    Aim. An analysis of polemic and open topics in epidemiology, pathogenesis, clinic, diagnosis and treatment of postinfectious irritable bowel syndrome (IBS).Key points. The incidence of postinfectious IBS links to aetiological factors underlying the development of acute infectious gastroenteritis (bacteria, viruses or protozoans). The risk factors of postinfectious IBS have been identified: female gender, excessive anxiety, depression and somatisation, a severe sequence of acute infectious gastroenteritis, etc. The diagnosis of postinfectious IBS implies exclusion of organic diseases with similar clinical presentations. A specific postinfectious IBS treatment is currently lacking and adheres to non-postinfectious IBS strategies.Conclusion. The postinfectious IBS problematic is poorly understood and requires further research

    Dilation of the Infarct-Related Coronary Artery to Reduce the Incidence of the No-Reflow Phenomenon in STEMI Patients

    Background: The pathogenesis of slow/no-reflow phenomena is a critical socio-medical problem due to high mortality and work disability rates in patients with ST-segment elevation myocardial infarction (STEMI). Slow/no-reflow phenomena are multifactorial involving 4 key elements: 1) distal embolization of the coronary bed of the infarct-related coronary artery; 2) ischemic damage to the myocardium; 3) reperfusion injury of the heart muscle; 4) individual (genetic) susceptibility of the microcirculation to injury.   Objective: To analyze the outcomes of percutaneous coronary interventions (PCI) in patients with STEMI and TIMI 0 blood flow of an infarct-related coronary artery based on the strategy to restore antegrade blood flow (balloon predilation or dilation of an infarctrelated artery).   Materials and methods: We analyzed treatment outcomes of 209 patients with STEMI and TIMI 0 blood flow. The patients were grouped based on the PCI strategy: group 1 included 147 patients who underwent balloon angioplasty to restore antegrade blood flow, and group 2 included 62 patients who underwent dilation of an infarct-related coronary artery.   Results: Our study found that direct stenting in STEMI patients was associated with statistically significantly lower risk of slow/noreflow phenomena (P = 0.001, Pearson’s χ2) and, as a result, better functional outcomes of treatment (chronic heart failure grade classified according to Strazhesko-Vasilenko and by left ventricular ejection fraction) that were also statistically significant (P = 0.001, Pearson’s χ2).   Conclusions: Our study demonstrated that the risk of slow/no-reflow phenomena in patients with TIMI 0 blood flow of an infarctrelated coronary artery was statistically significantly lower (P = .001, Pearson’s χ2) in the group of patients who underwent dilation of an infarct-related coronary artery to restore antegrade blood flow. Functional outcomes (chronic heart failure grade and overall survival) were also better in this group of patients (P = .001, Pearson’s Chi-square). Moreover, dilation of an infarct-related coronary artery was associated with preserved left ventricular ejection fraction compared with the group of patients who underwent balloon angioplasty to restore antegrade blood flow (P < 0.001, Pearson’s χ2)

    Structure and bonding in WCn (n = 2–5) clusters

    Stochastic explorations of the configurational spaces for WC n (n = 2–5) clusters lead to densely populated spin states at each molecularity. We found 8, 16, 42, and 68 well-defined minima for n = 2, 3, 4, 5, respectively, in spin states ranging from singlets to quintuplets. The lowest energy isomers are triplets in all cases, except for n = 2 where there is competition between a quintuplet and a triplet state for the global minimum. The transition from planar to 3D structural preferences occurs between n = 4 and n = 5. For the global minima, the structures may be considered as the result of the interaction between two fragments: a tungsten cation and a covalently bonded anionic carbon chain. We found that spin–orbit (SO) effects reduce energy differences among isomers. Likewise, SO effects diminish as a function of the carbon content in the clusters to the point that for n = 5 they become negligible

    Comparative of relation of American, German, French and Russian guidelines on irritable bowel syndrome management

    Aim of review. To compare the American, German, French and Russian guidelines on irritable bowel syndrome (IBS) management. Summary. According to the American and French guidelines, the diagnosis of IBS should be made mainly according to compliance of clinical symptoms to Rome criteria of IBS, whereas German and Russian guidelines determine these symptoms as nonspecific and consider the diagnosis of IBS as the diagnosis of exclusion which can be made only after careful investigation of patients. At definition of treatment approaches the American guidelines do not include general actions, consider dietic modifications as insufficiently effective as well as with such drugs as antispasmodics, polyethyleneglycol and probiotics. On the contrary, the German, French and Russian guidelines pay special attention to general recommendations and dietary modification and consider above-mentioned pharmaceuticals as quite effective. Conclusion. Presented guidelines are similar to each other in respect of their plot; however significantly differ in diagnostic approach to IBS and assessment of efficacy of series of drugs.
