXR [NR1I2]-rs1054190, VDR-rs7299460). The calculated genetic score successfully discriminated patients with different survival probabilities (p < 0.0001 log-rank test). These findings provide new insight on the prognostic value of genetic determinants, such as IL15RA and SMAD3 markers, and could offer a new decision tool to improve the clinical management of patients with mCRC receiving FOLFIRI

    Preserving Local Food Traditions: A Hybrid Participatory Approach for Stimulating Transgenerational Dialogue

    Local food traditions are an essential part of culture and society, reflecting a community’s history, values, and beliefs. Elders play a major role in passing on local food knowledge to younger generations, ensuring local food traditions and cultural identity do not disappear over time. To preserve these traditions, it is essential to engage older and younger generations of a community in a transgenerational dialogue. From this perspective, the study utilizes a hybrid participatory approach, composed of design thinking and learning-by-doing. Results of the case study underline the effectiveness of the approach in stimulating both the transfer of knowledge, as well as the involvement of younger generations, in the preservation of local food traditions

    Discussing the Role of ICT in Sustainable Disaster Management

    This paper analyses the role of information and communications technology (ICT) in supporting the implementation of the actions to be undertaken by the various actors of the Quadruple Helix (science, policy, industry, and society) in case of a disaster through the lens of sustainable development. A two-step methodology consisting of an online forum discussion with experts from SSH and ICT fields and a desk analysis of sustainable development goals and ICT tools for disaster management has been adopted. A set of actions that Quadruple Helix actors should implement under the ten sustainable development goals related to disaster risk reduction is provided in the paper. The role of ICT in favouring the implementation of the actions for achieving the sustainable development goals is underlined

    Online Health Information Seeking: An Italian Case Study for Analyzing Citizens’ Behavior and Perception

    This study aims to understand people’s behavior when searching for online health information (and COVID-19 information) and their perception of the trustworthiness and credibility of the searched information, the actors, and sources used to obtain it. A questionnaire addressed to people who permanently live in Italy between ages 19 and 60 has been used to collect data. Data extracted from the analysis are reassuring from the point of view of trust and credibility both in the actors and in the sources used to obtain information on health and COVID-19. A correlation between the analyzed individual features, the online health information-seeking behavior, and perception resulted from the analysis. The study also underlined a positive correlation between the perception of the influence of information on the knowledge of health problems and the ability to identify false online health information, and between the experience in detecting false health online information and the ability to detect it. Finally, a positive correlation also resulted between the experience in finding online health information and the experience in finding false COVID-19 information

    Evaluation of Sustainable Accounting Practices in THE Italian Bioenergy Sector

    Bioenergy is a vital component of the energy sector in Italy, contributing to the country’s commitment for reduction in green house gas emission and enhancing its self reliance on energy production. At the same time, however, there are risks that the bioenergy sector may generate negative impacts on the environment and society, if it is not properly managed. Sustainable accounting is one of the important instruments by which, through stakeholder pressure, responsible actions by organizations can be enforced. For this reason it is necessary to develop better sustainable accounting practices, to report and to address the concerns over the sustainability of the bioenergy sector in order to avoid detrimental results masked as renewable energy. In addressing the above needs, the aim of this research is to evalu- ate current sustainable accounting practices in the Italian Bioenergy sector and to construct a conceptual framework for enhanced sustainable accounting practices. This research begins with an analysis of a few small and medium Italian bioenergy enter- prises as regards their current accounting practices for sustainability using the content analysis method to identify the gaps in reporting. In this step, a considerable lack of reporting about the environmental and social impacts has been identified in a sample of small-and-medium-sized firms in the wood biomass sector. Based on the above findings, the study then develops a conceptual framework to fill the gap iden- tified, with due consideration of the practical limitations for the companies to adopt it. The basis of this framework is the regulatory framework of the European Union and sector-specific selective indicators have been proposed for use. This set of indicators, providing information on bioenergy’s sustainability impacts, will render a better picture of the company’s action for the stakeholders

    Non-Financial Disclosure and Intra-Industry Comparability: A Macro, Meso and Micro Analysis

    The inadequacy of financial reports for meeting the information needs of stakeholders has prompted companies to adopt non-financial communication systems (also called non-financial disclosure—NFD). Comparability of NFD is an issue as it allows making sense of the information. Nevertheless, while some argue that comparability is particularly difficult to pursue in practice on a general level, it can be achieved among companies belonging to the same industry. This study aims to understand whether, at the empirical level, the comparability of NFD is achieved and to what extent (macro, meso or micro). To achieve this aim, a text analysis of the NFD was performed. The object of analysis is represented by the NFD published by the listed companies belonging to the energy and banking industries, and that is part of the Dow Jones sustainability index. The main results are the following. First, there is a de facto comparability in terms of adopted standards, but not in terms of how the standards are applied. Second, the phenomenon of label creativity represents a relevant barrier. Third, although content standardization is lacking, common information dimensions in the reports seem to emerge. This appears to be an invitation to policymakers to transpose virtuous behavior and to implement the desired harmonization of jure
