    Neurodiversity-Affirming Practices are a Moral Imperative for Occupational Therapy

    The term neurodiversity encompasses neurological differences such as clinical labels of autism, learning disabilities, synesthesia, hyperactivity disorders, and more. Proponents of the neurodiversity movement argue that current therapeutic and medical practices often attempt to “normalize” behaviors and ways of participation that originate from these differences in neurology and contribute to an individual’s sense of identity. This paper argues that an ethical and morally just occupational therapy practice should affirm neurodivergent ways of being, and that occupational therapists must be active agents of change by listening to and collaborating with their neurodiverse clientele. We focus the discussion on our work with autistic individuals and consider past and current practice trends, including applied behavioral analysis, in light of various ethical mandates for occupational therapy. We conclude with suggestions for core tenets of neurodiversity-affirming occupational therapy practice with the hope that clinicians can apply these concepts to their clinical work and recognize how meaningful participation can be achieved by creating goals and interventions through a neurodiversity framework

    Occupational Disruption: The Influence of the COVID-19 Pandemic on the Behavioral Inflexibility and Anxiety of Autistic Children

    Background: The COVID-19 pandemic caused unprecedented changes to the lives of many. The aim of this paper was to understand how the COVID-19 pandemic impacted behavioral inflexibility (BI) and anxiety among autistic children and how autistic children and their families have adapted to COVID-19-related routine changes. Methods: This sequential mixed-method study included two phases. During the first phase, parents of autistic children (N = 48) completed an online survey consisting of the Behavioral Inflexibility Scale (BIS) and the Parent-Rated Anxiety Scale – Autism Spectrum Disorder (PRAS-ASD). During the second phase, a subset of parents (parents of adolescents, N = 11) was invited to participate in a virtual focus-group. Results: The parents reported a wide range of BI during the pandemic (BIS M = 2.03, SD = 1.02, range = 0.21 – 3.86). Child BI was a significant predictor of anxiety (t[40] = 5.56, p \u3c .0001). From the focus groups, 155 codes were organized into four themes, two of which are discussed in this paper: child client factors that have changed during the pandemic and changes to family routines. Conclusions: In this preliminary study, the parents identified problematic BI and anxiety among their autistic children. The outcomes of this small-scale study indicate that some of the changes to routines brought about by the pandemic may be helpful for autistic children after the pandemic restrictions have ended. We provide a unique perspective on how to leverage occupational changes that resulted from the COVID-19 pandemic to aid autistics during non-pandemic times

    Double Averaging Analysis Applied to a Large Eddy Simulation of Coupled Turbulent Overlying and Porewater Flow

    Freestream turbulence in rivers is a key contributor to the flux of dissolved nutrients, carbon, and other ecologically important solutes into porewater. To advance understanding of turbulent hyporheic exchange and porewater transport, we investigate flow over and through a rough bed of spheres using large eddy simulation (LES). We apply double averaging (combined space and time averaging) to the LES results to determine the mean velocity distribution, momentum balance, and drag forces. Our simulations show large-scale freestream structures interacting strongly with vortices generated at the surfaces of individual spheres to control turbulent momentum fluxes into the bed. The transition between turbulent flow and Darcy flow occurs over the first row of spheres, where turbulence decays rapidly and turbulent kinetic energy, Reynolds stress, and drag forces peak. Below this region, turbulence is only present in the high-velocity flow in open pore throats. Experimental observations suggest that minimum mean porewater velocity occurs in the first open pore space below the transition region, but our results show that the minimum occurs between the first and second pore spaces. The simulation mean porewater velocities are approximately half those captured in measurements because the model resolves the entire flow continuum while measurements can access high-velocity fluid in open pores. The high-resolution dual time-space averaging of the LES resolves both turbulent and mean flow features that are important to interfacial solute and particle fluxes, providing a means to include turbulent hyporheic exchange in upscaled river models, which has not been achieved to date
