    STRETCHing HIV treatment: A Replication Study of Task Shifting in South Africa

    The Streamlining Tasks & Roles to Expand Treatment and Care for HIV (STRETCH) program was developed to increase the reach of antiretroviral therapy (ART) for HIV/AIDS patients in Sub-Saharan Africa by training nurses to prescribe, initiate, and maintain ART. Fairall and colleagues conducted a cluster-randomized trial to determine the effects/impact of STRETCH on patient health outcomes in South Africa between 2008 and 2010. The purpose of our replication study is to evaluate Fairall and colleagues\u27 findings. We conducted push button and pure replication studies and measurement and estimation analyses (MEA). Our MEA validates the original findings: (1) overall, time to death did not differ between intervention (STRETCH) and control (ART) patients; (2) in a subgroup analysis of patients with CD4 counts of 201-350 cells per μL, the intervention group patients had a 30% lower risk of death than those in the control group, when controlling for baseline characteristics; (3) in a subgroup analysis of patients with CD4 counts of ≤200 cells per μL, time to death did not differ between the two groups; and (4) rates of viral suppression one year after enrollment did not differ between the intervention and control groups. This set of results have more caveats in the MEA. Although the intervention did not lead to improvements in the main outcomes, the effectiveness of STRETCH was proven to be similar to standard care while increasing the pool of prescribers, expanding their geographical range, and improving the quality of care for patients. Therefore, our analyses support the implementation of task shifting of antiretroviral therapy from doctors to trained nurses, which enhances confidence in the implementation of the intervention program and policymaking not only in South Africa but also in other developing countries that have similar circumstances

    Zero-Inflated Models for RNA-Seq Count Data

    One of the main objectives of many biological studies is to explore differential gene expression profiles between samples. Genes are referred to as differentially expressed (DE) if the read counts change across treatments or conditions systematically. Poisson and negative binomial (NB) regressions are widely used methods for non-over-dispersed (NOD) and over-dispersed (OD) count data respectively. However, in the presence of excessive number of zeros, these methods need adjustments. In this paper, we consider a zero-inflated Poisson mixed effects model (ZIPMM) and zero-inflated negative binomial mixed effects model (ZINBMM) to address excessive zero counts in the NOD and OD RNA-seq data respectively in the presence of random effects. We apply these methods to both simulated and real RNA-seq datasets. The ZIPMM and ZINBMM perform better on both simulated and real datasets

    Fingertips detection for human interaction system

    Fingertips of human hand play an important role in hand-based interaction with computers. Identification of fingertips\u27 positions in hand images is vital for developing a human computer interaction system. This paper proposes a novel method for detecting fingertips of a hand image analyzing the concept of the geometrical structural information of fingers. The research is divided into three parts: First, hand image is segmented for detecting hand, Second, invariant features (curvature zero-crossing points) are extracted from the boundary of the hand, Third, fingertips are detected. Experimental results show that the proposed approach is promising

    Mobile Banking: The Bangladesh Experience

    This study is about the Mobile Banking in Bangladesh. Major objectives of this study are to give an overview about cost, usage and all other benefits of Mobile Banking as well as to produce a scenario of Mobile Banking in Bangladesh. Both the primary and secondary data have been used for the study purpose.  The major findings of this study are 94% respondents think that it saves time than traditional banking, the highest number of respondents use Mobile Banking for ‘Air-time top-up’ service, out of 50 respondents 92% have replied it is less costlier than traditional banking, 100% respondents have agreed that it is speedy, and 100% respondents have opined any type of set can be used for Mobile Banking. Besides, 64% respondents have mentioned that DBBL plays a significant role in mobile baking sector, whereas 28% respondents have agreed that Bkash performs a vital role. Although this concept is new in Bangladesh but its potentiality is high and already it has started to contribute in the economy significantly.     Statistical Modeling of the Number of Deaths of Children in Bangladesh

    Efforts to reduce the number of children’s death in developing countries through health care programs focus more to the prevention and control of diseases than to determining the underlying risk factors/predictors and addressing these through proper interventions. This study aims to identify socioeconomic and demographic predictors of the number of children’s death to women aged 12-49 from the Bangladesh Health and Demographic Survey (BDHS) administered in 2011. The number of children’s death in a family is a non-negative count response variable. The average number of children’s death is found to be 28 per 100 women with a variance of 44per 100 women. Thus Poisson regression model is not a proper choice to predict the mean response from the BDHS data due to the presence of over-dispersion. In order to address over-dispersion, we fit a Negative Binomial Regression (NBR), a Zero-Inflated Negative Binomial Regression (ZINBR) and a Hurdle Regression (HR) model. Among these models, ZINBR fits the data best. We identify respondent’s age, respondent’s age at 1st birth, gap between 1st birth and marriage, number of family members, region, religion, respondent’s education, husband’s education, incidence of twins, source of water, and wealth index as significant predictors for the number of children’s death in a family from the best fitted model. Identification of the risk factors of the number of children’s death is an important public health issue and should be carried out correctly for the much needed intervention
