    An Investigation of Nurses' Knowledge, Attitudes, and Practices Regarding Disinfection Procedures in Italy

    <p>Abstract</p> <p>Background</p> <p>This study assessed the level of knowledge, attitudes, and practice regarding disinfection procedures among nurses in Italian hospitals.</p> <p>Methods</p> <p>A face-to-face interview gathered the following information: demographic and practice characteristics; knowledge about the healthcare-associated infections (HAIs) and the disinfection practices; attitudes towards the utility of guidelines/protocols and perception of the risks of acquiring/transmitting HAIs; compliance with antisepsis/disinfection procedures; and sources of information.</p> <p>Results</p> <p>Only 29% acknowledged that urinary and respiratory tract infections were the two most common HAIs and this knowledge was significantly higher in those with a higher level of education. Attitudes towards the utility of guidelines/protocols for disinfection procedures showed a mean score of 9.1. The results of the linear regression model indicated a more positive attitude in female nurses, in those with a lower number of years of activity, and in those needing additional information about disinfection procedures. Nurses with higher educational level and with a higher perception of risk of transmitting an infectious disease while working were more likely to perform appropriate antisepsis of the surgical wound and handwashing before and after medication.</p> <p>Conclusions</p> <p>Plan of successful prevention activities about HAIs and provide pointers to help optimize disinfection procedures and infection prophylaxis and management are needed.</p

    The effect of cigarette smoking, alcohol consumption and fruit and vegetable consumption on IVF outcomes: A review and presentation of original data

    Background - Lifestyle factors including cigarette smoking, alcohol consumption and nutritional habits impact on health, wellness, and the risk of chronic diseases. In the areas of in-vitro fertilization (IVF) and pregnancy, lifestyle factors influence oocyte production, fertilization rates, pregnancy and pregnancy loss, while chronic, low-grade oxidative stress may underlie poor outcomes for some IVF cases. Methods - Here, we review the current literature and present some original, previously unpublished data, obtained from couples attending the PIVET Medical Centre in Western Australia. Results - During the study, 80 % of females and 70 % of male partners completed a 1-week diary documenting their smoking, alcohol and fruit and vegetable intake. The subsequent clinical outcomes of their IVF treatment such as quantity of oocytes collected, fertilization rates, pregnancy and pregnancy loss were submitted to multiple regression analysis, in order to investigate the relationship between patients, treatment and the recorded lifestyle factors. Of significance, it was found that male smoking caused an increased risk of pregnancy loss (p = 0.029), while female smoking caused an adverse effect on ovarian reserve. Both alcohol consumption (β = 0.074, p < 0.001) and fruit and vegetable consumption (β = 0.034, p < 0.001) had positive effects on fertilization. Conclusion - Based on our results and the current literature, there is an important impact of lifestyle factors on IVF clinical outcomes. Currently, there are conflicting results regarding other lifestyle factors such as nutritional habits and alcohol consumption, but it is apparent that chronic oxidative stress induced by lifestyle factors and poor nutritional habits associate with a lower rate of IVF success

    Clinical and pathological characteristics of Chinese patients with BRCA related breast cancer

    Breast cancers related to BRCA mutations are associated with particular biological features. Here we report the clinical and pathological characteristics of breast cancer in Chinese women with and without BRCA mutations and of carriers of BRCA1 mutations compared to BRCA2 mutations. Two hundred and 26 high-risk Hong Kong Chinese women were tested for BRCA mutations, medical information was obtained from medical records, and risk and demographic information was obtained from personal interviews. In this cohort, 28 (12.4%) women were BRCA mutation carriers and among these carriers, 39.3% were BRCA1 and 60.7% were BRCA2 mutations. Mutation carriers were more likely to have a familial history of breast and ovarian cancer, high-grade cancers, and triple negative (TN) cancers. Prevalence of TN was 48.3% in BRCA carriers and 25.6% in non-carriers and was 67.7% in BRCA1 and 35.3% in BRCA2 carriers. Estrogen receptor (ER) negative cancer was significantly associated with BRCA1 mutations, especially in those under 40 years of age. BRCA-related breast cancer in this Chinese population is associated with family history and adverse pathological/prognostic features, with BRCA2 mutations being more prevalent but BRCA1 carriers having more aggressive and TN cancers.     Clustering of cancer among families of cases with Hodgkin Lymphoma (HL), Multiple Myeloma (MM), Non-Hodgkin's Lymphoma (NHL), Soft Tissue Sarcoma (STS) and control subjects

    <p>Abstract</p> <p>Background</p> <p>A positive family history of chronic diseases including cancer can be used as an index of genetic and shared environmental influences. The tumours studied have several putative risk factors in common including occupational exposure to certain pesticides and a positive family history of cancer.</p> <p>Methods</p> <p>We conducted population-based studies of Hodgkin lymphoma (HL), Multiple Myeloma (MM), non-Hodgkin's Lymphoma (NHL), and Soft Tissue Sarcoma (STS) among male incident case and control subjects in six Canadian provinces. The postal questionnaire was used to collect personal demographic data, a medical history, a lifetime occupational history, smoking pattern, and the information on family history of cancer. The family history of cancer was restricted to first degree relatives and included relationship to the index subjects and the types of tumours diagnosed among relatives. The information was collected on 1528 cases (HL (n = 316), MM (n = 342), NHL (n = 513), STS (n = 357)) and 1506 age ± 2 years and province of residence matched control subjects. Conditional logistic regression analyses adjusted for the matching variables were conducted.</p> <p>Results</p> <p>We found that most families were cancer free, and a minority included two or more affected relatives. HL [(OR<sub>adj </sub>(95% CI) <b>1.79 (1.33, 2.42)]</b>, MM <b>(1.38(1.07, 1.78))</b>, NHL <b>(1.43 (1.15, 1.77)</b>), and STS cases <b>(1.30(1.00, 1.68)) </b>had higher incidence of cancer if any first degree relative was affected with cancer compared to control families. Constructing mutually exclusive categories combining "family history of cancer" (yes, no) and "pesticide exposure ≥10 hours per year" (yes, no) indicated that a positive family history was important for HL <b>(2.25(1.61, 3.15))</b>, and for the combination of the two exposures increased risk for MM <b>(1.69(1.14,2.51))</b>. Also, a positive family history of cancer both with <b>(1.72 (1.21, 2.45)) </b>and without pesticide exposure <b>(1.43(1.12, 1.83)) </b>increased risk of NHL.</p> <p>Conclusion</p> <p>HL, MM, NHL, and STS cases had higher incidence of cancer if any first degree relative affected with cancer compared to control families. A positive family history of cancer and/or shared environmental exposure to agricultural chemicals play an important role in the development of cancer.</p

    A produção de plosivas por crianças de três anos falantes do português brasileiro Infant voices: the production of plosives by three-year-olds

    OBJETIVO: apresentar as características fonético-acústicas de produção das consoantes plosivas vozeadas e não-vozeadas por crianças de três anos. MÉTODOS: análise qualitativa baseada na inspeção das características fonético-acústicas de eventos fônicos ocorridos no intervalo de obstrução, na liberação da plosão e na transição para a vogal e análise quantitativa das medidas do Voice Onset Time (VOT), de três repetições das seis consoantes plosivas do Português Brasileiro (PB), por quatro crianças, sendo três meninos e uma menina, falantes nativos do PB, por meio de análise fonético-acústica, em espectrograma de banda larga. Os achados foram comparados com estimativas de valores para a faixa adulta de falante nativo do PB. RESULTADOS: as crianças apresentaram produções semelhantes às que são encontradas na fala adulta, tanto para as plosivas vozeadas como para as não-vozeadas, bilabiais, alveolares e velares. Apresentaram também plosivas vozeadas com interrupção de barra de sonoridade e ocorrência de VOT positivo, e qualidade diferenciada de burst. Para as não-vozeadas, ocorreram a presença da breathy vowel no período de obstrução das plosivas e a presença de aspiração no segmento correspondente ao VOT positivo, para os três pontos de articulação pesquisados. CONCLUSÃO: as diferenças encontradas na produção das plosivas vozeadas, quanto às características da barra de sonoridade e qualidade do burst, a presença da breathy vowel nas plosivas não-vozeadas e aspiração, caracterizam a produção das plosivas nessa faixa etária. A diversidade das medidas acústicas, verificada pelos valores dos desvios-padrão sugerem que as crianças realizaram diversas tentativas, com diferentes ajustes para produzir tais sons.<br>PURPOSE: to present the phonetic-acoustic features of the production of both voiced and voiceless plosives by three-year-old children. METHODS: qualitative analysis based on the inspection of phonetic-acoustic characteristics of phonic events occurring in the obstruction interval, in the burst, and in the transition to the vowel as well as quantitative analysis of the measures of the voice onset time (VOT), of three repetitions of the six plosive consonants of Brazilian Portuguese (BP) by four native-speaker children in the three-year-old age bracket, three boys and one girl, by means of an acoustic-phonetic analysis, in a wide-band spectrogram. The findings were compared to estimates of values for the adult group of native speakers of Brazilian Portuguese (BP). RESULTS: the children showed production of plosives with VOT similar to that of adult speech both for voiced and unvoiced plosives, bilabials, alveolars and velars. They also showed productions dissimilar to adults, such as voiced plosives with interruption of the voice bar and positive VOT, besides a particular quality of burst. As regards the unvoiced plosives, there were episodes of breathy vowel during obstruction as well as of aspiration in the segment corresponding to positive VOT, in the three articulation points under study. CONCLUSION: differences found in the production of voiced plosives as regards spectral features of the voice bar and burst quality as well as the occurrence of breathy vowel in unvoiced plosives along with aspiration, characterize the production of plosives in this age bracket. The diversity of acoustic measures, observed by the values of shunting line-standards, suggest that children made several attempts, with various degrees of adjustment, to produce such sounds
