    Hepatic artery thrombosis after liver transplantation: Radiologic evaluation

    Hepatic artery thrombosis after liver transplantation is a devastating event requiring emergency retransplantation in most patients. Early clinical signs are often nonspecific. Before duplex sonography (combined real-time and pulsed Doppler) capability was acquired in October 1984, 76% of all transplants in this institution referred for angiography with a clinical suspicion of hepatic artery thrombosis had patent arteries. In an effort to reduce the number of negative angiograms, CT, real-time sonography, and pulsed Doppler have been evaluated as screening examinations to determine which patients need angiography. Of 14 patients with focal inhomogeneity of the liver architecture detected by CT and/or real-time sonography, 12 (86%) had hepatic artery thrombosis, one had slow arterial flow with hepatic necrosis, and one had a biloma with a patent hepatic artery. In 29 patients undergoing duplex sonography of the hepatic artery, six (21%) had absence of a Doppler arterial pulse. All six had abnormal angiograms: Four had thrombosis, one had a significant stenosis, and one had slow flow with biopsy-proven ischemia. Of 23 patients with a Doppler pulse, two had hepatic artery thrombosis at surgery. However, real-time sonography demonstrated focal inhomogeneity in the liver in both cases. Our data demonstrate that pulsed Doppler of the hepatic artery combined with real-time sonography of the liver parenchyma currently is the optimal screening test for selecting patients who require hepatic angiography after liver transplantation.     A Review of Sustainable Agroforestry Practices as Climate Change Adaption and Mitigation Strategy in Nigeria

    Agroforestry is one of the most conspicuous land use systems across landscapes and agroecological zones in Nigeria. With food shortages and increased threats of climate change, interest in agroforestry is gathering for its potential to address various on-farm adaptation needs, and fulfill many roles in mitigation pathways. Agroforestry provides assets and income from carbon, wood energy, improved soil fertility and enhancement of local climate conditions; it provides ecosystem services and reduces human impacts on natural forests. Most of these benefits have direct benefits for local adaptation while contributing to global efforts to control atmospheric greenhouse gas concentrations. This paper presents recent findings on how agroforestry as a sustainable practice helps to achieve both mitigation and adaptation objectives while remaining relevant to the livelihoods of the poor smallholder farmers in Nigeria.     Maternal Obesity in Early Pregnancy and Subsequent Pregnancy Outcome in a Nigerian Population

    Despite a rising prevalence worldwide, there is limited data on pregnancy outcome among African women with prepregnancy or early pregnancy obesity. This was a case-control study to determine the prevalence of maternal obesity in early pregnancy and compare the subsequent pregnancy outcome between 201 women with obesity and 201 non-obese controls in a University Teaching Hospital in Nigeria. The prevalence of obesity in early pregnancy was 9.63%. Obesity was significantly associated with advanced maternal age and parity ≥ 1. It was also a risk factor for pregnancy induced hypertension, admissions during pregnancy, caesarean delivery and associated with 5th minute apgar score ≤3 (0.044). Obesity in early pregnancy is a risk factor for adverse pregnancy outcome among pregnant Nigerian women. This information should be utilised by physicians to improve the outcome of pregnancy and promote safe motherhood

    Pneumocephalie sous durale expansive apres une ventriculo- cisternostomie endoscopique

    La pneumocéphalie sous durale est une complication classique de la ventriculo-cisternostomie endoscopique (VCE). Il s’agit le plus souvent d’une pneumocéphalie simple sans traduction clinique. . Nous rapportons le cas d’une femme de 38 ans, ayant subi une VCE pour une hydrocéphalie par sténose de l’aqueduc du mésencéphale. Elle a développé une hémiparésie gauche postopératoire avec un retard de réveil. Le scanner cérébral de contrôle a montré une importante pneumocéphalie sous-durale expansive (signe du Mont Fuji) prédominante à droite. Une surveillance en position TRENDELENBURG, une réhydratation et une oxygénation au masque ont permis d’obtenir une régression complète de l’hémiparésie et la résorption de la pneumocéphalie sur le scanner à J3 postopératoire.Mots clés : Ventriculocisternostomie ; Pneumocéphalie ; Complication

    O Lúpus Eritematoso Sistémico (LES) é uma doença multissistémica, auto-imune, caracterizada por inflamação vascular e do tecido conjuntivo, com anticorpos antinucleares. As manifestações clínicas são variáveis, com uma história natural progressiva e imprevisível. Apresentamos o caso de uma adolescente, com antecedentes de artrite dos joelhos, febre e astenia de etiologia não esclarecida, com um ano de evolução. Internada na Unidade de Cuidados Intensivos Pediátricos por pneumonia bilateral a Streptococcus pneumoniae complicada com derrame pleural, anasarca e hematúria macroscópica. Iniciou antibioticoterapia, com evolução favorável, após o que se verifica agravamento clínico, com reaparecimento do derrame pleural, lesões vesiculares disseminadas sugestivas de etiologia herpética, hipertensão arterial sintomática e convulsão tónico-clónica generalizada. Da investigação, destaca-se estudo imunológico compatível com LES em actividade, poliserosite, proteinúria nefrótica, nefrite lúpica classe IV e anemia grave

    Direct and Inverse Variational Problems on Time Scales: A Survey

    We deal with direct and inverse problems of the calculus of variations on arbitrary time scales. Firstly, using the Euler-Lagrange equation and the strengthened Legendre condition, we give a general form for a variational functional to attain a local minimum at a given point of the vector space. Furthermore, we provide a necessary condition for a dynamic integro-differential equation to be an Euler-Lagrange equation (Helmholtz's problem of the calculus of variations on time scales). New and interesting results for the discrete and quantum settings are obtained as particular cases.     Gravitational lensing in the Kerr-Randers optical geometry

    A new geometric method to determine the deflection of light in the equatorial plane of the Kerr solution is presented, whose optical geometry is a surface with a Finsler metric of Randers type. Applying the Gauss-Bonnet theorem to a suitable osculating Riemannian manifold, adapted from a construction by Naz\i m, it is shown explicitly how the two leading terms of the asymptotic deflection angle of gravitational lensing can be found in this way.Comment: 7 pages, 1 figure.     Amplified and Homozygously Deleted Genes in Glioblastoma: Impact on Gene Expression Levels

    BACKGROUND: Glioblastoma multiforme (GBM) displays multiple amplicons and homozygous deletions that involve relevant pathogenic genes and other genes whose role remains unknown. METHODOLOGY: Single-nucleotide polymorphism (SNP)-arrays were used to determine the frequency of recurrent amplicons and homozygous deletions in GBM (n = 46), and to evaluate the impact of copy number alterations (CNA) on mRNA levels of the genes involved. PRINCIPAL FINDINGS: Recurrent amplicons were detected for chromosomes 7 (50%), 12 (22%), 1 (11%), 4 (9%), 11 (4%), and 17 (4%), whereas homozygous deletions involved chromosomes 9p21 (52%) and 10q (22%). Most genes that displayed a high correlation between DNA CNA and mRNA levels were coded in the amplified chromosomes. For some amplicons the impact of DNA CNA on mRNA expression was restricted to a single gene (e.g., EGFR at 7p11.2), while for others it involved multiple genes (e.g., 11 and 5 genes at 12q14.1-q15 and 4q12, respectively). Despite homozygous del(9p21) and del(10q23.31) included multiple genes, association between these DNA CNA and RNA expression was restricted to the MTAP gene. CONCLUSIONS: Overall, our results showed a high frequency of amplicons and homozygous deletions in GBM with variable impact on the expression of the genes involved, and they contributed to the identification of other potentially relevant genes
