    Delivering Safe Surgical Care While Simultaneously Caring for Patients With COVID-19; Assessment of Patient Selection, Volume and Outcomes in a Tertiary Care Hospital

    Objectives: Compare patient selection and postoperative outcomes after surgical treatment for gastrointestinal disorders before and during the SARS-CoV-2 pandemic. Methods: We assessed gastrointestinal surgeries conducted at a tertiary center from 2017-2021 for differences in patient populations and procedures before (up to February 2020) and during the pandemic (March 2020 to December 2021). We analyzed mortality, Intensive Care Unit (ICU) length of stay, admission to ICU and postoperative complications for complex procedures using descriptive statistics and regression models. Results: 7309 procedures were analyzed, showing a caseload reduction in March and October 2020, but no statistical evidence for fewer overall procedures overall. Population characteristics differed with lower Body Mass Indices in 2020 and 2021, more patients smoking and with diabetes treated in 2020. There was no increased mortality, ICU length of stay and in 1,144 complex procedures assessed low overall morbidity at 90 days postoperative. Conclusion: Delivering surgical care while treating patients for COVID-19 in the same hospital was safe. Healthcare officials should consider continuing surgical care during future health crises as consequences of limiting surgical treatment for gastrointestinal disorders may be fatal for patients

    Tyrosine hydroxylase deficiency: a treatable disorder of brain catecholamine biosynthesis

    Tyrosine hydroxylase deficiency is an autosomal recessive disorder resulting from cerebral catecholamine deficiency. Tyrosine hydroxylase deficiency has been reported in fewer than 40 patients worldwide. To recapitulate all available evidence on clinical phenotypes and rational diagnostic and therapeutic approaches for this devastating, but treatable, neurometabolic disorder, we studied 36 patients with tyrosine hydroxylase deficiency and reviewed the literature. Based on the presenting neurological features, tyrosine hydroxylase deficiency can be divided in two phenotypes: an infantile onset, progressive, hypokinetic-rigid syndrome with dystonia (type A), and a complex encephalopathy with neonatal onset (type B). Decreased cerebrospinal fluid concentrations of homovanillic acid and 3-methoxy-4-hydroxyphenylethylene glycol, with normal 5-hydroxyindoleacetic acid cerebrospinal fluid concentrations, are the biochemical hallmark of tyrosine hydroxylase deficiency. The homovanillic acid concentrations and homovanillic acid/5-hydroxyindoleacetic acid ratio in cerebrospinal fluid correlate with the severity of the phenotype. Tyrosine hydroxylase deficiency is almost exclusively caused by missense mutations in the TH gene and its promoter region, suggesting that mutations with more deleterious effects on the protein are incompatible with life. Genotype-phenotype correlations do not exist for the common c.698G>A and c.707T>C mutations. Carriership of at least one promotor mutation, however, apparently predicts type A tyrosine hydroxylase deficiency.     The invention relates to an apex-locating method and device for determining the depth position of the apex in a dental root canal. It uses a device that makes it possible to form a circuit comprising a first electrode-probe inserted in the root canal of a tooth, a second electrode in conductive contact with a buccal mucous membrane, frequency generator means capable of producing alternating electrical signals at several frequencies, and means for measuring the electrical magnitude of alternating signals in the circuit. The invention makes provision for exciting the circuit and for measuring the levels of magnitude of the alternating signals (I), respectively, at low frequency (f) and at high frequency (F) and for detecting a crossing point (C1M) where the two levels (If, IF) measured at the low and high frequencies (f, F) meet and become substantially equal, these frequencies (f, F) being far enough apart so that this crossing point (C) exists.     Interaction Between Eutectic Intermetallic Particles and Dispersoids in the 3003 Aluminum Alloy During Homogenization Treatments

    International audienceThe transformation of primary eutectic Al-6(Mn,Fe) intermetallics into alpha-Al(Mn,Fe)Si and the precipitation of dispersoids were studied in the commercial in the form of 3003 series cast aluminum alloys, mainly under isothermal conditions between 673 K and 873 K (400 A degrees C and 600 A degrees C). After solidification, both the solid solution and the primary eutectic intermetallics were far from equilibrium. During further heat treatment, the precipitation of fine dispersoids and eutectoid transformation of the primary eutectic particles occurred simultaneously. Having characterized these evolutions under industrial homogenization conditions, the evolution of the microstructure (in terms of its nature, and the quantity, size, and chemical composition of the phases) was characterized during isothermal heat treatment, using scanning electron microscopy (SEM) and transmission electron microscopy (TEM) observations, quantitative image analysis, and transmission electron microscopy-energy-dispersive spectroscopy (TEM-EDS). The experimental results are analyzed, and changes in chemical composition are discussed and compared with the calculated equilibrium compositions. It is shown that (1) the chemical composition of eutectic intermetallics evolves and tends toward an equilibrium composition; (2) during precipitation, the chemical composition of dispersoids is constant, and close to the expected equilibrium composition when the initial mean composition of the solidification cell is taken into account; (3) after the formation of dispersoids, the quantity of alpha-Al(Mn,Fe)Si formed from the initial eutectic intermetallics increased, with the kinetics being controlled by long-range manganese diffusion; and (4) the latter evolution is associated with the dissolution of dispersoids located close to eutectic intermetallics and contributes to the formation of a dispersoid-free zone (DFZ)

    Podocotyloides stenometraPritchard, 1966 (Digenea: Opecoelidae) is the only trematode known to infect anthozoan corals. It causes disease in coral polyps of the genus Porites Link (Scleractinia: Poritidae) and its life-cycle depends on ingestion of these polyps by butterflyfishes (Perciformes: Chaetodontidae). This species has been reported throughout the Indo-Pacific, from the Seychelles to the Galápagos, but no study has investigated whether multiple species are involved. Here, we recollect P. stenometra from its type-host and type-locality, in Hawaiian waters, and describe four new species from examination of 768 butterflyfishes from French Polynesia. On the basis of morphology, phylogeny and life-history, we propose Polypipapiliotrema Martin, Cutmore & Cribb n. gen. and the Polypipapiliotrematinae Martin, Cutmore & Cribb n. subf., for P. stenometra (Pritchard) n. comb., P. citerovarium Martin, Cutmore & Cribb n. sp., P. hadrometra Martin, Cutmore & Cribb n. sp., P. heniochi Martin, Cutmore & Cribb n. sp., and P. ovatheculum Martin, Cutmore & Cribb n. sp. Given the diversity uncovered here and the ubiquity, abundance and diversity of butterflyfishes on coral reefs, we predict that Polypipapiliotrema will prove to comprise a rich complex of species causing disease in corals across the Indo-Pacific. The unique life-cycle of these taxa is consistent with phylogenetic distinction of the group and provides evidence for a broader basis of diversification among the family. We argue that life-cycle specialisation, in terms of adoption of disparate second intermediate host groups, has been a key driver of the diversification and richness of the Opecoelidae, the largest of all trematode families and the group most frequently encountered in coral reef fishes
