    Childbirth care practices in public sector facilities in Jeddah, Saudi Arabia: A descriptive study

    Objectives: To explore reported hospital policies and practices during normal childbirth in maternity wards in Jeddah, Saudi Arabia, to assess and verify whether these practices are evidence-based. Design: Quantitative design, in the form of a descriptive questionnaire, based on a tool extracted from the literature. Setting: Nine government hospitals in Jeddah, Saudi Arabia. These hospitals have varied ownership, including Ministry of Health (MOH), military, teaching and other government hospitals. Participants: Key individuals responsible for the day-to-day running of the maternity ward. Measurements: Nine interviews using descriptive structured questionnaire were conducted. Data were analysed using SPSS for Windows (version 16.0). Findings: The surveyed hospitals were found to be well equipped to deal with obstetric emergencies, and many follow evidence-based procedures. On average, the Caesarean section rate was found to be 22.4%, but with considerable variances between hospitals. Some unnecessary procedures that are known to be ineffective or harmful and that are not recommended for routine use, including pubic shaving, enemas, episiotomy, electronic foetal monitoring (EFM) and intravenous (IV) infusion, were found to be frequently practiced. Only 22% of the hospitals sampled reported allowing a companion to attend labour and delivery. Key Conclusions: Many aspects of recommended EBP were used in the hospitals studied. However, the results of this study clearly indicate that there is wide variation between hospitals in Jeddah, Saudi Arabia in some obstetric practices. Furthermore, the findings suggest that some practices at these hospitals are not supported by evidence as being beneficial for mothers or babies and are positively discouraged under international guidelines. Implications for practice: This study has specific implications for obstetricians, midwives and nurses working in maternity Units. It gives an overview of current hospital policies and practices during normal childbirth. It is likely to contribute to improving the health and well-being of women, and have implications for service provision. It could also help in the development of technical information for policy-makers, and health care professionals for normal childbirth care

    The place and barriers of evidence based practice: knowledge and perceptions of medical, nursing and allied health practitioners in malaysia

    <p>Abstract</p> <p>Background</p> <p>Despite a recent increase in activities to promote evidence-based practice (EBP), it was unclear how Malaysian hospital practitioners received this new approach in medicine. This study examines their confidence and perceptions on EBP.</p> <p>Findings</p> <p>We conducted cross-sectional surveys using a self-administered questionnaire during two EBP training courses in two Malaysian hospitals in January and June 2007. Our subjects (n = 144) were doctors and nursing and allied health staff (NAH) participating in the EBP courses. Our questionnaire covered three domains: confidence and understanding (six items), attitude (five items) and barriers to practice (four items). We presented simple descriptive statistics, including the sum ratings and the proportions with different responses for each item, and compared different groups using Mann-Whitney U test for scaled ratings and Chi-square test for dichotomous responses.</p> <p>Ninety-two doctors and 52 NAH staff completed the surveys. Overall, doctors expressed slightly higher confidence on EBP compared to NAH staff. Out of a maximum sum rating of 27 over six items, doctors reported an average of 18.3 (SD 3.2) and NAH staff reported an average of 16.0 (SD 3.4), p = 0.002. Doctors were also more positive in their views on EBP. For example, 67.4% of doctors disagreed, but 61% of NAH staff agreed that "the importance of EBP in patient care is exaggerated", and 79.3% of doctors disagreed, but 46.2% of NAH staff agreed that "EBP is too tedious and impractical". Similar responses were observed for other items in the domain.</p> <p>Doctors and NAH staff shared similar concerns on barriers to evidence-based practice. The highest proportions considered poor facilities to access evidence a barrier (76% of doctors and 90% of NAH), followed by poor awareness of evidence (62% of doctors and 70% of NAH) and time constraints (63% of doctors and 68% of NAH), p = 0.09 for the combined rating of four items in the domain.</p> <p>Conclusions</p> <p>The findings of our survey suggest a need for greater efforts in promoting EBP among Malaysian hospital practitioners especially for NAH staff. From the responses based on the barriers to EBP, improving facilities for accessing evidence and promoting more user-friendly resources to address time constraints appear to be the priorities.</p

    Knowledge of healthy diets among adolescents in eastern Saudi Arabia.

    BACKGROUND : Bad dietary habits, such as eating high-fat/high-energy food, can contribute to obesity in adolescents, which tends to persist into adulthood. The objective of this study was to determine the level and sources of knowledge about foods and healthy diets among male and female adolescents. METHODS : This was a cross-sectional study of a sample of male (n=1240) and female (n=1331) adolescents from third grade, intermediate and all three grades of secondary school students in the Al-Khobar Area, eastern province of Saudi Arabia. A self-administered questionnaire was used to collect information. RESULTS : Approximately 51&#x0025; of the male and 65&#x0025; of the female students recognized unsaturated fats as healthy foods. However, 10&#x0025; of the males and 8&#x0025; of females reported saturated fats as healthy food items. About 49&#x0025; of the males and 66&#x0025; of the females correctly defined cholesterol. Dietary knowledge of both male and female students on the dangers of unhealthy foods and the benefits of fiber-rich diets was found to be unsatisfactory. The main sources of knowledge about health and disease reported by the male and female respondents were television (58&#x0025; and 61&#x0025;, respectively), magazines (31&#x0025; and 39&#x0025;) and daily newspaper (33&#x0025; and 34&#x0025;). Primary health care centers (PHCCs) staff were the least source of knowledge (17&#x0025; and 16&#x0025;). CONCLUSIONS : Knowledge of healthy diets among school students was inadequate. It is recommended that health education and information about healthy eating habits and lifestyle be included in school curricula

    This case report discusses osteotome sinus floor elevation (OSFE) and immediate placement in 2 stages in severely resorbed alveolar bone height in which multiple implant placement is not otherwise feasible due to a lack of initial stability. The first implant placed using OSFE without bone grafting prepares the adjacent resorbed sites for further implant placement in the sinus areas, which allows for better initial stability and early functional loading. This process avoids the conventional extensive lateral approach for sinus lifting and bone grafting procedures even in extremely resorbed alveolar bone

    Nigella sativa: A potential natural protective agent against cardiac dysfunction in patients with type 2 diabetes mellitus

    Objectives: To study the effect of Nigella sativa supplementation on cardiac functions in Type 2 diabetic patients treated with oral hypoglycemic agents. Background: Diabetes mellitus is associated with a high risk of cardiovascular morbidity and mortality. A number of reported beneficial effects of N. sativa on cardiovascular function were the inspiration for this study. Materials and Methods: Sixty patients with uncontrolled diabetes (hemoglobin A 1c [HbA 1c ] >7%) and with no known cardiovascular complications were recruited from the outpatient diabetes clinic. They were assigned, by convenience, to two groups; the control group received activated charcoal as placebo while the test group received 2 g/day of powdered N. sativa for 1-year. All patients continued with their standard oral hypoglycemic agents. Echocardiography was used to evaluate the diastolic function, systolic function, and left ventricular mass (LVM) before the intervention and after 6 and 12 months of the treatment. Results: HbA 1c decreased significantly in the N. sativa group but did not change in the control group. Echocardiographic assessment in the control group showed impairment in diastolic function after 12 months, but there were no significant changes in fractional shortening (FS) or ejection fraction (EF). Furthermore, left ventricular (LV) dimensions at diastole and systole, LVM, and LVM index were significantly increased. In N. sativa group, no significant changes were found in diastolic function or LVM. LV dimension at systole was decreased while FS and EF were significantly increased after 6 and 12 months. Conclusion: N. sativa supplementation may protect the hearts of type 2 diabetic patients from diastolic dysfunction while improving LV systolic function

    Nigella sativa Improves Glycemic Control and Ameliorates Oxidative Stress in Patients with Type 2 Diabetes Mellitus: Placebo Controlled Participant Blinded Clinical Trial

    <div><p>Background and Objective</p><p>Oxidative stress plays an important role in pathogenesis of diabetes mellitus and its complications. Our previous study has shown glucose lowering effect produced by 3 months supplementation of Nigella sativa (NS) in combination with oral hypoglycemic drugs among type 2 diabetics. This study explored the long term glucose lowering effect (over one year) of NS in patients with type 2 diabetes mellitus on oral hypoglycemic drugs and to study its effect on redox status of such patients.</p><p>Methods</p><p>114 type 2 diabetic patients on standard oral hypoglycemic drugs were assigned into 2 groups by convenience. The control group (n = 57) received activated charcoal as placebo and NS group (n = 57) received 2g NS, daily, for one year in addition to their standard medications. Fasting blood glucose (FBG), glycosylated hemoglobin (HbA<sub>1c</sub>), C- peptide, total antioxidant capacity (TAC), superoxide dismutase (SOD), catalase (CAT), glutathione and thiobarbituric acid reactive substances (TBARS) at the baseline, and every 3 months thereafter were determined. Insulin resistance and β-cell activity were calculated using HOMA 2 calculator.</p><p>Results</p><p>Comparison between the two groups showed a significant drop in FBG (from 180±5.75 to 180±5.59 in control Vs from 195±6.57 to 172 ±5.83 in NS group), HbA1c (from 8.2±0.12 to 8.5±0.14 in control VS from 8.6±0.13 to 8.2±0.14 in NS group), and TBARS (from 48.3±6.89 to 52.9 ±5.82 in control VS from 54.1±4.64 to 41.9 ±3.16 in NS group), in addition to a significant elevation in TAC, SOD and glutathione in NS patients compared to controls. In NS group, insulin resistance was significantly lower, while β-cell activity was significantly higher than the baseline values during the whole treatment period.</p><p>Conclusion</p><p>Long term supplementation with Nigella sativa improves glucose homeostasis and enhances antioxidant defense system in type 2 diabetic patients treated with oral hypoglycemic drugs.</p><p>Trial Registration</p><p>Clinical Trials Registry-India (CTRI) <a href="https://clinicaltrials.gov/ct2/show/CTRI/2013/06/003781" target="_blank">CTRI/2013/06/003781</a></p></div
