Decomposition of variance in terms of conditional means
We test against two different sets of data an apparently new approach to the
analysis of the variance of a numerical variable which depends on qualitative
characters. We suggest that this approach be used to complement other existing
techniques to study the interdependence of the variables involved. According to
our method the variance is expressed as a sum of orthogonal components,
obtained as differences of conditional means, with respect to the qualitative
characters. The resulting expression for the variance depends on the ordering
in which the characters are considered. We suggest an algorithm which leads to
an ordering which is deemed natural. The first set of data concerns the score
achieved by a population of students, on an entrance examination, based on a
multiple choice test with 30 questions. In this case the qualitative characters
are dyadic and correspond to correct or incorrect answer to each question. The
second set of data concerns the delay in obtaining the degree for a population
of graduates of Italian universities. The variance in this case is analyzed
with respect to a set of seven specific qualitative characters of the
population studied (gender, previous education, working condition, parent's
educational level, field of study, etc.)Comment: 3 figure
The Bio-Molecular Dynamics of Dental Pulp in Different Clinical Scenarios
Dental pulp (DP) is a very dynamic tissue both in health and in disease. When exposed to stressors and pathological conditions. It undergoes a complex series of biological reactions whereby alterations affect the pulp tissue at tissue cellular and molecular levels.
The aim of this review is to update the reader on the various bio-molecular alterations in the dental pulp under different clinical conditions: orthodontic treatment (OT), caries, pulpitis and others.
The morphological changes in the composition of the DP rang from the reversible remodeling to apoptosis and sometimes necrosis. Many apoptotic factors are involved like Bcl2, Bax and the significant increase in Caspases 9 and 3, as well as, Hsp60, its possible role and its mitochodrial localization.
The inflammatory responses in dental pulp and the role of diffusible and cellular factors as well as DP stem cells were highlighted, in particular, where caries was involved in the pulpitis.Recent data report changes in tissue metabolism and homeostasis inside the DP caused by OT leading to increased levels of iNOS reactivity in the nerve fibers of the pulp.
Moreover, remodeling of the extra cellular matrix(ECM) is an important feature in clinical scenarios like OT and caries whereby alterations in MMP-2 and MMP-9 expression patterns are reported leading to degradation of type IV and V collagens in the ECM.
Furthermore, neurogenic factors are also modified after injuries and OT. Neuropeptides play a significant role not onlyin pain perception but also in vascular responses. Substance P increases in DP and enhances pain perception and so is the increase in CGRP which is correlated with concomitant gain in bone morphogenetic protein expression resulting in more dentin formation.
The role of stem cells and the possible molecular mechanisms of dentin genesis are presented in this review. They focus on important signaling proteins and the possible role of various scaffolds in this regeneration process.
In conclusion, most alterations inpulpal structure are reversible unless the pulp has a history of caries, restorations, trauma or prolonged heavy orthodontic forces. Pulpal symptoms arising from these clinical conditions should be treated appropriately and swiftly.Otherwise, exacerpation of pulpitis and the interplay of the various bio-molecular factors will lead to inhibition of repair and regeneration

Long-term occipital nerve stimulation for drug-resistant chronic cluster headache
Introduction Chronic cluster headache is rare and some of these patients become drug-resistant. Occipital nerve stimulation has been successfully employed in open studies to treat chronic drug-resistant cluster headache. Data from large group of occipital nerve stimulation-treated chronic cluster headache patients with long duration follow-up are advantageous. Patients and methods Efficacy of occipital nerve stimulation has been evaluated in an experimental monocentric open-label study including 35 chronic drug-resistant cluster headache patients (mean age 42 years; 30 men; mean illness duration: 6.7 years). The primary end-point was a reduction in number of daily attacks. Results After a median follow-up of 6.1 years (range 1.6–10.7), 20 (66.7%) patients were responders (≥50% reduction in headache number per day): 12 (40%) responders showed a stable condition characterized by sporadic attacks, five responders had a 60–80% reduction in headache number per day and in the remaining three responders chronic cluster headache was transformed in episodic cluster headache. Ten (33.3%) patients were non-responders; half of these have been responders for a long period (mean 14.6 months; range 2–48 months). Battery depletion (21 patients 70%) and electrode migration (six patients – 20%) were the most frequent adverse events. Conclusions Occipital nerve stimulation efficacy is confirmed in chronic drug-resistant cluster headaches even after an exceptional long-term follow-up. Tolerance can occur years after improvement
The chaperone system in cancer therapies: Hsp90
: The chaperone system (CS) of an organism is composed of molecular chaperones, chaperone co-factors, co-chaperones, and chaperone receptors and interactors. It is present throughout the body but with distinctive features for each cell and tissue type. Previous studies pertaining to the CS of the salivary glands have determined the quantitative and distribution patterns for several members, the chaperones, in normal and diseased glands, focusing on tumors. Chaperones are cytoprotective, but can also be etiopathogenic agents causing diseases, the chaperonopathies. Some chaperones such as Hsp90 potentiate tumor growth, proliferation, and metastasization. Quantitative data available on this chaperone in salivary gland tissue with inflammation, and benign and malignant tumors suggest that assessing tissue Hsp90 levels and distribution patterns is useful for differential diagnosis-prognostication, and patient follow up. This, in turn, will reveal clues for developing specific treatment centered on the chaperone, for instance by inhibiting its pro-carcinogenic functions (negative chaperonotherapy). Here, we review data on the carcinogenic mechanisms of Hsp90 and their inhibitors. Hsp90 is the master regulator of the PI3K-Akt-NF-kB axis that promotes tumor cell proliferation and metastasization. We discuss pathways and interactions involving these molecular complexes in tumorigenesis and review Hsp90 inhibitors that have been tested in search of an efficacious anti-cancer agent. This targeted therapy deserves extensive investigation in view of its theoretical potential and some positive practical results and considering the need of novel treatments for tumors of the salivary glands as well as other tissues

Salvage Procedure in Case of Urethrocavernous Fistula after Revision Surgery for Malfunctioning Three-Piece Penile Prosthesis
Urethrocavernous fistula is a rare complication of penile prosthesis. Literature lacks any materials regarding this complication’s treatment. We report our experience with a 66-year-old man who developed urethrocavernous fistula after penile prosthesis implant. Our technique involves the careful closure of urethral and corpus cavernosum defects with application of TachoSil® above the sutures. After the salvage procedure, no recurrence of fistula occurred and patient was able to have sexual intercourse. We believe that our technique may be successfully used in case of urethrocavernous fistula after penile prosthesis implant
Thyme and Savory Essential Oil Efficacy and Induction of Resistance against Botrytis cinerea through Priming of Defense Responses in Apple
The efficacy of thyme and savory essential oils were investigated against Botrytis cinerea on apple fruit. Apples treated with thyme and savory essential oils showed significantly lower gray mold severity and incidence. Thyme essential oil at 1% concentration showed the highest efficacy, with lower disease incidence and smaller lesion diameter. The expression of specific pathogenesis-related (PR) genes PR-8 and PR-5 was characterized in apple tissues in response to thyme oil application and B. cinerea inoculation. After 6 h of pathogen inoculation, thyme essential oil induced a 2.5-fold increase of PR-8 gene expression compared to inoculated fruits. After 24 h of inoculation, PR-8 was highly induced (7-fold) in both thyme oil-treated and untreated apples inoculated with B. cinerea. After 48 h of inoculation, PR-8 expression in thyme-treated and inoculated apples was 4- and 6-fold higher than in inoculated and water-treated apples. Neither thyme oil application nor B. cinerea inoculation markedly affected PR-5 expression. These results suggest that thyme oil induces resistance against B. cinerea through the priming of defense responses in apple fruit, and the PR-8 gene of apple may play a key role in the mechanism by which thyme essential oil effectively inhibits gray mold in apple fruit
HIV-Infected Children and Nutrition: The Friend and The Foe
The impact of nutrition on HIV-infected children has been evaluated in multiple studies. Our review of the current trends of nutrition-related studies revealed that the focus has moved from simply the disease consequences of HIV to ensuring that antiretroviral therapy-treated children are well nourished to ensure growth and development. This update aims to present the state of the art regarding nutrition of HIV-infected children and the real potential for nutrition to serve as a dynamic therapy in this group. Recent World Health Organization reports indicate that the HIV/AIDS disease is curbing in incidence worldwide despite the high 1.8 million children, less than 15 years, reported in 2017. In addition, the literature supports the complexity and bidirectional relation between nutrition and HIV. HIV infection has a substantial effect on the nutritional status, in particular, the gastrointestinal side effects, which, in turn, have a profound impact on HIV infection. Advances in the field have transformed the course of the disease into a chronic illness, where more attention was given to lifestyle and quality of life including nutrition. However, achievement of food security, nutrition accessibility, and appropriate handling of nutrition-related complications of HIV infection are remarkable challenges, particularly, in resource poor environments, where most HIV infections exist
Immunohistochemical expression and distribution of orexin, orphanin and leptin in the major salivary glands of some mammals
Abstract: The aim of the study was to assess the involvement of apoptotic factors, cytokeratins and metalloproteinase-
9 in the histogenesis of both Epithelialized Gingival Lesions (EGL) and Periapical Lesions (PAL).
55 consecutive patients, 30 with PAL and 25 with EGL, were selected for the study after clinical and radiological
examinations. The PAL patients had severe periapical lesions and tooth decay with exposure of the pulp chamber.
All PAL and EGL biopsies were surgically extracted, fixed in 10% buffered formalin, and processed for
routine light microscopy. Ten biopsies of each category were processed for immunohistochemistry (IHC). Serial
paraffin sections were stained by IHC with appropriate antibodies to detect cytokeratins (CKs) 1, 5, 8, 10 and 14,
caspase-3 and -9, metalloproteinase-9, and for PCNA and TUNEL assays. Both PAL and EGL showed a high
expression of the cytokeratin 1, 5 and 8 with higher expression in EGL. Moreover, CK10 was markedly less
intense expressed in EGL compared to PAL, while CK14 was almost three times stronger expressed in EGL.
The expression of caspase-3 and -9 was stronger in PAL compared to EGL, however, the difference was only
significant for caspase-9. In PAL apoptosis detected by TUNNEL method and the expression of MMP-9 were
higher than in EGL, whereas PCNA was significantly more expressed in EGL. The results clearly suggest that
both lesions have exclusively an epithelial origin and that epithelial proliferation was correlated with the degree
of apoptosis in both entities. PAL and EGL presented mostly similar cytokeratin expression except for CK10
and CK14, though with marked differences in the distribution and intensity of IHC reactions. Finally, the degradation
of extracellular matrix in both lesions could be partially attributed to the strong presence of MMP-9.

orphanin FQ in the major salivary glands (parotid, submandibular and sublingual) of rat, sheep and cow. These
peptides, originally synthesized in central nervous system, adipose tissue and peripheral tissues including gastrointestinal
tract, play an orexigenic (orphanin and orexin) or anorexigenic (leptin) roles in the intricate neuronal
network appointed to the control of nutritional homeostasis. Peptide-specific immunoreactivity was present
in the studied salivary glands with various intensities in different species, in the ductal epithelium, sometimes in
the acinar epithelium, and in nervous trunks spread in connective tissue stroma. The obtained data show that
salivary glands present an unexpected source of orexigenic and anorexigenic peptides which with their autocrine,
paracrine, and endocrine mechanisms of action may participate in the control of salivary gland function
