Batalin-Vilkovisky Integrals in Finite Dimensions
The Batalin-Vilkovisky method (BV) is the most powerful method to analyze
functional integrals with (infinite-dimensional) gauge symmetries presently
known. It has been invented to fix gauges associated with symmetries that do
not close off-shell. Homological Perturbation Theory is introduced and used to
develop the integration theory behind BV and to describe the BV quantization of
a Lagrangian system with symmetries. Localization (illustrated in terms of
Duistermaat-Heckman localization) as well as anomalous symmetries are discussed

Outcomes of Fusions From the Cervical Spine to the Pelvis.
Study designRetrospective cohort study.ObjectiveDetermine the indications, complications, and clinical outcomes in patients requiring fusions from the cervical spine to the pelvis. Several investigators have examined fusions from the thoracic spine to the sacrum, but no similar study has been performed for cervical-to-pelvis fusions.MethodsPatients from 2003 to 2014 with an upper instrumented vertebrae (UIV) in the cervical spine (any level) and a lower instrumented vertebrae (LIV) in the sacrum or pelvis were included in the study. Those with infectious or acute trauma-related deformities were excluded. Patient demographics, medical history, diagnosis, operative procedure, and health-related quality of life measures were analyzed. Student's t test, Kruskal-Wallis test, and χ2 test were used as appropriate; significance was set at P < .05 for all tests.ResultsFifty-five patients met inclusion criteria for the study. Average follow-up was 2.8 years. Proximal junctional kyphosis was the most common indication for cervical-to-pelvis fusions (36%). The most common UIV was C2 (29%) followed by C7 (24%). There was an average 31° correction in maximum kyphosis and a 3.3 cm improvement in sagittal vertical axis. In adults, the rate of complication was 71.4%, with a major complication rate of 39.3% and reoperation rate of 53.6%. There was significant improvement in the Scoliosis Research Society (SRS-22r) score (3.0 to 3.5; P < .01).ConclusionProximal junctional kyphosis is the most common indication for patients requiring fusion to the cervical spine. Adult patients incur a significant risk of major complications and reoperations. However, significant improvement in SRS-22r outcomes are noted in these patients
TrueImage: A Machine Learning Algorithm to Improve the Quality of Telehealth Photos
Telehealth is an increasingly critical component of the health care
ecosystem, especially due to the COVID-19 pandemic. Rapid adoption of
telehealth has exposed limitations in the existing infrastructure. In this
paper, we study and highlight photo quality as a major challenge in the
telehealth workflow. We focus on teledermatology, where photo quality is
particularly important; the framework proposed here can be generalized to other
health domains. For telemedicine, dermatologists request that patients submit
images of their lesions for assessment. However, these images are often of
insufficient quality to make a clinical diagnosis since patients do not have
experience taking clinical photos. A clinician has to manually triage poor
quality images and request new images to be submitted, leading to wasted time
for both the clinician and the patient. We propose an automated image
assessment machine learning pipeline, TrueImage, to detect poor quality
dermatology photos and to guide patients in taking better photos. Our
experiments indicate that TrueImage can reject 50% of the sub-par quality
images, while retaining 80% of good quality images patients send in, despite
heterogeneity and limitations in the training data. These promising results
suggest that our solution is feasible and can improve the quality of

Modeling and Correcting the Time-Dependent ACS PSF
The ability to accurately measure the shapes of faint objects in images taken with the Advanced Camera for Surveys (ACS) on the Hubble Space Telescope (HST) depends upon detailed knowledge of the Point Spread Function (PSF). We show that thermal fluctuations cause the PSF of the ACS Wide Field Camera (WFC) to vary over time. We describe a modified version of the TinyTim PSF modeling software to create artificial grids of stars across the ACS field of view at a range of telescope focus values. These models closely resemble the stars in real ACS images. Using 10 bright stars in a real image, we have been able to measure HST s apparent focus at the time of the exposure. TinyTim can then be used to model the PSF at any position on the ACS field of view. This obviates the need for images of dense stellar fields at different focus values, or interpolation between the few observed stars. We show that residual differences between our TinyTim models and real data are likely due to the effects of Charge Transfer Efficiency (CTE) degradation. Furthermore, we discuss stochastic noise that is added to the shape of point sources when distortion is removed, and we present MultiDrizzle parameters that are optimal for weak lensing science. Specifically, we find that reducing the MultiDrizzle output pixel scale and choosing a Gaussian kernel significantly stabilizes the resulting PSF after image combination, while still eliminating cosmic rays/bad pixels, and correcting the large geometric distortion in the ACS. We discuss future plans, which include more detailed study of the effects of CTE degradation on object shapes and releasing our TinyTim models to the astronomical community
Cumulative stress in childhood is associated with blunted reward-related brain activity in adulthood
Early life stress (ELS) is strongly associated with negative outcomes in adulthood, including reduced motivation and increased negative mood. The mechanisms mediating these relations, however, are poorly understood. We examined the relation between exposure to ELS and reward-related brain activity, which is known to predict motivation and mood, at age 26, in a sample followed since kindergarten with annual assessments. Using functional neuroimaging, we assayed individual differences in the activity of the ventral striatum (VS) during the processing of monetary rewards associated with a simple card-guessing task, in a sample of 72 male participants. We examined associations between a cumulative measure of ELS exposure and VS activity in adulthood. We found that greater levels of cumulative stress during childhood and adolescence predicted lower reward-related VS activity in adulthood. Extending this general developmental pattern, we found that exposure to stress early in development (between kindergarten and grade 3) was significantly associated with variability in adult VS activity. Our results provide an important demonstration that cumulative life stress, especially during this childhood period, is associated with blunted reward-related VS activity in adulthood. These differences suggest neurobiological pathways through which a history of ELS may contribute to reduced motivation and increased negative mood
Collateral donor artery physiology and the influence of a chronic total occlusion on fractional flow reserve
Background— The presence of a concomitant chronic total coronary occlusion (CTO) and a large collateral contribution might alter the fractional flow reserve (FFR) of an interrogated vessel, rendering the FFR unreliable at predicting ischemia should the CTO vessel be revascularized and potentially affecting the decision on optimal revascularization strategy. We tested the hypothesis that donor vessel FFR would significantly change after percutaneous coronary intervention of a concomitant CTO. Methods and Results— In consecutive patients undergoing percutaneous coronary intervention of a CTO, coronary pressure and flow velocity were measured at baseline and hyperemia in proximal and distal segments of both nontarget vessels, before and after percutaneous coronary intervention. Hemodynamics including FFR, absolute coronary flow, and the coronary flow velocity–pressure gradient relation were calculated. After successful percutaneous coronary intervention in 34 of 46 patients, FFR in the predominant donor vessel increased from 0.782 to 0.810 (difference, 0.028 [0.012 to 0.044]; P=0.001). Mean decrease in baseline donor vessel absolute flow adjusted for rate pressure product: 177.5 to 139.9 mL/min (difference −37.6 [−62.6 to −12.6]; P=0.005), mean decrease in hyperemic flow: 306.5 to 272.9 mL/min (difference, −33.5 [−58.7 to −8.3]; P=0.011). Change in predominant donor vessel FFR correlated with angiographic (%) diameter stenosis severity (r=0.44; P=0.009) and was strongly related to stenosis severity measured by the coronary flow velocity–pressure gradient relation (r=0.69; P<0.001). Conclusions— Recanalization of a CTO results in a modest increase in the FFR of the predominant collateral donor vessel associated with a reduction in coronary flow. A larger increase in FFR is associated with greater coronary stenosis severity
The posterior use of BMP-2 in cervical deformity surgery does not result in increased early complications: A prospective multicenter study
Study designProspective cohort study.ObjectivesTo describe the rate of short-term complications following the posterior use of recombinant human bone morphogenetic protein-2 (rhBMP-2) in cervical deformity (CD) surgery.MethodsCD patients from 2013 to 2015 were enrolled in a prospective, multicenter database. Patients were divided into those receiving rhBMP-2 (BMP) and no rhBMP-2 (NOBMP). The relationship between BMP use, demographic variables surgical variables, radiographic parameters and complications was evaluated.ResultsA total of 100 patients (47 BMP, 53 NOBMP) were included. Follow-up time averaged 7.6 months (range 3-12 months). An average of 13.6mg of BMP was used per person with 1.49 mg per level. Compared with the NOBMP group, patients in the BMP group were older (P = .03). BMP was more commonly used in patients that and had longer prior fusions (6.0 vs 2.5, P < .01). There were no differences between groups with regards to a history of surgery, Charlson Comorbidity Index, estimated blood loss, operation time, fusion levels, and surgical approach. The maintenance of radiographic parameters at 6-month follow-up was similar. There were no differences in terms of total complication incidence, total complications per person, major complications per person or any specific complication. Linear regression and Pearson correlation analysis did not reveal any strong r2 values (r2 = 0.09, 0.08, 0.06) between the use of BMP and complications (major or operative).ConclusionsBMP use was not directly associated with an increased incidence of early complications in this prospective cohort of operative adult CD patients. Predictors and consequences of altered mineral metabolism: The Dialysis Outcomes and Practice Patterns Study.BackgroundAltered mineral metabolism contributes to bone disease, cardiovascular disease, and other clinical problems in patients with end-stage renal disease.MethodsThis study describes the recent status, significant predictors, and potential consequences of abnormal mineral metabolism in representative groups of hemodialysis facilities (N = 307) and patients (N = 17,236) participating in the Dialysis Outcomes and Practice Patterns Study (DOPPS) in the United States, Europe, and Japan from 1996 to 2001.ResultsMany patients fell out of the recommended guideline range for serum concentrations of phosphorus (8% of patients below lower target range, 52% of patients above upper target range), albumin-corrected calcium (9% below, 50% above), calcium-phosphorus product (44% above), and intact PTH (51% below, 27% above). All-cause mortality was significantly and independently associated with serum concentrations of phosphorus (RR 1.04 per 1 mg/dL, P = 0.0003), calcium (RR 1.10 per 1 mg/dL, P < 0.0001), calcium-phosphorus product (RR 1.02 per 5 mg2/dL2, P = 0.0001), PTH (1.01 per 100 pg/dL, P = 0.04), and dialysate calcium (RR 1.13 per 1 mEq/L, P = 0.01). Cardiovascular mortality was significantly associated with the serum concentrations of phosphorus (RR 1.09, P < 0.0001), calcium (RR 1.14, P < 0.0001), calcium-phosphorus product (RR 1.05, P < 0.0001), and PTH (RR 1.02, P = 0.03). The adjusted rate of parathyroidectomy varied 4-fold across the DOPPS countries, and was significantly associated with baseline concentrations of phosphorus (RR 1.17, P < 0.0001), calcium (RR 1.58, P < 0.0001), calcium-phosphorus product (RR 1.11, P < 0.0001), PTH (RR 1.07, P < 0.0001), and dialysate calcium concentration (RR 0.57, P = 0.03). Overall, 52% of patients received some form of vitamin D therapy, with parenteral forms almost exclusively restricted to the United States. Vitamin D was potentially underused in up to 34% of patients with high PTH, and overused in up to 46% of patients with low PTH. Phosphorus binders (mostly calcium salts during the study period) were used by 81% of patients, with potential overuse in up to 77% patients with low serum phosphorus concentration, and potential underuse in up to 18% of patients with a high serum phosphorus concentration.ConclusionThis study expands our understanding of the relationship between altered mineral metabolism and outcomes and identifies several potential opportunities for improved practice in this area

Cognitive and functional outcomes are of primary interest in the design of efficacy trials in HIV-associated cognitive impairment. In a longitudinal cohort study, weak associations were found between measures of cognitive performance and commonly used measures of daily functioning (mostly self-report measures) in HIV-infected individuals. Modifications of current functional scales or new functional instruments are needed to assess the clinical relevance of cognitive changes in clinical trials of HIV-associated cognitive impairment
Weak Lensing from Space III: Cosmological Parameters
Weak gravitational lensing provides a unique method to directly map the dark
matter in the universe and measure cosmological parameters. Current weak
lensing surveys are limited by the atmospheric seeing from the ground and by
the small field of view of existing space telescopes. We study how a future
wide-field space telescope can measure the lensing power spectrum and skewness,
and set constraints on cosmological parameters. The lensing sensitivity was
calculated using detailed image simulations and instrumental specifications
studied in earlier papers in this series. For instance, the planned
SuperNova/Acceleration Probe (SNAP) mission will be able to measure the matter
density parameter Omega_m and the dark energy equation of state parameter w
with precisions comparable and nearly orthogonal to those derived with SNAP
from supernovae. The constraints degrade by a factor of about 2 if redshift
tomography is not used, but are little affected if the skewness only is
dropped. We also study how the constraints on these parameters depend upon the
