Velar activity in individuals with velopharyngeal insufficiency assessed by acoustic rhinometry
Acoustic rhinometry is routinely used for the evaluation of nasal patency. Objective: To investigate whether the technique is able to identify the impairment of velopharyngeal (VP) activity in individuals with clinical diagnosis of velopharyngeal insufficiency (VPI). Methods: Twenty subjects with repaired cleft palate and inadequate velopharyngeal function (IVF) and 18 non-cleft controls with adequate velopharyngeal function (AVF), adults, of both genders, were evaluated. Area-distance curves were obtained during VP rest and speech activity, using an Eccovision Acoustic Rhinometry system. Volume was determined by integrating the area under the curve at the segment corresponding to the nasopharynx. VP activity (ΔV) was estimated by the absolute and relative differences between nasopharyngeal volume at rest (Vr) and during an unreleased /k/ production (Vk). The efficiency of the technique to discriminate IVF and AVF was assessed by a ROC curve. Results: Mean Vk and Vr values (±SD) obtained were: 23.2±3.6 cm3 and 15.9±3.8 cm3 (AVF group), and 22.7±7.9 cm3 and 20.7±7.4 cm3 (IVF group), corresponding to a mean ΔV decay of 7.3 cm3 (31%) for the AVF group and a significantly smaller ΔV decay of 2.0 cm3 (9%) for the IVF group (

Speech resonance after septoplasty in a patient with bilateral cleft lip and palate
Purpose: The nasal airway is an important regulator of the pressures generated in speech when velopharyngeal function is altered. Patients with cleft palate often have nasal obstruction and increased nasal resistance, which can compensate a possible velopharyngeal dysfunction (VPD) and mask hypernasality. Clinical report: A 16 yearold patient, male, underwent nasometric and rhinomanometric assessment before and one year after septoplasty and bilateral inferior turbinectomy surgery. Nasometry was used to evaluate the nasalance scores (acoustic correlate of nasality) during the reading of a set of 5 sentences containing predominantly nasal sounds (nasal text) and a set containing exclusively oral sounds (oral text). Rhinomanometry permits the determination of the minimum nasal cross-sectional area (CSA) by the simultaneous measurement of the differential transnasal pressure and nasal air flow during resting breathing. Before surgery, the nasalance values were 40% and 26% in nasal and oral text, respectively, indicating hyponasality. Rhinomanometry showed that the values obtained for nasal area were 0.571cm2 and 0.094cm2 in the right and left sides respectively indicating a reduced nasal CSA of the left side. After surgery, there was an increase in nasalance to 55% in the nasal text and to 40% in the oral text, indicating hypernasality. Rhinomanometry indicated that nasal area were 0.237cm2 and 0.287cm2, in the right and left sides respectively. Conclusion: The surgery resulted in an increase of the nasal area and improving nasal patency. However, hypernasality was demonstrated. These results confirm what has already been reported by Warren et al (1992) in stating that in the presence of VPD "a good nose for breathing is often a bad nose for speech"
Speech nasality and nasalance following secondary Sommerlad palatoplasty in individuals with velopharyngeal insufficiency: a prospective study
Objetivo: A insuficiência velofarÃngea (IVF) é uma observação frequente em indivÃduos com fissura palatina, mesmo após a palatoplastia primária. Palatoplastias secundárias são, nestes casos, necessárias para promover o fechamento velofarÃngeo. O presente estudo teve como objetivo verificar a eficácia da palatoplastia secundária de Sommerlad no tratamento das desordens de fala caracterÃsticas da IVF, por se tratar de um procedimento mais fisiológico e menos obstrutivo. Material e métodos: Vinte e três indivÃduos com idade entre 6 e 23 anos, de ambos os sexos, portadores de fissura de palato±lábio operada(s), com diagnóstico de IVF, foram avaliados antes e após a palatoplastia secundária pela técnica de Sommerlad. As caracterÃsticas da fala relacionadas à função velofarÃngea (nasalidade, impressão global da função velofarÃngea e sintomas ativos de fala) foram investigadas por avaliação perceptivo-auditiva realizada por três fonoaudiólogos experientes na área, utilizando gravação digital em áudio de vocábulos e sentenças padronizados, e, por análise instrumental da nasalância da fala por meio de um nasômetro, utilizando leitura ou repetição de sentenças padronizadas. O sucesso cirúrgico foi interpretado como eliminação ou redução da hipernasalidade. Resultados: O sucesso cirúrgico foi observado em 14 dos 20 pacientes analisados na avaliação perceptivo-auditiva (70%), sendo a eliminação da hipernasalidade constatada em 11 (55%) dos pacientes e a redução, em 3 (15%). Parâmetros pós-operatórios normais foram observados apenas nas crianças (6-16 anos). Observou-se significante redução da média da nasalância para as sentenças orais e nasais após a cirurgia para o total de participantes. A análise individual dos dados mostrou que 57% dos pacientes apresentaram valores de nasalância sugestivos de eliminação ou redução da hipernasalidade após a cirurgia. A redução da nasalância após a cirurgia foi significativamente maior nos participantes de 6 a 12 anos, corroborando com os achados da avaliação perceptivo-auditiva. Nenhum paciente apresentou escores de nasalância sugestivos de hiponasalidade. Conclusão: A avaliação perceptivo-auditiva da fala e a análise da nasalância mostraram que, em indivÃduos com fissura de palato operada e IVF, a palatoplastia secundária de Sommerlad demonstrou ser um procedimento eficaz na eliminação da hipernasalidade, sem levar à hiponasalidade, que pode ser observada após a cirurgia de retalho farÃngeo.Objective: Velopharyngeal insufficiency (VPI) is frequently seen among individuals with cleft palate, even after primary palatoplasty. Secondary palatoplasties are necessary to favor velopharyngeal closure. The present study aimed to verify the efficacy of secondary Sommerlad palatoplasty (palate re-repair), a more physiological and less obstructive procedure for the treatment of VPI speech disorders. Material and methods: Twenty-three individuals, aged 6 to 23 years, of both sexes, with repaired cleft palate±lip and VPI, were evaluated before and after secondary palatoplasty by the Sommerlad technique. Speech characteristics related to velopharyngeal function (nasality, global impression of velopharyngeal function and active speech symptoms) were investigated by auditory-perceptual evaluation performed by three experienced speech therapists, using audio-digital recording of standardized words and sentences, and by instrumental analysis of speech nasalance, with a nasometer, using reading or repetition of standardized sentences. Surgical success was defined as absence or reduced hypernasality on speech assessment. Results: Surgical success was observed in 14 out of the 20 patients analyzed perceptuallly (70%). Hypernasality was eliminated in 11 (55%) patients and reduced in 3 (15%). Normal postoperative parameters were observed only among children (6-16 years). A significant reduction in mean nasalance was observed for oral and nasal sentences after surgery in the group of 23 participants. Individual analysis of data showed that 57% of the patients had postoperative nasalance values suggestive of elimination or reduction of hypernasality. The nasalance decrease observed after surgery was significantly greater in the participants aged 6 to 12 years, corroborating findings of the auditory-perceptual assessment. No patient presented nasalance scores suggestive of hyponasality. Conclusion: The auditoryperceptual assessment of speech and nasalance analysis have shown that in individuals with repaired cleft palate and VPI, the Sommerlad secondary palatoplasty proved to be effective in eliminating hypernasality, without causing the hyponasality typically seen after the pharyngeal flap surgery
Assessment of velar mobility in individuals with velopharyngeal insufficiency by acoustic rhinometry
Objetivo: Verificar se a rinometria acústica, usada de rotina para avaliar a patência nasal, é capaz de identificar a deficiência no movimento velar em indivÃduos com diagnóstico clÃnico de função velofarÃngea inadequada (FVI). Modelo: Estudo clÃnico prospectivo Local de Execução: Hospital de Referência Especializado. Participantes: Vinte indivÃduos com fissura de palato reparada e FVI residual e 18 indivÃduos-controle sem fissura de palato e função velofarÃngea adequada (FVA), adultos, de ambos os sexos. Variáveis analisadas: Curvas área-distância foram obtidas no repouso velar e na fala (fonema /k/), utilizando um sistema Eccovision AR, sendo o volume determinado pela integração da área sob a curva em segmento correspondente à nasofaringe. A mobilidade velar (V) foi estimada pela diferença absoluta e relativa entre o volume nasofarÃngeo no repouso velar (Vr) e na fala (Vk). A eficiência da técnica em discriminar FVI e FVA foi analisada pela curva ROC. Resultados: Os valores médios (±DP) de Vr e Vk obtidos foram: 23,2±3,6cm3 e 15,9±3,8cm3, no grupo FVA, e 22,7±7,9cm3 e 20,7±7,4cm3, no grupo FVI, correspondendo a uma redução média de 7,3cm3 (31%) no grupo FVA e a uma redução significativamente menor, de 2,0cm3 (9%), no grupo FVI (p<0,05). Constatou-se que 70% dos pacientes do grupo FVI apresentaram V sugestivo de elevação velar prejudicada (inferior ao limiar de corte que maximizou, simultaneamente, a sensibilidade e a especificidade do teste), confirmando o diagnóstico clÃnico. Conclusão: A rinometria acústica foi capaz de identificar, com bom poder discriminatório, o comprometimento da atividade velar que caracteriza a insuficiência velofarÃngea.Objective: To determine whether acoustic rhinometry, routinely used for evaluation of nasal patency, is able to identify impairment of velar movement in individuals with clinical diagnosis of inadequate velopharyngeal function (IVF). Dimensões nasofarÃngeas em indivÃduos sem anomalias craniofaciais: dados normativos
RESUMO Objetivo Determinar os valores controles da área de secção transversa mÃnima nasofarÃngea de indivÃduos sem anomalias craniofaciais e em diferentes faixas etárias. Material e Método Participaram do estudo 96 indivÃduos sem anomalias craniofaciais, de ambos os sexos, com Ãndice de massa corpórea e circunferência cervical normais, subdivididos em 4 grupos etários: crianças com idade entre 6 e 10 anos (G1), adolescentes de 11 a 17 anos (G2), adultos jovens entre 18 e 39 anos (G3) e adultos de meia-idade entre 40 e 59 anos (G4). A área seccional transversa mÃnima nasofarÃngea (área nasofarÃngea – ANF) foi determinada por meio de rinomanometria anterior modificada (técnica fluxo-pressão), utilizando o sistema PERCI-SARS (versão 3.50 – Microtronics Corp.). Resultados Os valores médios±DP da ANF foram de 1,025±0,054cm2, 1,055±0,081cm2, 1,050±0,083cm2 e 1,054±0,081cm2, respectivamente, para G1, G2, G3 e G4, não havendo diferença entre as 4 faixas etárias. Conclusão Os valores controles da ANF foram determinados para indivÃduos sem anomalias craniofaciais de diferentes faixas etárias e servirão de referência na rotina clÃnica e em estudos envolvendo diagnóstico de obstrução nasofarÃngea, principalmente na presença de anomalias craniofaciais
Análise comparativa da atividade velofarÃngea aferida por rinometria acústica e rinomanometria
RESUMO Objetivo: Analisar a atividade velofarÃngea (VF) de indivÃduos com disfunção velofarÃngea (DVF) aferida por rinometria acústica, comparativamente à rinomanometria. Métodos: Estudo clÃnico prospectivo em 41 adultos, de ambos os gêneros, com fissura de palato±lábio previamente operada e DVF residual ao exame clÃnico, sem articulação compensatória nas plosivas surdas [p], [t] e [k]. Variáveis analisadas: (1) variação volumétrica da nasofaringe (∆V) na produção das três plosivas, relativamente ao repouso, por rinometria acústica (reduções <3 cm3 foram consideradas como ausência de atividade velofarÃngea); (2) área do orifÃcio velofarÃngeo (área VF), por rinomanometria anterior modificada; áreas ≥0,05 cm2 foram consideradas como fechamento inadequado. Na comparação das técnicas foi utilizada a plosiva [p] (n=24). Resultados: Observou-se: (1) ∆V médio de 18% no [k], significantemente menor (p<0,05) que a redução relatada para normais (30%); valores de ∆V sugestivos de DVF constatados em 59% dos casos. Resultados similares foram obtidos no [p] e [t], mostrando-se mais apropriados para o exame rinométrico, por não envolverem a participação da lÃngua no fechamento velofarÃngeo, diferentemente da plosiva velar [k]; (2) fechamento VF inadequado em 85% dos casos. Não houve correlação significativa entre o ∆V e a área do orifÃcio velofarÃngeo. A concordância de diagnóstico entre os métodos ocorreu em 51% dos casos. Conclusão: A rinometria acústica não apresentou boa acurácia como método de diagnóstico da DVF frente ao método padrão. Demonstrou, contudo, potencial como método de acompanhamento dos resultados de intervenções clÃnico-cirúrgicas que levem à maior atividade velar e farÃngea
