    Rebuilding the Iraqi State: The Regional Dimension of Ethno-Sectarian Conflict (2003-2016)

    This thesis addresses the relationship between state-building and ethno-sectarian conflict in Iraq from 2003 to 2016 in the regional context among Iraq’s core neighbours: Iran, Syria, Saudi Arabia and Turkey. The purpose of this study is to examine how and why Iraq's neighbours engaged in the process of state-building in Iraq after the fall of Saddam's regime. Part of this research’s significance lies in the fact that there is a lack of research projects that examine Iraq’s state-building process in its regional dimension, and of studies that address internal and external factors that shape security policies in the Persian Gulf. The majority of previous studies so far have addressed the state/nation-building process in Iraq as an internal issue among Iraq’s different sectarian, ethnic, and political factions. However, this study has found that the involvement of Iraq's neighbours in the ethno-sectarian conflict has been an enduring part of the state-building process in post-2003 Iraq. Furthermore, one of the central issues that this study has demonstrated regarding the involvement of Iraq's neighbours is the constant interaction of three main variables: security complex dynamics, ethno-sectarian conflict, and the state-building process in post-2003 Iraq. Based on the theoretical contribution of RSC, this research has found that the engagement of Iraq’s neighbours in the state-building process and ethno-sectarian conflict following the U.S. invasion of Iraq is rooted in the RSC dynamics of Iraq with its neighbours in the region, on the one hand, and among the neighbours themselves, on the other hand. The engagement of Iraq's neighbours in the state-building process in Iraq has been driven by both internal and external dimensions of the regional security complex. Moreover, the thesis found that both Turkey and Iran have built institutional bases for their leverage in post-2003 Iraq. Turkey through the KRG and a part of the Arab Sunnis, and Iran through the Shi'a-centric state and part of the Iraqi Kurds, have built institutional links with Iraq's components. However, both Saudi Arabia and Syria lacked the capability to build such institutional relations with post-2003 Iraq, and this has been a main cause for their ineffective positions in the process of state-building in Iraq after 2003.Thus, the ethno-sectarian conflict in post-2003 Iraq has been a form of intervention by Iraq's neighbours in the state-building process

    A Modified Hopfield Neural Network Algorithm (MHNNA) Using ALOS Image for Water Quality Mapping

    Decreasing water pollution is a big problem in coastal waters. Coastal health of ecosystems can be affected by high concentrations of suspended sediment. In this work, a Modified Hopfield Neural Network Algorithm (MHNNA) was used with remote sensing imagery to classify the total suspended solids (TSS) concentrations in the waters of coastal Langkawi Island, Malaysia. The adopted remote sensing image is the Advanced Land Observation Satellite (ALOS) image acquired on 18 January 2010. Our modification allows the Hopfield neural network to convert and classify color satellite images. The samples were collected from the study area simultaneously with the acquiring of satellite imagery. The sample locations were determined using a handheld global positioning system (GPS). The TSS concentration measurements were conducted in a lab and used for validation (real data), classification, and accuracy assessments. Mapping was achieved by using the MHNNA to classify the concentrations according to their reflectance values in band 1, band 2, and band 3. The TSS map was color-coded for visual interpretation. The efficiency of the proposed algorithm was investigated by dividing the validation data into two groups. The first group was used as source samples for supervisor classification via the MHNNA. The second group was used to test the MHNNA efficiency. After mapping, the locations of the second group in the produced classes were detected. Next, the correlation coefficient (R) and root mean square error (RMSE) were calculated between the two groups, according to their corresponding locations in the classes. The MHNNA exhibited a higher R (0.977) and lower RMSE (2.887). In addition, we test the MHNNA with noise, where it proves its accuracy with noisy images over a range of noise levels. All results have been compared with a minimum distance classifier (Min-Dis). Therefore, TSS mapping of polluted water in the coastal Langkawi Island, Malaysia can be performed using the adopted MHNNA with remote sensing techniques (as based on ALOS images)

    The Future of Minorities in Post-ISIL Iraq

    The period after June 2014 has seen forces from the Islamic State of Iraq and the Levant (ISIL), an armed group, spread quickly across Iraq. This process has led to massive displacements in which affected over two million people. Indeed, the primary targets of ISIL have been religious and ethnic minorities. Some of these groups include Yazidis, Shabak, Kaka’i, and Christians. Also, ISIL has abducted or injured many people, with thousands also killed. Examples of adverse actions that the armed group has committed include severe human rights abuses such as the looting of property, the abduction of children, the destruction of places of worship, sexual enslavement, rape, forced conversion, and summary executions. Whereas the period before the insurgency group’s entry still witnessed minorities exist as a vulnerable group, the violence led by ISIL has threatened to eliminate them permanently from regions such as the Nineveh Plains, have lived in these areas for several centuries. The main question purpose of this paper is to examine the future status of minorities in post-ISIL Iraq? ISIL. I will argue that although ISIL had been defeated in Iraq, their impact will be critical on Iraqi Minorities Even after the demise of ISIL. Currently, informal settlements, abandoned buildings, and camps in which the displaced persons live reflect deteriorating humanitarian conditions. With international agencies experiencing limited resources and also the government failing to offer an effective response, most of the international displaced people (IDPs)[1] do not have enough shelter, health care, water, food, and other essential items. Indeed, most vulnerable groups include children and women. At a time when the majority are contemplating emigration out of Iraq, their survival in the immediate and far future rests upon collaboration among four groups that include the international community, the Iraqi government, Kurdish authorities, and minorities themselves. Some of the specific areas that need to be addressed include the asylum dilemma, reconciliation and restoration, preventing future abuses, legislation, and humanitarian issues

    This project involves the transfer of knowledge between a university community and an orphanage. The project applies the hybrid learning method which is a combination of the traditional and computer-assisted approaches to enhance the Quranic Recital, Fardhu Ain and Arabic language programs. This KTP project is combining the theories of multimedia learning by Mayer (2001) and hybrid learning by Nicole and Retta (2006) in the activities. Mayer (2001) states that " people learn more deeply from words and pictures than from words alone", while Nicole and Retta (2006) mentioned that the hybrid learning approch is effective in carrying out 'in-depth learning'. An orphanage was chosen as the partner for this project because the orphans are in dire need of attention and support. The project helps to ensure that they are not left behind. The researchers chose to teach Quranic recital and Fardhu Ain since the skills could help to make them a better person. The religious and moral values which are embedded in these two courses may help towards producing ethical manpower in the future. This paper presents some of activities that were held in the first quarter of 2014. It was successfully run with a few modifications to the strategies and timing of the activities. Some of the children were found to be extremely weak in their Jawi script writing skill and Arabic language proficiency. Hence more time was needed to assist them during the the learning activities. This project implies the need for the government to set up a special care center for orphans under its supervision which integrates education and welfare in ensuring a brighter future for this needy group

    Optimising the outcomes of index admission laparoscopic cholecystectomy and bile duct exploration for biliary emergencies: a service model

    Aims: The rate of acute laparoscopic cholecystectomy remains low due to operational constraints. The purpose of this study is to evaluate a service model of index admission cholecystectomy with referral protocols, refined logistics and targeted job planning. Methods: A prospectively maintained dataset was evaluated to determine the processes of care and outcomes of patients undergoing emergency biliary surgery. The lead author has maintained a 28 years prospective database capturing standard demographic data, intraoperative details including the difficulty of cholecystectomy as well as postoperative outcome parameters and follow up data. Results: Over five thousand (5555) consecutive laparoscopic cholecystectomies were performed. Only patients undergoing emergency procedures (2399,43.2% of entire group) were analysed for this study. The median age was 52 years with 70% being female. The majority were admitted with biliary pain (34%), obstructive jaundice (26%) and acute cholecystitis (16%). 63% were referred by other surgeons. 80% underwent surgery within 5 days (40% within 24 h). Cholecystectomies were performed on scheduled lists (44%) or dedicated emergency lists (29%). Two thirds had suspected bile duct stones and 38.1% underwent bile duct exploration. The median operating time was 75 min, median hospital stay 7 days, conversion rate 0.8%, morbidity 8.9% and mortality rate 0.2%. Conclusion: Index admission cholecystectomy for biliary emergencies can have low rates of morbidity and mortality. Timely referral and flexible theatre lists facilitate the service, optimising clinical results, number of biliary episodes, hospital stay and presentation to resolution intervals. Cost benefits and reduced interval readmissions need to be weighed against the length of hospital stay per episode

    Tocilizumab in patients admitted to hospital with COVID-19 (RECOVERY): a randomised, controlled, open-label, platform trial

    Background: In this study, we aimed to evaluate the effects of tocilizumab in adult patients admitted to hospital with COVID-19 with both hypoxia and systemic inflammation. Methods: This randomised, controlled, open-label, platform trial (Randomised Evaluation of COVID-19 Therapy [RECOVERY]), is assessing several possible treatments in patients hospitalised with COVID-19 in the UK. Those trial participants with hypoxia (oxygen saturation <92% on air or requiring oxygen therapy) and evidence of systemic inflammation (C-reactive protein ≥75 mg/L) were eligible for random assignment in a 1:1 ratio to usual standard of care alone versus usual standard of care plus tocilizumab at a dose of 400 mg–800 mg (depending on weight) given intravenously. A second dose could be given 12–24 h later if the patient's condition had not improved. The primary outcome was 28-day mortality, assessed in the intention-to-treat population. The trial is registered with ISRCTN (50189673) and ClinicalTrials.gov (NCT04381936). Findings: Between April 23, 2020, and Jan 24, 2021, 4116 adults of 21 550 patients enrolled into the RECOVERY trial were included in the assessment of tocilizumab, including 3385 (82%) patients receiving systemic corticosteroids. Overall, 621 (31%) of the 2022 patients allocated tocilizumab and 729 (35%) of the 2094 patients allocated to usual care died within 28 days (rate ratio 0·85; 95% CI 0·76–0·94; p=0·0028). Consistent results were seen in all prespecified subgroups of patients, including those receiving systemic corticosteroids. Patients allocated to tocilizumab were more likely to be discharged from hospital within 28 days (57% vs 50%; rate ratio 1·22; 1·12–1·33; p<0·0001). Among those not receiving invasive mechanical ventilation at baseline, patients allocated tocilizumab were less likely to reach the composite endpoint of invasive mechanical ventilation or death (35% vs 42%; risk ratio 0·84; 95% CI 0·77–0·92; p<0·0001). Interpretation: In hospitalised COVID-19 patients with hypoxia and systemic inflammation, tocilizumab improved survival and other clinical outcomes. These benefits were seen regardless of the amount of respiratory support and were additional to the benefits of systemic corticosteroids.     Convalescent plasma in patients admitted to hospital with COVID-19 (RECOVERY): a randomised controlled, open-label, platform trial

    Background: Many patients with COVID-19 have been treated with plasma containing anti-SARS-CoV-2 antibodies. We aimed to evaluate the safety and efficacy of convalescent plasma therapy in patients admitted to hospital with COVID-19. Methods: This randomised, controlled, open-label, platform trial (Randomised Evaluation of COVID-19 Therapy [RECOVERY]) is assessing several possible treatments in patients hospitalised with COVID-19 in the UK. The trial is underway at 177 NHS hospitals from across the UK. Eligible and consenting patients were randomly assigned (1:1) to receive either usual care alone (usual care group) or usual care plus high-titre convalescent plasma (convalescent plasma group). The primary outcome was 28-day mortality, analysed on an intention-to-treat basis. The trial is registered with ISRCTN, 50189673, and ClinicalTrials.gov, NCT04381936. Findings: Between May 28, 2020, and Jan 15, 2021, 11558 (71%) of 16287 patients enrolled in RECOVERY were eligible to receive convalescent plasma and were assigned to either the convalescent plasma group or the usual care group. There was no significant difference in 28-day mortality between the two groups: 1399 (24%) of 5795 patients in the convalescent plasma group and 1408 (24%) of 5763 patients in the usual care group died within 28 days (rate ratio 1·00, 95% CI 0·93–1·07; p=0·95). The 28-day mortality rate ratio was similar in all prespecified subgroups of patients, including in those patients without detectable SARS-CoV-2 antibodies at randomisation. Allocation to convalescent plasma had no significant effect on the proportion of patients discharged from hospital within 28 days (3832 [66%] patients in the convalescent plasma group vs 3822 [66%] patients in the usual care group; rate ratio 0·99, 95% CI 0·94–1·03; p=0·57). Among those not on invasive mechanical ventilation at randomisation, there was no significant difference in the proportion of patients meeting the composite endpoint of progression to invasive mechanical ventilation or death (1568 [29%] of 5493 patients in the convalescent plasma group vs 1568 [29%] of 5448 patients in the usual care group; rate ratio 0·99, 95% CI 0·93–1·05; p=0·79). Interpretation: In patients hospitalised with COVID-19, high-titre convalescent plasma did not improve survival or other prespecified clinical outcomes.
