    Backward scattering of low-energy antiprotons by highly charged and neutral uranium: Coulomb glory

    Collisions of antiprotons with He-, Ne-, Ni-like, bare, and neutral uranium are studied theoretically for scattering angles close to 180∘^{\circ} and antiproton energies with the interval 100 eV -- 10 keV. We investigate the Coulomb glory effect which is caused by a screening of the Coulomb potential of the nucleus and results in a prominent maximum of the differential cross section in the backward direction at some energies of the incident particle. We found that for larger numbers of electrons in the ion the effect becomes more pronounced and shifts to higher energies of the antiproton. On the other hand, a maximum of the differential cross section in the backward direction can also be found in the scattering of antiprotons on a bare uranium nucleus. The latter case can be regarded as a manifestation of the screening property of the vacuum-polarization potential in non-relativistic collisions of heavy particles.Comment: 14 pages, 5 figure

    Analysis of the RC catalog sample in the region overlapping with the regions of the FIRST and SDSS surveys: I. Identification of sources with the VLSS, TXS, NVSS, FIRST, and GB6 catalogs

    Radio sources of the RC catalog produced in 1980--1985 at RATAN-600 radio telescope based on a deep survey of a sky strip centered on the declination of the SS 433 source are optically identified in the region overlapping with FIRST and SDSS surveys (about 132 square degrees). The NVSS catalog was used as the reference catalog for refining the coordinates of the radio sources. The morphology is found for about 75% of the objects of the sample and the ratio of single, double and multicomponent radio sources is computed based on FIRST radio maps. The 74, 365, 1400, and 4850 MHz data of the VLSS, TXS, NVSS, FIRST, and GB6 catalogs are used to analyze the shape of the spectra.Comment: 16 pages, 3 figure

    General practitioner workforce planning: assessment of four policy directions

    <p>Abstract</p> <p>Background</p> <p>Estimating the supply of GPs into the future is important in forecasting shortages. The lengthy training process for medicine means that adjusting supply to meet demand in a timely fashion is problematic. This study uses Ireland as a case study to determine the future demand and supply of GPs and to assess the potential impact of several possible interventions to address future shortages.</p> <p>Methods</p> <p>Demand was estimated by applying GP visit rates by age and sex to national population projections. Supply was modelled using a range of parameters derived from two national surveys of GPs. A stochastic modelling approach was adopted to determine the probable future supply of GPs. Four policy interventions were tested: increasing vocational training places; recruiting GPs from abroad; incentivising later retirement; increasing nurse substitution to enable practice nurses to deliver more services.</p> <p>Results</p> <p>Relative to most other European countries, Ireland has few GPs per capita. Ireland has an ageing population and demand is estimated to increase by 19% by 2021. Without intervention, the supply of GPs will be 5.7% less than required in 2021. Increasing training places will enable supply to meet demand but only after 2019. Recruiting GPs from overseas will enable supply to meet demand continuously if the number recruited is approximately 0.8 per cent of the current workforce per annum. Later retirement has only a short-term impact. Nurse substitution can enable supply to meet demand but only if large numbers of practice nurses are recruited and allowed to deliver a wide range of GP services.</p> <p>Conclusions</p> <p>A significant shortfall in GP supply is predicted for Ireland unless recruitment is increased. The shortfall will have numerous knock-on effects including price increases, longer waiting lists and an increased burden on hospitals. Increasing training places will not provide an adequate response to future shortages. Foreign recruitment has ethical considerations but may provide a rapid and effective response. Increased nurse substitution appears to offer the best long-term prospects of addressing GP shortages and presents the opportunity to reshape general practice to meet the demands of the future.</p

    The impact of clerkships on students' specialty preferences: what do undergraduates learn for their profession?

    OBJECTIVE: Clinical experiences and gender have been shown to influence medical students' specialty choices. It remains unclear, however, which aspects of experiences make students favour some specialties and reject others. This study aimed to clarify the effects of clerkships on specialty choice and to identify explanatory factors. METHODS: We carried out a longitudinal cohort study to collect data on career preferences and attitudes towards future careers among 3 cohorts of students before and after clerkships in surgery (n = 200), internal medicine (n = 277) and general practice (n = 184). Regression analyses were performed to identify the determinants of career choice and the role of gender. RESULTS: Exposure to clinical settings encourages students to opt for a career in the corresponding specialty. Men were more stimulated than women by the general practice clerkship. Gender had no clear role as a predictor of career preference. The major predictor of career choice in all 3 specialties was positive evaluation of work-intrinsic factors. A preference for working with acute patients and technology-oriented work, prestige orientation and insignificance of a controllable lifestyle were determinants of a preference for surgery. Students with a preference for general practice had almost opposite preferences. Those who chose internal medicine favoured a controllable lifestyle. DISCUSSION: Factors other than gender appear to drive specialty decisions. Work content, type of patients and lifestyle options play major roles. Consequently, along with teaching about the practice of medicine, the matching of specialty preferences with reality is an essential outcome of clerkships. (aut. ref.

    Shortages in general practice despite feminisation of medicine: a seeming paradox? A cohort study.

    Background: Female medical students often prefer primary care specialties, while male students appear to be attracted to hospital specialties. Notwithstanding the steady feminisation of medicine, in many countries there are still difficulties in recruiting trainees for general practice. This seeming paradox raises the question on what specific role gender plays in a specialty choice. The authors looked at the (a) the role of gender in general practice specialty choice of Dutch medical students, (b) the decisive factors in career choice and relation of gender to these, and (c) differences in how male and female students are influenced by the GP clerkship. Methods: A cohort of 206 final year medical students at the Maastricht University, the Netherlands were asked to complete a questionnaire focusing on career preferences before and after a 12-week general practice clerkship and at graduation, a couple of months later. Results: gender was significantly related to willingness to become a GP in bivariate analysis. Adding variables in multivariate analysis made this effect disappear. While females expressed overall higher preference for general practice than males, after the GP clerkship likelihood of choosing general practice increased with 38% among male and 22% among female students. After graduation, interest in general practice had dropped, mainly among females. Attitudes predicting a GP career choice were: extrinsic career motivation before the clerkship, and the content of GP work (patient contacts, treatments) and motivation to work with chronic and palliative patients after the clerkship. Conclusions: Gender ‘as such’ appeared not to be a distinctive predictor of specialty choice. It is students’ attitudes towards GP work and preferred patient category that determine the career choice in general practice. However, more male students were positively influenced by the GP clerkship than female students. The motivating effect of the clerkship is not long lasting. Especially female graduates change their interest in favour of other specialties, which may explain why eventually few students choose general practice. It might be worthwhile to reinforce an initial preference for general practice by motivational guidance throughout the whole period of clerkships. (aut. ref.
