    Biological approaches to the repair and regeneration of the rotator cuff tendon-bone enthesis: a literature review

    Entheses are highly specialised organs connecting ligaments and tendons to bones, facilitating force transmission, and providing mechanical strengths to absorb forces encountered. Two types of entheses, fibrocartilaginous and fibrous, exist in interfaces. The gradual fibrocartilaginous type is in rotator cuff tendons and is more frequently injured due to the poor healing capacity that leads to loss of the original structural and biomechanical properties and is attributed to the high prevalence of retears. Fluctuating methodologies and outcomes of biological approaches are challenges to overcome for them to be routinely used in clinics. Therefore, stratifying the existing literature according to different categories (chronicity, extent of tear, and studied population) would effectively guide repair approaches. This literature review supports tissue engineering approaches to promote rotator cuff enthesis healing employing cells, growth factors, and scaffolds period. Outcomes suggest its promising role in animal studies as well as some clinical trials and that combination therapies are more beneficial than individualized ones. It then highlights the importance of tailoring interventions according to the tear extent, chronicity, and the population being treated. Contributing factors such as loading, deficiencies, and lifestyle habits should also be taken into consideration. Optimum results can be achieved if biological, mechanical, and environmental factors are approached. It is challenging to determine whether variations are due to the interventions themselves, the animal models, loading regimen, materials, or tear mechanisms. Future research should focus on tailoring interventions for different categories to formulate protocols, which would best guide regenerative medicine decision making

    Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: multispecialty, prospective cohort study in 25 countries

    Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures.Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge.Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P < 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (beta coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not.Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction. Opioid-free discharge analgesia should be adopted routinely

    The study aimed to identify the relationship between the manifestations of attention and the accuracy of the skill of handling and shooting basketball for female students. As for the research sample, the research community was identified as students of the College of Physical Education and Sports Sciences - University of Al-Qadisiyah for the academic year 2021-2022. Their number is (39) students, where the proportion of the sample from the community reached 100%. In light of the preceding, the results were a positive relationship between the concentration of attention and the skills of free and peaceful scoring among students of physical education and sports sciences, as well as a positive relationship between the intensity of attention and the skills of free and peaceful scoring among students of physical education and sports sciences

    Protective Effects of Selenium in Tacrolimus-induced Lung Toxicity: Potential Role of Hemeoxygenase-1

    Abstract The present study aimed to evaluate the protective effects of selenium administration against tacrolimus (Tac) induced lung toxicity and to assess the relation between heme oxygenase 1 (HO-1) and these effects. The study was conducted on 36 Wistar male albino rats equally divided into 4 groups: (1) normal control, (2) Selenium (Sel 0.1mg/kg/day p.o for four weeks), (3) TAC 3mg/ml as single oral dose on 27th day (4) Tac + Sel. Lung tissues, lung homogenate, and bronchoalveolar lavage of the sacrificed animals were investigated biochemically, histopathologically, by immunohistochemistry or by PCR. Tac group showed significantly lower expression of HO-1. Administration of selenium was associated with increased HO-1 expression. Oxidative (malondialdehyde: MDA, reduced glutathione: GSH, superoxide dismutase: SOD, myeloperoxidase: MPO, glutathione peroxidase activity: GPx) and nitrosative stress (Nitric oxide: NO) markers and markers of inflammation (Interleukins: IL1β, 6 and 10) showed changes corresponding to HO-1 levels in rats groups. Tac group showed the highest expression of caspase-3.     Mechanisms of Thymoquinone Hepatorenal Protection in Methotrexate-Induced Toxicity in Rats

    To investigate mechanisms by which thymoquinone (TQ) can prevent methotrexate- (MTX-) induced hepatorenal toxicity, TQ (10 mg/kg) was administered orally for 10 days. In independent rat groups, MTX hepatorenal toxicity was induced via 20 mg/kg i.p. at the end of day 3 of experiment, with or without TQ. MTX caused deterioration in kidney and liver function, namely, blood urea nitrogen, creatinine, alanine aminotransferase, and aspartate aminotransferase. MTX also caused distortion in renal and hepatic histology, with significant oxidative stress, manifested by decrease in reduced glutathione and catalase, as well as increase in malondialdehyde levels. In addition, MTX caused nitrosative stress manifested by increased nitric oxide, with upregulation of inducible nitric oxide synthase. Furthermore, MTX caused hepatorenal inflammatory effects as shown by increased tumor necrosis factor-α, besides upregulation of necrosis factor-κB and cyclooxygenase-2 expressions. MTX also caused apoptotic effect, as it upregulated caspase 3 in liver and kidney. Using TQ concurrently with MTX restored kidney and liver functions, as well as their normal histology. TQ also reversed oxidative and nitrosative stress, as well as inflammatory and apoptotic signs caused by MTX alone. Thus, TQ may be beneficial adjuvant that confers hepatorenal protection to MTX toxicity via antioxidant, antinitrosative, anti-inflammatory, and antiapoptotic mechanisms

    Three new Co (II), Ag (I), and Cd (II) ion complexes were created from the ligand, 2-(phenylglycyl)-N-(p-tolyl)hydrazine-1-carbothioamide (H2LB). The structural makeup of the new compounds was clarified using analytical and spectroscopic techniques. Then, using the Gaussian09 software, geometry optimization was done for each synthesis. Furthermore, the compound's antibacterial and anticancer properties were evaluated against different types of bacteria and the HepG2 cell line using non-irradiated and irradiated complexes, where the complex (A1) is higher than them. It was proposed that only an intercalation or replacement technique was used when the ligand and complexes [Co(H2L)(NO3)2]H2O (B1, A1) interacted with CT-DNA. Calculations were made for the intrinsic binding constant Kb. According to a molecular docking study, the ligands and complexes revealed fascinating interactions with the amino acids in the ribosyltransferase active site. (Code: 3GEY)

    Assessment of On-Site Treatment Process of Institutional Building’s Wastewater

    This study is conducted to investigate the characteristics of outflow wastewater of the 1 m3 on-site wastewater treatment unit on the basis of the testing and measurement data of the samples that were taken during the study monitored period (August 2017 to January 2018). For this purpose, samples were taken on a weekly basis from the treated wastewater effluent and five quality parameters (biochemical oxygen demand (BOD), chemical oxygen demand (COD), total suspended solids (TSS), pH, E-coli counts) were monitored and measured. The average values of the five parameters were compared with the Jordanian standard maximum values, and water reuse in irrigation of plants classifications have been assessed and investigated. Average values of BOD, COD, TSS, pH, and E-coli in treated wastewater were 11 mg/L, 104 mg/L, 15 mg/L, 7.51, and 387 counts, respectively. The installation of in-line ultraviolet (UV) unit in recirculating delivery system played a vital role in the reduction of counts far below the permissible maximum level (1000 counts). Based on national and international standards and criteria, results showed that the treated wastewater is suitable for the irrigation of two classifications of plants: (i) Fruit trees, road-green sides outside cities, and green landscape; (ii) Crops, commercial crops, and forest trees. Hence, such very low water flow rate treatment system can be utilized in refugees’ camps and water scarce residential areas in Jordan

    Background: Several studies suggested a significant role of epigenetic changes, including alterations in miRNA, histone modifications, and DNA methylation of α-synuclein (SNCA) in Parkinson’s disease (PD) pathogenicity. As of yet, only very few studies have been carried out in this field in Africa and none in Sudan. Materials and methods: We collected DNA from 172 Sudanese individuals (90 cases, 82 controls) who donated saliva for DNA extraction (mean age of onset: 40.6 ± 22.4 years). A family history of PD was evident in 64 patients. DNA preparation and bisulfite sequencing of SNCAintron1 was performed as described earlier. Results: Of the fourteen analyzed CpGs of SNCAintron1, CpGs 16-23 were hypomethylated in PD (P-value ranged from 0.023 to 0.003). P–values improved, when sporadic cases were excluded from the analysis. Conclusion: We identified the presence of a specific pattern of DNA methylation in a young Sudanese cohort of familial PD, which confirms the importance of the methylation of SNCAintron1 for PD. This phenomenon appears to be independent of ethnicity, the impact of environmental factors, drug history, or familial clustering
