    A Mixed Methods Study Of The Implementation Of Collaborative Technology Tools For Enhancing Collaboration And Student Engagement In Online Learning: Faculty Experiences And Student Perspectives

    The appropriate implementation of collaborative technology tools in online courses leads to a culture of social learning where technology empowers students to take central roles in their learning. Yet, critical questions still exist about how faculty design, develop, implement collaborative eLearning activities using technology tools that support collaboration and student engagement in online courses, and what perspectives students have toward their experiences while participating in these activities. The purpose of the study is to explore the experiences of faculty members implementing collaborative technology tools in online courses to support collaboration and student engagement, in addition, to obtain the perspectives of students toward their experiences while participating in these activities. The study attempts to better understand the potential and use of technology for enhancing collaboration and student engagement in online settings and the factors that influence the selection of collaborative technology tools for incorporating collaborative eLearning activities in online courses. An explanatory sequential mixed methods approach was utilized to collect data from a total of 210 faculty and student participants who met the participation criteria and volunteered to participate in the study at a large Midwestern state university. Out of the 210 participants, 29 faculty members and 181 students were surveyed, and after a review of the results, follow-up interviews were conducted with four faculty members and two students. The findings of this study confirmed that collaborative technology tools have the potential to create a virtual collaborative environment that enables instructors to establish a learning community within online courses where students can synchronously or asynchronously work together toward a common task, in which each student adds to an emerging pool of knowledge of the group. This study provides evidence that the use of collaborative technology tools positively affects studentsā€™ experiences with collaborative eLearning activities in online learning. The instructor\u27s ability to successfully select and implement collaborative technology tools that effectively support collaborative eLearning and student engagement in online courses is a primary concern. This concern raises the demand for online instructors who are well-prepared and fully-supported to integrate collaborative technology tools into online settings and design eLearning activities that engage students and foster interaction and collaboration. Possible implications of the study and practical recommendations drawn from the findings of the study for professional and meaningful practice are discussed

    The effect of COVID-19 on the characteristics of adult emergency department visits:A retrospective cohort tertiary hospital experience in Riyadh

    BACKGROUND: On March 2, 2020, Saudi Arabia identified the first positive COVID-19 case. Since then, several aspects of the COVID-19 impact on Emergency Departments (EDs) use have been reported. The objective of this study is to describe the pattern and characteristics of Emergency Department visits during the COVID-19 pandemic period, compared with the same period in the previous year, including the patientsā€™ demographic information, acuity level, length of stay, and admission rate. METHODS: Data were collected from King Abdulaziz Medical City in Riyadh, Saudi Arabia. The health records of all the patients who presented at the Emergency Department from January 2019 to September 2020 were retrospectively reviewed. The variations in the patient and the visit characteristics were described for the periods before and during COVID-19. RESULTS: The records of 209,954 patients who presented at the Emergency Department were retrieved. In contrast to 2019, the number of visits during the pandemic period reduced by 23%. A dramatic decrease was observed after the announcement of the first COVID-19 diagnosed case in Saudi Arabia, and subsequently the numbers gradually increased. The patients who presented at the Emergency Department during the pandemic period were slightly older (mean age, 43.1 versus 44.0 years), more likely to be older, more urgent and had a higher admission rate compared to the pre-pandemic period. There was a slight increase in visits during the daytime curfew hours and a decrease during the nighttime. CONCLUSION: We report a considerable decrease in the number of Emergency Department visits. The reduction was higher in non-urgent and less urgent cases. Patients presenting at the Emergency Department during the curfew times were more likely to stay longer in the Emergency Department and more likely to be admitted, compared with the pre-pandemic period

    Oralne manifestacije u pedijatrijskih bolesnika zaraženih virusom humane imunodefi cijencije na antiretrovirusnoj terapiji i bez nje: presječno istraživanje

    The aim of the study was to assess and compare oral manifestations of human immunodefi ciency virus (HIV) infected pediatric patients undergoing antiretroviral therapy (ART) and those not undergoing ART. A cross sectional study included HIV positive children (receiving and not receiving ART) aged 5-15 years, registered at the District Hospital ART Centre, Udaipur, and HIV negative schoolchildren. HIV related oral lesions were diagnosed according to the World Health Organization criteria. Sociodemographic and other related information were also recorded. CD4+ cell count was determined in all study subjects. The Ļ‡2 -test, stepwise multiple linear regression and logistic regression were used on statistical analysis. In all tests, confi dence interval and p-value were set at 95% and ā‰¤0.05, respectively. A greater proportion of HIV patients receiving treatment had CD4+ cell count of more than 750 cells/mm3 . The majority of HIV patients receiving ART for more than three years and only 20% of those not receiving ART were free from any oral lesions. The results of the present study demonstrated ART to be eff ective in reducing the prevalence of HIV related oral lesions, as already described in the literature; however, unlike previous studies, the present paper reports more valid fi ndings for having included most of the confounding variablesCilj istraživanja bio je procijeniti i usporediti oralne manifestacije u pedijatrijskih bolesnika zaraženih virusom humane imunodefi - cijencije (HIV) koji primaju antiretrovirusnu terapiju (ART) i onih koji ne primaju ovu terapiju. Ovo presječno istraživanje uključilo je HIV pozitivnu djecu (djecu koja primaju ART i djecu koja ne primaju ART) u dobi od 5-15 godina, registriranu pri Centru za ART u Pokrajinskoj bolnici u Udaipuru te HIV negativnu Å”kolsku djecu. Oralne promjene povezane s HIV-om dijagnosticirane su prema kriterijima Svjetske zdravstvene organizacije. Prikupljeni su socio-demografski te ostali podaci za svu djecu uključenu u ispitivanje. U svih je određen broj CD4+ stanica. U statističkoj analizi primijenjen je Ļ‡2 -test te postupna viÅ”estruka linearna regresija i logistička regresija. U svim testovima interval vjerodostojnosti i vrijednost p utvrđeni su na razini od 95% odnosno ā‰¤0,05. Broj CD4+ stanica veći od 750 stanica/mm3 zabilježen je u većem postotku HIV bolesnika na ART. Bez oralnih promjena bila je većina HIV bolesnika koji su primali ART duže od tri godine, ali samo 20% onih koji nisu primali ART. Rezultati ovoga istraživanja pokazuju da ART učinkovito snižava učestalost oralnih promjena povezanih s HIV-om, Å”to je već prije opisano u literaturi. Međutim, za razliku od prijaÅ”njih ispitivanja, nalazi prikazani u ovom radu imaju veću vrijednost, jer uključuju većinu čimbenika koji bi mogli utjecati na rezultate

    Acute Appendicitis among Saudi and Non-Saudi Patients: A Cross-Sectional Based Study

    Objective: We conduct this study to discuss the differences between Saudi and non- Saudi patients with acute appendicitis.Background: Acute appendicitis is one of emergency surgeries in developing and developed countries. Its symptoms are vomiting, lower abdominal pain and decreased appetite. Appendicitis needs urgent surgical prouder to avoid its perforation and associated complications which may lead to death. Method: We conduct cross-sectional based study in one of khamis Mushayt, Saudi Araba. 136 patients diagnosed with acute appendicitis were included and their medical records were reviewed after getting their informed consent.Results: We included 136 patients, 90 were non-Saudi and 46 were Saudi. There were no statistically differences in their diagnosis but the distribution of the diagnosis was different.Conclusion: Acute appendicitis was more prevalent among non-Saudi patients, the diagnosis between both was with no significant differences.     Imbalanced Prostanoid Release Mediates Cigarette Smoke-Induced Human Pulmonary Artery Cell Proliferation

    BackgroundPulmonary hypertension is a common and serious complication of chronic obstructive pulmonary disease (COPD). Studies suggest that cigarette smoke can initiate pulmonary vascular remodelling by stimulating cell proliferation; however, the underlying cause, particularly the role of vasoactive prostanoids, is unclear. We hypothesize that cigarette smoke extract (CSE) can induce imbalanced vasoactive prostanoid release by differentially modulating the expression of respective synthase genes in human pulmonary artery smooth muscle cells (PASMCs) and endothelial cells (PAECs), thereby contributing to cell proliferation.MethodsAqueous CSE was prepared from 3R4F research-grade cigarettes. Human PASMCs and PAECs were treated with or without CSE. Quantitative real-time RT-PCR and Western blotting were used to analyse the mRNA and protein expression of vasoactive prostanoid syhthases. Prostanoid concentration in the medium was measured using ELISA kits. Cell proliferation was assessed using the cell proliferation reagent WST-1.ResultsWe demonstrated that CSE induced the expression of cyclooxygenase-2 (COX-2), the rate-limiting enzyme in prostanoid synthesis, in both cell types. In PASMCs, CSE reduced the downstream prostaglandin (PG) I synthase (PGIS) mRNA and protein expression and PGI2 production, whereas in PAECs, CSE downregulated PGIS mRNA expression, but PGIS protein was undetectable and CSE had no effect on PGI2 production. CSE increased thromboxane (TX) A synthase (TXAS) mRNA expression and TXA2 production, despite undetectable TXAS protein in both cell types. CSE also reduced microsomal PGE synthase-1 (mPGES-1) protein expression and PGE2 production in PASMCs, but increased PGE2 production despite unchanged mPGES-1 protein expression in PAECs. Furthermore, CSE stimulated proliferation of both cell types, which was significantly inhibited by the selective COX-2 inhibitor celecoxib, the PGI2 analogue beraprost and the TXA2 receptor antagonist daltroban.ConclusionsThese findings provide the first evidence that cigarette smoke can induce imbalanced prostanoid mediator release characterized by the reduced PGI2/TXA2 ratio and contribute to pulmonary vascular remodelling and suggest that TXA2 may represent a novel therapeutic target for pulmonary hypertension in COPD

    Pattern of Strabismus in Children and Adolescents in Hail, KSA

    Background: Strabismus (Squint), abnormal ocular alignment could occur constantly or intermittently. Double vision, poor vision or abnormal head positioning may accompany it. A child with squint may stop using the affected eye. This can lead to visual loss, which can become permanent unless treated early in childhood. Objective: to estimate the prevalence of squint, types and treatment characteristics in the studied children and adolescents in Hail city, Saudi Arabia. Methods: A cross-sectional study conducted in Hail city, Saudi Arabia. The study included 299 participants; 148 male and 151 female children and adolescents from 6 months to 19 years. The study period was from 1 January to 30 May 2018. Data collected by personal interview using a pre-designed questionnaire, which distributed among mothers of children and adolescents to be self-reported. Results: Squint found in 17.1% of the studied sample. Squint was right sided in 37.3% of the cases, left sided in also 37.3% and in both eyes in 25.5% of the studied cases. About half (47.1%) of cases had inward squint (esotropia) and 15.7% outward squint (exsotropia), 21.6% of the cases had Intermittent squint and 52.2% had permanent squint. In most (70.7%) of cases, squint affected the visual acuity. As regards treatment, 45.1% received medical treatment and 13.7% received surgical treatment. Only 19.6% of cases completely cured and 39.2% had recurrence. There was insignificant relation with sex, squint in parents, other hereditary diseases and consanguinity between parents (P>0.05). Conclusion: in this study, the prevalence of squint in the studied children and adolescents in Hail city, Saudi Arabia was 17.1%. No significant difference between males and females. After treatment, only 19.6% of cases completely cured and 39.2% had recurrence. Health education of the public about importance of early treatment is mandatory. Furthermore, the article evaluates various treatment options, from pharmacological interventions to psychotherapy, and their applicability in primary care settings. The integration of technological advancements, such as electronic health records and telemedicine, is also discussed in the context of enhancing patient care. The review concludes by highlighting the importance of educating primary care physicians to not only pay close attention to the mental health of their patients, but also to be able to accurately screen and diagnose mental health issues using various diagnostic tools. Moreover, it is critical to develop more useful tools that can be used in the context of primary care settings

    Introduction: Antipsychotic agents are categorized usually as either ā€œtypical antipsychoticsā€ or ā€œatypical antipsychoticsā€. Typical antipsychotic pharmacological agents, which are also called first-generation antipsychotics or traditional antipsychotics, include chlorpromazine and haloperidol. On the other hand, atypical antipsychotic pharmacological agents, which are also called second generation antipsychotics, include risperidone, quetiapine, and olanzapine. Both typical and atypical antipsychotics work in similar mechanisms by inhibiting the activation of specific receptors in the dopamine pathway.Aim of work: In this review, we will discuss the antipsychotics- classification, uses, and adverse effects, Methodology: We did a systematic search for antipsychotics- classification, uses, and adverse effects using PubMed search engine (http://www.ncbi.nlm.nih.gov/) and Google Scholar search engine (https://scholar.google.com). All relevant studies were retrieved and discussed. We only included full articles.Conclusions: The prescription of atypical antipsychotic agents, like quetiapine and olanzapine, has been significantly increasing and in many conditions, they are sometimes prescribed ā€œoff-labelā€ ā€“ which is a worrying trend due to their possible risk of developing harmful adverse events. Therefore, atypical antipsychotic agents must only be used in their specific indications and with caution, especially among older patients and young adults. Atypical antipsychotic agents can be indicated for schizophrenia treatment and other associated conditions and in some cases to treat the behavioral and psychological manifestations related to dementia (risperidone only). Antipsychotics are not first-line treatments for patients with anxiety and are not generally recommended for the use in patients with posttraumatic stress disorder or insomnia.     Overview on Juvenile Primary Fibromyalgia Syndrome

    JPFS (juvenile primary fibromyalgia syndrome) is a musculoskeletal pain illness that affects children and adolescents. The intricacy of the clinical picture in JPFS has not been adequately characterized in the literature. JFMS symptoms are sometimes difficult to compare to adult fibromyalgia syndrome since many of them are "medically unexplained" and frequently overlap with other medical disorders.  The etiology of the illness is multifaceted, with impaired central pain processing being a significant contributor. Musculoskeletal pain that is severe and pervasive is the defining symptom. Other signs and symptoms include headaches, stiffness, subjective joint swelling, sleep and mood disorders, and headaches. Multiple sensitive spots might be found during a physical examination. The diagnosis has certain criteria and is clinical. Early detection and treatment are crucial. The gold standard of care combines a variety of modalities, but most significantly, exercise and cognitive behavioral therapy. The outlook varies, and symptoms might last well into adulthood. Discussing the epidemiology, etiology, pathophysiology, clinical symptoms, diagnosis, and management of JPFS is the goal of the review

    Perspectives, practices, and challenges of online teaching during COVID-19 pandemic: A multinational survey

    The result of the movement restrictions during the COVID-19 pandemic was an impromptu and abrupt switch from in-person to online teaching. Most focus has been on the perception and experience of students during the process. The aim of this international survey is to assess staffs' perspectives and challenges of online teaching during the COVID-19 lockdown. Cross-sectional research using a validated online survey was carried out in seven countries (Brazil, Saudi Arabia, Jordan, Indonesia, India, the United Kingdom, and Egypt) between the months of December 2021 and August 2022, to explore the status of online teaching among faculty members during the COVID-19 pandemic. Variables and response are presented as percentages while logistic regression was used to assess the factors that predict levels of satisfaction and the challenges associated with online instruction. A total of 721 response were received from mainly male (53%) staffs. Most respondents are from Brazil (59%), hold a Doctorate degree (70%) and have over 10 years of working experience (62%). Although, 67% and 79% have relevant tools and received training for online teaching respectively, 44% report that online teaching required more preparation time than face-to-face. Although 41% of respondents were uncertain about the outcome of online teaching, 49% were satisfied with the process. Also, poor internet bandwidth (51%), inability to track students' engagement (18%) and Lack of technical skills (11.5%) were the three main observed limitations. Having little or no prior experience of online teaching before the COVID-19 pandemic [OR, 1.58 (95% CI, 1.35ā€“1.85)], and not supporting the move to online teaching mode [OR, 0.56 (95% CI,0.48ā€“0.64)] were two main factors independently linked with dissatisfaction with online teaching. While staffs who support the move to online teaching were twice likely to report no barriers [OR, 2.15 (95% CI, 1.61ā€“2.86)]. Although, relevant tools and training were provided to support the move to online teaching during COVID-19 lockdown, barriers such as poor internet bandwidth, inability to track studentsā€™ engagement and lack of technical skills were main limitations observed internationally by teaching staffs. Addressing these barriers should be the focus of higher education institution in preparation for future disruptions to traditional teaching modes
