    Measurements of nonlinear harmonic generation at the Advanced Photon Source's SASE FEL

    SASE saturation was recently achieved at the Advanced Photon Source's SASE FFL in the low-energy undulator test line at 530 nm and 385 urn. The electron beam microbunching becomes more and more prominent until saturation is achieved. This bunching causes nonlinear harmonic emission that extends the usefulness of a SASE system in achieving shorter FEL wavelengths for the same electron beam energy. We have investigated the intensity of the fundamental and second harmonic undulator radiation as a function of distance along the undulator line and present the experimental results and compare them to numerical simulations. In addition, we have measured the single-shot second harmonic spectra as well as the simultaneous fundamental and second harmonic spectra and present the experimental results.     Evaluation of the risk of a stripping perforation with gates-glidden drills: serial versus crown-down sequences

    The aim of this study was to evaluate the remaining dentine/cementum thickness using Gates-Glidden burs in serial and crown-down sequences and to observe which of the two sequences is the safest for preparing mesial roots of molars. Thirty-six left and right human mandibular first molars were selected. Standard access cavities were made and initially explored with Flexofiles sizes 10 and 15 until the tip was visible at the apex. The teeth were embedded in a muffle specially developed for this study using a PVC tube with two parallel metal rods in its lid. Each tooth-block was sectioned 3 mm apically to the furcation using a low-speed saw with a diamond disc. The tooth-block was examined under a microscope and an initial image was captured by a digital video system with 8 X and 12 X magnifications. Finally, the tooth-blocks were reassembled in the muffle so that the canals could be instrumented. After instrumentation the area of each mesial canal as well as the smallest distance to the root furcation were measured again. The mesio-buccal canals (crown-down order) and the mesio-lingual canals (serial sequence) presented an average area of 0.46 ± 0.16 mm² and 0.88 ± 0.27 mm² (P < 0.01), respectively. The mean values of the smallest distance to the furcation for the mesio-buccal and mesio-lingual canals were 0.66 ± 0.19 mm and 0.39 ± 0.13 mm (P < 0.01), respectively. The remaining dentine/cementum thickness using Gates-Glidden burs was greater in the crown-down sequence than in the serial sequence

    A survey of adoption of endodontic nickel-titanium rotary instrumentation part 2: community and hospital dental practitioners in Wales

    Objectives: To assess adoption of endodontic nickel-titanium rotary technology (NiTi) by community and hospital dental clinicians within Wales and identify factors that may restrict uptake. Design: Postal questionnaire. Setting: Community and hospital-based dentists in Wales. Methods: Community and hospital-based dentists with a remit for provision of restorative dentistry (community dental setting = 32; hospital dental setting = 36) were approached regarding their usage or otherwise of nickel-titanium rotary instrumentation for endodontic treatments. The postal questionnaire took the form of an anonymous survey comprising 12 questions. These questions covered usage parameters, satisfaction and training and broached reasons for NiTi avoidance. Results: The response rate was 77%. NiTi rotary instruments were used routinely by an encouraging 82% of those in the hospital-based restorative dental services but only 13% of community staff. Factors cited as being implicated in the decision to avoid their use included cost (62% of responses) lack of training and the perceived lack of benefit. Conclusion: The adoption of rotary NiTi endodontic technology by the hospital dental practitioners of Wales is encouraging with the majority having converted to such systems in excess of three years prior to the survey. There was, however, a significant disparity in NiTi usage between community and hospital settings, the implications for which and possible solutions for increased training and uptake are discussed

    Present status and recent results from the APS SASE FEL

    The Low-Energy Undulator Test Line (LEUTL) at the Advanced Photon Source, Argonne National Laboratory, is intended to demonstrate the basic operation of a SASE-based free-electron laser. Goals include comparison of experimental results With theoretical predictions and scaling laws, identification of problems relevant to fourth-generation light source construction and operation and the means of addressing them, the development of operational and diagnostic techniques to optimize SASE FEL performance and increase repeatability from run to run. and performance of initial pioneering experiments capable of exploiting the unique properties of the laser. The basic layout and operational philosophy of the LEUTL experiment is presented. A summary of past results, including saturation, is reviewed, and a description of recent results is presented. We conclude with future plans, which include pressing to shorter wavelengths and incorporating user experiments into the LEUTL experimental program

    Subcutaneous emphysema in patients undergoing root canal treatment: a systematic review of the factors affecting its development and management

    Background: Subcutaneous emphysema is an infrequent mishap during root canal treatment which, in rare cases, can lead to severe complications. Aim: To systematically review the literature on the factors affecting the development of subcutaneous emphysema during root canal treatment, and on its management. Data sources: An electronic search was conducted in EMBASE (1947–2018), LILACS (1982–2018), PubMed (1950–2018), SciELO (1997–2018), Scopus (1970–2018), Web of Science (1900–2018) and two grey literature databases. Moreover, all issues of nine journals and four endodontic textbooks were hand-searched. Study eligibility criteria, participants, interventions: The retrieved studies were screened by two reviewers to select clinical studies, case reports or case series describing subcutaneous emphysema that developed during or immediately after root canal treatment in adult patients. Study appraisal and synthesis methods: Included studies were critically appraised according to a custom list of quality requirements. The extracted data were arranged in tables, and combined through a narrative synthesis. Results: The search retrieved 99 unique articles. Thirty six case reports and 15 case series describing a total of 65 cases of subcutaneous emphysema were included in this review. The methodological quality was medium. Reported cases of emphysema more often involved females and maxillary teeth. Drying of the root canal system with air under pressure, inadvertent extrusion of hydrogen peroxide through the apical foramen, the air-water spray produced by handpieces or laser devices, and the use of ozone gas were the most commonly suspected causes. Its management involved prescription of antibiotics and NSAIDs/analgesics, local application of ice packs or compresses and hospitalization. The signs and symptoms resolved completely within 1-17 days. Limitations: Case reports and case series are a low level of evidence. Conclusions and implications of key findings: Subcutaneous emphysema can develop during both nonsurgical and surgical root canal treatment. Pressurized air streams or air-water sprays should not be directed towards the root canals or areas with mucosal discontinuity. None of the management approaches were clearly related to expedited recovery. Guidelines should be developed in order to avoid unnecessary or potentially harmful interventions
