Az anaemiák epidemiológiája
Világszerte magas az anaemia prevalenciája, a vashiányos anaemia az ötödik leggyakoribb eltérés a Globális betegség-
teher című tanulmány szerint. Hátterében számos tényező, összetett patomechanizmus állhat, az etiológia nagy vál-
tozatosságot mutat életkor, nem és földrajzi eloszlás szerint. Az anaemia prevalenciája emelkedik az életkorral. A demográfiai változások, a népesség idősödése jelenleg nagyobb léptékű, mint a korábbi évtizedekben, és ez jelentős
kihívás elé állítja a társadalmakat és az egészségügyi rendszereket. Az időskori anaemia átlagos prevalenciája 17%, de ennél lényegesen magasabb az ápolási otthonokban élő (47%) és a hospitalizációra került idősek (40%) között. Oka
általában multifaktoriális, és gyakran a komorbiditások miatt egyidejűleg több mechanizmus is szerepet játszik a ki-
alakulásában. A preoperatív anaemia prevalenciája magasabb (35%), mint az anaemia gyakorisága az átlagpopuláció-

Equine-like G3 rotavirus in Hungary, 2015 – Is it a novel intergenogroup reassortant pandemic strain?
Novel, intergenogroup reassortant G3 rotavirus strains are spreading in at least three continents: Asia, Australia, and Europe. The present study provides evidence that a closely related G3P[8] strain circulated in Hungary during 2015. Whole genome sequencing and phylogenetic analysis showed that the identified strain continues to evolve by reassortment. This observation demonstrates the genomic plasticity of the novel strain, which is thought to be a prerequisite of the success of emerging rotavirus genotypes

Diabetes mellitus: endotheldiszfunkció és haemostasiselváltozások = Diabetes mellitus: endothelial dysfunction and changes in hemostasis
Absztrakt:
A diabetes mellitus magas vércukorszinttel járó krónikus metabolikus
kórelváltozások heterogén csoportját jelöli. A diabetes az élethosszig tartó
kezelés, gondozás és a markáns cardiovascularis szövődmények medicinális,
szociális, társadalmi következményeinek ellátása vagy éppen el nem látása miatt
az egyik leginkább költségigényes megbetegedés, az egészségügyi erőforrások
2–20%-át foglalja le. Cardiovascularis profilú belgyógyászati és sebészeti
betegellátó rendszerekben a cukorbetegek aránya a 30–40%-ot is elérheti.
Diabetes mellitusban a hyperglykaemia, az inzulinrezisztencia és a megváltozott
lipidanyagcsere kóros metabolikus miliőt teremt. Az endothelsejtben kóros
biokémiai, sejt- és szöveti szintű elváltozások alakulnak ki, melyek
endotheldiszfunkcióhoz, majd a micro- és a macrovascularis keringés
károsodásához vezetnek. Diabetes mellitusban thrombocytadiszfunkció is kialakul,
mert a glükóz a vérlemezkék sejtmembránján is inzulinfüggetlen transzporterrel
jut át. A vérplazmaalvadási faktorok koncentrációja a gyulladásos folyamatok
miatt megemelkedik, és fenotípusuk az oxidáció, nitrolizáció, glikáció miatt
megváltozik. Cukorbetegségben tehát direkt és indirekt hatások következtében a
prothromboticus folyamatok erősödnek, az antithromboticus tendenciák gyengülnek,
ez a haemostasis kóros thrombogen eltolódásával jár. Az endotheldiszfunkció és a
haemostasis egyensúlyvesztésének „from bench to clinical basics” ismeretei
megteremtik a lehetőséget, hogy alaposan meg tudjuk érteni a terápiás lépéseket,

Abstract:
Diabetes mellitus involves a group of chronic metabolic disorders with elevated
blood glucose concentrations. Since this disease needs lifelong treatment and
care, the medical and social aspects present major public health concerns and
pose a global challenge for health care providers. The number of aged patients
with degenerative diseases undergoing surgical procedures is continuously
increasing, resulting in an overwhelming dominance of diabetes in the
perioperative care. There is a particular need for an increased awareness of
diabetic patients in cardiovascular units, where the incidence of this disease
reaches as high as 30–40%. The main hallmarks of the pathologic metabolic milieu
of diabetes are hyperglycaemia, insulin resistance and pathologic lipid
metabolism. The biochemical, cellular and organ-level pathophysiological changes
lead to endothelial dysfunction including a low-grade prothrombotic balance,
inflammatory state and, as a consequence, impaired micro- and macrocirculation.
Diabetes is also followed by platelet dysfunction resulting from intracellular
hyperglycaemia, because thrombocytes have insulin-independent glucose
transporters in their cell membrane. The levels of the coagulation factors of
the plasma are increased, and these factors are also modified by oxidation and
glycation. Diabetes mellitus is a prothrombotic condition resulting from direct
and indirect tendencies of the endothelial platelet and the plasma coagulation
factors. The basic “bench to clinical basics” knowledge of the endothelial
dysfunction and prothrombotic balance in diabetes may contribute to the better
understanding of the clinical focuses in the perioperative care of patients with

Differences Between Central Venous and Cerebral Tissue Oxygen Saturation in Anaesthetised Patients With Diabetes Mellitus
Association of tumor location with anxiety and depression in childhood brain cancer survivors: a systematic review and meta-analysis
Abstract Objective This study aimed to evaluate the association between the location (supratentorial or infratentorial) of brain tumors and the development of depression and anxiety in childhood cancer survivors. Understanding the risk factors for the development of depression and anxiety disordersin these patients is crucial for early diagnosis and successful treatment. Methods The meta-analysis included articles that listed patients diagnosed with an intracranial tumor before the age of 18 years, provided the location of the tumor, had exact data on the prevalence of anxiety and depression, or measured these disorders using different assessment tools. The search was conducted in five different databases (MEDLINE, Embase, Web of Science, Scopus, and Cochrane Library). Risk of bias was assessed using QUIPS-2. Outcome measures used were prevalences and standardized means. Results The analysis included 42 eligible articles with a total number of 1071 patients. Relevant articles were cohort studies, cross-sectional studies, and case series. Based on the available data infratentorial brain tumor survivors had significantly higher scores on various assessment tools measuring anxiety (MRAW (raw mean scores): 36.24 [CI (confidence interval): 28.81–43.67]; versus MRAW: 23.21 (CI 0.91–45.51); p = 0.02, and depression (MRAW: 27.57 (CI 14.35–40.78) versus MRAW: 13.84 (CI 11.43–16.26); p < 0.01. Conclusion Childhood infratentorial cancer survivors have more impairments in terms of depression and anxiety; these children and adults should be monitored more frequently and may require closer follow-up on their mental health. The main limitation of our study originates from the lack of data on follow-up times used by different studies
