A qualitative examination of barriers against effective medical education and practices related to breastfeeding promotion and support in Lebanon.
Background: Insufficient breastfeeding promotion and support by physicians contribute to suboptimal breastfeeding rates globally. Understanding setting-specific barriers against breastfeeding promotion and support from the perspective of medical students and addressing those that can be modified through undergraduate medical education may help improve learning outcomes, medical practice, and ultimately health outcomes associated with breastfeeding.Objectives: We selected the underserved and under-supported public medical school in Lebanon to explore psychosocial, institutional, and societal barriers hindering effective preventative medicine practices using breastfeeding promotion and support as an exemplar case.Methods: One-on-one semi-structured interviews, each lasting around 60 min, were conducted with medical interns (in Med III and Med IV) at their training hospitals. Interviews were voice-recorded, transcribed verbatim, coded, and analyzed thematically based on Theory of Planned Behavior.Results: Interns (n= 49; 96% response rate) completed the study. Five major themes emerged addressing barriers at various levels. At the health care system level at large, interns identified the predominant focus on pathophysiology and treatment rather than on disease prevention and health promotion as a barrier. At the level of trainees and their education experiences, interns reported limited and optional clerkship training in obstetrics/gynecology and in neonatology which contributes to their insufficient knowledge and low self-efficacy. Competing financial interests from infant formula companies and social pressures to promote infant formula were identified as two main barriers at the level of physicians and clinical practice.Conclusions: Our work using breastfeeding as an exemplary case highlights how undergraduate medical education and its learning outcomes and how medical practices and patient behavior are highly intertwined with psychosocial, institutional, and social drivers and constraints. Re-evaluating the success of undergraduate medical curricula in light of overcoming these constraints and not only based on meeting national accreditation and certification guidelines might prove helpful in improving medical education and ultimately clinical practice

A Galerkin boundary element method for high frequency scattering by convex polygons
In this paper we consider the problem of time-harmonic acoustic scattering in two dimensions by convex polygons. Standard boundary or finite element methods for acoustic scattering problems have a computational cost that grows at least linearly as a function of the frequency of the incident wave. Here we present a novel Galerkin boundary element method, which uses an approximation space consisting of the products of plane waves with piecewise polynomials supported on a graded mesh, with smaller elements closer to the corners of the polygon. We prove that the best approximation from the approximation space requires a number of degrees of freedom to achieve a prescribed level of accuracy that grows only logarithmically as a function of the frequency. Numerical results demonstrate the same logarithmic dependence on the frequency for the Galerkin method solution. Our boundary element method is a discretization of a well-known second kind combined-layer-potential integral equation. We provide a proof that this equation and its adjoint are well-posed and equivalent to the boundary value problem in a Sobolev space setting for general Lipschitz domains

Effects of Work Permits on Illegal Employment Among Youth Workers: Findings of a School-Based Survey on Child Labor Violations
We compared self-reported child labor violations between teenagers with and without work permits. Data were obtained from a school-based survey of working teenagers in 16 randomly selected high schools in North Carolina. We examined associations between work hour violations, hazardous order violations (performance of illegal tasks), and possession of a work permit. Work permits appear to be protective against performance of illegal tasks but not against work hour violations, demonstrating the need for stricter enforcement policies and improvements in work permit screening processes
Design of encrypted transceiver Non-coherent OFDM with ability to correct coding bits of information
A differential encrypted transceiver was designed in the signal-to-noise ratio systems. A time"-"spread" frequency-"encoded (TSFE) algorithm was proposed using the orthogonal frequency multiplexing multiplier OFDM. The proposed design was verified under the Doppler frequency effect and there was an improvement in the performance of the signal-to-noise ratio system. As a result, the reliability of decoded data was increased and achieves the ability to correct coding bits of information
Balancing Compression and Encryption of Satellite Imagery
With the rapid developments in the remote sensing technologies and services, there is a necessity for combined compression and encryption of satellite imagery. The onboard satellite compression is used to minimize storage and communication bandwidth requirements of high data rate satellite applications. While encryption is employed to secure these resources and prevent illegal use of image sensitive information. In this paper, we propose an approach to address these challenges which raised in the highly dynamic satellite based networked environment. This approach combined compression algorithms (Huffman and SPIHT) and encryptions algorithms (RC4, blowfish and AES) into three complementary modes: (1) secure lossless compression, (2) secure lossy compression and (3) secure hybrid compression. The extensive experiments on the 126 satellite images dataset showed that our approach outperforms traditional and state of art approaches by saving approximately (53%) of computational resources. In addition, the interesting feature of this approach is these three options that mimic reality by imposing every time a different approach to deal with the problem of limited computing and communication resources
Immediate effect of kinesiology tape on ankle stability
BackgroundLateral ankle sprain is one of the most common musculoskeletal injuries, particularly among the sporting population. Due to such prevalence, many interventions have been tried to prevent initial, or further, ankle sprains. Current research shows that the use of traditional athletic tape can reduce the incidence of sprain recurrence, but this may be at a cost to athletic performance through restriction of motion. Kinesiology tape, which has become increasingly popular, is elastic in nature, and it is proposed by the manufacturers that it can correct ligament damage. Kinesiology tape, therefore, may be able to improve stability and reduce ankle sprain occurrence while overcoming the problems of traditional tape.AimTo assess the effect of kinesiology tape on ankle stability.Methods27 healthy individuals were recruited, and electromyography (EMG) measurements were recorded from the peroneus longus and tibialis anterior muscles. Recordings were taken from the muscles of the dominant leg during induced sudden ankle inversion perturbations using a custom-made tilting platform system. This was performed with and without using kinesiology tape and shoes, creating four different test conditions: barefoot(without tape), shoe(without tape), barefoot(with tape) and shoe(with tape). For each test condition, the peak muscle activity, average muscle activity and the muscle latency were calculated.ResultsNo significant difference (p>0.05) was found by using the kinesiology tape on any of the measured variables while the wearing of shoes significantly increased all the variables.ConclusionKinesiology tape has no effect on ankle stability and is unable to nullify the detrimental effects that shoes appear to have
