    Synthesis, Characterization, Antimicrobial Evaluation, and Computational Investigation of Substituted Imidazo[2,1-b][1,3,4]Thiadiazole Derivatives

    In this study, a novel series of 2,6-disubstituted and 2,5,6-trisubstituted imidazo[2,1-b][1,3,4]thiadiazole derivatives were synthesized starting from 2-amino-1,3,4-thiadiazole derivatives. Structures of the synthesized compounds were characterized using various analysis techniques. Then, in vitro biological activity tests were carried out for all synthesized compounds and they were found to show moderate to good activity against all bacteria and fungi tested. Next, molecular docking simulations were performed to observe the inhibition effect of the synthesized compounds on the 3R9C receptor and support their biological activity results. Finally, the pharmacokinetic, ADME and toxicity properties of all compounds were examined using FAF-Drugs and ProTox webservers and it was concluded that they had acceptable toxicity and ADME properties. © 2020 Wiley-VCH Gmb

    WOS: 000376564200037Non-traumatic elbow pain is rarely seen in children and pre-adolescents. Osteochondral lesions may be the source of chronic elbow pain, swelling, and loss of motion in children or adolescents. Osteochondritis dissecans (OCD) is described as a lesion of subchondral bone resulting in separation of the articular cartilage and subchondral bone. It is found primarily in the knee, ankle, and elbow joints. Since osteochondrosis of the elbow primarily involves capitellum, few papers involving osteonecrosis of the trochlea have been reported. This paper discusses a pre-adolescent boy with clinical and radiographic signs consistent with unilateral osteochondral lesion of the trochlea humeri, with no history of recent trauma. The patient had insidious onset of right elbow pain during daily activities for the last 3 weeks. After usage of long arm splint for 2 weeks, persistence of the symptoms necessitated MRI of the affected elbow. After the diagnosis, non-operative management was achieved. Care should be taken for the affected children to recognize any residual deformity and to treat it properly at follow up.Travma olmaksızın dirsek ağrısı çocuklarda ergenlik öncesi dönemde nadir görülür. Bu dönemde kronik dirsek ağrısı, şişlik ve eklem hareket kaybı osteokondral lezyonlara bağlı olabilir. Osteokondritis dissekans, subkondral kemik ile eklem kıkırdağının subkondral kemikteki bir lezyona bağlı ayrışması olarak tanımlanır. Özellikle diz, ayak bileği ve dirsek eklemlerinde görülür. Dirsek osteokondrozunda birincil olarak kapitellum etkilenmekteyken, troklea osteonekrozu ile ilgili az sayıda olgu bildirilmiştir. Bu yazıda yakın zamanda travma öyküsü olmayan, ergenlik öncesi dönemde bulunan, klinik ve radyolojik olarak tek taraflı troklea osteokondral lezyonu ile uyumlu olan bir olgu sunulmuştur. Hastada son 3 haftadır daha önce belirti vermeyen, günlük aktivitelerde ortaya çıkan sağ dirsek ağrısı mevcuttu. 2 hafta uzun kol atelde takip edilen hastada şikayetlerin devamı üzerine dirsek MR görüntülemesine ihtiyaç duyuldu. Tanı konduktan sonra cerrahi dışı tedavi ile başarılı sonuç elde edildi. Çocuklarda rezidüel deformitenin tanınması ve uygun tedavisi için takipte dikkatli olunmalıdır

Menopozda görülen değişiklikler aterojenik lipit profili ile ilişkilidir. Statinlerin kardivasküler hastalıkları azaltmadaki etkinlikleri birçok randomize, plasebo kontrollü çalışmada gösterilmiştir. Bu derlemede, statinlerin menopoz geçirmiş kadınlarda hem kardiyovasküler hastalıklar, hem de bazı önemli endikasyonlardaki etkinlikleri irdelendi.Cardiovascular disease is one of the leading causes of mortality and morbidity in postmenopausal women. Menopausal changes have been shown to be related with an atherogenic lipid profile. The efficiency of statins in reducing the incidence of cardiovascular diseases has been well-documented in a variety of randomized, placebo-controlled trials. This review outlines the effectiveness of statins both in cardiac events and in some noticeable indications in postmenopausal women

    Gouty arthritis at interphalangeal joint of foot after sildenafil use: A case report

    INTRODUCTION: Interphalangeal joint of foot is a very unusual location for gouty arthritis and sildenafil use may cause this phenomenon. PRESENTATION OF CASE: A 58-year-old hypertensive man was admitted to hospital with pain and swelling over interphalengeal joint of his right great toe. His health history included the use of diuretics for a long time and his last gout attack was two years ago at first metatarsophalengeal joint of right foot. Sildenafil, a selective inhibitor of phosphodiesterase type 5 (PDE5), was administered in case of erectile dysfunction for two months. Subsequently, he had several episodes of pain and swelling at first interphalangeal joint of right great toe. Both the onset and recurrence of symptoms were just seen the day after sildenafil use. The patient was free of symptoms after discontinuation of the drug. After an initial evaluation, gout was diagnosed on the basis of synovial fluid analysis. DISCUSSION: This case demonstrates a rare location of gouty arthritis with an uncommon etiology: sildenafil. Regarding the clinical data, the discussion was made to expand the horizon for diagnosis of patients with similar symptoms, to identify risk factors for gout relevant to elderliness, and to review the management of gout. CONCLUSION: Sildenafil use may cause gouty arthritis, and surgical decompression may be helpful for definitive diagnosis and symptom relief in atypical presentation of gout

    Perinatal outcomes of pregnancies with prenatally diagnosed foetal congenital heart disease

    We aimed to assess the types of prenatally diagnosed congenital heart disease (CHD) and their association with structural and chromosomal abnormalities and to evaluate the perinatal outcomes according to the type of the heart defect. We retrospectively reviewed 377 pregnancies with prenatally diagnosed CHD. The main outcome measure was to evaluate the pregnancy outcomes of CHD according to the type of the heart defect and associated structural or chromosomal abnormalities. Of 377 foetuses with major structural CHD, 214 (56.8%) were isolated, 49 (13%) had additional cardiac anomalies, 58 (15.4%) had extracardiac malformations with normal karyotype and 56 (14.9%) had chromosomal abnormalities. The most common chromosomal abnormality was trisomy 21 (55.4%). Prenatal detection of CHD allows early workup to identify chromosomal abnormalities and detailed anatomic evaluation of extracardiac malformations. Prognostication of each heart defect at diagnosis and facilitating patients with isolated surgically correctable CHD for targeted postnatal care is essential.IMPACT STATEMENT What is already known on this subject? CHD is the most common structural anomaly and is strongly associated with chromosomal anomalies and genetic syndromes. What do the results of this study add? Survival of the prenatally diagnosed CHD depends on the type and severity of the condition and coexisting extracardiac structural or chromosomal abnormalities. What are the implications of these findings for clinical practice and/or further research? Prenatal detection of CHD allows early workup to identify chromosomal abnormalities, detailed anatomic evaluation of extracardiac malformations and time to refer the parents to tertiary cardiac care centres and prepare for planned delivery, as well as to establish an appropriate perinatal and postnatal therapeutic plan

    The effect of the use of complex molecular profiling in advanced solid organ tumours on clinical decision: Turkey Molecular Profiling in Advanced Cancers Trial (TUMPACT)

    Background: Molecular genetic profiling (MGP) which has started to take an important part in cancer diagnosis and treatment in recent years, has entered the routineclinical practice much faster than expected. We aimed to evaluate the use of thistechnology in routine practice, the effect of changing the treatment decision.Methods: Two hundred and thirty-four patients who received treatment from 21different centers with OncoDeep MGP platform tests were included, which uses acombination of new generation sequencing (NGS), immunohistochemistry (IHC) andother specific tests (Package Plus).Results: Summary was given in the table. Physician waited for test results in 27%(n¼61) of patients for treatment decision. Prior to MGP analysis patients receivedmedian 2-lines of treatment. The median time between sending the sample abroadfor testing and reaching the result by the physician was 14 days (range:5-71). With thetest results, the physician changed the treatment decision in 51.8% of the patients(n¼118). The most frequent way of drug supply of patients whose treatment decisionwere 64.4% (n ¼ 67) from their own budget. When the treatment responses wereevaluated, the disease control rate was 31.1% and the drug discontinuation wasapplied due to toxicity in 4 patients (3.4%). 63.6% (n ¼ 75) of the patients were foundto be alive with a median follow-up of 18.0 months.
