    Clinico-epidemiological profile and predictors of in-hospital outcome of acute poisoning cases in children of Northern India

    Background: Childhood poisoning is a common but preventable problem worldwide with incidence varying from 0.3% to 7.6%. Demography, socioeconomic status, education, local belief and customs, occupation, religious, and cultural influences determine the cause of poisoning. Aim: This study was conducted to understand the recent changes increasing urbanization and rapid socioeconomic development in India during the past decade and to find out the change in pediatric due to poisoning profile. Materials and Methods: This study was conducted in a pediatric ward of a tertiary care hospital in North India during the period of January 2016–December 2016. A total of 174 children admitted with acute poisoning in the hospital during the study tenure were enrolled for the study. Clinical and demographic data were recorded in a predesigned proforma and results were compared with the previous studies from the region. Results: Mean age at presentation was 3.7 years, wherein males outnumbered females. Accidental mode (97%) was the most common mode of poisoning while ingestion (99%) was the most common route of exposure. The common agents incriminated in decreasing order of frequency were corrosives (24.71%), pyrethroids (12.64%), and kerosene (9.77%). Majority of patients belongs to lower middle class (57%) and urban areas (61%). Most children present with mild symptoms, of them vomiting was the most common (65%) and 12.6% developed complications. Mean duration of hospital stay was 2.5±1.91 days with a mortality of 2.16%. Conclusion: There is changing pattern in acute poisoning in children with decline in incidence of kerosene and pesticide poisoning while increase in corrosives and parathyroid poisoning. Low Glasgow Coma Scale and time lapse between poisoning and presentation to the hospital can be a predictor of high mortality

    Odontogenic carcinoma is rare group of malignant epithelial odontogenic neoplasms with characteristic clinical behavior and histological features, which requires an aggressive surgical approach. The pathogenesis of this rare group remains still controversial and there have been many varied opinions over the classification of this rare group of lesions. As there have not been many reviews on odontogenic carcinoma, the existing knowledge is mostly derived from the published case reports. This review is discussing the pathogenetic mechanisms and is updating the knowledge on nomenclature system of less explored odontogenic carcinomas. This review might throw light on the pathogenesis and nomenclature system of odontogenic carcinoma and this knowledge may be applied therapeutically

    Process intensification for the small footprint compact heat transfer device

    Process intensification for the development of compact heat exchanger with small footprint is greatest challenge of the heat exchanger technology today. In the present study, a heat transfer device, coiled flow inverter (CFI) is revamped for the better heat transfer efficiency with a smaller footprint. The proposed small foot-print coiled flow inverter (SFCFI) is fabricated by bending of helical coil at 90° with equal arm lengths before and after the bend with variable curvature radius. In integration to the improved centrifugal force due to variable curvature, the SFCFI additionally offers a complete 90 flow inversion caused by each 90 bend, which results in higher radial mixing and heat transfer. The velocity and temperature flow fields depict the improved radial mixing under the laminar flow regime for the Dean number ranges from 8 to 1581. The performance of existing CFI of same heat transfer area (0.17 m2) was studied and compared with the novel SFCFI device. The results suggest, the proposed SFCFI device provides three-fold heat transfer enhancement as compared to the straight tube of same heat transfer area at Dean number 400. Additionally, heat transfer coefficient in SFCFI enhanced by 48 % as compared to helical coil. Furthermore, SFCFI provides 18 % higher value of Nusselt number as compared to the CFI. The reason for improved heat transfer may be the enhanced centrifugal force due to additional curvature effect provided in each arm of SFCFI in the plane of vortex formation. It was interesting to note that the proposed device provides 11 % lower pressure drop as compared to the CFI.     Classification based on fuzzy logic techniques can handle uncertainty to a certain extent as it provides only the fuzzy membership of an element in a set. This paper implements the extension of fuzzy logic: Neutrosophic logic to handle indeterminacy, uncertainty effectively. Classification is done on various techniques based on Neutrosophic logic i.e. Neutrosophic soft set, rough Neutrosophic set, Neutrosophic ontology to provide better results in comparison to fuzzy logic based techniques. It is proved that rough neutrosophic soft set will handle indeterminacy effectively that exists in the medical domain as it provides the minimum and maximum degree of truth, indeterminacy, falsity for every element

    An unusual case of multifocal central serous chorioretinopathy with low serum cortisol managed using eplerenone

    In this report, we describe a rare case of a 44-year-old Asian male with acute central serous chorioretinopathy (CSC) with bullous exudative retinal detachment. Endocrinology evaluation revealed hypothalamic–pituitary–adrenal axis suppression with low serum cortisol. Furthermore, neuroimaging revealed the presence of a pituitary microadenoma. He was treated with systemic eplerenone and hydrocortisone. After 12 weeks, bullous detachment completely resolved. Our case is a unique description of acute CSC with underlying low serum cortisol levels that responded to treatment with mineralocorticoid antagonist. This case highlights the various endocrine abnormalities other than the raised serum cortisol that can occur in patients with CSC
