    Factors Affecting Academics’ Involvement in TEL Continuing Professional Development (CPD)

    Reinforcing the level of essentiality of understanding the factors that influence the involvement in TEL-oriented CPD and the challenges to the sustained expansion of their expertise not only for academics, but also professional bodies and educational developer for effective integration of digital technologies in teaching and learning remains is the current emphasis on strategic continuing professional development (CPD) programmes of technology enhanced learning (TEL) within higher education. A naturalistic approach and making use of qualitative method were applied in achieving the inquiry, using semi-structured interviews. The perceptions and views of twelve academics from one college in Qassim University (QU) in Saudi Arabia is going to be explored and discussed during this study; although academics had both internal and external motivations towards the value of involvement into TEL CPD opportunities, the findings indicated that significant challenges existed to the active participation of TEL programmes, including time and workload, relevant and realistic programme content that address needs, opportunities to practise the use of TEL, and accessibility and awareness. Furthermore, the fundamental conclusion of this paper that should be taken into account is that the vast majority of academics would like to involve and also value the importance of TEL CPD when their inhabiting factors are addressed; so as to impact positively on their academics into TEL CPD, the sufficient role of management support and institutional culture within Saudi organisations of higher education are highly recommended.     A prospective randomized clinical trial compared the effect of various types of local anesthetics cartridges on hypertensive patients during dental extraction

    Objectives: To evaluate hemodynamic changes of blood pressure and heart rate on hypertensive patients under - going tooth extraction using various types of local anesthesia (LA). Study Design: A prospective randomized clinical trial was conducted on 45 hypertensive patients who were divided equally into 3 parallel groups according to LA received. Group 1: Lidociane 2% with epinephrine 1: 80,000. Group 2: Prilocaine 3% with Felypressin 0.03 IU/ml. Group 3: Mepivacaine 3% plain. Inclusion criteria: hypertensive patients, under medical management with blood pressure = 159/99. Exclusion criteria: Blood pressure =160 /100 and patients receiving Ăź blockers. Negative aspiration was mandatory before the injection of 2 cartridges of LA. Blood pressure and heart rate were evaluated by Electronic Sphygmomanometer and Pulse Oximeters in 3 different time-points; 3 minutes before LA, three minutes after LA and three minutes after extraction. Results: The mean of systolic blood pressure (SBP) had increased after LA injection, and then decreased after ex - traction in the 3 groups of patients. Increase of SBP after extraction with (Mepivacaine plain) was higher than (Li - dociane with Epinephrine) and the difference was statistically significant using ANOVA ( p =0.037). The differences in the mean heart rates and mean diastolic blood pressures in the 3 groups were not significant. Conclusions: The increase of blood pressure with Epinephrine and Felypressin is negligible. Therefore, it is safe to use 2 cartridges of Lidociane 2% with Epinephrine 1:80,000 or Prilocaine 3% with Felypressin 0.03 IU/ml for hypertensive patients whose blood pressure = 159/99 provided negative aspiration is verified before injection

    Seroprevalence of antibodies to SARS-CoV-2 among blood donors in the early month of the pandemic in Saudi Arabia

    Background: Serologic testing provides better understanding of SARS-CoV-2 prevalence and its transmission. This study was an investigation of the prevalence of antibodies to SARS-CoV-2 among blood donors in Saudi Arabia. Objective: To estimate the seroprevalence of anti-SARS-CoV-2 antibodies among blood donors in Saudi Arabia during the early phase of the COVID-19 pandemic. Methods: Serology results and epidemiological data were analyzed for 837 adult blood donors, with no confirmed SARS-CoV-2 infection, in Saudi Arabia from 20th to 25th May 2020. Seroprevalence was determined using electrochemical immunoassay to detect anti-SARS-CoV-2 antibodies. Results: The overall seroprevalence of anti-SARS-CoV-2 antibodies was 1.4% (12/837). Non-citizens had higher seroprevalence compared with citizens (OR 13.6, p = 0.001). Secondary education was significantly associated with higher seroprevalence compared with higher education (OR 6.8, p = 0.005). The data showed that the highest seroprevalence was in Makkah (8.1%). Uisng Makkah seroprevalence as the reference, the seroprevalence in other areas was: Madinah 4.1% (OR 0.48, 95% CI 0.12-1.94), Jeddah 2.3% (OR 0.27, 95% CI 0.31-2.25), and Qassim 2.9 % (OR 0.34, 95% CI 0.04-2.89) and these were not statistically different from seroprevalence in the Makkah region. Conclusions: At the early months of the COVID-19 pandemic in Saudi Arabia, the seroprevalence of antibodies to SARS-CoV-2 among blood donors was low, but was higher among non-citizens. These findings may indicate that non-citizens and less educated individuals may be less attentive to preventive measures. Monitoring seroprevalence trends over time require repeated sampling

    Risk-Based Inspection and Rehabilitation Planning of Service Connections in Intermittent Water Supply Systems for Leakage Management in Arid Regions

    Most of the leakage in water distribution systems operating with plastic pipes occurs at service connections (SCs), while the existing tools plan rehabilitation of pipes. With limited water resources, intermittent supplies in arid regions further enhance the failure vulnerability of metal fittings on water mains due to scale formation and large pressure transients. The present research developed a risk-based methodology for the proactive maintenance of SCs in intermittent water supply systems. A five-generation bottom-up hierarchical approach aggregated the basic hydraulic, physical, and water quality factors to determine the vulnerability of structural failures of SCs. Hydraulic parameters (pressure and velocity) were estimated by simulating a distribution network of 366 water mains of diameters ranging from 110 mm to 225 mm serving 371 SCs in a residential neighborhood located in the Qassim region of Saudi Arabia. Age, depth, and length of SCs’ estimated the condition index, while soil corrosivity and condition of the water mains were also counted when assessing the structural failure index for each SC. Water quality parameters, e.g., pH, turbidity, and iron, that can contribute to the vulnerability of an SC’s failure were also included. Fuzzy-based methods first assessed the relative importance weights of the basic input parameters at the bottom of the hierarchy and the risk factors in the middle of the hierarchy. Subsequently, the performance and condition scores were aggregated to develop respective indices. As the consequence of structural failure is high for the SCs serving households with a large number of residents, the final risk index aggregates the vulnerability and consequence at the hierarchy’s top. The developed model was effectively validated by comparing the SCs of high priority with the leaking and repaired SCs in the past. The method will be a useful tool for planning proactive inspection and rehabilitation of SCs of intermittent supply systems to minimize water losses (less than 8% of the national benchmark) in Saudi Arabia and elsewhere

    Psychotic Experiences and Alternate Dimensions: A Thematic Analysis Exploring Frameworks of Psychotic Symptoms Among Saudis

    Psychotic expression is influenced by unique contexts, including the individual’s culture. The majority of research on psychotic experiences is quantitative and from Western, democratic societies. This article explores the explanatory models used by Saudis to describe psychotic experiences (i.e., hallucinations and delusions). Using open-ended responses to a structured psychosis screener embedded within a comprehensive mental health survey instrument, we conducted thematic analysis on data representing the psychotic experiences of 59 individuals. We found that Saudis report religious (e.g., Jinns) and cultural (e.g., modest clothing) frameworks alongside biological, psychological, and social mechanisms which potentially trigger an alternative reality for the affected individual. Our findings suggest it may be helpful for health care professionals to consider individual differences and work with religious leaders (e.g., Shaykhs ) to prevent misdiagnosis and mistreatment. In-depth qualitative studies are needed to examine trajectories of psychotic symptomatology among Saudis and the specific language used to describe such occurrences

    Angiotensin II Exaggerates SARS-CoV-2 Specific T-Cell Response in Convalescent Individuals following COVID-19

    Dysregulation of renin−angiotensin systems during coronavirus disease 2019 (COVID-19) infection worsens the symptoms and contributes to COVID-19 severity and mortality. This study sought to investigate the effect of exogenous angiotensin II (Ang-II) on severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)-specific T-cells response in recovered COVID-19 patients. Human peripheral blood mononuclear cells (PBMCs) were treated with Ang II and then stimulated with a SARS-CoV-2 peptide pool. T-cell responses were measured using flow cytometry, while enzyme-linked immunosorbent assay (ELISA) and intracellular cytokine staining (ICS) assays determined functional capability and polarization. Additionally, the relative level of protein phosphorylation was measured using a phosphokinase array. Our results showed that Ang II treatment significantly increased the magnitude of SARS-CoV-2-specific T-cell response in stimulated PBMCs with a SARS-CoV-2 peptide pool. Moreover, the phosphorylation levels of numerous proteins implicated in cardiovascular diseases, inflammation, and viral infection showed significant increases in the presence of Ang II. The mitogenic stimulation of PBMCs after Ang II and SARS-CoV-2 peptide pool stimulation showed functional polarization of T-cells toward Th1/Th17 and Th17 phenotypes, respectively. Meanwhile, ELISA showed increased productions of IL-1β and IL-6 in Ang II-stimulated PBMCs without affecting the IL-10 level. To our knowledge, this study is the first to demonstrate that Ang II exaggerates SARS-CoV-2-specific T-cells response. Therefore, during COVID-19 infection, Ang II may aggravate the inflammatory response and change the immune response toward a more inflammatory profile against SARS-CoV-2 infection

    Demographic Characteristics and Status of Vaccinated Individuals with a History of COVID-19 Infection Pre- or Post-Vaccination: A Descriptive Study of a Nationally Representative Sample in Saudi Arabia

    Background: Saudi Arabia expedited the approval of some COVID-19 vaccines and launched mass vaccination campaigns. The aim of this study was to describe the demographics of vaccinated COVID-19 cases and compare the mortality rates of COVID-19 cases who were infected post-vaccination in Saudi Arabia. Methods: This was a retrospective cohort study. We retrieved data for COVID-19 cases who were infected pre- or post-vaccination and had received at least one injection of the Oxford–AstraZeneca or Pfizer–BioNTech vaccine from 4 December 2020 to 15 October 2021. Results: The number of patients who were infected and had received at least one dose of a COVID-19 vaccine was 281,744. Approximately 45% of subjects were infected post-vaccination, and 75% of subjects had received the Pfizer–BioNTech vaccine. Only 0.342% of the patients who were infected post-vaccination died, and 447 patients were admitted to ICUs. Most of the patients who were infected with COVID-19 post-vaccination and were admitted to ICUs (69.84%) had received only one dose of the vaccine (p < 0.0001). The mean time to infection for patients who had received one and two doses of the Oxford–AstraZeneca vaccine were 27 and 8 days longer than their counterparts who had received one and two doses of Pfizer–BioNTech vaccine, respectively. No difference in the odds of mortality between the Pfizer–BioNTech and Oxford–AstraZeneca vaccines was found (OR = 1.121, 95% CI = [0.907–1.386], p-value = 0.291). Patients who had received two doses of the vaccine had significantly lower odds of mortality compared to those who had received one dose (p < 0.0001). Conclusions: Vaccines are vital in combating the COVID-19 pandemic. The results of this study show no difference between the Pfizer–BioNTech and Oxford–AstraZeneca vaccines in the rate of mortality. However, the number of vaccine doses was significantly associated with a lower risk of mortality. Future studies should examine the effectiveness of different COVID-19 vaccines using real-world data and more robust designs
