    Unsupervised Learning from Narrated Instruction Videos

    We address the problem of automatically learning the main steps to complete a certain task, such as changing a car tire, from a set of narrated instruction videos. The contributions of this paper are three-fold. First, we develop a new unsupervised learning approach that takes advantage of the complementary nature of the input video and the associated narration. The method solves two clustering problems, one in text and one in video, applied one after each other and linked by joint constraints to obtain a single coherent sequence of steps in both modalities. Second, we collect and annotate a new challenging dataset of real-world instruction videos from the Internet. The dataset contains about 800,000 frames for five different tasks that include complex interactions between people and objects, and are captured in a variety of indoor and outdoor settings. Third, we experimentally demonstrate that the proposed method can automatically discover, in an unsupervised manner, the main steps to achieve the task and locate the steps in the input videos.Comment: Appears in: 2016 IEEE Conference on Computer Vision and Pattern Recognition (CVPR 2016). 21 page

    Sternoclavicular instability - reconstruction of sternoclavicular ligament using semitendinosus autograft

    Background: Injury to the sternoclavicular joint is rare with an incidence of 3% of all the injuries around the shoulder girdle. Most of them heal with conservative treatment. Rarely some progress to chronic instability associated with pain.Methods: We present a small series of 4 such cases of chronic symptomatic sternoclavicular joint instability. We performed reconstruction of the sternoclavicular ligament using semitendinosus autograft, with excellent result, with minimum of 15 months follow up.Results: All the patients returned to pre injury level of activity at the end of 6 months. There was significant improvement in the DASH score following surgery. Our results are comparable with that of Castropil et al, who had performed a similar technique. Conclusions: Reconstruction of sternoclavicular ligament using the semitendinosus autograft is a safe, reproducible and functionally good surgical option in patients with chronic sternoclavicular instability

    Effect Of Non-Ballistic Active Knee Extension In Neural Slump Position On Hamstring Flexibility

    PURPOSE :- To assess how neural slump mobilization affects hamstring flexibility BACKGROUND :- Thirty adult individuals in good health with restricted right hamstring flexibility (minimum of 15 degrees less active knee extension assessed with femur held at 90 degrees of hip flexion) were divided into two groups at random. METHOD:- Subjects in Group A will engage in non-ballistic static stretching for 30 seconds, which will be followed by static stretching for 30 seconds after a 5-minute rest period for two weeks. Group-B: Over the course of two weeks, the subject will complete static stretches maintained for 30 seconds, repeated five times, with a 1-second break in between each stretch. STATICAL ANALYSIS :- To compare the mean value for pre-treatment and post-treatment ROM across groups as well as the comparison of the mean values for pre-treatment and post-treatment ROM within the group, independent sample t-test and paired t-test were employed, respectively. A value of p was chosen.05 RESULT:- According to a within-group analysis, group A's mean range of motion was larger after therapy than it was before treatment (53.83 2.43), and this difference was determined to be statistically significant (p=0.00). The difference between the mean post-treatment range of motion value for group B (51.44 2.63) and the mean pre-treatment range of motion value (38.49 3.26) was determined to be statistically significant (p=0.00). The results of the between-group analysis showed that group A's mean post-treatment range of motion value was greater than group B's (53.83 2.43 vs. 51.44 2.63), and this difference was judged to be statistically significant (p=0.016). CONCLUSION :- The results of the current study showed that static stretching combined with the neural slump posture improved hamstring flexibility in adults more than static stretching alone, resulting in changes in the accuracy of the active knee extension measuremen

    Seismic Response Control Systems for Structures

    Structures constructed in developing world are typically RC frames with masonry infill. These structures have little resistance for lateral loads caused by earthquake and wind. Even for  adequately designed structures also, due to permissible deformation beyond elastic limits, failure of masonry causes severe loss of life and property. In the case of structures designed to sustain excessive deformation such as of defence establishments, functioning and serviceability of machines and equipment installed therein are adversely affected. This co-lateral damage may be reduced by adopting another design philosophy of structure response control. In this methodology, a supplementary damping device is incorporated in the primary structure, which absorbs most of the seismic energy imparted to it, restricting the structural response within serviceable limits. These devices may be passive, active, semi-active or hybrid types. Other than passive all options are technology-intensive and dependent on external energy source, not a favourable proposition for developing nations. Among all the passive devices, tuned liquid dampers (TLDs) promise to be most suitable. Here, existing overhead water tanks (OHWT) may be used as TLD with slight adjustment and modification. This method will be able to control the structural response without putting any extra load on the existing or newly-designed buildings. This paper reviews various types of dampers and discusses evolution of tuned liquid dampers. A method has also been proposed for incorporating TLDs in existing and new structures.     A Large Nonmetastatic Anaplastic Thyroid Cancer with Complete Thyroidal Confinement

    Anaplastic thyroid cancer (ATC) is rare but extremely aggressive, which accounts for about 2% of all thyroid cancers yet nearly 50% of thyroid-cancer-associated deaths in the United States. The median survival time from diagnosis is 5 months, with a 1-year survival rate of only 20%. We report here a case of ATC in a 56-year-old man who survived a large ATC. Preoperative fine-needle aspiration biopsy study to a large right thyroid mass suggested ATC. Total thyroidectomy with radical lateral neck and central neck dissection removed a well-circumscribed 9.5 cm tumor without extrathyroidal extension or lymphovascular invasion. All 73 lymph nodes removed were negative for metastasis. The tumor consisted of highly pleomorphic, undifferentiated cells with large zones of necrosis and loss of thyroid transcription factor-1 and thyroglobulin expression. A focal well-differentiated component and PAX8 expression confirmed its thyroid follicular cell origin. Nine months after postsurgical adjuvant concurrent radiation therapy and chemotherapy, the patient remained well without clinical, biochemical, and radiographical evidence for cancer recurrence. This is an unusual case of ATC in that it is one of the largest ATC tumors reported to display mild pathologic behavior and relatively long-term patient survival

Creating such assistants, however, is non-trivial and requires understanding of visual and verbal content of a video.     Comparison of admission GCS score to admission GCS-P and FOUR scores for prediction of outcomes among patients with traumatic brain injury in the intensive care unit in India

    Background This study aimed to determine the predictive power of the Full Outline of Unresponsiveness (FOUR) score and the Glasgow Coma Scale Pupil (GCS-P) score in determining outcomes for traumatic brain injury (TBI) patients. The Glasgow Outcome Scale (GOS) was used to evaluate patients at 1 month and 6 months after the injury. Methods We conducted a 15-month prospective observational study. It included 50 TBI patients admitted to the ICU who met our inclusion criteria. We used Pearson's correlation coefficient to relate coma scales and outcome measures. The predictive value of these scales was determined using the receiver operating characteristic (ROC) curve, calculating the area under the curve with a 99% confidence interval. All hypotheses were two-tailed, and significance was defined as P<0.01. Results In the present study, the GCS-P and FOUR scores among all patients on admission as well as in the subset of patients who were mechanically ventilated were statistically significant and strongly correlated with patient outcomes. The correlation coefficient of the GCS score compared to GCS-P and FOUR scores was higher and statistically significant. The areas under the ROC curve for the GCS, GCS-P, and FOUR scores and the number of computed tomography abnormalities were 0.912, 0.905, 0.937, and 0.324, respectively. Conclusions The GCS, GCS-P, and FOUR scores are all excellent predictors with a strong positive linear correlation with final outcome prediction. In particular, the GCS score has the best correlation with final outcome

    Adjuncts for the evaluation of potentially malignant disorders in the oral cavity

    BACKGROUND: Oral squamous cell carcinoma is the most common manifestation of malignancy in the oral cavity. Adjuncts are available for clinicians to evaluate lesions that seem potentially malignant. In this systematic review, the authors summarized the available evidence on patient-important outcomes, diagnostic test accuracy (DTA), and patients' values and preferences (PVPs) when using adjuncts for the evaluation of clinically evident lesions in the oral cavity. TYPES OF STUDIES REVIEWED: The authors searched for preexisting systematic reviews and assessed their quality using the Assessing the Methodological Quality of Systematic Reviews tool. The authors updated the selected reviews and searched MEDLINE, Embase, and the Cochrane Central Register of Controlled Trials to identify randomized controlled trials and DTA and PVPs studies. Pairs of reviewers independently conducted study selection, data extraction, and assessment of the certainty in the evidence by using the Grading of Recommendations Assessment, Development and Evaluation approach. RESULTS: The authors identified 4 existing reviews. DTA reviews included 37 studies. The authors retrieved 7,534 records, of which 9 DTA and 10 PVPs studies were eligible. Pooled sensitivity and specificity of adjuncts ranged from 0.39 to 0.96 for the evaluation of innocuous lesions and from 0.31 to 0.95 for the evaluation of suspicious lesions. Cytologic testing used in suspicious lesions appears to have the highest accuracy among adjuncts (sensitivity, 0.92; 95% confidence interval, 0.86 to 0.98; specificity, 0.94; 95% confidence interval, 0.88 to 0.99; low-quality evidence). CONCLUSIONS AND PRACTICAL IMPLICATIONS: Cytologic testing appears to be the most accurate adjunct among those included in this review. The main concerns are the high rate of false-positive results and serious issues of risk of bias and indirectness of the evidence. Clinicians should remain skeptical about the potential benefit of any adjunct in clinical practice
