    A timely computer-aided detection system for acute ischemic and hemorrhagic stroke on CT in an emergency environment

    Standalone Presentations: no. LL-IN1105BACKGROUND: When a patient is accepted in the emergency room suspected of stroke, time is of the most importance. The infarct brain area suffers irreparable damage as soon as three hours after the onset of stroke symptoms. Non-contrast CT scan is the standard first line of investigation used to identify hemorrhagic stroke cases. However, CT brain images do not show hyperacute ischemia and small hemorrhage clearly and thus may be missed by emergency physicians. We reported a timely computer-aided detection (CAD) system for small hemorrhages on CT that has been successfully developed as an aid to ER physicians to help improve detection for Acute Intracranial Hemorrhage (AIH). This CAD system has been enhanced for diagnosis of acute ischemic stroke in addition to hemorrhagic stroke, which becomes a more complete and clinically useful tool for assisting emergency physicians and radiologists. In the detection algorithm, brain matter is first segmented, realigned, and left-right brain symmetry is evaluated. As in the AIH system, the system confirms hemorrhagic stroke by detecting blood presence with anatomical and medical knowledge-based criteria.     Highly stable, ligand-clustered “patchy” micelle nanocarriers for systemic tumor targeting

    A novel linear-dendritic block copolymer has been synthesized and evaluated for targeted delivery. The use of the dendron as the micellar exterior block in this architecture allows the presentation of a relatively small quantity of ligands in clusters for enhanced targeting, thus maintaining a long circulation time of these “patchy” micelles. The polypeptide linear hydrophobic block drives formation of micelles that carry core-loaded drugs, and their unique design gives them extremely high stability in vivo. We have found that these systems lead to extended time periods of increased accumulation in the tumor (up to 5 days) compared with nontargeted vehicles.     Effect of N and K fertilizers on nutrient leaching and groundwater quality under mature oil palm in Sabah during the monsoon period

    Problem statement: The oil palms are mainly grown in the humid tropics with high rainfall. Soluble Nitrogen (N) and Potassium (K) fertilizers are commonly required by the oil palm plantations to maximize palm productivity due to the highly weathered soils with low fertility. Thus, leaching losses of N and K nutrients may be unavoidable and these nutrients may move further downward and eventually cause groundwater pollution. This study reports the leaching of N and K nutrients in a mature oil palm field as affected by fertilizer rates and soil depths and its effect on groundwater quality during the monsoon period in Tawau, Sabah. Approach: The sources of N and K fertilizer were Ammonium Chloride (AC) and Muriate Of Potash (MOP), respectively. Soil water samplers were installed at depths of 30, 60 and 120 cm in four fertilizer treatments, namely, N0P0K0 (Control plot, no N and K), N0P2K1 (K1 = 4.5 kg MOP palm-1 year-1), N1P2K1 (N1 = 3.75 AC kg palm-1 year-1) and N1P2K0. Three replications were used in the experiment. Monitoring wells were installed in the above treatment plots and in another treatment, N2P2K1 (N2 = 7.5 kg AC palm-1 year-1) to investigate the effect of excessive N rate on groundwater quality. Samplings were done at 15 day intervals for duration of 150 days from October 2008-February 2009 to cover the entire monsoon period in North Borneo. Water samples were analyzed for NH4-N by automated phenate method, NO3-N + NO2-N and NO2-N by automated hydrazine reduction method on Auto Analyzer 3 and K by flame photometric method using flame photometer. Results: The mean NH4-N concentration of N1P2K1 at 33.69 mg L-1 was significantly higher than N1P2K0 at 8.15 mg L-1. In the presence of K, NH4-N concentrations increased 4.1 fold when N fertilizer was applied and 3.5 times in the absence of N application. The mean NH4-N concentration was 17.89 mg L-1 at 30 cm depth declining to 12.19 and 6.52 mg L-1 at soil depths of 60 and 120 cm, respectively. The transformation of NH4-N to NO3-N was not a major process during the monsoon period. The leaching losses of inorganic N were 1.0 and 1.6% of the applied fertilizer for N1P2K0 and N1P2K1 respectively. For K, the leaching losses were 5.3 and 2.4% for N0P2K1 and N1P2K1 respectively. The concentrations of NH4-N, NO3-N and K in groundwater ranged from 0.23-2.7, 0.07-0.25 and 0.63-9.54 mg L-1, respectively. Conclusion/Recommendations: N and K concentrations in the soil solution decreased with soil depth and their leaching losses were related to rainfall pattern, fertilizer treatment and nutrient uptake by roots. Groundwater quality was not affected by the applications of N and K fertilizers at the optimum rates for mature oil palms

    Ultimate behavior of idealized composite floor elements at ambient and elevated temperature

    This paper is concerned with the ultimate behavior of composite floor slabs under extreme loading situations resembling those occurring during severe building fires. The study focuses on the failure state associated with rupture of the reinforcement in idealized slab elements, which become lightly reinforced in a fire situation due to the early loss of the steel deck. The paper describes a fundamental approach for assessing the failure limit associated with reinforcement fracture in lightly reinforced beams, representing idealized slab strips. A description of the ambient-temperature tests on isolated restrained elements, carried out to assess the influence of key material parameters on the failure conditions, is firstly presented. The results of a series of material tests, undertaken mainly to examine the effect of elevated temperature on ductility, are also described. A simplified analytical model is employed, in conjunction with the experimental findings, to assess the salient material parameters and their implications on the ultimate response at both ambient and elevated temperature. © 2009 Springer Science+Business Media, LLC

    Identification of H3K4me1-associated proteins at mammalian enhancers.

    Enhancers act to regulate cell-type-specific gene expression by facilitating the transcription of target genes. In mammalian cells, active or primed enhancers are commonly marked by monomethylation of histone H3 at lysine 4 (H3K4me1) in a cell-type-specific manner. Whether and how this histone modification regulates enhancer-dependent transcription programs in mammals is unclear. In this study, we conducted SILAC mass spectrometry experiments with mononucleosomes and identified multiple H3K4me1-associated proteins, including many involved in chromatin remodeling. We demonstrate that H3K4me1 augments association of the chromatin-remodeling complex BAF to enhancers in vivo and that, in vitro, H3K4me1-marked nucleosomes are more efficiently remodeled by the BAF complex. Crystal structures of the BAF component BAF45C indicate that monomethylation, but not trimethylation, is accommodated by BAF45C's H3K4-binding site. Our results suggest that H3K4me1 has an active role at enhancers by facilitating binding of the BAF complex and possibly other chromatin regulators

    Coherent optical wavelength conversion via cavity-optomechanics

    We theoretically propose and experimentally demonstrate coherent wavelength conversion of optical photons using photon-phonon translation in a cavity-optomechanical system. For an engineered silicon optomechanical crystal nanocavity supporting a 4 GHz localized phonon mode, optical signals in a 1.5 MHz bandwidth are coherently converted over a 11.2 THz frequency span between one cavity mode at wavelength 1460 nm and a second cavity mode at 1545 nm with a 93% internal (2% external) peak efficiency. The thermal and quantum limiting noise involved in the conversion process is also analyzed, and in terms of an equivalent photon number signal level are found to correspond to an internal noise level of only 6 and 4x10-3 quanta, respectively.Comment: 11 pages, 7 figures, appendi

    Outcome of anal symptoms and anorectal function following two obstetric anal sphincter injuries (OASIS)-a nested case-controlled study.

    INTRODUCTION AND HYPOTHESIS: Obstetric anal sphincter injury (OASI) is a significant risk factor for developing anal incontinence. It can therefore be hypothesised that recurrent OASI in a subsequent delivery may predispose women to further anal sphincter dysfunction. METHODS: A nested case-controlled study based on data collected prospectively between 2006 and 2019. Women matched for age and ethnicity, with a history of one OASI and no sphincter damage in a subsequent delivery (control) were compared to women sustaining a second OASI. Assessment was carried out using the St Mark's score (SMIS), anorectal manometry and endoanal ultrasound scan (findings quantified using the modified Starck score). RESULTS: Eighty-four women were included and equally distributed between the two groups, who were followed up 12 weeks postnatally. No difference in SMIS scores was found. Maximum resting pressure (MRP, mmHg) and maximum squeeze pressure (MSP, mmHg) were significantly reduced in the study group. Median (IQR) MRP in the study group was 40.0 (31.3-54.0) versus 46.0 (39.3-61.5) in the control group (p = 0.030). Median (IQR) MSP was 73.0 (58.3-93.5) in the study group versus 92.5 (70.5-110.8) (p = 0.006) in the control group. A significant difference (p = 0.002) was found in the modified Starck score between the study group (median 0.0 [IQR 0.0-6.0]) and control group (median 0.0 [IQR 0.0-0.0]). CONCLUSIONS: We have demonstrated that women with recurrent OASI do not have significant anorectal symptoms compared to those with one OASI 12 weeks after delivery, but worse anal sphincter function and integrity. Therefore, on long-term follow-up, symptoms may possibly develop. This information will be useful when counselling women in a subsequent pregnancy
