Cognitive Functioning and Hippocampal Connectivity in Patients With Longstanding Type 1 Diabetes and Apolipoprotein E ε4
OBJECTIVE: While the apolipoprotein E ε4 allele (ApoE-ε4) is related to cognitive and brain decline in the general population, its effect on the brain in type 1 diabetes mellitus (T1DM) remains unclear. Therefore, the aim was to determine the interaction between ApoE-ε4 and T1DM on cognitive performance and hippocampal structure and connectivity as the brain area most vulnerable to ApoE-ε4 effects in adult patients with T1DM. RESEARCH DESIGN AND METHODS: Blood sampling was performed in 104 patients with T1DM and 49 control subjects for ApoE genotyping, neuropsychology, and neuroimaging to determine hippocampal volume and resting-state connectivity. The interaction between T1DM status and ApoE-ε4 presence was investigated and adjusted for age and mean systolic blood pressure. RESULTS: ApoE genotyping could not be performed for three patients with T1DM. Significant interaction effects, indicating a differential effect of ApoE-ε4 between both groups, were found for overall cognitive functioning and for the subdomains of information processing speed and attention. Additionally, interaction effects were present for right hippocampal connectivity with the right posterior cingulate and supramarginal gyri. Subsequent group analysis showed that patients with T1DM with ApoE-ε4 performed worse on these cognitive domains with increased connectivity, relative to their counterparts without ApoE-ε4. In contrast, no cognitive effects, but decreased connectivity, were observed in control subjects with ApoE-ε4. In patients with T1DM, higher right hippocampus connectivity with the posterior cingulate gyrus was related to poorer overall cognitive functioning. CONCLUSIONS: The results may suggest that ApoE-ε4 presence leaves our patients with T1DM more susceptible to cognitive decrements at a younger age, possibly through vascular pathways, warranting further longitudinal studies

Health-related quality of life of child and adolescent retinoblastoma survivors in the Netherlands
To assess health-related quality of life (HRQoL) in children (8-11 years) and adolescents (12-18 years) who survived retinoblastoma (RB), by means of the KIDSCREEN self-report questionnaire and the proxy-report version. This population-based cross-sectional study (participation rate 70%) involved 65 RB survivors (8-18 years) and their parents. Child/adolescents' and parents' perception of their youth's HRQoL was assessed using the KIDSCREEN, and the results were compared with Dutch reference data. Relations with gender, age, marital status of the parents, and visual acuity were analyzed. RB survivors reported better HRQoL than did the Dutch reference group on the dimensions "moods and emotions" and "autonomy". Increased ratings of HRQoL in RB survivors were mainly seen in perceptions of the younger children and adolescent girls. RB survivors with normal visual acuity scored higher on "physical well-being" than visually impaired survivors. Age was negatively associated with the dimensions "psychological well-being", "self-perception" (according to the child and parent reports) and "parent relations and home life" (according to the child). "Self-perception" was also negatively associated with visual acuity (according to the child). Only parents of young boys surviving RB reported lower on "autonomy" than the reference group, and parents of low visual acuity and blind RB survivors reported higher on "autonomy" than parents of visually unimpaired survivors. Survivors' perceptions and parents' perceptions correlated poorly on all HRQoL dimensions. RB survivors reported a very good HRQoL compared with the Dutch reference group. The perceptions related to HRQoL differ substantially between parents and their children, i.e. parents judge the HRQoL of their child to be relatively poorer. Although the results are reassuring, additional factors of HRQoL that may have more specific relevance, such as psychological factors or coping skills, should be explore
One More Awareness Gap? The Behaviour–Impact Gap Problem
Preceding research has made hardly any attempt to measure the ecological impacts of pro-environmental behaviour in an objective way. Those impacts were rather supposed or calculated. The research described herein scrutinized the ecological impact reductions achieved through pro-environmental behaviour and raised the question how much of a reduction in carbon footprint can be achieved through voluntary action without actually affecting the socio-economic determinants of life. A survey was carried out in order to measure the difference between the ecological footprint of “green” and “brown” consumers. No significant difference was found between the ecological footprints of the two groups—suggesting that individual pro-environmental attitudes and behaviour do not always reduce the environmental impacts of consumption. This finding resulted in the formulation of a new proposition called the BIG (behaviour–impact gap) problem, which is an interesting addition to research in the field of environmental awareness gaps
Learning Gradient Fields for Shape Generation
In this work, we propose a novel technique to generate shapes from point
cloud data. A point cloud can be viewed as samples from a distribution of 3D
points whose density is concentrated near the surface of the shape. Point cloud
generation thus amounts to moving randomly sampled points to high-density
areas. We generate point clouds by performing stochastic gradient ascent on an
unnormalized probability density, thereby moving sampled points toward the
high-likelihood regions. Our model directly predicts the gradient of the log
density field and can be trained with a simple objective adapted from
score-based generative models. We show that our method can reach
state-of-the-art performance for point cloud auto-encoding and generation,
while also allowing for extraction of a high-quality implicit surface. Code is

Adverse prognostic and predictive significance of low DNA-dependent protein kinase catalytic subunit (DNA-PKcs) expression in early-stage breast cancers
Background: DNA-dependent protein kinase catalytic subunit (DNA-PKcs), a serine threonine kinase belonging to the PIKK family (phosphoinositide 3-kinase-like-family of protein kinase), is a critical component of the non-homologous end joining (NHEJ) pathway required for the repair of DNA double strand breaks. DNA-PKcs may be involved in breast cancer pathogenesis. Methods: We evaluated clinicopathological significance of DNA-PKcs protein expression in 1161 tumours and DNA-PKcs mRNA expression in 1950 tumours. We correlated DNA-PKcs to other markers of aggressive phenotypes, DNA repair, apoptosis and cell cycle regulation. Results: Low DNA-PKcs protein expression was associated with higher tumour grade, higher mitotic index, tumour de-differentiation and tumour type (ps<0.05). Absence of BRCA1, low XRCC1/SMUG1/APE1/Polβ were also more likely in low DNA-PKcs expressing tumours (ps<0.05). Low DNA-PKcs protein expression was significantly associated with worse breast cancer specific survival (BCCS) in univariate and multivariate analysis (ps<0.01). At the mRNA level, low DNA-PKcs was associated with PAM50.Her2 and PAM50.LumA molecular phenotypes (ps<0.01) and poor BCSS. In patients with ER positive tumours who received endocrine therapy, low DNA-PKcs (protein and mRNA) was associated with poor survival. In ER negative patients, low DNA-PKcs mRNA remains significantly associated with adverse outcome. Conclusions: Our study suggests that low DNA-PKcs expression may have prognostic and predictive significance in breast cancers

An Association of Multiple Well Differentiated Liposarcomas, Lipomatous Tissue and Hereditary Retinoblastoma
Well differentiated liposarcoma (atypical lipomatous tumour) is a low grade tumour, with no metastatic potential unless
dedifferentiation supervenes. When superficial, it recurs locally only occasionally after marginal excision. We present a
patient in whom bilateral childhood retinoblastoma was followed by later development of massive confluent areas of low
grade liposarcoma and lipomatous tissue affecting the upper extremities and trunk. We discuss the role of mutations in the
retinoblastoma gene (RB1) in linking these conditions and demonstrate the surgical management of an extremely unusual
and challenging case
Altered Eigenvector Centrality is Related to Local Resting-State Network Functional Connectivity in Patients with Longstanding Type 1 Diabetes Mellitus
Introduction: Longstanding type 1 diabetes (T1DM) is associated with microangiopathy
and poorer cognition. In the brain, T1DM is related to increased functional resting-state
network (RSN) connectivity in patients without, which was decreased in patients with
clinically evident microangiopathy. Subcortical structure seems affected in both patient
groups. How these localized alterations affect the hierarchy of the functional network in
T1DM is unknown. Eigenvector centrality mapping (ECM) and degree centrality are
graph theoretical methods that allow determining the relative importance (ECM) and
connectedness (degree centrality) of regions within the whole-brain network hierarchy.
Methods: Therefore, ECM and degree centrality of resting-state functional MRI-scans
was compared between 51 patients with, 53 patients without proliferative retinopathy,
and 49 controls, and associated with RSN connectivity, subcortical gray matter volume,
and cognition.
Results: In all patients versus controls, ECM and degree centrality were lower in the
bilateral thalamus and the dorsal striatum, with lowest values in patients without
proliferative retinopathy (PFWE<0.05). Increased ECM in this group versus patients with
proliferative retinopathy was seen in the bilateral lateral occipital cortex, and in the right
lateral cortex versus controls (PFWE<0.05). In all patients, ECM and degree centrality
were related to altered visual, sensorimotor, and auditory and language RSN connectivity
(PFWE0.05).
Conclusion: Our findings suggest reorganization of the hierarchy of the cortical
connectivity network in patients without proliferative retinopathy, which is lost with
disease progression. Centrality seems sensitive to capture early T1DM-related functional
connectivity alterations, but not disease progression
